2.A Case of Cutaneous Metastasis of Male Breast Cancer.

Male breast cancer is a rare disease, representing approximately 1% of breast cancer and less than 1.5% of malignant tumors occurring in men. We report a case of cuaneous metastasis of male breast cances. a 64-year-old male who presented with multiple nodules and erythematous plaque on the right anterior chest and enlargement of the right axillary lymplinodes. The histopathologic findings of the nodule showed atypical tumor cells with a somew eat gland-like pattern. The cells showed potive reactions in PAS, DPAS and alcian blue stains, and also positive for estrogen and roiesterone receptors. Biopsy specimens taken from the plaque showed cellular arrangement in show line, referred to as Indian filing, between thiclenecin collagen bundles. He was treated not, tamoxifen, 5-fluorouracil, adriamycin, cyclophosphamide and megestrol acetate. 5 months after nitiation of the treatment, the skin lesions were improved vvith a decreased number and size or dules and decreased intensity of erythema.

3.A Case of Multiple Bowen' s Disease in a Leprosy Patient.

We report a case of multiple Bowen's disease associated with leprosy. A 68-year-old man was seen because of slightly pruritic multiple erythematous scaly patches and plaques on the trunk and the both extremities for 5 years. He has been treated for leprosy with many kinds of drugs consisting of the herb medications for over 40 years. He also showed decreased cell-mediated immunity in skin tests. It is considered that depressed cellular immunity may be an important predisposing factor in the development of mutiple Bowen's disease in this case, .although arsenics can not be completetly excluded.

4.Livedo Reticularis Idiopathica Associated with Mononeuropathy Multiplex Syndrome and Bilateral Ulnar-median Nerve Anastomosis.

A 31-year-old woman had a livedo reticularis of reticulated, bluish discoloration on both arms and legs for 18 months with a burning pain in the right hand and numbness in both lower legs. The findings of the electromyography were consistent with mononeuropathy multiplex and bilateral ulnar-median nerve anastomosis. A biopsy specimen from the right sural nerve showed perivascular lymphocytic infiltration and occasional myelin digestion chambers, which were consistent with vasculitic neuropathy. We present a patient with livedo reticularis idiopathica associated with mononeuropathy multiplex syndrome who also has bilateral ulnar-median nerve anastomosis.

5.The Expression of IL-4 and Interferon-gamma Genes in Peripheral Blood Mononuclear Cells of Patients with Severe Atopic Dermatitis:Evaluation in Proportion to Serum IgE Levels.

BACKGROUND: It is not yet clear whether the abnormal cytokine production in relation to serum IgE levels in atopic dermatitis (AD) is associated with the amount of mRNA of cytokine gene. OBJECTIVE: Our purpose was to delineate the effect of reciprocal correlation in the level of mRNA between interleukin-4 (I") and interferon-gamma (IFN-γ) in severe AD. METHODS: We examined 15 cases including 5 AD patients with high serum IgE (>2,000 kU/liter), 5 AD patients with low serum IgE (<100 kU/liter), and 5 healthy controls. Using semi quantitative reverse transcription-polymerase chain reaction, IL-4 and IFN-γ gene expressions in peripheral mononuclear cells (PBMC) were examined. RESULTS: 1) IL-4 gene expression in spontaneous PBMC was higher in AD patient groups than in control group, significantly higher only in AD patient group with high serum IgE level (p < 0.05). 2) IFN-γ gene expression in spontaneous PBMC showed increased tendency in AD patient groups than in control group without statistical significance. 3) IL-4 and IFN-γ gene expressions in stimulated PBMC were not different among all three groups. CONCLUSION: In light of our results, high and low IgE subgroups in AD can exist and AD may not be R characterized by the shift in the reciprocal relationship between IL-4 and IFN-γ when T cells are stimulated under antigen presenting cell-independent conditions.

6.Diseases and Health Service Utilization Patterns of Geriatric Inpatients.

BACKGROUND: Recently elderly people have increased in Korea and increased demands of medical service. This study was carried out to identify diseases and health service utilization patterns of geriatric inpatients to provide better understanding and basis planning to meet the needs of expanding geriatric population. METHOD: We use data of `Patient Survey in Korea` in 1999 and analyzed characteristics of disease structure and utilization patterns of health care service of 54,013 cases of discharged patients aged 65 years or more. RESULT: The proportion of the elderly in total discharged patients is 14.7%. Male to female ratio is 1:1.3 and most common age group was between 65 and 69 years old. 84.2% of elderly patients were admitted at secondary or tertiary health care facilities. The most common disease in geriatric inpatients is malig- nancy in male and vascular disease in female. The average duration of hospitalization is 14.2 days. They stay longer than the nation wide average hospital length and stay longest when they are admitted by mental and behavioral disorders. CONCLUSION: Geriatric patients are admitted mostly with chronic degenerative diseases such as neoplasm and vascular disease. Elders prefer to be admitted to tertiary hospitals and oriental hospitals. They stay more in hospital.

7.A Case of Papulonecrotic Tuberculid Combined with Erythema Nodosum.

We report an extremely rare case in which two tuberculids, papulonecrotic tuberculid and erythema nodosum, occurred together. A 16-year-old girl had two types of clinically distinct lesions; asymptomatic necrotic papules on the upper and lower extremities and painful subcutaneous nodules on the legs. A Mantoux test showed a strong positive reaction. Histopathologic findings of the necrotic papule and subcutaneous nodule were consistent with papulonecrotic tuberculid and erythema nodosum, respectively. There was no extracutaneous focus of tuberculosis. Polymerase chain reaction for tuberculous bacilli in the papulonecrotic tuberculid lesion revealed a negative result. Antituberculous therapy brought a rapid clinical improvement of both lesions.

9.Treatment of Radial Neck Fractures in Children.

In children, the radial neck fractures are treated in various ways, including long arm cast without closed reduction, long arm cast following C/R by Patterson's or Pesudo's method, open reduction with or without K-wire fixation. We treated 25 children with radial neck fracture who visited DaeDong General Hospital from March 1992 to February 1995, and followed up average 17.4 months (from 13 months to 27 months). The results of clinical analysis are as follows: l. Age distribution was from 3 to 15 years old, and the mean was 9.4 years old. 2. Twelve cases showing less than 30degrees initial angulation were treated with long arm cast without reduction, and all of them resulted in excellent result. 3. Patterson s C/R method was useful in cases with angulation between 30degrees ~45degrees. In patients with more than 45degreesangulation, Pesudo method was more effective. 4. 3 cases with 90 angulation were treated by open reduction and internal fixation, and all of them showed unexpected excellent results. 5. Premature epiphyseal closure was present in 2 cases but both of them revealed good functional result.

10.The Effect of Epidermal Growth Factor in Wound Healing.

BACKGROUND: Epidermal growth factor (EGF) , a potent stimulant of epithelialization, has been noted to increase the tickness of the epidermis, increase the epithelial cell proliferartion and keratinization, and accelerate wound contraction. OBJECTIVE: Our purpose was to evaluate the efficacy of topical application of recombinant human epidermal growth factor (rhEGF) in the healing of full-thickness excision and burn wound. METHODS: Full-thickness excision and burn wound were made on the back of the male Wistar rat. Recombinant human epidermal growth factor was applied twice a day and the size of the wound was measured with planimetry every other day for 21 days. The keratinocytes of the circumcised foreskin were cultured in different concentrations of recombinant human epidermal growth factor and proliferation of the keratinocytes was evaluated. RESULTS: 1. Regardless of wound types or base types, wound healing in the experimental groups (rhEGF 10, 30, 50g/g) was generally better than that in the control or vehicle group. 2. The duration of wound healing was decreased as follows in order; in full-thickness excision wound, rhEGF 50 g/g group, rhEGF 30 g/g group, rhEGF 10 g/g group, vehicle group, and control group and in full-thickness burn wound, rhEGF 30 g/g group, rhEGF 50g/g group, rhEGF 10g/g group, vehicle group, and control group. 3. In the biopsy specimen taken from the wound at 9th and 13th day, neodermis, neovascularization, the thickness and maturation of the collagen bundles, and reepithelithelialization were more increased in the experimental groups than in the control or vehicle group. 4. In vitro culture of epidermal cells showed similar proliferation in the concentration of rhEGF higher than 10 ng/ml. CONCLUSION: these findings suggest that topical application of recombinant human epidermal growth factor in the healing of full-thickness excision and urn wound.
