4.Role of Oxidative Stress in the Pathogenesis of Myocardial Stunning in Dogs and Effects of Aminoguanidine

Back ground Superoxide anion produced by oxidative stress combined with nitric oxide(NO)released by cellular NO synthase(NOS)generates peroxynitrite(ONOO-)which was one of reactive oxygen species.Objective To study the role of peroxynitrite in the pathogenesis of myocardial stunning(MS)and the effects of aminoguanidine(AMD).Methods Twenty-four dogs were randomly divided into four groups:(1)Short-time myocardical stunning(MS)group:the animals underwent 15 min occlusion of left anterior descending coronary artery(LAD),followed by 120 min of reperfusion.(2)Long-time MS group:60 min of LAD occlusion,followed by 120 min of reperfusion.(3)AMD group:Dogs were subjected to the same ischemia/reperfusion management as Long-MS group,but AMD was administrated(total 100 mg/kg).(4)Sham-operation group.Left ventricular function was evaluated by echocardiography and blood samples were taken from coronary venous sinus for the examination of NO at different time points.The stunned myocardium was examined immunohistochemically and electronic microscope.Results(1)Systolic thickening of stunned myocardium region and left ventricular ejection fraction were markedly declined during LAD occlusion in the 3 experimental groups,but progressively improved with the time of reperfusion.The improvement was faster in short-MS and AMD groups than in long-MS group.(2)Plasma NO concentrations in coronary venous sinus were greatly increased during reperfusion in short-and long-MS group,while no significant elevation was found in AMD group.(3)A marked increase in the immunoreactivity to nitrotyrosine,a specific "footprint" of peroxynitrite,was shown in the stunned myocardium both in short-and long-MS group,with larger and stronger positive foci in long-MS group.The positive staining was located in myocardial cytoplasm,especially at myofibrils and contractile bands.Treatment with AMD significantly reduced the nitrotyrosine staining in the stunned myocardium.(4)No ultrastructural changes of myocardial cells were noted in stunned myocardium in short-MS group except a slight loss of mitochondrial granules.On the contrary,myocyte edema,myofilament fractures,granule loss and swelling of mitochondrias were significant in long-MS group.AMD significantly ameliorated the ultrastructural abnormalities.Conclusion Myocardial stunning was associated peroxynitrite with production which mainly deteriorate the contractile proteins of myofibrils.AMD significantly inhibit the peroxynitrite formation leading to attenuation of the ultrastructural and functional injuries of stunned myocardium.

OBJECTIVE: The aim of this study was to estimate the feasibility, safety and surgical outcomes of single port access laparoscopic myomectomy (SPA-M). METHODS: We reviewed the medical records of 29 patients with uterine leiomyoma who underwent SPA-M in Gumi CHA hospital between March 2010 and August 2010. We performed SPA-M with conventional rigid straight laparoscopic instruments in all cases. RESULTS: In this study, the mean of leiomyoma weight, operating time, and estimated blood loss were 55.43 gm(+/-54.79, range 5~220 gm), 69.68 min (+/-32.99, range 20~120 min.), 100 mL (+/-104.26 range minimal~300 mL), respectively. Transfusion was done in the one case. CONCLUSION: SPA-M using conventional rigid straight laparoscopic instruments was feasible and could be an alternative to conventional multi-port access laparoscopic myomectomy (MPA-M).

7.The Clinical Significance of Reuse Disposable Instruments for Laparoscopic Surgery.

PURPOSE: Laparoscopic instruments have been remarkably developed through many trials. Various studies and experiments on laparoscopic instruments are underway in other countries. Laparoscopic surgery is also very actively applied in Korea. However, research on the use and safety of the instruments is stagnant. Furthermore, reuse of some disposable laparoscopic instruments is frequently observed, but there are only rare studies on the safety of this. Thus, we tried to provide study cases on the safety of repeated use of disposable laparoscopic instruments. METHODS: To investigate the effectiveness of sterilization and a re-package procedure, we divided the laparoscopic instruments that are commonly used in our institution into 10 types. Among all the available instruments, 32 instruments were selected for the simulation experiment. Each instrument was sterilized using ethylene oxide gas or glutaraldehyde 2%, and then packaged. Then, each was observed grossly and microscopically under aseptic conditions and we looked for any remnant foreign body or contaminant. When remnant foreign body or contaminant was found, they were collected and separately cultured. RESULTS: Residual contaminants were found in 15 instruments (46.9%) out of a total of 32 and microorganisms, including coagulase-negative staphylococcus and gamma-hemolytic streptococcus, were cultured from (9.38%), and each had different types of microorganisms. CONCLUSION: It is remarkable that the bacteria were cultured from recycled laparoscopic instruments after sterilization. The reuse of laparoscopic instruments might be cost-effective, but further studies on its safety are required. Moreover, careful inspection on the method of surgical instrument sterilization in each institution will be necessary.

Metaplastic carcinoma is a rare form of breast carcinoma in which a variety of metaplastic changes occur. These commonly involve squamous or spindle cells, but pure chondroid metaplasia is relatively uncommon. We report a case of metaplastic carcinoma of the breast which mainly involved chondroid metaplasia and in which chondroid calcifications were seen on mammograms.

9.Histopathological Evaluation of Collagen Sheet (Permacol®) after Augmentation Rhinoplasty.

In Asian rhinoplasty, allograft materials are often required due to insufficient amount of autologous sources for dorsal augmentation. Nowadays, Silicon and polytetrafluoroethylene are commonly used allograft materials although these can often lead to postoperative complications such as protrusion, migration or infection of implanted graft. Permacol® is a collagen sheet which is indicated for soft tissue reinforcement or repair of the head and face area in plastic and reconstructive surgery. Its clinical safety and stability is well established with low complication rates. However, its histopathology has rarely investigated, especially in human. Thus, we reviewed histopathology of two graft-removal cases of patients who had undergone rhinoplasty using Permacol®. In conclusion, Permacol® can lead to fibrosis of imbedded tissues with or without absorption in long-term follow-up. Although Permacol® is gradually absorbed as time passes, the implanted graft can maintain its volume by replaced fibrotic connective tissues.

10.Rhino-Orbito-Cerebral Mucormycosis Complicated as Facial Paralysis and Ophthalmoplegia without Rhinologic Manifestation.

Rhinocerebral mucormycosis is a type of opportunistic infections, which can often be lethal in immunocompromised patients. The infection usually occurs in the nose and can spread to adjacent structures. The most typical symptom is facial pain, followed by headache, fever and bloody tinged rhinorrhea. We experienced three cases of rhino-orbito-cerebral mucormycosis, which showed atypical initial presentations. In our cases, patients visited hospital for unilateral ophthlamoplegia and facial palsy without having any nasal symptoms. After evaluating neurologic and ophthalmic problems, they were referred to the Otolaryngology Head and Neck Surgery Department for confirming nasal invasions. Despite using early a parenteral antifungal agent with immediate surgical debridement, patients died due to a residual cerebral lesions and multiple organ failure. In this article, we present the prognosis and management of rhinocerebral mucormycosis patients with atypical symptoms, as well as a review of the published literatures.
