2.Study on cumulative effect of acupuncture on Fenglong (ST 40) to hemodynamics of PCA

Objective To discuss the cumulative effect of acupuncture on left ST40 to hemodynamic of PCA.Methods Sixteen healthy young people were randomly chosen to be acupunctured on left ST40 and parameters of TCD such as Vs,Vd,Vm,PI and RI were managed to be recorded sequentially at the 5 points below to make out an statistical analysis:10 min before acupuncture; the time of inserting the needles; 15 min after retaining the needles; the time when taking the needles out; 10 min after the acupunture.Results The parameters of TCD Vs,Vd,Vm 10min after withdrawing the needle [Were ( 84.56± 19.72) cm/s、( 39.75 ±10.15 ) cm/s、(56.13 ± 14.73 ) cm/s each] all showed a decline compared with the parameters 10min before acupuncture(P<0.05),while other parameters turned out to be almost the same (P>0.05).Conclusion Acupuncture on left ST40 has cumulative effect on Vs,Vd and Vm,which suggest us the needle retention time should be noted.
3.Efficacy observation of acupuncture and tapping therapy in the treatment of type 2 diabetes of yin deficiency pattern combined with stasis in the patients.

OBJECTIVETo explore the clinical efficacy and mechanism of acupuncture and tapping therapy in the treatment of type 2 diabetes of yin deficiency pattern combined with stasis in the patients.
METHODSOne hundred and twelve patients were treated with acupuncture, followed by tapping method. The reinforcing method with lifting, thrusting and rotating needle technique was used at Taixi (KI 3), Shenmen (HT 7), Taibai (SP 3), Taiyuan (LU 9), Sanyinjiao (SP 6) and Zusanli (ST 36); and the even-needling technique was used at Taichong (LR 3), Hegu (LI 4), Xuehai (SP 10) and Fenglong (ST 40). The needles were remained for 30 min. After needle removal, the tapping method with plum-blossom needle was applied to Geshu (BL 17), Yishu (EX-B 3), Ganshu (BL 18), Shenshu (BL 23) and Pishu (BL 20), once every two days. The treatment of 3 months was required. The changes were observed before and after treatment in the relevant indices of fat-islet axis such as fasting plasma glucose (FPG), fasting leptin (FLP), fasting insulin (FINS), insulin sensitivity index (ISI), insulin resistance (Homa IR), insulin secretion index (Homa-beta) and the indices of obesity and blood lipids in the patients.
RESULTSAfter treatment, the total effective rate was 84.8% (95/112), in which the efficacy on the stasis predominated syndrome was better than that of yin deficiency predominated syndrome [92.9% (52/56) vs 76.8% (43/56), P < 0.05]. Before treatment, FPG, FLP, FINS, Homa-IR, total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), body mass, body mass index (BMI), fat percentage (F%) and obesity degree in the patients were higher obviously than those in the normal group (all P < 0.01); high-density lipoprotein cholesterol (HDL-C) and ISI were lower than those in the normal group (both P < 0.01). After treatment, FPG, FLP, FINS, Homa-IR, TC, TG, LDL-C, body mass, BMI, F% and obesity degree were all reduced obviously as compared with those before treatment (all P < 0.01). ISI and HDL-C were increased apparently (both P < 0.01). The efficacy on mild obesity was superior to that on moderate and severe obesity, and the efficacy in the young patients was better than that in the elderly, the efficacy in the males was better than that in the females, but the differences were not significant (all P > 0.05). The efficacy in the cases with 10 years duration of sickness was superior to those with over 10 years duration of sickness (P < 0. 01).
CONCLUSIONAcupuncture and tapping therapy achieves the significant efficacy on type 2 diabetes of yin deficiency pattern combined with stasis and its effect mechanism is related to the positive regulation of glucose, lipid metabolism and fat-islet axis.

4.Efficacy and safety of minimally invasive percutaneous surgery for treatment of thoracolumbar fracture combined with posterior ligament injury

Objective To investigate the efficacy and safety of the minimally invasive percutaneous surgery in the treatment of thoracolumbar fracture combined with posterior ligament injury.Methods The 94 cases of thoracolumbar fracture combined with posterior ligament injury who were admitted into our hospital from Septenber 2012 to January 2016 were selected and equally divided into the observation group and the control group(47 cases in each group) according to different methods.Patients of the observation group were treated with short segment of minimally invasive percutaneous surgery,while patients of the control group were underwent single segment of minimally invasive percutaneous surgery.Recorded the outcomes and prognosis of the two groups.Results There was no significant difference in the operation time and blood loss between the two groups(P > 0.05).The postoperative volume of drainage and the length of stay in the observation group were significantly shorter than those in the control group(P < 0.05).The JOA score of the observation group 1 month after operation was significantly better than that of the control group(P < 0.05).The incidence of complications including pedicle screw breakage,pedicle screw loosening,incision infection,and pulmonary infection in the observation group was 4.3%,which was significantly less than 21.3% in control group (P < 0.05).The Cobb's angle of the obervation group and control group 1 month after operation were (8.55 ± 1.34) ° and (16.30 ± 2.33) ° respectively,which were significantly lower than (32.19 ± 1.98) ° and (32.08 ± 2.08) ° before operation (P < 0.05).And the Cobb's angle 1 month after operation in the observation group were significantly lower than that of the control group(P < 0.05).Conclusion The short segment of minimally invasive percutaneous surgery for the treatment of thoracolumbar fracture combined with posterior ligament injury can reduce the incidence of postoperative complications,promote the patient's rehabilitation,correct the kyphosis and promote the recovery of neural function,which was safer and more efficient.
5.Electrical stimulation of the pudendal nerve for neurogenic bladder dysfunction after spinal cord injury:a literature research on functional reconstruction

BACKGROUND:Stimulation of the pudendal nerve as a target can improve neurogenic bladder dysfunction after spinal cord injury, by adjusting the frequency, intensity, pulse width, as wel as regulating synergistic effect of detrusor and urethral sphincter. OBJECTIVE: To assess the research status of electrical stimulation of the pudendal nerve in the treatment of neurogenic bladder dysfunction after spinal cord injury. METHODS: We searched the folowing databases for articles addressing electrical stimulation of the pudendal nerve for neurogenic bladder dysfunction after spinal cord injury: PubMed, Cochrane Central Register of Controled Trials (CENTRAL), China National Knowledge Infrastructure, Wanfang Database, and VIP Database. The search was updated to July 2014, and the reference lists of the identified studies were manualy screened for additional studies. The study selection and data extraction were independently conducted by two reviewers. RESULTS AND CONCLUSION:Fourteen studies were included in this review. We derived the folowing points by analyzing the included studies: Electrical stimulation of the pudendal nerve has positive effects on rehabilitation of neurogenic bladder dysfunction after spinal cord injury; pudendal nerve electrical stimulation can modulate the coordination of the detrusor and the external urethral sphincter, improve the bladder compliance and restore bladder function by regulating pudendal-to-bladder reflex and spinal reflexes to the bladder. Electrical stimulation of the pudendal nerve may be potentialy implemented as a feasible treatment of neurogenic bladder dysfunction after spinal cord injury. More high quality researches should be conducted to clarify the efficacy and the potential active mechanisms of pudendal nerve electrical stimulation for neurogenic bladder dysfunction after spinal cord injury because the number of clinical reports published in this study area is limited, neurophysiological mechanisms underlying biphasic regulation of frequency on bladder function are not wel known, and which intensity of pudendal nerve electrical stimulation is more effective than others for the treatment of neurogenic bladder dysfunction after spinal cord injury remains unknown.
6.APOE Genotypes Frequency of Chinese Old People in Rural Area

Objective To know the apolipoprotein E(APOE)genotypes frequency of Chinese old people in the rural area. Methods 2000 Chinese aged 65 years or older from four sites in China were enrolled in this study in 2004-2008.Two sites were from the Sichuan province in southwestern China,and another two sites were from the Shandong province in eastern China.The finger blood samples on filter paper were collected from all the investigated people in the end of the interview.The genotype for apolipoprotein E(gene symbol,APOE)was determined by eluting DNA from a dried blood spot,followed by HhaI digestion of amplified products.Results Through statistical analysis,APOE genotypes frequency of Chinese people aged 65 years or older in the rural area,as for ?/?2,?2/?3,?2/?4,?3/?3,?3/?4,?4/?4,they were 1.1%,13.55%,2.05%,68.80%,13.35% and 1.15% respectively.4 carriers and no 4 carriers were 16.55% and 83.45%.Conclusion The distribution of APOE 4 genotype is more widely in Chinese people aged 65 years or older in the rural area.
7.Curative Effect of Underatured Lactalbumin Treatment in Children with Nephrotic Syndrome

Objective To investigate the clinical efficacy of nephrotic syndrome in children adjunctively treated by underatured lactalbumin.Methods Thirty-six cases of nephrotic syndrome were studied for about 1 year.The changes in red blood cells of reduced glutathione(GSH) were analyzed in 18 cases of underatured lactalbumin treatment group and 18 cases of control group.The clinical presentations were observed in both groups.Results GSH levels increased in underatured lactalbumin treatment group when compared with control group.The frequency of relapse and upper respiratory tract infection in underatured lactalbumin treatment group were significantly reduced compared with control group.Conclusion Underatured lactalbumin can provide support for the potential use of an antioxidant therapy in these patients.
8.Efficacy and safety of different antiplatelet drugs on coronary heart disease with hyperhomocysteinemia

Objective:To investigate the efficacy and safety of ticagrelor and clopidogrel in patients with coronary atherosclerotic heart disease (CHD) and hyperhomocysteinemia (Hhcy).Methods:A total of 101 patients with CHD complicated with HHcy who had successfully undergone percutaneous coronary intervention (PCI) were enrolled. They were divided into ticagrelor group ( n=49) and clopidogrel group ( n=52) according to the different antiplatelet drugs used. The clinical data of the two groups were collected, and the incidence of major cardiovascular and cerebrovascular adverse events (MACCE) and bleeding events in one year were compared between the two groups. Results:There was no statistical difference in baseline datas and PCI datas between the clopidogrel and ticagrelor groups ( P>0.05). Compared with clopidogrel group, ticagrelor can reduce the total MACCE (8.16% vs 32.69%, P<0.05) and the incidence of unstable angina pectoris events (0 vs 13.46%, P<0.05) in patients with hyperhomocysteinemia PCI 1 year after operation. The incidences of ischemic stroke, unstable angina pectoris, recurrent myocardial infarction, and cardiogenic death were compared separately between the two groups, and the difference was not statistically significant ( P>0.05). The incidence of major bleeding events, minor bleeding events, and minimal bleeding events were similar between the two groups, and the difference was not statistically significant (4.08% vs 0; 4.08% vs 1.92%; 20.41% vs 9.62%; P>0.05). Conclusions:In patients with CHD and Hhcy, ticagrelor can play a better antithrombotic effect, reduce the incidence of ischemic events, and has good security, which is more worthy of clinical recommendation.

10.Magnetic resonance imaging combined with diffusion tensor tractography can improve prognosis after ische-mic stroke

Objective To explore the pattern of functional reorganization in the cortex after corticospinal tract ( CST) injury and its relationship with the recovery of upper limb motor function. Methods Fifteen patients with complete paralysis on one side after acute cerebral infarction were studied. Within 1 week after the onset, func-tional magnetic resonance imaging ( fMRI ) and diffusion tensor tractography ( DTT) were performed in parallel with timed finger flexion and extension movements in all subjects. The number of nerve fibers in corticospinal tract ( CST) in the affected and healthy sides was measured by using Dtv.Ⅱ. R2 software. One and three months later, fMRI was performed while the affected fingers were flexed and extended passively and any cortical activation was observed. In addition, Fugl-Meyer arm motor function scores were assessed one week, one month and three months after the stroke. Results According to the reconstructed nerve fiber number in CST on the affected side, the patients were classified into three types. Type I:the number of newly-built CST nerve fibers is more than 2/3 of that on the healthy side;type II:the ratio is between 1/3 and 2/3;and type III:the ratio is less than 1/3. For typeⅠpatients, blood oxygenation level-dependent fMRI ( bold-fMRI) showed initial activation of the bilateral sensorimotor cortex ( SMC) and the sup-plementary motor area ( SMA) on the affected side. That was followed by a gradual decrease in the activity in the healthy SMC and an increase in the affected SMC at 1 and 3 months. Among the type II patients bold-fMRI indicated activation of the SMC and SMA on the affected side initially, significant activation of the bilateral SMC and SMA one month later and then stronger activation in the SMC on the healthy side and a weakening of activation in the SMC on the affected side. For typeⅢpatients, initially the SMA and the posterior parietal cortex were found to be slightly ac-tivated. One month later SM1 on the unaffected side was slightly activated, and 3 months later neither the SMC nor the SMA on either side was activated. One week after the onset, the average upper extremity FM scores of the three types of subjects were not significantly different. After one month the three groups′averages were all significantly dif-ferent from one another. But after three months the averages for types I and II were again not significantly different, but significantly better than the average of the type III patients. Conclusion Different CST injuries induce different modes of cortical reorganization. The reorganization is a dynamic process, and different activation patterns are closely correlated with clinical prognosis.
