1.A Case Report of Retroperitoneal Teratoma.

A case of retroperitoneal teratoma was presented in a 40-year-old Korean Male. The radiologic studies were evaluated a space occupying mass containing tooth like a density in the retroperitoneal region. Surgical removal of mass wag performed and histological finding was conformed the benign teratoma.

2.Clinical Analysis of Twelve Cases of Sporotrichosis : Report of two cases and ten cases appeared in literatures of Korea.

Two cases of lymphatic type of Sporotrichosis were presented, 36-year-old and 46-year-old Korean farmers were affected over the right hands and arms. Authors made clinical analysis of ten cases of cutaneous sporotrichosis appeared in Korean literatures with additional two authors cases, and types of the disease reported were shown 9 cases of localized lymphatic cutaneous sporotrichosis and 3 cases of fixed ulcerative cutaneous sporotrichosis. Most highest instances of the diaease according to group of age, occupation, sex and involved region were found to be as follows: 25% in the age of 30 to 39 and under 10, 91.7% in farmer or among their family, 75% in male and 50% over right hands and arms, respectively. These results may indicate that sporotrichosis is one of occupational fungus diseases nf the farme: in Korea and whose age group is active worker or player in the farm.

3.The role of G protein in the activation of phospholipase C from bovine brain.

The objective of the present study was to identify the characteristics of phospholipase C (PLC) isozymes purified from bovine brain and to investigate their interrelationship with G protein. The purified PLC isozymes β, γ and δ were obtained and the characteristics of PLC activity on various concentrations of free Ca²⁺ were observed. The activity of PLC was increased with increasing Ca²⁺ concentration and the activity PLC δ was increased higher in the presence of phosphatidyl choline (PC) than in the absence of PC. For vesicle formation as the structure of cell membrane, cholic acid and deoxycholic acid as detergent on phosphatidylinositol bisphosphate (PIP₂) substrate containing PC were used, and then the activity of PLC isozymes were increased with increasing concentration of cholate, from 0.2% to 1% and were increased slightly in deoxycholate. In the PIP₂ containing phospholipid and glycolipid as brain extract, the activity of PLC isozymes were checked in 0.2-1% cholic acid. The activities of PLC isozymes were continuously increased up to 1% cholic acid. The quantitation of PLC isozymes from several bovine organs by radioimmunoassay was made. Brain was the most sufficient organ in terms of amount of PLC β and δ. A large amount of PLC δ was existed in adrenal gland. The binding capacity of GTPrS and G protein was observed and other observations of the binding effect of GTPrS-G protein and PLC monoclonal Ab-Protein A from tissue homogenate with PLC were made. From the observation the binding capacity was revealed the range of 0.11-1.49%. The effects of each type of G protein on the percent activity of purified PLC isozymes were observed. From the observation, activities of isozymes were increased in Goa & Gmix, and the activities of PLC β and δ were increased in Gβγ and Gia. Activities of PLC β and γ were decreased in Gta but PLC δ increased.

4.A Case of Sparganosis, Formed Giant Hematoma in Scrotum.

Sparganum infestation in human body was reported over 20 cases in Korea. Infestation of sparganosis in spermatic cord was reported a case in Korea. Three cases of sparganosis at Keu Je island in Korea was reported by Dr. Weinstein, Krawczyc and Peers 1954. We experienced a case of the sparganum infestation in scrotum. The patient, 41 year-old male who was suffering with pulmonary tuberculosis moderate advanced, visited 3rd Army Hospital urologic clinic complained with acute swelling of the scrotum in baby head size abruptly without any specific causes. severe tenderness and walking disturbance. The laboratory findings revealed no specific findings except ascaris ova in stool. Operation revealed massive hematoma formation and proliferated connective tissue where noted several infested parasites in length of 120cm, 75cm, 70cm around the tunica vaginalis in scrotum Extracted parasites revealed milkish-whitish color, flat, fragile, and tendon like glistening appearance. There is not any specific sign and symptoms by parasite infestation on the other organs. Patient had an episode of intake of raw snake as a tonic 9 years ago at Keu Je island in Korea.

5.Endocrine Study on Menstrual Irregularities in Wilson's Disease.

We have investigated endocrine functions of 2 patients with Wilson's disease who showed oligomenorrhea or amenorrhea, The serum basal levels of hypothalamic, pituitary, thyroid, adrenal cortical and ovarian hormones were determined. Then stimulation tests were performed with GnRH(50 microgram/m2), TRH(7 microgram/Kg) and insulin(0.l U/Kg) in one intravenous bolus, Levels of LH, FSH, prolactin, GH, TSH, total testosterone, l7 beta-estradiol, free T3, T4, DHEA-S and cortisol were measured by standard radioimmunoassays. The endocrine profiles which represent functions of hypothalamus, pituitary, thyroid and adrenal cortex was all proved to be normal. Serum estradiol and total testosterone levels were 59.0 microgram/ml ( normal ; 60-130 microgram/ml) and 2.9 ng / dl (normal ; 0.l5-1.1 ng/dl) in one case, and 20.5 microgram / ml and 0.69 ng / dl in the other respectiviely. Low estradial and high total testosterone levels seen in these cases suggested ovarian dysfuncton. Interference of ovarian follicular aromatase activity due to passible copper intoxication could explain these findings as the cause of the menstrual irregularity of patients with Wilson's disease.

6.A Case of Patau Syndrome Diagnosed in Early Pregnancy.

Patau syndrome, or Trisomy 13 is one of the most common autosomal aberration associtated with multiple congenital abnormalities. We report a case with trisomy 13 mosacism which was found during an amniocentesis performed due to the age of the mother and abnormal nuchal translucency. The clinical features of fetus included cleft lip and palate, low set ears, polydactily, small ""micro"" penis, and Rocker-bottom feet. After termination of the pregnancy, the fetus was sent for an autopsy. The autopsy report was compatible with the gross findings and pulmonary hypoplasia, microophthalmia, hypoplasia of left ventricle of heart were found.

7.Retrocaval Ureter: A Case Report.

Retrocaval ureter is a rare embryologic venous system anomaly of genitourinary tract and its essentially to the right side. We have reported diagnosed preoperatively in 41 years old male with complaints of intermittent right flank pain, with gross hematuria, urinary frequency, nocturia, tenesmus, and high fever. I.V.P. revealed delayed dye excretion on the right side with pyelocalyectasis and ureterectasis of upper ureter and stenotic portion beneath ureter was non visualization. R.G.P. revealed same as I.V.P. and right ureter was `sickle shape` course and its descent alonged over the right side of 4th and 5th lumbar vertebra column. On post operation 8th days, I.V.P. revealed slightly reduced of previous marked dilated right pelviocalyceal system with upper ureter and preoperative placed ureter was corrected to normal course.

8.Adolescent menstrual disorders: Comparison Between 1988 and 1998.

OBJECTIVE: To compare the age of menarche, changing patterns of the menstrual characteristics in adolescent girls between 1988 and 1998. METHODS: A survey was undertaken for 1,500 highschool girls and compare with those of pervious study for 5,000 middle and highschool girls performed by same author in 1998 by means of questionnaire. RESULTS: During a decade, the mean menarcheal age decreased from 13.5+/-1.0 to 12.7+/-1.0 years(p=0.001). Gynecologic age, Body mass index were associated with age of menarche in both 1988 and 1998. The interval of menstruation was 30.6 3.5dyas in 1998 and shortened compared with 32.1+/-6.3 days in 1988(p=0.001). The duration of menstruation was lengthened in 1998 than in 1988(6.1+/-1.1 days vs 5.8+/-2.2 days, p=0.001). The incidence of irregular cycle(28.2% vs 58.2%, p=0.001), dysmenorrhea(73.9% vs 77.8%, p=0.001), and secondary amenorrhea(16.2% vs 27.2%, p= 0.001) decreased in 1998 compared with those of 1988. The incidence of agonies about menstruation was also decreased in 1998 than in 1988(26.2% vs 69.1%, p=0.001). The most common agony about menstruation in 1988 was irregular cycle(28.4%), but dysmenorrhea( 64.3%) in 1998. CONCLUSIONS: From the above results, the mean age of menarche was advanced since last decade but most of the adolescent girls still have suffered from dysmenorrhea.

9.Clinical Significance of Ultrasonography for Detection of Hip Joint Effusion in Children: Analysis of the 23 Cases of Transient Synovitis of the Hip

Transient synovitis of the hip in children is a non-specific inflammatory and self-limited condition. It is also the most common cause of painful hip in children under ten years of age. Despite of the benign prognosis, there are many difficulties in distinguishing it from other diseases of the hip joint. It remains a common diagnostic problem for clinician because the clinical symptoms, physical findings, and conventional radiography is not pathognomonic of the condition. The authors paid attention to the increase of effusion in the affected hip and studied the value of the hip ultrasonography in 24 cases of transient synovitis from August 1985 to July 1987. The results are summerized as follows : 1. The ratio of male to female was 7 to 1, average age was 7.9 yrs, average hospitalization period was 4.5 days. 2. In simple X-ray studies, no bony change was detectable except for soft tissue signs in 68% of the cases. 3. Capsule-to-bone distance in sagittal ultrasonographic section revealed abnormal increase in 87.5% of the affected hip. 4. Average capsule-to-bone distance of affected hip joint was 7.50 mm, while that of the normal hip was 4.26 mm. 5. As shown in the above studies, ultrasonography can be considered good noninvasive technique in detection and follow-up of hip effusion. So, it is thought to be a valuable method in the diagnosis of transient synovitis of the hip in children.

10.A Case Report of Cerebral Embolism from a Left Atrial Myxoma.

Transient ischemic attacks or embolic strokes caused by the cardiac myxoma are very rare in its freguency. To detect cardiac myxoma, as a source of cerebral embolism originating from the heart, has a great significance because of the occurrence and recurrence of the possible stroke can be prevented by surgical procedure. We present a patient who showed typical clinical symptoms and signs of the left atrial myxoma which was subsequently diagnosed by 2-dimensional real time echocardiography and confirmed by sugery. Following the successful removal of the left atrial myxoma, now, the patient is in stable neurological condition.
