1.Osteoporotic Lumbar Compression Fracture in Patient with Ankylosing Spondylitis Treated with Kyphoplasty.

Ankylosing spondylitis (AS) is an inflammatory disease primarily affecting the spine. Osteoporosis can be a complication of AS and associated with low bone mineral density. As well, spinal fractures in the AS are usually unstable and may cause neurologic deficit at the mainly cervical region with low energy trauma. However, reports of lumbar compression fracture in AS are very rare. Thus, we report a 73-year-old male patient with osteoporotic L3 compression fracture with AS treated with kyphoplasty which has no symptom improvement with conservative treatment. Kyphoplasty is a useful procedure option in the treatment of the lumbar compression fracture in AS.

2.Analysis of Clinical Features of Hip Fracture Patients with or without Prior Osteoporotic Spinal Compression Fractures.

BACKGROUND: To analyze and compare the clinical characteristics including bone mineral density (BMD) in a group who had operation of hip fracture with or without prior osteoporotic spinal compression fractures. METHODS: Two hundred forty patients who had undergone operation of hip fractures were evaluated, 127 patients who had with prior osteoporotic spinal compression fractures were in group I, and 113 patients without prior spinal fractures were in group II. In each group, we measured age, gender, body mass index (BMI, kg/m2), BMD (mg/cm3), type of hip fractures, concomitant diseases, presence of secondary hip fracture and history of percutaneous vertebroplasty. RESULTS: The mean age of group I was 79.4 years (male/female: 28/99) and that of group II was 77.6 years (male/female: 37/76). The mean BMI of group I was 21.3 kg/m2 and that in group II was 22.0 kg/m2. The mean BMD and T-score of group I were 41.1 mg/cm3 and -4.45 and those in group II were 51.0 mg/cm3 and -4.17 (P<0.05). The numbers of patients of neck and intertrochanter fracture of group I were 31 and 96 patients and those in group II were 61 and 52 patients. Sixty in group I and 45 in group II patients had concomitant diseases. Thirteen patients had undergone percutaneous vertebroplasty and 18 patients (7.5%) had second hip fractures. CONCLUSIONS: The hip fracture patients who had with prior osteoporotic spinal compression fractures had lower BMD compared to the hip fracture patients without previous spinal compression fractures.

3.Superficial Fibromatosis Mimicking Glomus Tumor of the Second Toe.

Various types of tumor can occur in the subungual space, including glomus tumors, subungual exostosis, hemangioma, epidermal cysts, and malignant tumors. While fibromatosis can occur at various sites throughout the body, it is very rarely seen in the toe. Here, we are the first to report a case of superficial fibromatosis mimicking a glomus tumor in the subungual space of the second toe. The presentation of this condition shows the possibility of encountering uncommon superficial fibromatosis in the distal phalanx of the toe, and suggests that superficial fibromatosis should be included in the differential diagnosis of a glomus tumor in the toe.

4.The Epidemiologic Study on Prevalence of Hypertension by Classification of JJNC-5 in Adult Korean Male Workers Resident in Pohang.

OBJECTIVES: To establish Korean prevalence of hypertension, we surveyed male workers resident in Pohang by classification of JNC-5(FIFTH JOINT NATIONAL COMMITTEE CLASSIFICATION OF BLOOD PRESSURE) and further analysed association with age, working condition, body weight, fasting blood sugar, total serum cholesterol, tryglyceride, uric acid, drinking habit, smoking habit and amout of physical exercise. METHODS: We studied 13,052 male workers resident in Pohang from January 1993 to Novemver 1993. Each male worker completed a medical and occupational questionaire and blood pressure was taken. All blood sample were taken at venous blood under fasting state at morning. RESULTS: 1) The overall prevalence rate of hypertension by classification of JNC-5 was 16A percent and most of all hypertensives were in stage 1 and 2. 2) The systolic and diastolic blood pressure increased significantly with age. 3) The prevalence of hypertension in shift workers was significantly higher than that of nonshift workers. 4) The prevalence of hypertension in the subjects of overweight, high fasting blood sugar, cholesterol, triglyceride and uric acid level was significantly higher than that of normal criteria group. 5) The prevalence of hypertension in smokers was not significantly higher than that of nonsmokers. 6) The prevalence of hypertension in nonalcoholics was significantly lower that that of heavy alcoholics. 7) The prevalece of hypertension of nonexerciser was significantly lower than that of heavy exerciser. CONCLUSION: The prevalence of hypertension by classitication of JNC-5 at male workers was 16.4percent. Increasing age, shift work, overweight, high fasting blood sugar, high serum total cholesterol and triglycerides ad uric acid level, heavy alcohol drinking significantly increased prevalence of hypertension.

5.A Multi-institutional Study on Histopathological Characteristics of Surgically Treated Renal Tumors: the Importance of Tumor Size.

PURPOSE: The incidence of accidentally detected small renal tumors is increasing throughout the world. In this multi-institutional study performed in Korea, histopathological characteristics of contemporarily surgically removed renal tumors were reviewed with emphasis on tumor size. MATERIALS and METHODS: Between January 1995 and May 2005, 1,702 patients with a mean age of 55 years underwent surgical treatment at 14 training hospitals in Korea for radiologically suspected malignant renal tumors. Clinicopathological factors and patient survival were analyzed. RESULTS: Of the 1,702 tumors, 91.7% were malignant and 8.3% were benign. The percentage of benign tumors was significantly greater among those < or = 4cm (13.2%) than those > 4cm (4.5%) (p < 0.001). Among renal cell carcinoma patients, the percentage of tumors classed as stage > or = T3 was significantly less among tumors < or = 4cm (5.2%) than those > 4cm (26.8%) (p < 0.001). The percentage of tumors classed as Fuhrman's nuclear grades > or = 3 was also significantly less among tumors < or = 4cm (27.3%) than tumors > 4cm (50.9%) (p < 0.001). The 5-year cancer-specific survival rate was 82.7%, and T stage (p < 0.001), N stage (p < 0.001), M stage (p = 0.025), and Fuhrman's nuclear (p < 0.001) grade were the only independent predictors of cancer-specific survival. CONCLUSION: In renal tumors, small tumor size is prognostic for favorable postsurgical histopathologies such as benign tumors, low T stages, and low Fuhrman's nuclear grades. Our observations are expected to facilitate urologists to adopt function-preserving approach in the planning of surgery for small renal tumors with favorable predicted outcomes.
