1.The Early to Emergency Surgery on the Deteriorating Aneurysms.

The decision of proper time for operation on ruptured aneurysm is a very important tactic of treatment, because of the following major complications after subarachnoid hemorrhage. Recently it is widely accepted opinion that the ruptured aneurysm should receive an early operation for lower mortality and morbidity by preventing possible complications after subarachnoid hemorrhage such as rebleeding, symptomatic vasospasm and even hydrocephalus. Since 1985, we applied early to emergency surgery to the clinically deteriorating or non improving patients after aneurysm rupture and delayed surgery to the ameliorating patients who were placed under close observation. The early to emergency surgery on poor risk patient was performed within 48 hours after recent subarachnoid hemorrhage and delayed surgery was done after 2 weeks. Of the selective 95 poor-condition patients admitted to Kyungpook National University Hospital over the past 4 years from 1984, 60 got early surgeries and 35 delayed surgeries. Among the 60 patients in the early group, 22 patients with considerable intracranial hemorrhage were exempted from this comparative study. A few cases of vertebrobasilar aneurysms were not included in this paper for a little value in significance. In this prospective study, we evaluated the results of early and delayed surgeries of poor risk patients through clinical analyses and comparison, and we could gain better results from early surgeries.

2.Bleeding Gastric Ulcer - Clinical Observation of 120 Cases of Bleeding Gastric Ulcer).

Analyses were done on one hudred and twenty caaes of bleeding gastric ulcers diagnosed by emergency endoscopy for the past five years. The reaults are as the following: Much more cases were found in male than female and the most prevalent age group was 6th decade. The body of stomach along the lesser curvature was the most common bleeding site. No age related difference was noted in bleeding sites. Most common type of ulcer was in the round and shallow form. Five of six cases with exposed vessels showed atrophic changes in surrounding mucosa. Among the probable precipitating factors, analgesica, alcohols and certicosteroids were found in such order,

3.Improvement plans and future tasks for the governance of the Korean Medical Association

The purpose of this study is to suggest improvement measures to solve the problems of governance within the Korean Medical Association (KMA), and to outline future tasks to implement these measures. The following measures are proposed to improve the governance of the KMA: a new representative system, the revision of the term of officers, new qualification requirements for representatives and officers, a clear business division for vice presidents, the organization and operation of councils by professional fields, and an organization of a top decision making committee. Future tasks to apply these measures in practice include the establishment of the direction of improvement of the KMA‘s governance, organization and operation of the presidential committee, and active participation and support of the KMA members. In order to improve the KMA‘s governance, the bodies of the KMA must avoid conflicts and criticisms; taking collective decisions about all tasks and policies should be made an essential principle. In conclusion, the bodies of the KMA must cooperate with each other while constantly considering and discussing the purpose of the establishment of the KMA. These measures will ensure the improvement of KMA’s governance.
4.Practical strategies for the improvement of the Korean Medical Association’s governance

This study aims to suggest practical strategies for improving the governance system of the Korean Medical Association (KMA). To this end, we used survey studies, statistical analysis, and literature reviews. The statistical analysis was performed using an analytic hierarchy process and frequency analysis. In our previous study, we proposed some measures to improve the governance system of the KMA. These measures included the organization of top decisionmakers’ meetings, recruitment of non-member officers, new qualification requirements for representatives and officers, commitment of functional roles to six vice-presidents, and organization of professional committees supporting the roles of six vice-presidents. In this study, we proposed practical strategies for the improvement measures and an implementation organization to achieve the suggested practical strategies. For the implementation of the practical strategies, the members of the KMA should understand the issue and the importance of the KMA’s governance. Additionally, the strong support of the members should be required to solve the internal problems of the KMA and to improve its governance system.

10.Glaucomatous Optic Nerve Head Progression and its Risk Factor in Glaucoma:Longitudinal Study of Heidelberg Retina Tomograph.

This study evaluated the risk factor of glaucoma progression despite of adequate intraocular pressure[IOP]and that of glaucomatous optic nerve head progression before visual field progression with minimum treatment regimen. Inclusion criteria included subjects with elevated intraocular pressure and glaucomatous optic nerve head in fundus examination. Patients were main-tained their IOP by medication or filtering surgery. Baseline assessment included routine ocular examination, Heidelberg Retina Tomograph[HRT]and visual field. All patients were reexamined 6 months after the first examina-tion. We review the results of 61 patients[118 eyes]. In multiple logistic regression analysis, the risk factors of glaucoma progression were age[p=0.027] and the intraocular pressure at the first HRT examination[p=0.0216]. Glaucomatous visual field progression correlated significantly with HRT progres-sion[p=0.0216]. If the early glaucoma progression is detected with HRT, progression of glaucoma can be prevented. In the groups with the risk factors, the target pressure should be lower to prevent further loss or blindness with agressive treatment. In groups without risk factor, the quality of patient's life can be improved with minimum treatment. We may be able to prevent the progression of glaucomatous field defect, if we can recognize the early change of their optic nerve head morphology. Their findings may help us to differentiate non-risk group from risk group. We hope that we would improve our patient's life quality by minimizing glaucoma treatment especially in the non-risk group.
