1.Effects of a Marital Relationship Enrichment Program on Communication, Conflict Resolution, and Marital Satisfaction in Multicultural Couples.

PURPOSE: This study was conducted to examine the effects of a marital relationship enrichment program on communication, conflict resolution and marital satisfaction in multicultural couples. METHODS: A quasi-experimental study with a non-equivalent control group pre-post test design was used. Participants were sampled from couples registered at 2 local centers and 2 multicultural family support centers in Seoul and Gyeonggi Province. Sixteen pairs each were randomly assigned to the experimental or control group. The experimental group received the marital relationship enrichment program for 120 minutes/session/week for 6 weeks. Data were analyzed using chi2-test, Fisher's exact test, Mann-Whitney U test, and t-test for subject homogeneity verification, and ANCOVA to examine the hypotheses. RESULTS: Communication and marital satisfaction increased significantly after the program in experimental group compared with control group. Among conflict solution styles, collaborating and compromising increased and avoiding decreased significantly after the program in the experimental group. Competing decreased significantly after the program for wives in the experimental group. Accommodating did not change significantly in either experimental or control groups. CONCLUSION: Results of the study confirm that marital relationship enrichment programs are effective in promoting multicultural couples' communication and marital satisfaction, and causing positive changes in conflict resolution.

2.Outcome of Prenatally Diagnosed Hydronephrosis: One Center Experience.

PURPOSE: The detection of hydronephrosis(HN) with antenatal ultrasonography was first reported in the 1970s. Prenatal HN is diagnosed with an incidence of 1:100 to 1:500 on antenatal screening. Recently, the purpose of antenatal screening has changed from simple detection to selection for specific diagnosis-based management. this study is to evaluate the usefulness of antenatal sonography for HN and to investigate the differential causes of HN and their clinical outcomes. PATIENTS AND METHODS: 11,783 live neonates with prenatal ultrasonographic examination at Ajou University School of Medicine, from Sep. 1994 to Aug. 2001 were analyzed. RESULTS AND CONCLUSION: Hydronephrosis (>10 mm) was detected in 119 (1.0%) cases antenatally and among these, 91 were proved to have HN postnatally. Males were three times more affected than females. Additional imaging studies revealed that ureteropelvic junction obstruction was the most common postnatal diagnosis (47%), followed by multicystic dysplastic kidney, vesicoureteral junction obstruction and vesicoureteral reflux. During 20 months' follow-up (3 to 72 months), 58(48%) renal units showed spontaneous resolution and surgical interventions were necessary in 10 (7.4%) of postnatally confirmed hydronephrotic renal units.

3.A Case of Neuromyelitis Optica Misdiagnosed as Cervicogenic Headache.

Neuromyelitis optica (NMO) is an inflammatory demyelinating disease of the central nervous system associated with longitudinally extensive myelitis and optic neuritis. It is characterized by relapses that lead to blindness and paralysis sequelaes. But, this is rare disease; therefore high clinical suspicion for a correct diagnosis and proper examinations are not easy. However, early diagnosis is essential to prevent sequelae. We report the case of NMO with headache. A 30-year male patient who suffered headache visited our pain clinic because of aggravated pain despite treatment. The cause of the pain was revealed as NMO by more detailed previous history and examination.

5.Impact of Health-related Quality of Life in Firefighters: Focused on Posttraumatic Stress, Health-related Work Limitations, and Family Support.

PURPOSE: The purpose of this descriptive, cross-sectional study was to identify the factors affecting health-related quality of life by examining the relationships among posttraumatic stress, health-related work limitations, and family support in firefighters. METHODS: The data were collected from 390 firefighters working in four fire stations located in the G district using self-report questionnaires. The collected data were analyzed using descriptive statistics, t-test, one-way ANOVA along with the Scheffé test, and multiple regression. RESULTS: As per the results of multiple regression analysis, the factors affecting health-related quality of life in firefighters were posttraumatic stress, health-related work limitations, and family support. This model explained 31% of the variance in health-related quality of life (F=30.03, p<.001). CONCLUSION: Considering the findings of this study, there is a need to reduce posttraumatic stress and health-related work limitations and develop family support toward firefighters.

6.Smoking-attributable mortality among Korean adults in 2019

OBJECTIVES:
Tobacco use ranks among the leading preventable causes of death worldwide. This study was conducted to calculate the mortality rate attributable to smoking in Korea for 2019 and to highlight the importance of tracking and monitoring smoking-related deaths for public health purposes.
METHODS:
Population attributable risk (PAR) was used to estimate the number of deaths related to smoking in 2019. PAR percentages were applied to the estimated mortality figures for various diseases, with PAR determined based on relative risk (RR). Levin’s formula was used to calculate PAR, and RR was adjusted for age and alcohol consumption using Cox proportional hazards regression model to derive disease-specific regression coefficients. The analysis incorporated previously determined smoking rates from 1985, and use rates of novel tobacco products were not considered.
RESULTS:
The findings revealed a total of 67,982 smoking-attributable deaths in Korea in 2019, 56,993 of which occurred in men and 11,049 in women. The PAR of smoking for various causes of death in adult men was highest for lung cancer at 74.9%, followed by pneumonia (29.4%), ischemic heart disease (42.3%), and stroke (30.2%). For women, the PAR for smoking-related death was highest for lung cancer (19.9%), followed by stroke (7.6%), pneumonia (5.7%), and ischemic heart disease (9.1%).
CONCLUSIONS
In countries experiencing rapid fluctuations in smoking rates, including Korea, regular studies on smoking-related mortality is imperative. Furthermore, it is necessary to investigate smoking-related deaths, including the prevalence of novel tobacco product use, to accurately gauge the risks associated with emerging tobacco products.
7.Two Cases of Salmonella typhi Liver Abscess after Transarterial- Chemoembolization.

Hepatocellular carcinoma (HCC) is one of the most common malignancies in Korea. Transarterial chemoembolization (TACE) is an effective modality for treating HCC with a few complications. Liver abscess is most common infectious complication during post-TACE period. However, liver abscess caused by Salmonella typhi has never been reported. We experienced two cases of liver abscess following TACE, caused by S. typhi. Th clinical manifestations of S. typhi liver abscess are similar with other pyogenic liver abscesses. The CT scans showed liver abscesses mixed with necrotic materials after TACE, and the cultures of blood and abscess aspirates yielded S. typhi. The patients did not respond well only with the antibiotic treatment and was controlled after percutaneous drainage. A possible pathogenesis of these liver abscesses is considered as secondary infection in the necrotic liver tissues in biliary carrier of S. typhi.

8.A Case of Heterotopic Pregnancy in a Natural Cycle.

Heterotopic pregnancy is the coexistency of intrauterine and extrauterine pregnancy. The incidence of heterotopic pregnancy is about 1 to 30,000 pregnancy in a natural cycle. However, the frequency of heterotopic pregnancy has steadily increased because of rising incidence of pelvic inflammatory disease, pelvic surgery and the development of ovulation induction and assisted reproduction. The clinical diagnosis of heterotopic pregnancy is difficult due to low incidence. Many of the cases are diagnosed by rupture of ectopic mass that makes serious complication to mother and fetus. We experienced a case of heterotopic pregnancy in a 32-year old woman who presented with acute abdominal pain in a natural cycle and report this case with a brief review of literature.

9.Association of the polymorphisms in CYP1A1 and CYP1A2 genes with preterm premature rupture of membranes.

OBJECTIVE: Preterm delivery (PTD) is the leading cause of perinatal mortality and morbidity. However, its etiology and pathogenesis remain unknown in most cases. Recently, some research put forth the hypothesis that PTD results, at least in part, from a genetic predisposition. This study was undertaken to elucidate whether polymorphisms of cytochrome 450 (CYP) 1A1 and 1A2 are associated with PTD caused by preterm premature rupture of membrane (PPROM) in Korean pregnant women. METHODS: From August 2002 to October 2003, in the affiliated hospitals in the Catholic University of Korea, we have collected the samples from the 264 women who delivered after 37 weeks and from 26 women who delivered following spontaneously ruptured membranes before 37 weeks. RESULTS: There was no significant difference in the genotype frequency as well as in the allelic frequency of CYP1A1*m2 in PPROM group compared with the control group (54% vs. 66%, P=0.224; 0.29 vs. 0.40, P=0.111, respectively). The genotype frequency of CYP1A2*C was significantly higher in PPROM group than in the control group (69% vs. 49%, P=0.047). However, the allelic frequency of CYP1A2*C was not significantly higher in PPROM group than in the control group (0.4 vs. 0.275, P=0.45). CONCLUSION: These results suggest that CYP1A2*C may be, at least in part, associated with PPROM.

10.An Imported Case of Babesiosis.

Babesiosis is a tick-borne, malaria-like illness caused by Babesia species that infect erythrocytes of mammals incidentally. The family Babesiidae is characterized by consisting of non-pigmented intraerythrocytic parasites that reproduce within erythrocytes by asynchronous, asexual budding into two or four daughter cells (tetrad). We experienced a case of human babesiosis presenting fever and chills. The patient was a 49-year old man, who had been in Africa (Ethiopia, Uganda). Three weeks before admission intermittent spiking fever had developed, which had been accompanied by severe chills. The peripheral blood smear (Giemsa-stain) revealed characteristic forms of an intracellular quadruplet parasite compatible with Babesia. The patient was improved significantly by the treatment with quinine and clindamycin for a week.
