1.We Anesthetized a Patient with Unexpected Pheochromocytoma, Diagnosed during Operation: 1 case report.

Pheochromocytoma is a tumor which secretes catecholamine and produces remarkable hemodynamic changes during the perioperative period. It is reported that in cases where a patient with undiagnosed pheochromocytoma is operated on, the mortality rate can reach 25% to 50%. The subject in this study was a 55-year-old female patient who was diagnosed having a retroperitoneal mass which looked like a neurogenic tumor. During the manipulation, serious hypertension and tachycardia were developed. The authors at that time suspected a pheochromocytoma, interrupted the operation, and after insertion of arterial line and sodium nitroprusside infusion, the rest of the operation was restarted. After the extirpation of the tumor, as the patient suffered serious hypotension, the concentration of the inhalation agent was reduced, ephedrine was injected, the proper amount of fluid and blood were administered, and, as a result, the operation was carried out safely, and postoperative course was unremarkable.

2.Clinical Significance of p53 Gene Expression with Immunohistochemical Staining in Ovarian Cancer.

Mutation of p53 gene is one of t.he commonest genetic changes in the development of human cancer including ovary. We intvestigated the diagnostic usefulness of the demonstration of p53 gene immunoreactivity in ovarian cancers, because immunochemical demonstration of p53 immunoreactivity is inexpensive, easily controlled and can be applied in routine pathology laboratories. p53 immunoreactivity was not identified in any patients in whom there was no morphological evidence of neoplasia. ln contrast, in contrast, in 46% of patients of ovarian cancer, p53 immunoreactivity was identified. Overexpression of gene correlated with advanced stage but did not corre1ate with grade, cell type and tumor size.

3.The Effects of Electrical Stimulation Therapy with Microchip for the Treatment of Bladder Irritability Symptoms in Stress Urinary Incontinent Women.

PURPOSE: This study was carried out to evaluate the efficacy of intravaginal pelvic floor electrical stimulation (FES) therapy on bladder irritability symptoms in stress urinary incontinent women. MATERIALS AND METHODS: The evaluation before and after treatment included the Blaivas's female Bladder Questionnaire, the quality of life questionnaires and the overall satisfaction with present voiding state using visual analogue test (VAT). All patient were treated for 20 minutes, twice a week for 6 weeks, using a new intravaginal electrical stimulator with microchip (PIC16C74). RESULTS: After the FES therapy, the overall success rate of the SUI was 50.4.% at 9 months. The bladder irritability symptoms such as frequency, nocturia, urgency, residual urine sensation and lower abdominal discomfort were improved. In particular, the symptoms of frequency, urge incontinence, dysuria were significantly improved after the FES therapy at 9 months. The VAT score of the overall satisfaction with the present voiding state was significantly lower after the FES therapy. 73.7% of patients were satisfied with the FES therapy and complications such as hemorrhage, vaginitis, urinary tract infection and pain were not encountered. CONCLUSION: These results suggest that FES therapy with microchip improves some bladder irritability in SUI women. Therefore, electrical stimulation therapy should be considered as a first line therapy in SUI women with bladder irritability symptoms.

4.A Case of Neurilemmoma of the Penis and the Perineum.

5.A Case of acute Exophthalmos due to Spreading of Chronic Frontal Sinusitis.

It is not common that formation of retrobulbar abscess by inflammatory spreading of chronic frontal sinusitis and even rare that the abscess cause acute exophthalmos and ocular pain. But, if the acute exophthalmos with ocular pain arise from the patient associated with chronic rhinitis or paranasal sinusitis, it should be suspected that peri bulbar abscess was formed by spreading of these disease and otorhinologic examination should be taken. The authors experienced a case of acute exophthalmos with ocular pain in a 27-years old woman and founded that abscess arising from the chronic frontal sinusitis disrupted the superomedial wall of orbit and spread to the retrobulbar area by computed tomography and other radiologic evaluation. We had good result by performing incision and drainage and some otorhinologic therapy.

6.The Role of Calcium Affecting Signal Pathway Related to Nitric Oxide-induced Cytotoxicity in H9c2 Cardiac Myoblast.

Nitric oxide (NO) elevates intracellular calcium. But the actions of calcium in NO-induced cell death are not well understood. This study was carried out to investigate the signal transduction pathways of calcium and NO-induced cytotoxicity in H9c2 cardiac myoblasts by using NO donor compounds such as sodium nitroprusside (SNP) and S-nitroso-N-acetylpenicillamine (SNAP). Pretreatment of intracellular calcium chelating agent (BAPTA/AM) or L-type calcium channel blockers (nicardipine, nifedipine, diltiazem and veraparmil) or T-type calcium channel blocker (flunarizine) blocked SNP-induced cytotoxicity respectively only in a three hours. However, thapsigargin (TG), which inhibits endoplasmic reticulum dependent Ca(2+)-ATPase and thereby increases cytosolic Ca(2+), augmented SNP-induced cytotoxicity. The protective effect of BAPTA/AM was inhibited by treatment of protein synthesis inhibitor, cyclohexamide. In addition, pyrrolidine dithiocarbamate (PDTC), NF-kB inhibitor, attenuates the protective effect of BAPTA/AM against SNP-induced cytotoxicity. It is indicated that the protective effect of BAPTA/AM against NO-induced cytotoxicity might be due to the expression of protein related to activation of NFkB. From these results, it is concluded that SNP-induced cytotoxicity is mediated by calcium in a 3 hours via down regulation of protein expression rleated to activation of NFkB.

7.Protective Effect of Nitric Oxide Against Lipopolysaccharide-induced Cytotoxicity in C6-glial Cell.

Nitric oxide (NO) is mainly involved in brain ischemic damage to elucidate the protective mechanism of NO pretreatment on ischemic-induced cytotoxicity. This study was investigated whether NO pretreatment inhibits the increase of iNOS expression by lipopolysaccharide (LPS) combined phorbol 12-myristate 13-acetate (PMA) via regulating NF-kB activation in C6 glial cells. C6 glial cells with LPS and PMA for 72 hours markedly induced NO, but sodium nitroprusside (SNP) (100 nM) pretreatment before exposure of LPS and PMA significantly supressed NO production, iNOS expression and NF-kB activation by LPS and PMA. In addition, LPS and PMA treatment for 72 hours induced severely cell death and LDH release from cell into media in C6 glial cells. However SNP pretreatment before treatment of LPS and PMA significantly protected LPS and PMA induced cytotoxicity. Treatment with LPS and PMA induced caspase 3 activation follewed by chromosomal condensation, and fragmentation of nuclei in C6 glial cells. SNP pretreatment before exposure to LPS and PMA supressed caspase 3 activation and inhibited chromosomal condensation and fragmentation of nuclei. From these above results, it is suggest that the protective effects of SNP pretreatment against LPS and PMA induced cytotoxicity may be mediated by inhibiting the expression of iNOS via regulating NF-kB activation.

8.The Clinical Investigation of Pediatric Caudal Anesthesia .

Many studies have cited occurrences of severe and long-lasting emotional disturbances of almost every kind after surgical procedures, particularly due to the sedatives or narcotics which were administered to control the post-operative pain. In order to control this post-operative pain, pediatric caudal analgesia was performed in 15 infants and children from the age 2 months to 6 years, who were to undergo lower abdomen, perineum and lower extremity surgery. They were induced with inhalation anesthoeia, using nitrous oxide, oxygen and halothane by mask before and during the block. Following the block, infants and children were turned to supine position and the initial concentration of halothane was reduced. All cases were given 10mg/kg of 1% solution of lidocaine. On examination of the level of the analgesia, 13 out of 15 cases had spread higher level than T10, with effective block for surgical procedures. The identification of the sacral cornu is easy and technical failureis lees in comparison than with the lumbar epidural block for infants and children. Thus we can perform this block successfully for infants and children without any complications or accidents. It is especially helpful in the management of postoperative cases due to the effect in control of the postoperative pain. Thus it gives satisfaction to their parents, nurses and doctors. Despite no follow-up study on postoperative emotional reactions, it was considered that there were minimal occurrences of emotional and behavioral sequelae following the hospital and surgical experiences.

Leiomyoma is a neoplasm of smooth muscle relatively common in the stomach, but very rare in the duodenal location. Recurrent bouts of severe melana remain the most frequent presenting symptom for leiomyoma. A 69-year-old man presented to us with a 4-day history of melena. Endoscopy revealed norinal mucosa covered ovoid elevated lesion with central linear depression and ulcer at the second portion of duodenum. Endoscopic biopsy showed chronic inflammation only. Duodenal mass was excised through a duodenotomy. Pathological diagnosis was leiomycena.
