1.Emergency anesthesia in elderly patients with septic shock: a case report and literature review

Objective:To investigate perioperative management of anesthesia for septic shock.Methods:Anesthesia performed on an elderly patient with septic shock who underwent emergency surgery in Beijing Hospital was analyzed and summarized, with a literature review.Results:Etomidate, Ketamine, cis Atracurium and Remifentanil are preferred for anesthesia induction, and Remifentanil and Sevoflurane are the first choices for anesthesia maintenance.Combined application with Dexmedetomidine may improve patients' prognosis.For septic shock patients with new-onset atrial fibrillation, β-blockers are preferred for perioperative anti-arrhythmia.If necessary, propafenone or amiodarone can be used for cardioversion.Perioperative ultrasound evaluation may be used to guide perioperative fluid therapy and vasoactive drug administration for septic shock in the future.Conclusions:Anesthesiologists should place a high value on and have a good command of the main aspects of perioperative management of anesthesia for septic shock.
2.Quality evaluation of clinical practice guidelines for frail elderly people based on AGREEⅡ and AGREE-REX

Objective:To evaluate and analyze the methodology and quality of recommendations in clinical practice guidelines for frail elderly people.Methods:We systematically searched 4 databases including the PubMed, China National Knowledge Infrastructure (CNKI) , China Biology Medicine Database, Wanfang Data, as well as 5 guideline websites involving Guidelines International Network, Canadian Medical Association Clinical Practice Guideline, Scottish Intercollegiate Guidelines Network, National Institute for Health and Care Excellence, New Zealand Guidelines Group, and supplemented to search Medlive. The Appraisal of Guidelines for Research and EvaluationⅡ (AGREEⅡ) and the Appraisal of Guidelines for Research and Evaluation-Recommendation Excellence (AGREE-REX) were used to evaluate the overall quality of the included guidelines and the quality of the recommendations respectively.Results:A total of 862 documents were retrieved, and 5 guidelines were finally included after screening. Among the overall scoring results of the guideline, there were 4 articles in Grade B and 1 article in Grade C. The average scores of 6 fields in AGREEⅡ were scope and purpose 82.2%, stakeholder involvement 54.5%, rigor of development 38.7%, clarity of presentations 83.3%, applicability 30.8% and independence 21.1%. The average scores of 3 fields in AGREE-REX were clinical applicability 73.3%, values and preferences 20.8%, and implementability 65.0%. The guideline issued by the International Conference on Frailty and Sarcopenia Research in 2019 was of the highest quality.Conclusions:Most of the guidelines come from developed countries in Europe and America, and there are no clinical practice guidelines for frail elderly people in China. The newly released guidelines have improved the quality of the methodology and guideline recommendations, but the four fields of rigor, applicability, editorial independence, values and preferences need to be improved.
3.Effects of distal tibial tuberosity-high tibial osteotomy on ankle angle on coronal plane

BACKGROUND:Distal tibial tuberosity-high tibial osteotomy is a surgical treatment for knee osteoarthritis,but there is still a lack of clinical studies on its effect on ankle joints. OBJECTIVE:To observe the effects of distal tibial tuberosity-high tibial osteotomy on ankle angle on coronal plane of the radiography of the full length of lower limb in weight loading. METHODS:Data of 40 patients(41 knees)with distal tibial tuberosity-high tibial osteotomy from March 2021 to March 2022 were retrospectively analyzed,including 31 females and 9 males,20 left knees and 21 right knees,aged 49-75 years,mean(63.44±6.57)years.The radiographic data of the full length of the lower limb in weight loading were collected before,week 2 and week 48 postoperatively.Hip-knee-ankle angle,talar tilt angle,tilt angle of the ankle,tibiocrural angle,and tibial articular surface angle were measured before and after surgery. RESULTS AND CONCLUSION:(1)Hip-knee-ankle angle improved from(-6.24±3.69)° before operation to(2.59±3.49)° week 2 postoperatively and(2.15±3.49)° week 48 postoperatively.The tilt angle of the ankle changed from(-7.90±3.11)° before operation to(-2.51±2.59)° week 2 postoperatively and(-2.46±2.42)° week 48 postoperatively,with statistically significant difference(P<0.001).(2)There was no significant difference in talar tilt angle,tibiocrural angle,and tibial articular surface angle before and week 2 postoperatively.(3)No significant difference in the angle changes was detected between week 2 and week 48 postoperatively.(4)It is indicated that distal tibial tuberosity-high tibial osteotomy can not only correct genu varus but also improve ankle angle.This result remains stable after 48 weeks of weight-bearing activities.
4. Value of Oxford classification and ISKDC classification in the prognosis of children with Henoch-Schönlein purpura nephritis

Objective:
To analyze the Oxford classification (MESTC) and the International Study of Kidney Disease in Children (ISKDC) classification for evaluating the clinical manifestations, histological lesion and short-term prognosis of children with Henoch-Schönlein purpura nephritis (HSPN).
Methods:
According to the Oxford classification and ISKDC classification, the histological lesions of children with HSPN diagnosed by renal biopsy from Beijing Children's Hospital affiliated to Capital Medical University from January 2018 to December 2018 were re-evaluated. The renal biopsy specimens of the selected subjects were scored according to the Oxford classification and the ISKDC classification. According to whether the first symptom was combined with renal performance, MESTC score and ISKDC classification, children were grouped. The differences in clinicopathological manifestations between the groups were compared. Correlation between MESTC and ISKDC grades was analyzed by nonparametric test rank correlation. Kaplan-Meier survival curve and Log-rank test were used to compare the difference of proteinuria remission rate between the two groups. Univariate and multivariate Cox regression equations were used to analyze the influencing factors of the proteinuria remission rate.
Results:
A total of 78 children with HSPN were enrolled. There were 37 male patients (47.4%) with age of (10.4±2.9) years. When the patients were divided according to MESTC scores and ISKDC classification, the results showed that the proportion of children with nephrotic-range proteinuria in the group of endocapillary hypercellularity (E1,
