1.Clinical Study of Infantile Hypertrophic Pyloric Stenosis.

BACKGROUND: Infantile Hypertrophic Pyloric Stenosis (IHPS) is one of the most common surgical problems of early infancy and one for which an eminently successful surgical treatment has been available since the work of Ramstedt in 1912. A clinical study was begun to access further the accuracy of ultrasonography in identifying hypertrophic pylorus. METHODS: This study is a retrospective clinical analysis of 31 cases of IHPS treated at the Department of Surgery of Pohang St. Mary's Hospital from Jan. 1990 to Dec. 1997. RESULTS: (1) The most prevalent age group was between 3 weeks and 8 weeks in 24 cases (77.4%), and the ratio of males to females was 5.2:1. (2) Among the 31 cases, new born babies were 21 cases (67.7%). (3) In 30 cases (96.8%), the gestational age was between 37 weeks and 42 weeks, and the birth weight was more than 3.5 kg in 21 cases (67.7%). The body weight percentile at admission was lower than the 50 percentile in 31 cases. (4) Among the 31 cases, breast-fed infants were 15 cases (48.4%), milk-fed 13 cases (41.9%), and mixed-fed 3 cases (9.7%). B type blood group was 23 cases (74.2%), and O type was 4 cases (12.9%). (5) In 4 cases (12.9%), an inguinal hernia was noted as an associated anomaly. (6) The onset of symptoms was neonatal (1 week-12 weeks) in all 31 cases, and the duration of the symptoms was between 1 week and 2 weeks in 18 cases (58.1%). (7) Non-bile stained, projectile vomiting was noted in all 31 cases (100.0%), an olive-shaped mass in right upper quadrant of the abdomen was felt in 27 cases (87.1%), and visible peristalsis on the epigastrium was noted in 25 cases (80.6%). (8) On laboratory tests, 17 patients had leukocytosis, and anemia was observed in 2 cases. Hypokalemia was observed in 9 cases (29.0%), hypochloremia in 4 cases (12.9%), and moderate to severe alkalosis (CO2 content > 25 mEq) in 7 cases. (9) Among the 25 cases, for which an the ultrasonographic evaluation was performed, the length of the stenotic canal was from 16 mm to 20 mm in 23 cases (92.0%), and the thickness of the stenotic portion was from 5 mm to 6 mm in 21 cases (84.0%). (10) All 31 cases were surgically treated by using a Fredet-Ramstedt pyloromyotomy, and the mortality was nil. The average hospitalization was 9.4 days. (11) There was 1 case of duodenal perforation and 1 case of intermittent non-projectile vomiting after the operation. CONCLUSIONS: We conclude that early accurate diagnosis, adequate preoperative preparation of the fluid & electrolyte imbalance, immediate surgical correction, and scheduled careful oral feeding are important in treatment of IHPS. Ultrasonographic determination of pyloric muscle length and thickness is the most accurate of the currently available techniques. A Fredet-Ramstedt pyloromyotomy is a safe and successful surgical procedure.

2.Successful desensitization for treatment of an exfoliative dermatitis to allopurinol.

Allopurinol is widely used for chronic tophaceous gout as a uric acid lowering agent. Hypersensitivity to allopurinol occurrs in about 10% of patients, which limits the usage of allopurinol. The successful oral and intravenous desensitization of allopurinol has been reported worldwide since 1976. We recently experienced a 51-year-old male patient with gouty arthritis and hyperuricemia, who had previously experienced skin rash after allopurinol treatment. When allopurinol was retried, erythematous and foliative skin rash developed on entire body. Because allopurinol was essential in controlling hyperuricemia, the oral desensitization of allopurinol was tried. We report successful rapid oral allopurinol desensitization in the patient with chronic tophaceous gout, who exhibited exfoliative dermatitis as allopurinol hypersensitivity.

3.A Case of Bilateral Atrial Myxoma.

As is well known, myxoma of the heart is very rare(incidence 0.03%), constituting nearly 50% of all primary cardiac tumors. It is important because it can be abolished by surgical removal but usually fatal if unrecognized or untreated. Recently a wider use of echocardiogram as the screening test of valve lesions can be expected to increase the number of myxomas found preoperatively. We have experienced a case of bilateral atrial myxoma which simulated mitral valvular disease clinically and had typical three cardinal manifestations such as obstructive, embolic and constitutional symptoms. The patients was 39 years old woman. The left atrial myxoma was diagnosed preoperatively by echocardiography and cardiac angiography and the remainder during open heart surgery under the E.C.C. Both of them were removed successfuly and confirmed as typical myxoma microscopically. This is the first case report of bilateral atrial myxoma in Korea to our knowledge.

4.Role of HGF/c-Met in Serum-Starved ARPE-19 Cells.

PURPOSE: Hepatocyte growth factor (HGF) and its receptor (HGFR/c-Met) regulate motility, mitogenesis, and morphogenesis in a cell type-dependent fashion. We report the role of HGF and c-Met on stress-induced ARPE-19 human retinal pigment epithelial (RPE) cells in this study. METHODS: The cells were cultured either with or without serum. Southern and Western blot analyses were done to determine the expression patterns of HGF/c-Met in serum-starved ARPE-19 cells. The cell proliferation pattern in serum-starved condition was analyzed using MTS assay. Inhibition level of cell proliferation was analyzed using a neutralizing monoclonal antibody against c-Met (2 microgram/ml). RESULTS: Abnormal cell proliferation and scattering of ARPE-19 cells was observed under serum starvation. HGF/c-Met were expressed in serum-starved ARPE-19 cells. ARPE-19 cell proliferation was also enhanced with recombinant HGF treatment. Neutralization against c-Met inhibited the proliferation of serum-deprived ARPE-19 by 64.5% (n=9, S.D. 5.5%). Serum starvation appears to induce epithelial-mesenchymal transition of ARPE-19 cells, resulting in scatter, and the expression of alpha-smooth muscle actin (alpha-SMA), a marker for fibrosis. CONCLUSIONS: In conclusion, c-Met induced under non-physiologic conditions has significant effects on the activation of RPE cells.

5.Review of domestic and international intervention studies to improve oral health in pregnant women

Objective:
This study aimed to review domestic and international literature on oral health interven-tions for pregnant women and provide basic data for the development of oral health intervention programs.
Methods:
We used domestic and international search databases, such as Medline, Embase, Re-search Information Sharing Service, Science On, Korea Citation Index, and DBpia. Among papers published between 2003 and 2023, 22 met the selection criteria by combining “pregnant women,” “oral hygiene,” and “oral health.”
Results:
Since 2007, foreign literature has steadily increased; however, domestic literature has decreased. Overseas research designs were mostly randomized control trials, whereas domestic designs were mostly non-equivalent control group pretest-posttest. The intervention methods in-cluded three preventive programs, five specialized oral care programs, and 14 oral health education programs.
Conclusions
In-depth research should be conducted to increase the understanding and interest of pregnant women. Systematic intervention programs that consider the effectiveness and sus-tainability of intervention methods should be developed, and policy support should be provided to strengthen the importance of oral health education by experts and institutional improvements.
6.Influence of varying cement types and abutment heights on pull-off force of zirconia restorations

Purpose:
The purpose of this study is to evaluate Ti-base abutment’s three different heights and three different cement types on the pull-off force of zirconia-based restorations.
Materials and Methods:
A total of 90 fixture lab analogs were embedded in auto polymerizing resin bloack. 90 Ti-base abutments heights of 3 mm, 5 mm, 7 mm were scanned and zirconia restoration were prepared from scanned files. Zirconia restoration were cemented with three different types of cements (temporary, semi-permanent, permanent) following manufacturer’s instructions. All 90 specimens were placed and tested in a universal testingmachine for pull-out testing. Retention was measured by recording the force at load drop. Statistical analysis was performedusing Kruskal-Wallis test for detecting whether there are any statistical significance along cement types or abutment heights.After that, Mann-Whitney test was used for figuring out differences regarding abutment height and the comparison between 3cements.
Results:
Temp bond showed significantly lower pull-off force compared to Fujicem regardless of any abutment height.However, there were significant differences between Cem-implant and Fujicem in abutment height of 3 mm and 7 mm, but there was no significant difference in 5 mm. Temp bond and Cem-implant had significant differences only in abutment height of 5 mm.
Conclusion
Although Ti-base abutment height did not influenced zirconia restorations’ retentiveness, cement types showed significant differences.

9.The Effect of Photodynamic Therapy with Verteporfin Retreatment on Corneal Neovascularization in Rabbits.

PURPOSE: This study evaluated the effect and safety of photodynamic therapy (PDT) with verteporfin retreatment on patients with corneal neovascularization. METHODS: Corneal neovascularization was induced with a silk suture of the corneal stroma in 24 white rabbits (48 eyes). Four rabbits were examined histologically before performing PDT. Ten rabbits were chosen randomly, one eye from each rabbit was treated with PDT at 50 J/cm2, and the other eye was used as a control. Both eyes of the remaining 10 rabbits were treated with PDT at 150 J/cm2. One week later, one eye was further retreated with PDT at the same intensity. The area of corneal neovascularization was measured and evaluated histologically using light and electron microscopies. RESULTS: The least neovascularized area was observed in the retreated group (P=0.04). The histologic examination revealed fewer new corneal vessels in the retreated group, but the corneal epithelium, stroma, and endothelium showed a normal appearance. Results from electron microscopic examination demonstrated thrombi as well as destruction and nuclear fragmentation of the vascular endothelial cells. However, there were no other changes in the cornea except for vascular endothelial cells, even in the retreated group. CONCLUSIONS: In conclusion, these results suggest that PDT with verteporfin retreatment appears to be safe and significantly more effective in patients with regression of corneal neovascularization.

10.Efficacy of Non-standard Surface Electrodes for Detecting Epileptiform DLwharges in Patients with Temporal Lobe Epilepsies.

This study was aimed for comparison of efficacy in detecting epileptiform discharges, which was originating from mesiobasal temporal lobe, among the four non-standard non-mvasive skm surface electrodes such as ear lobe(Al/A2), zygomatic(Zl/Z2), surface sphenoidal(SS1/SS2) and true anterior temporal electrode(Tl/T2). The study was performed with simultaneous placement of the nonstandard electrodes including standard electrodes(F7/F8). The resul ts were as follows. Among the non-standard electrodes, zygomatic and true anterior temporal electrodes detected more epileptiform discharges compared to the other two electrodes, although there was no statistical significancy. Compared to the standard electrodes, zygomatic and true anterior temporal electrode showed statistically high significant detecting rate (p<0. 01). Also average detection rate of epileptiform discharges of the whole non-standard electrodes which was compared to the rate of the standard electrodes showed statistically significance (p<0.05). In conclusions, non-standard surface electrodes, especially zygomatic and true anterior temporal electrode, are considered as valuable to detect the epileptiform discharges from temporal lobe if they were used at the routine initial EEG examination.
