2.Comparison of Bone Mineral Density and Lipid Profiles in Pre and Postmenopausal Women.

BACKGROUND: Estrogen deficiency accelerates loss of bone mass and changes lipid profile in the postmenopausal women, so that the osteoporosis and astherosclerosis were developed. But it has not enough studies including the premenopausal women. So we have investigated about the differences of body mass index(BMI), lipid profile and bone mineral density (BMD) with pre- and postmenopausal women. METHODS: We have evaluated 201 premenopausal women and 322 postmenopausal women out of total 651 who had visited Health Screening Center in the hospital of Eul-Ji Medical College from November, 1995 to July, 1996. RESULTS: The mean age of total subjects, premenopausal women, postmenopausal women were 51.9, 43.9, 56.8 years, respectively. The mean period after menopause was 8.1 years. Significant difference was seen in BMI, lipid profile and BMD according to age and menopause(P<0.01). BMI was related to lipid profile(P<0.01), but not to BMD(P>0.1). In postmenopausal women BMI, BMD and lipid profile were related to postmenopausal period (P<0.05). In viewing their correlations BMD had strong adverse correlations with factors such as age, menopause, and postmenopausal period. Lipid profile had weak positive correlations with factors such as age, menopause, BMI(P<0.001). CONCLUSIONS: The lipid profile are related to factors such as age, BMI, menopause, and postmenopausal period. The BMD is related to above factors except BMI. Prospective study is needed to evaluate the influence of estrogen on BMD and lipid metabolism. Thus, it helps to the prevention and treatment of the osteoporosis and hyperlipidemia in the postmenopausal women.

3.Expression of the Ki-67 antigen Using Monoclonal Antibody MIB-1 in Children with Glomerulonephritis.

The aim of the present study is to demonstrate the usefulness of intraoperative fine needle aspiration (FNA) of pancreatic lesions in 30 patients. A conclusive diagnosis was done in 27 patients and the diagnoses of three patients were deferred. No complications followed the procedure. Based on histologic findings of the resected specimens in 20 cases and of cell blocks in 10 cases, the final diagnoses were adenocarcinoma in 19 cases, chronic pancreatitis in nine cases and tuberculosis in two cases. The sensitivity, specificity and diagnostic accuracy were 95%, 100% and 96% resepectively and there were no false positives. The smear of aspirate was stained with toluidine blue and examined by light microscope. The presence of there-dimensional clusters of disoriented cells and the increased nuclear/cytoplasmic ratio with large prominent nucleoli were the most helpful criteria for a diagnosis of malignancy in the pancreas. The intraoperative FNA of pancreatic lesions was considered as a simple, safe, and highly specific and sensitive tool in differentiating benign from malignant lesions. The intraoperative FNA can be recommended as the first tool of choice of intraoperative diagnostic procedure in lesions of the pancreas.

4.A Study on Expression of p53 Protein according to Histologic Types, Degree of Malignancy and Differentiation of the Ovarian Surface Epithelial Tumors.

p53 gene alterations in the ovarian cancers are regarded as early events in the whole process of carcinogenesis. This study is intended to compare p53 protein expression rate in the ovarian surface epithelial tumors according to histologic types, degree of malignancy and differentiation. 134 cases of ovarian epithelial tumors including 26 cases of serous cystadenoma, 7 cases of serous borderline malignancy, 15 cases of serous cystadenocarcinoma, 40 cases of mucinous cystadenoma, 21 cases of mucinous borderline malignancy, 20 cases of mucinous cystadenocarcinoma, 4 cases of endometrioid carcinoma, and 1 case of clear cell carcinoma were studied. Immunohistochemistry using monoclonal p53 antibody(DO-7) was applied to the routine formalin-fixed paraffin embedded tissue. The results were as follows; 1. No immunohistochemical positivity of p53 protein was found in all 66 cases of benign serous and mucinous tumors studied. 2. There was no significant difference of p53 protein expression between serous and mucinous malignant tumors. 3. The expression rate of p53 protein exhibited a statistically significant difference between borderline(42%) and malignant(74%) ovarian surface epithelial tumors (p<0.05). 4. The expression rate of p53 protein of poorly differentiated carcinomas(100%) was higher than those of moderately (88%) and well differentiated carcinomas(60%), but there was no statistical significance. In summary, p53 protein expression might be a good indicator of malignant transformation of the ovarian surface epithelial tumors.

5.Two Cases of Invasive Aspergillosis of Sino-nasal Origin .

Cerebral aspergillosis is a rare condition that affects primarily the immunocompromised host. Most of cerebral aspergillosis is developed by hematogenous dissemination from extracranial foci, but aspergillosis of sino-nasal origin rarely affects the CNS. In case 1, wel symptom of tumor recurrence was unilateral numbness of the chin. A 65-year-old male was admitted because of paresthesia around the left chin and left lower lip. Neurologic examination revealed hypesthesia on the left side of chin, lower lip and buccal mucous mem-brane. Bone scan (Tc-99m MDP) showed focal hot uptakes on the left mandible and left first rib. Brain CT with bone window setting showed a focal osteolytic lesion in the bone marrow of the left mandibular canal without destruction of bone cortex. Both coronal T1 weighted image and axial T2 weighted image showed focal low signal intensities on the left ramus. The pathophysiologic mechanism could be understood by identification of the pathologic focus.

7.Usefulness of Family Counseling.

BACKGROUND: Family counseling is an essential part of family practice in which patients are cared in psychosocial dimension as will as in physical dimension and the family is dealt with as a whole unit. However, it is little applied in practice. The department of family medicine has made efforts to activate family counseling. This study was done to estimate the patients response on the effect of family counseling that we have performed, and to investigate what clinical problems counseling was held and in what situations the counseling was perceived to be useful by the patients. We hope this study will be useful to establish some useful data and ideas for the development of family counseling in family practice in Korea. METHODS: The subjects of this study are 53 households who received family counseling at the department of family medicine from Oct. 1, 1994 to May 31, 1995. 64 households received counseling during this peroid, but 11 persons who could not be reached by the phone were excluded. Patients demographic charateristics, patients chief complaints, physicians assessrnent on the complaints and major prolems presented in the counseling sessions were identified from the medical records. Patients own estimation of the effect of counseling and the reasons for termination of counseling were inquired through the telephone interview. Finally the association between the usefulness of counseling and some factors was statistically analyzed. RESULTS: Subjects were predominently women(84.9%), especially middle aged. The most common chief complaint was headache complained by 10 subjects (18.9%), and the second and the third were abdominal pain complained by 8 (15.1%) and chest discomfort complained by 7(13.2 %), respectively. The list of physicians assessment included depression(34.0%), physical symptom per se without any assessment(20.8%), family problem(15.1%), somatization(13.2%), and anxiety(9.45). More than half of the subjects(60.4%) had only one session, 26.4% two sessions and 7.5% more than 4 sessions. The most common problem presented in the counseling was marital conflict(28.3%), the second was trouble with in-laws(15.1%), and the third was chronic illness in the family(9.4%). 32.1% replied that the counseling was very helpful, 28.3% somewhat helpful, 17% little helpful, 20.8% never helpful, and 1.9% not sure. The session was experienced to be more useful in the group of duration of symptom less than 6 months, than in the group of longer duration(p<0.05), and in the group with motivation for counseling than in the group without motivation(p<0.05). CONCLUSIONS: About 60% of subjects replied the counseling was useful to them. Symptom duration and patients motivation to participate in counseling showed significant association with the effect of family counseling.

9.A Statistical Review of Benign Tumors of the Skin for the Past 10 Years.

We have reviewed the 503 cases of benign skin tumors which were confirmed by histopathological study from January 1977 to July 1987 in the Dermatologic Department of Kyungpook National University Hospital. The result were summarized as follows . l. Among the total of 26,690 dermatologic out patients, 503 cases(1.9%) were diagnosed as having benign skin tumors. The number of benign skin tumors has shown a noticeable increase in the last several years. 2. The tumors were found mainly in ages ranging from the 2nd to 5th decade, most commonly in the 3rd decade(124 cases, 24.7%). 3. Aceording to Lever's elasaification, the tumors were divided into the following 7 groupa . tumors and cyets of the epidermis(35.2%), melanocytic tumors(16.5%), tumors of vaecular tissues(13.3%), tumora of epidermal appendages(11.1%), tumors of fibrous tissue(8.9%), tumors of neura.l tiasue(7.6%) and tumors of fatty, muscular and osseous tiseue(7.4%) in decreasing orders. 4. The 14 common tumors in order of frequency were as follows epidermal cyst (17.l% ),:melanocytic nevus(16.5%), neurofibroma(7.6%), congenital hemangioma(6.4%), nevus sebaceous(6.0%), lipoma(5.2%),seborrheic keratosis(4,8%), linear epidermal nevus(4.6%), steatocystoma mutiplex(2.8%), dermatofibroma(2.6%), hypertrophic scar and keloid.(2.6%), granuloma pyogenicum(2.4% ), keratoacanthoma(2.0%) and syringoma (2,0%). 5. The predilection sites of seborrheic keratosis, keratoacanthoma, syringoma and melanocytic nevus were the face, linear epidermal nevus, dermatofibroma and granuloma pyogenicum were located on the extremities. 6. The duration of tumors from the presumed time of onset to the first visit was within 1 year for epidermal cyst, keratoacanthoma and granuloma pyogenicum, over 10 years for nevus sebaceous, congenital emangioma and neurofibroma. 7. Diagnostic accuracy was higher in melanocytic nevus, congential hemangioma, nevus sebaceous and neurofibroma and lower in steatocystoma multiplex, dermatofibroma, keratoacanthoma, lipoma and epidermal cyst.

10.Primitive Neuroectodermal Tumor Associated with Mature Cystic Teratoma of the Ovary: A case report.

Malignant transformation of benign cystic teratomas of the ovary is rare, with an incidence rate of 1-3%. The most common malignancy associated with benign cystic teratoma of the ovary is squamous cell carcinoma, which represents 90% of malignant transformations, followed by adenocarcinoma and carcinoid. Primary malignant neuroectodermal tumor in a mature cystic teratoma is extremely rare and microscopically can be subclassified as differentiated, primitive, and anaplatic. We experienced a case of primitive neuroectodermal tumor (PNET) associated with mature cystic teratoma of the ovary in a 11-year-old girl. Grossly, the ovary was a unilocular cyst which contained sebum material and disclosed several large, yellow to gray-tan polypoid excresences attached to the inner cystic wall. Except a large, fish-flesh, soft mass, the other polypoid masses were covered with sebum and contained small pieces of cartilage and tooth, and microscopically represented as mature teratoma. The large, fish-flesh, soft mass disclosed sheets and lobules of closely packed small cells with thin fibrovascular septa, which frequently replacing and encroaching the mature cystic teratoma. The small tumor cells were uniform and made up of round to oval nuclei with coarse chromatin and prominent nucleoli, very similar to those of PNET of the central nervous system.
