3.Serum Insulin-like Growth Factors and their Binding Proteins in the Women With Polycystic Ovary.

OBJECTIVE: The involvement of IGF system in hyperandrogenism and abnormal follicular development is controversial. This study is to assess whether IGF system contribute to it in the women with polycystic ovary(PCO). METHODS: Baseline serum levels of luteinizing hormone (LH), follicle stimulating hormone (FSH), estradiol (E2), testosterone (T), androstenedione (ADD), prolactin, thyroid stimulating hormone (TSH), free insulin-like growth factor(IGF)-I, free IGF-II, insulin-like growth factor binding protein(IGFBP)-1, and IGFBP-3 were measured in twelve healthy regularly cycling volunteers and forty-two women with PCO then, the changes of baseline serum levels were evaluated after laparoscopic ovarian electrocauterization in nine PCO patients. In addition, the expression pattern of IGF-I and IGF-II was examined in the ovary of control and PCO group. RESULTS: Baseline levels of LH, ADD, free IGF-II, and IGFBP-3 were significantly higher in PCO group. However, there were no significant differences in the levels of free IGF-I and IGFBP-1, although free IGF-I showed decreasing tendency in PCO group. And there was a significant positive correlation between the LH and free IGF-II level in the PCO(P=0.011, r2=0.3899), but not in the control. After ovarian electrocauterization, LH, T, and ADD levels decreased, and free IGF-I and IGFBP-3 level increased. While free IGF-II and IGFBP-1 level showed no significant changes. In the ovary, expression of both IGFs showed similar pattern in normal and PCO ovaries. CONCLUSIONS: The elevated IGFBP-3 level may alter the bioavailability of IGF(s) in the PCO. The change in IGF-I level and resumption of ovulation after electrocauterization, suggest a possible role of IGF system in the impairment of follicular development in the PCO.

4.Effect of Calcium Intake and Health Status on Blood Lipids in Middle-aged Women.

The purpose of this study was to confirm that calcium intake and healthy status play a role in reducing the risk of cardiovascular disease, to assure that calcium intake and regular exercise are important in reducing serum lipid levels in middle-aged women. Daily nutrient intakes were analyzed by convenient method. Anthropometric measurement such as body weight and height and blood pressure were measured. The subjects divided into two groups : women in group I under 50 years of age(< or = 49 yr group) and those in group II 50 years up(> or = 50 yr group). Average ages of group I and II were 43.87 and 53.46 years. The nutrients intake of subjects were higher than Korean recommended dietary allowances(KRDA) except calorie, iron, calcium, vitamin B1. They showed significant difference each other in vitamin B2(p<0.05), calcium(p<0.01) intake. Mean daily calcium intake was 540.88mg in < or = 49 yr group and 519.50mg in > or = 50 yr group. The levels of serum cholesterol, triglyceride, blood pressure, Atherogenic Index(AI), LDL/HDL and Cardiac Index(CI) were increased with age. The triglyceride and total cholesterol concentrations and LDL/HDL in > or = 50 yr group were significantly higher than those of < or = 49 yr group. There was a highly significantly positive correlation between age and triglyceride, total cholesterol and Atherogenic Index(AI). Triglyceride, total cholesterol, LDL-cholesterol concentration and Atherogenic Index(AI) were positively correlated with body weight and WHR(waist-hip ratio). Also ther was a significantly negative correlation between exercise and Cardiac Index(CI). Also a significantly negative correlation was found between Ca intake and LDL-cholesterol. Exercise and Cardiac Index(CI) also had a significantly negative relation. The results suggest that increased habitual physical activity and calcium intake should be recommended as a way of decreasing blood lipids and blood pressure in middle-aged women.

Delayed rupture of the extensor pollicis longus tendon mostly occur after plate fixation of distal radius fracture. However, delayed rupture of the extensor pollcis longus tendon following intramedullary nailing of radial shaft fracture is very rare. We report one case of delayed rupture of the extensor pollicis longus tendon following intramedullary nailing, presenting abnormal signs from adhesion of the extensor pollicis longus with extensor indicis at the nail insertion site.

7.Effectiveness of Medication Videos as Educational Instruments in the Education of Fundamental Nursing Skills.

The effectiveness of dedication video tapes in teaching fundamental nursing skills was investigated using experimental research methodology. Data was collected from 153 students attending fundamental nursing practice classes of two junior colleges and two universities. The subjects were randomly assigned to two groups, one was an experimental group of 74 subjects and the other was a control group of 79 subjects. The in dependent variable was learning type and the dependent variables were the degree of knowledge achievement and nursing skills achievement regarding medication. The research hypotheses of this study were as follows : 1) There will be a significant difference between the experimental group and the control group in the degree of knowledge achievement about medication. 2) There will be a significant difference between the experimental group and the control group in the nursing skills achievement about medication. The data was analyzed with the SPSSWIN program and the t-test, number of subjects and percentiles were then examined. The findings of this study are as follows : 1) There was a significant difference between the experimental group and the control group in the knowledge achievement about medication using P<.05. 2) There was no significant difference between the experimental group and the control group in the nursing skill achievement about medication using P<.05. 3) This study suggests that medication video tapes in the education of fundamental nursing skills are effective for knowledge achievement. As a result, medication video tapes can be effectively utilized for the education of nursing skills regarding medication.

Non-invasive positive pressure ventilation (NIPPV) was mainly applied to acute respiratory failure in chronic obstructive pulmonary disease (COPD) and to intrinsic lung disease such as neuromuscular disorders. It has been reported that this maneuver reduces morbidity, mortality and hospital stay in patients in the intensive care unit. We observed a 32-year-old female renal transplantation patient who developed pulmonary edema during the perioperative state. The mainteance of an endotracheal tube in intubated patients who were on immunosuppresive therapy might increase nosocomial pneumonia significantly. In this case, the mortality increase was expected, therefore we considered that early extubation and mask continuous positive airway pressure (mask CPAP) could help. By using mask CPAP, the patient showed a good response and the pulmonary edema was improved. We report a case about the benefits of NIPPV in the treatment of pulmonary edema in an immunosuppressed patient of which the result was very satisfactory.

10.A Study on Iron Status and Anemia of Rural and Urban Middle School Students in Ulsan.

PURPOSE: This study was aimed at assessing the difference of the prevalence of iron deficiency and iron deficiency anemia among rural and urban middle school students in relation to dietary habit. METHODS: With a questionnaire, blood samples were obtained from 439 apparently healthy rural and urban middle school students residing in Ulsan. Anemia was defined as hemoglobin level of 12.6 g/dL or less for boys and 11.9 g/dL or less for girls. Iron deficiency was defined as serun ferritin level less than 12 micrograms/L or/and transferrin saturation less than 14%. Iron deficiency anemia was defined as iron deficiency plus low hemoglobin. RESULTS: 1) In boys, the prevalence rate of anemia was 17.2%. Among these anemias, 5.4% were found to be iron deficiency anemia. In girls, the prevalence of anemia increased with age. The prevalence of iron deficiency anemia was 6.9%. 2) In girls, the prevalence rate of anemia in rural area was higher than that of anemia in urban area (12.6% in rural, 6.1% in urban, P<0.01). 3) The prevalence of anemia and iron deficiency in the students with menstruation was 10.6% and 33.1%, which was higher than the prevalence of 2.5% and 7.5% in those who did not have the menarche (P<0.001 and P<0.001, respectively). 4) Dietary intake of rural and urban middle school students was estimated lower in energy, iron than the recommeded dietary allowance (RDA). In girls, dietary intake of rural middle school students was estimated lower in iron, niacin, and vitamin C than that of urban middle school students. 5) Nutritional factors such as energy, carbohydrate, protein, and phosphorus showed positive correlation with RBC, hemoglobin (P<0.05). CONCLUSION: It is recommended to enforce the nutritional education to take enough iron in middle school students to reduce the high prevalence rate of anemia among pubertal students.
