1.Osteochondroma in the Rib: A Case Report Reviewing Magnetic Resonance Imaging Findings

Osteochondroma is a benign bone tumor that is most commonly found in the extremities and is prevalent in adolescents, with a peak incidence before the age of 20 years. Herein, we present a rare case of a 33-year-old man with an asymptomatic tumor on the right anterior rib that was pathologically confirmed to be a rib osteochondroma after surgical excision. We describe the computed tomography and magnetic resonance imaging (MRI) procedures and highlight the importance of MRI as a critical diagnostic tool for osteochondroma.

With the exception of choriocarcinoma, the central nervous system (CNS) is rarely associated with metastases from gynecological malignancy. Cerebral involvement in ovarian carcinoma is generally regarded as occurring in 1% of cases. Overall incidence of cerebellar metestases is 27% of CNS metastases. However higher rates have been reported following initial platinum-based chemotherapy. We experienced a cerebellar metastatic cancer patient treated with craniectomy and radiation therapy. So we report the case with a brief review of literature.

3.A Case of Multiple Primary Malignancy.

Multiple primary cancer means that more than two cancers are independently developed in one individual. The frequency of multiple primary neoplasm is increasing as a result of the lengthening average lifetime, progress in cancer screening technique, and improved cancer therapy. Gynecologic malignancies are especially prone to associated with primary neoplasms elsewhere. The frequency of multiple primary cancers was significantly higher in patients with uterine corpus cancer than in those suffering from cervical cancer. This case has different cell types of cancer in uterine cervix, thyroid and colon simultaneously. So, we report the case with a brief review of literatures.

4.The Effect of Venoarterial Extracorporeal Lung Assist ( ECLA ) Using a Small - sized Membrane Oxygenator in Hypoxic Dogs.

After the heart-lung machine was used for the open heart surgery, it was hypothesized that the extracorporeal circulation might be applicable to the management of acute respiratory failure. The development of silicone membrane minimized the possible physical or chemieal damages to blood perfused into the oxygenator. Extracorporeal lung assist(ECLA) using a membrane oxygenator has been recognized as a therapeutic modality for acute respiratory failure, To simplify and reduce the voulme of the ECLA circuit, a small oxygenator, surface area 0.3 m2, was developed by Kurare Co., Japan. It was composed of non-microporous hollow fibers. We performed venoarterial(VA) ECLA to evaluate the gas transfer of the Kurare oxygenator. The priming volume of the ECLA circuit was about 150 ml. Venous blood was drained via the right external jugular vein. The maximum bypass flow rate was about l060 ml/min. Oxygenated blood was perfused into aortic arch via the right carotid artery. The increase of arterial oxygen tension was about 58 mmHg during VA ECLA. It was confirmed that Kurare oxygenator was adequate for the oxygenation support on hypoxic dogs. We tried this ECLA circuit on a postoperative cardiac patient on May, 20, 1991. After 90 hours VA ECLA, she recovered without any complicatioris.

Leiomyomas of the vagina are rare benign solid tumors. A leiomyoma arises frommuscle in the round ligament and appears as a firm movable tumor deep in the substanceof the labium majus, but minority of these tumors occur in vaginal wall. A case of a 39year old woman with leiomyoma of the vaginal orifice was reported with a brief review ofliterature.

6.Study for the Outcome of Mid-Trimester, Emergency Cervical Cerclage in Patients with Painless Dilatation of Cervix.

A retrospective study was conducted to evaluate the clinical outcomes of midtrimester emergency cervical cerclage in patients with painless cervical dilatation with/without prolapsed amniotic sac, including cases of no previous history of cervical incompetence. 10 patients including 1 twin pregnancy were teviewed for study, All had received emergency cervical cerclage with Macdonald operation or Shirodka operation at 18 to 31 weeksgestation. The procedures were carried out without serious complication except for one patient who had received re-operation 3 days after Macdonald operation because of re-protrusion of amniotic sae. The mean procedure to delivery internal was 8.9+/-5.3 (range 1 to 17) weeks. The mean gestational age at delivery was 32.4+/-5.6 (range 22 to 39+6) weeks for the entire group, 36.1+/-2.6 (range 32+4 to 39+6) weeks for the 6 who achived viability, and 27.6+/-5.0 (range 22 to 29+4) weeks for the 3 who died during the neonatal period. 1 died during gestation and was delivered at 33+6 weeksgestation. The mean birthweight was 2181.6+/-971.6 (range 498 to 3500)g for the entire group, and 2712.9+/- 571.5 (range 1860 to 3500)g for the 7 infants who lived until after neonatal period. The total survival rate for 11 babies was 64%. This study demonstrates that midtrimester emergency cervical cerclage for the patients who have dilated cervix with/without prolapsed amniotic sac is valuable method to try in the cases predicted they would lost the baby with conservative treatment only.

Of the various types of ectopic pregnancies that have been reported, the retroperitoneal implantation probably represents the rarest. A search of the literature reveals only three other cases that appear to be true retroperitoneal pregnancies. We have experienced a case of the true retroperitoneal ectopic pregnancy, which was seen to be adherent to the anteromedial aspect of the left kidney. We report the case with a brief review of literatures.

8.Bioassay-Guided Isolation and Identification of Compounds from Arecae Pericarpium with Anti-inflammatory, Anti-oxidative, and Melanogenesis Inhibition Activities.

This study describes the anti-inflammatory, anti-oxidant, and melanogenesis inhibition activities of methanol extract and various organic solvent fractions of Arecae Pericarpium. We examined the inhibition of lipopolysaccharide (LPS)-induced nitric oxide (NO) production in RAW 264.7 cells, 1,1-diphenyl-2-picrylhydrazine (DPPH) scavenging activity, mushroom tyrosinase inhibition activity and melanin contents. The study showed that, among all tested fractions, methylene chloride fraction showed the strongest inhibition of LPS-induced NO production in RAW 264.7 cells (IC₅₀ value 8.89 µg/mL) and DPPH radical scavenging activity (EC₅₀ value 21.39 µg/mL). Methylene chloride and ethyl acetate fractions similarly inhibited mushroom tyrosinase activity. Methanol extract exhibited strongest reduction of melanin content in B16F10 melanoma cells. Based on the bioactivity assay results, methylene chloride and ethyl acetate fractions were further separated. Eight phenolic compounds were isolated, which are dimeric syringol (1), catechol (2), 4-hydroxybenzaldehyde (3), vanillin (4), 4-hydroxyacetophenone (5), apocynin (6), protocatechuic acid (7) and 4-hydroxybenzoic acid (8). Among the isolated compounds tested, catechol showed the strongest inhibition of LPS-induced NO production in RAW 264.7 cells. Catechol also showed the concentration-dependent NF-κB inhibition activity. Arecae Pericarpium might have potentials to be developed as anti-inflammatory agent or dermatological product for skin-whitening agent.

9.Nutritional status of Korean elderly with dementia in a long-term care facility in Hongseong

BACKGROUND/OBJECTIVES: This study investigated nutritional status of the elderly with dementia in a care facility with the aim of improving the meal quality of the facility. SUBJECTS/METHODS: Data were collected from 30 dementia patients aged more than 65 years in a long-term care facility in Hongseong. The data were obtained from questionnaires and medical records. The food intake data was obtained using food photographs and the nutrient intakes were calculated using the CAN-Pro 5.0. The data were compared with the dietary reference intakes for Koreans (KDRIs). The nutrient density, diet quality such as nutrient adequacy ratio (NAR), mean adequacy ratio (MAR), and index of nutritional quality (INQ), as well as dietary diversity score (DDS) were evaluated. The data were analyzed using SPSS statistical programs. RESULTS: The average daily energy intakes for men and women were much lower than the estimated energy requirements of the KDRIs. The average intakes of energy and most nutrients in the general diet group were significantly higher than those of the other two groups. Significant differences in diet quality and diet diversity were observed according to the meal type groups. The NARs of some minerals (calcium, iron, and zinc) and vitamins (vitamin B6 and folic acid) were less than 0.5 in all study groups. The NARs of protein, iron and MAR of the general diet group were significantly higher than those of the liquid diet group. The DDS scores of meats, fruits and diary food group were very low in all meal type groups, meaning that the diet qualities of the study subjects were not appropriate in all meal type groups. CONCLUSIONS: The food intakes of the study groups showed some limitations by a direct comparison with KDRIs because of the very low physical activities of the study subjects. The diet quality and diet diversity indices suggest the need for improvements in the nutritional quality in all types of diet. Overall, new intervention strategies targeting facility residents with dementia in Korea are needed as soon as possible.
