1.Sirenomelia: An autopsy case report.

Sirenomelia is a severe form of caudal regression syndrome that results in a fusion of the lower extremities, which is not compatible with life. A various spectrum of anomalies affecting primarily the musculoskeletal, genitourinary and gastrointestinal systems also can occur. This rare malformation has a reported incidence rate of approximately 1 in 60,000 births, with a range of 1 to 1 percent of all malformed infants. We experienced a sirenomelic case with combined anomalies of genitourinary, cardiovascular and gastrointestinal systems. Maternal obstetric history revealed oligohydramnios and intrauterine fetal growth retardation, and the baby was spontaneously delivered at 37 weeks of gestational age, but died I hour after birth.

3.Future Role For Motivational Interviewing In The Treatment Of Insomnia - An Opinion

Objective: This paper aims to explore the possibility of using motivational interviewing (MI) as a therapy for insomnia patients. Methods: We reviewed the current practice guidelines for insomnia, and noted the issues pertaining to types of treatment, notably CBT-I. We also reviewed some studies which seem to suggest that MI treats insomnia. Results: MI is a proven technique for other
psychiatric illnesses. There are currently not many studies done evaluating the efficacy of MI on insomnia. Of those published studies, they are either underpowered to draw any firm conclusions, or it is limited to a particular age group. Conclusion: There seems to be promise in the area of MI on insomnia. Given the paucity of data in this area, more research with bigger group of study participants are needed to fully conclude the effectiveness of this treatment.

4.A Case of Bilateral Atrial Myxoma.

As is well known, myxoma of the heart is very rare(incidence 0.03%), constituting nearly 50% of all primary cardiac tumors. It is important because it can be abolished by surgical removal but usually fatal if unrecognized or untreated. Recently a wider use of echocardiogram as the screening test of valve lesions can be expected to increase the number of myxomas found preoperatively. We have experienced a case of bilateral atrial myxoma which simulated mitral valvular disease clinically and had typical three cardinal manifestations such as obstructive, embolic and constitutional symptoms. The patients was 39 years old woman. The left atrial myxoma was diagnosed preoperatively by echocardiography and cardiac angiography and the remainder during open heart surgery under the E.C.C. Both of them were removed successfuly and confirmed as typical myxoma microscopically. This is the first case report of bilateral atrial myxoma in Korea to our knowledge.

5.Malignant Hyperthermia Syndrome.

MHS is a myopathic disorder transmitted by a dominant autosomal gene. The seriousness of the problem is underscored by the fact that the mortality rate, until recently, has ranked the highest among the complications of modern anesthesia. During halothane anesthesia we experienced a case of MHS with high temperature and rigidity after succinylcholine, observed change of serum electrolytes, and noted CPK and myoglobin in the urine. The patient was managed with surface cooling, bicarbonate, diurectics, mechanical ventilator and corticosteroids. The patient survived without any sequelae and was discharged after 10 days.

6.Clinical Observation on Antireflux Procedure.

Clinical observation was made on 37 cases (41 ureter) who had beet performed antireflux procedure at Department of Urology from October 1, 1968 to July 31. 1973. Clinical observation was included age and sex distribution, indication for operation. operation site, method of operation, additional procedure, duration of indwelling ureteral splint catheter and complication. The results were as follows; 1. Antireflux procedure was performed for ureteral stricture, bladder tumor. kidney transplantation. megaloureter and ureterovesical reflux. 2. 38 ureters were performed with the method of Politano-Leadbetter procedure and 3 ureters with Paquia procedure. 3. Majority cases were required additional procedure suck as partial cystectomy, kidney translantation. ureteral tailoring. psoas hitch, nephrostomy and ileocystoplasty. 4. Mean duration of indwelling ureteral splint catheter was 3 days on antireflux procedure and 10 days on antireflux procedure with ureteral tailoring or ileocystoplasty. 5. Complication bad beet observed in 9 cases, which included acute pyelonephritis, ureterovesical reflux, sepsis, uremia, urine leakage and vesicocutaneous fistula.

8.Passive Bowel Movement with a New Colostomy Device: An Acute Experiment in Dog.

In some clinical situations such as cerebrovascular accident, pelvic bone fracture or any bed ridden states patients do not have self control of their bowel movement. Nursing care around the perianal area is not an easy job. There is no devices which substitude the work because of the chracteristics of the anatomy of the anorectum and the fecal matter made of solid and gel state component. AIM: to evaluate the possibility of passive evacuation of the fecal matter from the rectum with a newly developed silicon device. MATERIAL AND METHODS: A New Colostomy Device (NCD; US Patent No. 5,569,216) for fixing in the stoma or rectum of human body, includes an internal balloon, a ring figured external balloon surrounding the internal balloon, a connecting tube disposed under the both internal and external balloons and supply tube containing a pair of air passages and an enema fluid passages. It is designed to be inserted into the rectum and is held in place by an inflatable external balloon and drains irrigated fecal matter through a thin collapsible connecting tube which exist in the anal canal. Six mongrel dog with 20~25 kg of body weight was used for the acute experiment. Three types (1.5, 2.0, 2.5 cm in luminal diameter of the solid portion) of NCD were applied in 3 consecutive every other days. For softening of the stool, normally harder than that of human, Psyllium dextrose 30 gm was added to the daily food. Average 750 cc of tepid water was administered through the device for bowel irrigation. Anesthesia was not used in each procedures. The amount evacuated fecal matter and remained solid stool in rectum were checked. To evaluate the rectal mucosal injury anoscopic examinations were performed. RESULTS: Stool evacuation was closely correlated with the intemal diameter of the device and stool component. The device with 2.5 cm in luminal diameter passed fecal matter well enough in 5 of 6 dogs however, smaller devices did not. Accidental prolapse of NCD were noted in 4 of 6 cases with 2.5 cm sized and all of 1.5 and 2.0 sized devices eventually until last push. CONCLUSION: The NCD with 2.5 cm of internal diameter could be used in selected clinical situations.

9.The Effects of a Weight Control Program with Competence.

PURPOSE: The purpose of this study was to identify the effects of a weight control program and compliancy in overweight women. METHOD: This program was composed of strategies to modify diet and exercise and to change compliance and self determination over an 8 week period. The subjects were 19 overweight women who participated in our project voluntarily. Data was collected from May 4 to Jun 30 of 2007. The program consisted of regular rapid walking exercise, diet, mobile phone messages and e-mail. The data was analyzed by Repeated Measures ANOVA using the SPSS WIN program. RESULT: According to 3 assessment periods, there were significant differences in body weight, body mass index, and compliance. There were no significant differences in self determination. CONCLUSION: These findings suggest that more intensive interventions may be needed to demonstrate a change in self determination.

10.Hemangiopercytoma of the Meninges: The immunohistochemical study for the relationship between hemangiopericytic meningioma and peripheral hemangiopericytoma.

Hemangiopericycic maningioma is clinically and pathologically similar to peripheral Hemangiopericytoma and now tends to be terned as hemangiopericytoma of central nervous system. The authors studied 3 cases of hemangiopericytic meningioma obtained from 3 patients, 1 case of meningotheliomatous meningioma, angioblastic meningioma and transitional meningioma, and 2 cases of peripheral hemangiopericytoma, which had operated from November 1988 to May 1989 at the department of neurosurgery, Pusan Inje University Hospital. The authors analysed and compared the immunohistochemical finding and light microscopic apearance. The results obtained were summarized as follows; 1) Classic maningioma (meningotheliomatous meningioma, transitional meningioma and angioblastic meningioma) shows reactivity to both EMA and vimentin. 2) Hemangiopercicytic meningioma and peripheral hemangiopericytoma are reactive only to vimentin, so the two tumors are suggested as same type of tumor. 3) One of 3 cases of hemangiopericytic meningioma shows whorling and interlacing bundles of spindle cells, the peculiar light microscopic features of transitional meningioma, suggesting transitional or mixed form of hemangiopericytic meningioma and transitional meningioma.
