1.Epidemiological studies on the Acaroid mite.

Epidemiological study on Acaroid mite in dust different sites were carried out and the following results were obtained. Tyrophagus dimidiatus, Chibidania tokyoensis and Ornithonyssus nagayoi were found in living room(46.2 %), bath room(36.7 %), garden(27.0 %), shopping-store(28.8 %) and ware-house (29.2 %) in the Pusan area. Dermanyssus gallinae and Rhizoglyphus echinopus were found mainly in class rooms of girl's high school(34.0 %) and class room of girl's middle school(40.0 %) as well as in primary school(13.2 %), but they are not found in class rooms of boys high school. Generally, the various mites were found in the living room(57.0 %), under the carpet(50 %), under the furniture(52.0 %), on the furniture(27.5 %), lavatory(35.7 %) and garden(23.8 %).

2.The Study of Hypoglycemia in Fullterm with Large for Gestational Age Newborns.

PURPOSE: This study is to determine the frequency, onset age, and the proper duration of evaluation in fullterm large for gestational age (LGA) newborn infant. We also compared risk factors between hypoglycemic and euglycemic newborns. METHODS: 77 term newborns from non-diabetic mothers who were greater than 90 percentile on Lubchencho growth curves were included in this study. Blood glucose levels were checked at age 1, 2, 3, 6, 12, 24, 36, 72, 96 hours and cord blood with rapid strip test. Hypoglycemia was defined as a serum glucose less than 35 mg/dL at less than 3 hours, less than 40 mg/dL between 3 to 24 hours, and less than 45 mg/dL at greater than 24 hours of age. RESULTS: In 77 fullterm neonates with LGA (4,185+/-224 g, 39+/-0.9 wk), frequency of hypoglycemia was 9 case (11.7%) and 3 case of them (3.9%) had clinical symptoms of hypoglycemia. The mean onset age and mean serum glucose level were 2.56+/-2.13 hour and 34+/-6 mg/dL. Glucose level started to decrease in 1 hour and showed lowest at 2 hours. No hypoglycemic patients were noticed after 6 hours of life. There were no significant differences in delivery type, sex, preeclampsia and meconium staining between hypoglycemic and euglycemic newborns except maternal body weight greater than 80 kg (P<0.05). CONCLUSION: Screening of hypoglycemia after 6 hours of age in fullterm LGA newborn infants might not be necessary. And neonatal hypoglycemia should be considered if maternal body weight greater than 80 kg.

3.Initial Results after Implantation of Coronary Artery Stents with Antiplatelet Agents.

Backgound: The placement of stents in coronary arteries has been shown to reduce acute closure and restenosis in comparison to balloon angioplasty. However, clinical use of intracoronary stents is impeded by the subacute stent thrombosis and hemorrhagic complications associated with the anticoagulant regimen. It's known that the complete stent deployment with high pressure inflation and new antiplatelet agents are effective in reduction of subacute thrombosis and hemorrhage. So we evaluated initial results (success and complication rate) after high pressure-stent deployment with new anticoagulation protocol. METHODS: One hundred and ninety one patients with 201 lesions were treated with 231 stents of various types. The high pressure balloon inflation and antiplatelets agents were used in all cases. Final high pressure balloon inflation guided by IVUS were performed in 23 consecutive cases with incomplete stent deployment according to angiographic findings. RESULTS: 1) The indications of stenting (n=210) were De novo in 124 (59%), bailout procedure in 57 (27%), suboptimal result after PTCA in 19 (8%), and restenosis after PTCA in 14 (6%). The location of lesions were LAD in 101, RCA in 67, circumflex in 28, ramus intermedius in 3, and LMT artery in 2 lesions. Angiographic morphologic characteristics were type A in 2, type B in 158 (B1: 57, B2: 101), and type C in 22 lesions. 2) The angiographic and clinical success rate was 96% (192/201) and 92% (186/201) respectively. 3) In angiographic analysis, the baseline average reference vessel dirmeter was 3.33+/-0.35 mm. Baseline minimum lumen diameter (MLD) was 0.58+/-0.29 mm, with baseline percent diameter stenosis of 82.86+/-8.64%. The final stent diameter was 3.37+/-0.29 mm, with mean final percent stenosis of 0.63+/-8.25. The mean MLD after stenting was significantly increased (p<0.001). The mean MLD within stent increased 14%, from 2.91+/-0.39 mm at the nominal balloon inflation (inflation pressure=7 atm) to 3.37+/-0.29 mm at high pressure balloon inflation (inflation pressure <0A65B>12atm) (p<0.001). The length of lesions in GR I (cook), GR II, and Micro II stents were significantly longer than ones in PS, Cordis, Wiktor, Nir (p<0.001). 4) In intravascular ultrasound analysis, the mean lumen CSA at the tightest point within stent increased 11%, from 8.4+/-2.4 mm2 at the intial intravascular ultrasound to 9.4+/-2.1 mm2 at the final intravascular ultrasound (p<0.001). 5) The procedural and postprocedural complications were 2 acute closures associated with AMI and emergent CABG, 1 subacute closure which was revascularized by bail out stenting, 5 major hemorrhage requiring transfusion associated with 1 CVA and 2 metabolic acidosis induced by acute renal failure, and 5 death. CONCLUSION: The high pressure stent deployment procedure and new anticoagulation protocol associating tidopidine and aspirin without coumadin or prolonged heparin infusion allow us to obtain an acceptably low subacute thrombosis or bleeding complication rate. These results are encouraging and allow a wide use of coronary stenting.

5.Impact of Social Distancing and Personal Hygiene on the Prevalence of Epidemic Keratoconjunctivitis during the COVID-19 Pandemic

Purpose:
To analyze the change in the weekly incidence of epidemic keratoconjunctivitis (EKC) per 1,000 outpatients during the coronavirus disease 2019 (COVID-19) pandemic by comparing the mean weekly proportion of EKC of 2020 with that from 2016 to 2019.
Methods:
Using data from the Korea Disease Control and Prevention Agency for 2016-2020, we analyzed the weekly proportion of EKC per 1,000 outpatients. The data were also analyzed according to age, semester and vacation periods, region, and social distancing stages. For the Daegu data, we also analyzed the effects of social distancing in an area.
Results:
The mean weekly proportion of EKC per 1,000 outpatients in 2020 was lower than in previous years for all ages (2016-2019 19.77 ± 7.17‰, 2020 7.28 ± 2.97‰; p < 0.001). During the semester, the mean difference between 2016-2019 and 2020 was significant, particularly for preschool children. In Daegu, the weekly proportion of EKC per 1,000 outpatients during the extra 12-18 weeks of social distancing was significantly lower (2016-2019, 18.78 ± 6.61‰; 2020, 8.94 ± 2.92‰; p < 0.001).
Conclusions
The public health interventions implemented during the COVID-19 outbreak not only reduced the prevalence of COVID-19 but also reduced the prevalence of EKC. Therefore, maintaining hygiene principles and standard precautions may help prevent EKC.
6.Invention of a Nasolacrimal Stent Retrieval Hook.

PURPOSE: To evaluate the clinical usefulness of a nasolacrimal stent retrieval hook. MATERIALS AND METHODS:The retrieval hook consisted of two parts, a hook and pusher. The head of the hook part was made of 0.7 mm stainless steel wire and was question mark-spaped, while the body of this part was constructed by soldering together four stainless steel pipes ranging in thickness from 1.25 to 3.5 mm, serially. The pusher part was made of two plastic tubes and was designed in such a way that the hook grasped the stent tightly. The device was used to remove 46 stents in 40 patients, and tested for 1) average number of trials ; 2) success rate; and 3) average time of the procedure. RESULTS: The average number of trials, success rate, and average time taken to remove the stent was 4.7, 91%, and 1.8 minutes, respectively. CONCLUSION: In the removal of nasolacrimal stents, the retrieval hook was easy to use, useful and effective.

7.Meningitis due to Listeria Monocytogenes Following Orthotopic Heart Transplantation.

We report a first case of meningitis due to listeria monocytogenes after cardiac transplantation in Korea. This patient is a 40-year-old man with Dilated cardiomyopathy, he presented with intermittent dyspnea and abdominal distension for about 1 year. After cardiac transplantation, he was treated with azathioprine, cyclosporine and prednisolone for graft rejection. He was presented with intermittent fever, headache and lethargy for about 10days after cardiac transplantation. Listeria monocytogenes was isolated from blood culture and CSF culture. He was treated with intravenous penicillin G for 10days successfully and changed with ampicillin for 10days, took oral ampicillin for 10days without any complication after discharge.

8.Predictive Factors for the Short-Term Efficacy of Epidural Injections in Lumbar Disc Herniation Treatment

Objectives:
The purpose of this study was to analyze several factors affecting the efficacy of epidural steroid injections as a nonoperative treatment modality in lumbar disc herniation patients.Summary of Literature Review: Despite several studies, the normal course of intervertebral disc herniation is not fully understood, and the optimal timing of surgery is unclear.
Methods:
From January 2017 to April 2019, among outpatients in our hospital, 64 patients diagnosed with 3-4 or 4-5 single-level lumbar disc herniation and followed up for at least 6 weeks were included. Epidural steroid injection was performed in all patients during their 1-week admission. Symptoms improved in 36 patients (group 1) and persisted or recurred in 28 patients (group 2), who were finally treated surgically. Demographic factors, magnetic resonance imaging findings, and clinical assessments were analyzed.
Results:
The only demographic factor with a significant difference between groups was symptom duration. In magnetic resonance imaging, significant differences were found in disc sequestration and the degree of the herniated disc. At 6 weeks after the injection, significant differences were found between the 2 groups in visual analog scale scores and the Korean version of the Oswestry Disability Index.
Conclusions
Several factors must be considered when treating patients with lumbar disc herniation. Of note, longer symptom duration was found to be associated with a lower effectiveness of conservative treatment. The effects of treatment could also be influenced by the disc form and the degree of disc herniation.
9.Effect of Transcranial Direct Current Stimulation on Postural Stability and Lower Extremity Strength in Hemiplegic Stroke Patients.

OBJECTIVE: To evaluate the effect of anodal transcranial direct current stimulation (tDCS) over the lesioned leg motor cortex, which can enhance the strength and coordination of the contralateral lower extremity and furthermore, enhance the postural stability of the hemiplegic subject. METHODS: Anodal or sham stimulation on the lesioned cortex of a lower extremity was delivered to 11 ambulatory hemiplegic patients. The stimulation intensity was 2 mA. All subjects took part in two 10-minute tDCS sessions consisting of anodal stimulation and sham stimulation. The interval period between real and sham stimulation was 48 hours. The order was counter-balanced among the subjects. Before and after each stimulation session, static postural stability was evaluated with eyes opened and closed. Also, the isometric strength of the hemiplegic side of the treated knee was measured before and after each stimulation session. Repeated measure ANOVA was used to determine the statistical significance of improvements in postural stability and strength. RESULTS: There was significant improvement for overall stability index with eyes opened and closed after anodal tDCS (p<0.05). Isometric strength of the lesioned quadriceps tended to increase after anodal tDCS (p<0.05). Postural stability and quadriceps strength were not changed after sham stimulation. CONCLUSION: Anodal tDCS has potential value in hemiplegic stroke patients to improve balance and strengthen the affected lower extremity.

10.Incomplete Intertrochanteric Fractures Based on Multiplanar Reconstruction Computerized Tomography : A Report of Three Cases.

We experienced 3 cases of incomplete intertrochanteric fracture that were detected by multiplanar reconstruction computerized tomography (MPR CT). On the plain radiographs, two cases had only greater trochanteric fracture and the other case had a localized intertrochanteric fracture. There were linear hot uptakes in the intertrochanteric area in all cases on the bone scintigrams. On MPR CT, cortical breakage was found only in the anterior cortex, and the medial, lateral and posterior cortices were intact in all cases. Two cases were treated surgically with using compression hip screws and the other case was managed conservatively because of the patient's poor general condition. The findings of MPR CT were definitely different from those MRI findings of the previously reported incomplete intertrochanteric fractures.
