2.The Treatment of Infected Nonunited Fractures of Long Bone

Many difficult problems are encountered in the treatrnent of infected nonunion of long bone. To solve these difficult fractures, authors adopted an active approach-thorough debridement of the infected and necrotic soft tissue and bone, rigid internal fixation with autogenous cancellous bone graft and opening of the wound to ensure continuous drainage until union of the fracture. Authors treated 28 cases of infected nonunions with this method from Mar. 1980 to Feb. 1989, and the results are as follows ;1. Femur(54%) and tibia(43%) were most frequently involved sites. 2. Most of the cases(86%) were treated by rigid internal fixation and cancellous bone graft. 3. In most cases(64%), infection was controlled spontaneously by the time of bony union, but in 5 cases which showed persistent drainage, implant removal controlled the problem, and residual soft tissue and bony defect were solved by Papineau's technique or musculocutaneous flap. 4. Revision was required in 5 cases(18%) because of insercure fixation. 5. Union could be obtained in average 4.8 months, and infection could be controlled in all except 1 case.

7.A Case Report of Slipped Capital Femoral Epiphysis Associated with Hypogonadism and Diabetes Insipidus

The slipped capital femoral epiphysis is the condition in which the femoral head slips downward and backward on the femoral neck at the epiphyseal plate. The underlying cause of this disease is unknown. In general, it is believed that endocrine factors may play a part as shown in experimental work. But few endocrine abnormalities have been proved. It is rare in Korea and eight cases have been reported and only one case was associated with diabetes insipidus in hypogonadal Turner mosaicism. Authors report a case of mild, acute on chronic slipped capital femoral epiphysis in a 19 year old male with hypogonadism and diabetes insipidus. It was treated by internal fixation with three Steinmann pins and the result was good.

8.An Experimental Study of the Corneal Epithelial Damage by Electric Welding Light on the Rabbit Cornea.

The cellular alteration following the electric welding light on the rabbit corneal epithelial cells were studied in several conditions wearing various corneal contact lenses on the corneal surface exposed to the light for 5 minutes at 50 cm in each. The enucleated rabbit eyes 5 hours following the application of electric welding light were histopathologically investigated on the cellular structures of the cornea. The result were as follows: 1. The cellular alterations in a group of soft contact lens wearing consisted of multiple vacuole enlargement, separation of interdigitation and Desmosome, intracellular edema, and loss of organelles in superficial and wing cells layer. Loss of microvilli of the corneal superficial cells were observed. 2. Loss of cytoplasmic microorganelles and microvilli intercellular edema in superficial cell layer, and slight vacuoles enlargement in wing cells layer were demonstrated in a group of the direct light exposure group without contact lenses. 3. Loss of microvilli and intercellular edema with separation of interdigitation in superficial cell layer but no specific changes in wing cells layer were exhibited in a group of hard contact lens wearing. 4. The most destructive features of the corneal epithelial cells were observed in a group of soft contact lens wearing, and it was suggested that the doubtful effects of protection on the corneal superficial tissue in the case of soft contact lens wearing were noteworthy.

9.A Case of Corneal Amyloidosis.

Amyloidosis may be defined as a group of disorders of extracellular deposition of the fibrous protein amyloid in one or more sites of body with variable causes and pathogenesis which have in common characteristic histopathologic features. Primary localized corneal amyloidosis is confined in the cornea and has not the evidence of apparent preexisting disorder or systemic involvement. The incidence has been reported rare. We have experienced one case of corneal amyloidosis which is confirmed by the histopathologic evaluation. We report this rare case with literatures reviews.

10.The Study of Education and Qualification for the Maintenance of Clinical Research Associate (CRA) Professionalism.

BACKGROUND: The aim of the current study is to investigate perspectives on the quality and validity of the current clinical research associate (CRA) education program. The trainee program of other healthcare professions including medical doctors, registered nurses, and pharmacists, has been already reviewed as an advance research. Thus, the current study evaluates the CRA education system to assist in building a more suitable CRA education infrastructure in the future. METHODS: A questionnaire was handed out to CRAs with at least 3 months working experience among 30 pharmaceutical corporations, and 10 Contract Research Organizations (CROs) affiliated with the Korean Pharmaceutical Association and the Korean Clinical Research Committee. A total of 118 questionnaires were utilized for data analysis. RESULTS: The poll shows that 93.2 % of the survey participants consider the role of a CRA as a specialized healthcare profession and 58.6 % of those respondents reason that becoming a CRA requires specialized knowledge. 95.9 % of survey participants attend a CRA trainee program for the maintenance of CRA professionalism. The respondents are inclined to attend the Government hosted CRA education program. 59.3 % of survey participants answered that implementing a CRA qualification system is necessary to establish a CRA specialty validation. CONCLUSION: The current study demonstrates the importance of qualification and what is necessary for being a CRA. The study result will provide a guideline for those who wish to become a CRA as part of their career. In addition, the result will also be used in developing an improved CRA education program in the future.
