2.Comparison Between HLA-DR Serological Typing and O1igotyping.

BACKGROUND: In renal transplantation, a good HLA-DR match Is associated with successive graft outcome. But due to a number of technical problems, reliable serological DR typing cannot always be obtained. To compare the serological DR typing with DRBI DNA typing, we tested 103 specimens that had been frozen after serological typing, by PCR-SSOP typing method. METHODS: Serological DR typing was performed by complement-dependent microlymphocytotoxicity technique using commercial antisera kits, and DNA gyp ins was performed by PCR-SSOP, using one of the methods recommended by 12th International Histocompatibility Workshop. DNA amplification was done by DRBAMP-A and DRBAMP-B primers, and hybridization by 18 oligonucleotides labelled with digoxigenin.. RESULTS: The concordance rate between serologic typing and DNA typing was 76.7%. Most (79.0%) of discordant results were due to serological blanks turning out to be definable antigens by DNA typing and these antigens consisted of mainly DR5 splits but none of DR1, DR2, or DR7. CONCLUSIONS: In spite of technical improvement, serological typing method often can not define the accurate HLA-DR type. It is thought that combining serological typing with DNA typing Is necessary to achieve a higher success rate of graft outcome.

5.A Case of Heterotopic Pregnancy Following in vitro-fertilization.

Heterotopic pregnancy occurs when an intrauterine pregnancy co-exists with an ectopic pregnancy. It is a potentially fatal condition, rarely occuring in natural conception cycle. But its incidence is increased since the rise in PID, pelvic surgery, IUD, and advent of assisted reproductive technology involving use of superovulatory drugs and/or in vitro-fertilization. We present a case of intrauterine twin pregnancy and right tubal pregnancy following treatment with in vitro-fertilization. The right tubal pregnancy was diagnosed after rupturing at 6th gestational weeks, and resected via laparoscopy. And healthy twin babies were delivered without complication at 38th gestational week. So we report this case with a brief review of the literatures.

6.A Bumpy and Winding but Right Path to Domestic Drug-Eluting Coronary Stents.

Restenosis and stent thrombosis remain major concerns after percutaneous coronary intervention for the treatment of coronary artery disease. The present review was undertaken in order to highlight the various coronary stents that have been investigated in our Heart Research Center, and how far we have come from the first heparin-coated stent first used in the late 1990s. Thereafter, from the abciximab-coated stent to the current gene-delivery stent and other newer agents, our group has applied a range of techniques in this field. However, in groups similar to ours, the restenosis rates of such stents are still high for second-generation drug-eluting stents (DESs). Moreover, our nation imports almost all of these types of stents from other countries. Thus, we need to develop domestic coronary stents. Research into newer DESs are warranted in Korea so as to achieve improved safety and efficacy outcomes.

7.A Socio-Psychiatric Study on Family Violence by Delinquent Adolescents.

OBJECTIVES: This research was performed to identify family violence by male adolescencts, and to examine the characteristics of male adolescent delinquents in terms of domestic viloence. METHODS: We surveyed 677 male adolescents including 238 delinquents from BBS(Big brothers and sisters) in Pusan, 209 criminal adolescents from adolescent correctional institution in Pusan, 230 high school students through questionnaire, MMPI(Minnesota Multiphasic Personality Inventory), and PBI(Parental Bonding Instrument), and then compared these three groups by Chi-square test, T-test, and Anova. RESULTS: In most cases, male delinquent adolescents were placed in poorer socio-economic conditions than those of students, but in better socio-economic conditions than those of criminal adolescents. In general, the assaulters were senior family members, physically stronger than sufferers, and have bad brotherliness. Compared with normal students, the serious problem of violence including violence to the ancestors in delinquent and criminal adolescents. On results of MMPI and PBI test, there were significant difference between violent group and non-violent group on the Sc score and Passive aggressive index in adolescent delinquents and parental overprotection in normal students. CONCLUSION: Generally the characteristics of family violence by male delinquent adolescents were intermediate between those of criminal adolescent and students, these results were same as those of previous socio-psychiatric study, and so the intervention in criminal adolescents is needed for the pre-vention of family violence.

9.Clinical and Bacteriological Studies of Urinary Tract Infection in Children.

Clinical and bacteriologicaql studies on 133 cases of urinary tract infection who were admitted to the Dept. of Pediatrics, NMC during the period of Jan. 1974 to Jan. 1979 were subjected in this study. The resultes were as follows : 1. Among the total 133 cases, 100 cases(75.2%) were male and 33 cases(24.8%) were female with sex ratio of 3:1. 2. The highest incidence(42.1%) was in children below the age of one year. The rate then decreased with age. 3. Seasonal incidence was relatively high in summer months, but seasonal difference was not significant. 4. Edema, signs of URI, vomiting, hematuria, failure to thrive and feeding problem were common clinical features. 5. Urinalysis disclosed proteinuria in 43 cases(32.3%), hematuria in 36 cases(27.1%) and pyuria in 26 cases(19.6%). 6. Hematological findings showed low hemoglobin in 40 cases(30.0%), leucocytosis in 31 cases(23.3%) and elevated ESR in 42 cases(31.6%). 7. IVP was performed in 13 cases, and 3 cases of them showed abnormal findings. 8. Nephrotic syndrome, acute glomerulonephritis, hyperbilirubinemia and sepsis were the common concurrent illnesses. 9. E. coli predominated as the infecting organisms(36.8%). It was most sensitive to ;gentamycin(83.7%) and cefamezine(77.8%)
