Pseudoaneurysm or false aneurysm is one of the late complication of a missed or untreated arterial injury. Most traumatic aneurysms in the extremities are false, and they are usually caused by penetrating trauma. The diagnosis of pseudoaneurysm is suggested by the presence of one or more of the followings: an expanding ill defined mass with or without pulsation, a bruit, pain, paresthesia or paralysis due to nerve compression, an absent peripheral pulse, or signs of ischemia. Although plain radiographs may demonstrate a nonspecific soft tissue mass, arteriography is essential in defining the false aneurysm or aneurysms and differentiating them from other soft-tissue masses. We are reporting the case of a patient who had a false aneurysm of the posterior tibial artery after open fracture of tibial shaft.

We have reviewed the 503 cases of benign skin tumors which were confirmed by histopathological study from January 1977 to July 1987 in the Dermatologic Department of Kyungpook National University Hospital. The result were summarized as follows . l. Among the total of 26,690 dermatologic out patients, 503 cases(1.9%) were diagnosed as having benign skin tumors. The number of benign skin tumors has shown a noticeable increase in the last several years. 2. The tumors were found mainly in ages ranging from the 2nd to 5th decade, most commonly in the 3rd decade(124 cases, 24.7%). 3. Aceording to Lever's elasaification, the tumors were divided into the following 7 groupa . tumors and cyets of the epidermis(35.2%), melanocytic tumors(16.5%), tumors of vaecular tissues(13.3%), tumora of epidermal appendages(11.1%), tumors of fibrous tissue(8.9%), tumors of neura.l tiasue(7.6%) and tumors of fatty, muscular and osseous tiseue(7.4%) in decreasing orders. 4. The 14 common tumors in order of frequency were as follows epidermal cyst (17.l% ),:melanocytic nevus(16.5%), neurofibroma(7.6%), congenital hemangioma(6.4%), nevus sebaceous(6.0%), lipoma(5.2%),seborrheic keratosis(4,8%), linear epidermal nevus(4.6%), steatocystoma mutiplex(2.8%), dermatofibroma(2.6%), hypertrophic scar and keloid.(2.6%), granuloma pyogenicum(2.4% ), keratoacanthoma(2.0%) and syringoma (2,0%). 5. The predilection sites of seborrheic keratosis, keratoacanthoma, syringoma and melanocytic nevus were the face, linear epidermal nevus, dermatofibroma and granuloma pyogenicum were located on the extremities. 6. The duration of tumors from the presumed time of onset to the first visit was within 1 year for epidermal cyst, keratoacanthoma and granuloma pyogenicum, over 10 years for nevus sebaceous, congenital emangioma and neurofibroma. 7. Diagnostic accuracy was higher in melanocytic nevus, congential hemangioma, nevus sebaceous and neurofibroma and lower in steatocystoma multiplex, dermatofibroma, keratoacanthoma, lipoma and epidermal cyst.

There are many advantages in treating long bone fractures with interlocking intramedullary nail and its advantages are well known. Although it is used widely, many major and minor complication can develop in spite of meticulous operation techniques. Frist fifty consecutive interlooking intramedullary nail of two different surgeon, total of 100 cases that were operated in Seoul Paik Hospital from January, 1991 to July, 1994, were reviewed and the complications were analyzed with various factors. 1. Among the 100 cases, a total of 30(30%) major and minor complications had developed. But only 4 major complications which needed reoperation or poor result occurred. 2. Among the complications that had developed, angulation and rotation deformity being 10 cases were the most common, other complications were delayed union in 5 cases, nonunion in 2 cases, problem of screw in 4 cases, metal failure in 2 cases, misentry point in 2 cases and others in 5 cases. 3. There were no remarkable difference between surgeon A and surgeon B. The complications decreased abruptly after 30th case for each surgeon and only a few complications, probably due to complex fracture pattern, occurred. In conclusion, complications after interlocking intramedullary nailing can be reduced by meticulous preoperative planning and learning process of surgeon.

BACKGROUND: Trichophytosis gladiatorum is dermatophytosis transmitted by a physical contact between wrestlers. OBJECTIVE: To investigate the current status of the illness in Korean amateur wreatlers. METHOD: The epidemiological, clinical and mycological studies were performed on dermatophytosis in a total of 1, 394 Korean amateur wrestlers through a nation-wide group examination from July, 1995 to April, 1996. RESULTS: A number of 575 (42.5%) cases of tinea were detected throughout the country among 1, 394 wrestlers. The prevalence rate of tinea in wrestling students was 48.4% for middle school, 41.4% for high school, 30% for college and above. The head and neck area was the predilection site of the skin lesions, which decreased in number as moving towards legs. As a whole, the lesions affected the right half of the body 1.7 times as more frequently as the left half. The tendency of a predilection for the right side of the body was most prominent on the head, typified by auricle (2.8 times). In case of the scalp, the lesions developed on the right temporal, right occipital, left occipital and left temporal regions, in decreasing order. The scalp lesions were noted in 44.7% of the total tinea patients - erythematous scaly patches affecting 69.2% of the total tinea capitis patients, followed by alopecia, black dot ringworm, seborrheic dermatitis-like scales, folliculitits, cervical lymphadenitis, kerion celsi, in decreasing order. From 360 patients (62.6%), dermatophytes were isolated - 359 strains of Trichophyton tonsurans (207 of sulfureum variety, 137 of mahogany variety, 15 of both), 1 of Microsporum canis. The varieties of T. tonsurans showed a tendency to occur in groups in the same districts and teams. CONCLUSION: Trichophytosis gladiatorum due to T. tonsurans is prevailing among Korean wrestler. Proper epidemiological measures must be taken to prevent further spreading of the disease.

The cardiac function of the patients with chronic renal failure, who have had routine hemodialysis for long period is influenced by many factors including hypertension, anemia, artherosclerosis, interdialytic fluid collection and acidosis. Also the fluid shifts that occur during dialysis can alter left ventricular loading conditions. Hemodialysis may also influence left ventricular contractility by virture of its ability to correct electrolyte imbalances, BUN, hematocrit and concentration of inotrophics. Ten patients with chronic renal failure, who have hemodialysis during more than 6 months were studied for the effect of hemodialysis on left ventricular function through echocardiography. 1) Body weight and systolic blood pressure were decreased(P<0.05) and diastolic blood pressure mean arterial blood pressure and heart rate did not increased significantly. 2) Serum calcium increased and BUN and serum potassium decreased significantly(P<0.05). 3) The prolonged preejection period, decreased left ventricular ejection time, stroke volume, endsystolic wall stress and increased rate corrected velocity of circumferential fiber shortening, shortening fraction had statistical significance. In conclusion, hemodialysis can improve cardiac function as decreased preload and increased left ventricular contractility from the patients with chronic renal failure.

The purpose of this study was to analyze the minimum two year clinical and radiological results of revision total hip arthroplasties using allogenic chip bone graft and cement in acetabular bone deficiencies. Thirty six revision total hip arthroplasties that had been performed in thirty four patients between Sep. 1992 and May. 1994 at Seoul Paik Hospital and had followed more than two years were included in this study. The clinical result was evaluated by Harris hip score. The mean Harris hip score was 48 points preoperatively, 88 points at final follow-up, a mean of 3 years after revision. In radiological evaluation, osseous union between grafted bone and host bone was seen within 4 months in thirty two hips (89%), a complete grafted bone-cement radiolucent line of two millimeter or more in at least one zone was seen in seven hips (19%) and acetabular component migration was seen in six hips (17%) at postoperative 3 year follow-up radiograph. At the time of follow-up, five hips (14%) had been revised a second time: three for aseptic acetabular loosening, two for infection. In conclusion, we recommend the technique using allogenic chip bone graft and cement to reconstruct the acetabular bone deficiencies in revision total hip arthroplasties, though other technique will be recommend in severe segmental acetabular deficiencies or previous infection.

Common atrium is a rare congenital heart disease characterized by complete absence of the interatrial septum, and is commonly accompanied by malformation of the atrioventricular valve. Most patients with common atrium experience symptoms during childhood. Here, we describe a patient with common atrium who experienced his first obvious symptom at 48 years of age.
