2.Antibacterial Effect of Polyphosphates on Porphyromonas gingivalis.

Porphyromonas gingivalis is strongly implicated in the pathogenesis of adult periodontitis, the major cause of tooth loss in adults. Use of an antibacterial agent controlling P. gingivalis as a periodontal therapeutic agent has been rationalized. The present study was performed to observe the antibacterial effect of inorganic polyphosphates (polyP) on P. gingivalis. P. gingivalis 2561 was grown in half-strength brain-heart infusion broth containing hemin and vitamin K with or without polyP. Minimal inhibitory concentration (MIC) of polyP with various chain lengths was determined by measuring the absorbance of the grown cells at 540 nm. MIC of polyP for the bacterium was determined to be 0.05%. The effect of polyP with a chain length of 75 (polyP 75) was further examined. PolyP 75 added to the growing culture of P. gingivalis at its exponential phase was as effective in inhibiting the growth of P. gingivalis as polyP 75 added at the very beginning of the culture. More than 99% of the cells lost their viability determined by viable cell count when polyP 75 was added to the culture of growing P. gingivalis at the concentration of 0.06%, suggesting that polyP 75 has a bactericidal effect on the bacterium. Intracellular nucleotide release from the cells was increased by approx. 20% in the presence of polyP 75 but was not reversed by the addition of divalent cations like Ca++ and Mg++. Under the transmission electron microscope, only a small number of the growing P. gingivalis cells were actually lysed. However, the majority of the cells appeared to be atypical in their shape, demonstrating accumulation of highly electron-dense granules and bodies of condensed nucleic acid-like material in the cytoplasm. In the presence of polyP 75, the protein profile of P. gingivalis was changed as determined by SDS-polyacrylamide gel electrophoresis and immunoblot, and the proteolytic activity of the bacterium demostrated on the zymograms was decreased. The overall results suggest that polyP have a strong bactericidal activity against P. gingivalis in which lysis in relation to chelation may not play the major role but unknown mechanism that possibly affects the viability of the bacterium may be involved. PolyP may be used as an agent for prevention and treatment of periodontitis.

3.The transconjunctival approach to a large retrobulbar cavernous hemangioma of the orbit.

Cavernous hemangiomas are one of the most common benign tumors of the orbit in adults. We report a case of a longstanding retrobulbar hemangioma that was removed successfully through a temporal transconjunctival approach combined with lateral canthotomy. A 45-year-old female patient, with a 15-year history of slowly progressive proptosis and decreased visual acuity of the left eye, had a corrected visual acuity of finger count at 50 cm OS, compared with 1.0 OD. Exophthalmometry by the Nagle's method measured 15 mm OD by 26 mm OS. Magnetic resonance imaging (MRI) revealed a well-encapsulated retrobulbar main mass, 2.3 x 3.0 x 3.7 cm in size along with multiple small satellite nodules that were displacing the optic nerve and globe superiorly. The tumors were removed through a superotemporal transconjunctival approach combined with lateral canthotomy. Pathological examination revealed an intraorbital cavernous hemangioma. The patient was free of visible scars, proptosis and any other noticeable complications at her last follow-up, 6 months after surgery.

4.External Nasal Appearance Preferred by Koreans: Photo Analysis.

A sound understanding of the preferred appearance of the external nose is central to successful rhinoplasty. A survey using photographs was conducted among college students and employees aged 20 to 39 years in Inchon, Korea. Three hundred and twenty-one subjects filled out a questionnaire about their preferences regarding external nasal appearance. The most commonly preferred height of the nasion, measured from the corneal plane, was 6 mm among both sexes, and the most commonly preferred depth of the nasion, measured from the glabella plane, was 3 mm among both sexes. Regarding the nasofrontal angle, men preferred an angle of 135 degrees and women preferred an angle of 140 degrees. Men preferred a nasofacial angle of 33 degrees while women preferred an angle of 30 degrees. A straight nasal dorsum was most commonly preferred among both sexes and the most commonly preferred dorsal width was 13 mm among men and 10 mm among women. The most commonly preferred tip projection was 30 mm among men and 27 mm among women and the preferred tip angle was 105 degrees among both sexes. The angle of the apex in the most commonly preferred nasal base configuration was 85 degrees among men and 70 degrees among women. The angle of the apex in the most commonly preferred lobular configuration was 65 degrees among both sexes. The most commonly preferred ratio between nostril length and the distance from the subnasale to the tip was 45 percent among both sexes. The most commonly preferred angle between both nostril axes was 60 degrees among both sexes. The most commonly preferred nasolabial angle was 90 degrees among men and 95 degrees among women. The main difference in preference between Koreans and Caucasians seemed to center on the radix area rather than the tip. These results can be useful as basic guidelines for rhinoplasty.

Sir James Paget described the first case of Paget's disease of the breast in 1874, and Du-breuilh, in 1901, reported the first case of extramammary Paget's disease of the vulva. Paget's disease of vulva is a rare intraepithelial neoplasm of uncertain histogenesis. Recently we expe-rienced a case of Paget's disease of the vulva that we present a brief review of literature.

7.Blood Brain Barrier and Brain Tissue Injury by Gd-DTPA in Uremia-induced Rabbits.

PURPOSE: An experimental study was carried out to evaluate the morphological changes in the blood brain barrier and neighbouring brain tissue caused by Gd-DTPA in uremia-induced rabbits. MATERIALS AND METHODS: Bilateral renal arteries and veins of ten rabbits were ligated. Gd-DTPA(0.2 mmol/kg) was intravenously injectedinto seven rabbits immediately after ligation. After MRI, they were sacrificed 2 or 3 days after ligation in orderto observe light and electron microscopic changes in the blood brain barrier and brain tissue. RESULTS: MRIfindings were normal, except for enhancement of the superior and inferior sagittal sinuses on T1 weighted imagesin uremia-induced rabbits injected with Gd-DTPA. On light microscopic examination, these rabbits showed perivascular edema and glial fibrillary acidic protein expression ; electron microscopic examination showed separation of tight junctions of endothelial cells, duplication/rarefaction of basal lamina, increased lysosomes of neurons with neuronal death, demyelination of myelin, and extravasation of red blood cells. Uremia-induced rabbits injected with Gd-DTPA showed more severe changes than those without Gd-DTPA injection. CONCLUSION: Injuries to the blood brain barrier and neighbouring brain tissue were aggravated by Gd-DTPA administration inuremia-induced rabbits. These findings appear to be associated with the neurotoxicity of Gd-DTPA.

FISH is suggested as a possible method to detect the numerical and structural abnormalities of chromosomes in interphase nucleus. We performed this study to discuss the clinical usefulness of FISH in uncultured amniocytes and to set up the cut-off value for further study. We collected amniotic fluid samples from patients whose chromosome studies were recommended due to screen positive for Down and Edword syndrome in triple marker test using maternal serum. The centromeric probe for chromosome 18 and the locus-specific probe for chromosome 21 were used and the results were compared to their karyotypes. We could find 2 cases of trisony 21 and 2 cases of trisony 18 and the other cases represented normal karyotypes. The accuracies were 91% for disomy 18, 89% for trisomy 18, 92% for disomy 21 and 88% for trisomy 21. Therefore FISH technique is a possible method to detect the chromosomal abnormalities in uncultured amniocytes and the use of locus-specific probe for chromosome 21 would be more useful for detecting the aneuploidy of chromosome 21 than 13/21 centromeric probe.

9.Application of FISH(Fluorescence In Situ Hybridization) in uncultured chorionic villus cells.

OBJECTIVE: The cytogenetic analysis for earlier detection of fetal chromosome aneuploidies is performed from chorionic villus using either long-term culture or direct chromosome preparation. To analyze the cause of pregnancy loss, we also attempt the cytogenetic study in product of conception(POC) using chorionic villi or fetal tissue. But the failure of analysis often occurs in direct preparation of villus cells and product of conception(POC). We studied to evaluate the clinical usefulness of FISH in uncultured chorionic villus cells of culture-failed cases. METHODS: According to the patient's indication, we performed FISH for chromosome 18, 21, X and Y in chorionic villi as well as POC and compared FISH results with their chromosomal studies. RESULTS: We found one trisomy 18 and one trisomy 21 in Chorionic Villus Sampling and one trisomy 18 and one monosomy X(45, X) in POC. The averages for accuracy of FISH were 83-91% and all cases are represented consistent results with their chromosomal studies. Among them, we could analyze using FISH only in 5 cases of culture failure including one case of monosomy X in POC. CONCLUSION: We could detect aneuploidy with uncultured chorionic villus cells in case of culture failure, using FISH, it may be the potential method to assist the cytogenetic study.

10.Three Cases of Reconstructive Rhinoplasty Using a Local Flap or a Split Calvarial Bone Graft.

The nose is located centrally in the face and occupy an important place in the harmonic architecture of the face. Also, it plays important functions such as respiraton, olfaction and phonation. Therefore, reconstructive rhinoplasty is dealt with both cosmetic and functional aspect in mind. We have recently experienced three cases of reconstructive rhinoplasty for seborrheic keratosis of the nose, traumatic nasal defect, and basal cell carcinoma of the nose. Local flap and split calvarial bone graft were used for the reconstruction of the nose.
