Although there have been many reports about the importance of the spleen's role in hepatic fibrogenesis, the exact mechanism is still uncertain. The author designed this study to evaluate splenic function on hepatic fibrogenesis. The degree of hepatic fibrosis and the population of splenocyte subsets were studied in the experimental animal model with fibrosis produced by injecting normal swine serum intra-peritoneally into Sprague-Dawley rats. The animals were divided into three groups; group A was subjected to injection of swine serum only, group B swine serum and complete Freund's adjuvant and group C swine serum and cyclosporin A. The experimental hepatic fibrogenesis by swine serum was augumented by coinjection with the adjuvant and inhibited by cyclosporin A. The study of the splenocyte subset revealed increased percentages of spienic B cell and CD4+ cell and a decreased percentage of CD8+ cell, and these changes of splenocyte subset were also augumented by the adjuvant and inhibited by cyclosporin A. The percent of monocytes was not significantly altered, although a tendancy of early decrease by the adjuvant was noted.

4.Pseudohydronephrosis in Two-Phase Spiral CT of the Abdomen.

PURPOSE: To assess the incidence and causes of pseudohydronephrosis in the abdominal two-phase (arterial and portal) spiral CT. MATERIALS AND METHODS: This study included 115 renal pelvises of 62 patients (hepatocellular carcinoma in 36, gastric carcinoma in 10 and others in 16 patients) whose entire renal pelvises were imaged in both phases of abdominal spiral CT. Arterial and portal phase scans were obtained 30 and 65-70 seconds after the injection of 100 mL of non-ionic contrast media (370mg I/mL) at a rate of 2.5 mL/sec, respectively. The renal pelvis was considered to be dilated when the shortest diameter of renal pelvis exceeded 1cm as seen on CT. In each patient, a plain film was obtained 5-10 minutes after the injection of contrast media for CT to confirm the hydronephrosis. RESULTS: Seven renal pelvises were dilated on spiral CT. All of them were pseudohydronephrosis with prominent renal pelvis and two of them were extrarenal pelvises. In 115 renal pelvises, there was one case of true hydronephrosis due to a ureteral stone not suspected clinically. CONCLUSION: A 6% incidence of pseudohydronephrosis due to prominent renal pelvis should be considered in interpreting the abdominal two-phase spiral CT.

6.Standardization and Validity Study of Korean Nomenclatures of The Home Health Care Classification.

The purpose of this study was to develop and test the validity of standardized Korean nomenclature of the Home Health Care Classification(HHCC), developed by Saba at the University of Georgetown. The four phases of the study were : (1) Two professors and 15 graduate students, who were taking a nursing intervention course, translated the HHCC into Korean. (2)12 nurses with various clinical backgrounds reviewed each nomenclature taking into consideration of the definition and activities, and the Delphi method was used to determine the best appropriate nomenclature for each term; (3)20 academic and clinical experts in nursing were given a questionnaire to rate the validity of each Korean nomenclature using a 5 point Likert scale ranging from very inappropriate to very appropriate; (4) Five members of the Korean Nurses Association Research Committee reviewed the survey results and determined the most appropriate Korean nomenclature for each term of the HHCC. Most of the nomenclatures had a score of more than 4.0, but several terms had a score lower than 4.0. They are Ambulation therapy(3.1.1), Anticipatory grieving(3.11), Sexual patterns alteration(3.17), Nursing care coordination(3. 22), Physical therapist service(3.22), Regular diet(3.33), and Instrumental activities of daily living(3.44). These research results will be included in the nursing vocabulary dictionary to be published by the Korean Nurses Association.

7.Development of a Korean-translated Version of the 17 Nursing Diagnosis.

Nursing Diagnosis has evolved in the guest to define nursing and its functions. But for the application to clinical practice an unified system of terminology that help nurses to assess selected data and identify potential or actual client problems is required. Consistent terminology that captures the real meanings of the nursing diagnosis makes oral and written communication more accurate and efficient. Already we had performed to deliver a Korean -translated version of the 98 nursing diagnoses through the process of content validity tests and translations, and additionally performed to deliver a Korean-translated version of another 17 nursing diagnoses. The list of 17 nursing diagnoses were reviewed by our research team and two linguists, one specialized in Korean and the other in English. 17 diagnoses were mailed to 719 subjects to test content validity from December 10, 1997 to January 24, 1998, Among 719 questionnaires, 185 were used for analysis. Total mean score of the diagnoses was 3.72 on the 5 point likert scale. The diagnoses that acquired less than 3.50 were 'Risk for altered parent/infant/child attachment'(3.28), 'Potential for enhanced organized infant behavior'(3.40), 'Potential for enhanced community coping' & 'Risk for disorganized infant behavior'(3.49). We suggest to conduct tests for validation of related factors and characteristics of these 17 Nursing Diagnosis.

8.Improvement of the qualification system in Korea for foreign nurses eligible to take Korean nursing licensing examination

Purpose:
We aimed to review and provide a quality improvement for the document utilized by the relevant Korean government body to verify and evaluate foreign university/college graduates’ eligibility for nursing and qualification to take the Korean nursing licensing examination.
Methods:
This was a descriptive study. We analyzed the current Korean qualification system for foreign graduates to Korean nursing licensing examination and the same system utilized in some other countries. Then, we created a draft of the reviewed qualification standards document based on the 2 prior analyses and their comparisons, and applied a questionnaire in an open hearing with 5 experts to enhance the draft’s quality. Finally, we presented and discussed the final draft.
Results:
The reviewed criteria of the qualification standards included confirming whether the foreign graduate’s university has an accreditation provided by its relevant government body, the exclusion of foreign graduates’ provision of several documents previously required, a minimum number of credits (1,000 hours) for their original course, a 3-year minimum enrollment period for their original course, and a mandatory reassessment of the foreign graduates’ university recognition in a 5-year cycle.
Conclusion
We believe that by creating a review draft that addresses the flaws of the current document utilized to determine the qualification for foreign graduates to take the Korean nursing licensing examination, we have simplified it for a better understanding of the application process. We hope that this draft will contribute to a more objective and equitable qualification process for foreign university nurse graduates in Korea.
9.Visual Performance and Optical Quality after Implantation of a New Generation Monofocal Intraocular Lens

Purpose:
To evaluate the performance of two intraocular lenses (IOLs). The new monofocal IOL using a higher-order aspheric optic (Tecnis Eyhance ICB00) was compared to a standard monofocal IOL (Tecnis monofocal ZCB00) of the identical platform and material.
Methods:
The medical records of the patients who had undergone cataract surgery with implantation of either the ZCB00 or the ICB00 in the dominant eye from March 2020 to August 2020 and with available data from the 3-month visit were reviewed. Subjects with ocular comorbidities or corneal astigmatism greater than 1.00 diopters were excluded. The uncorrected near, intermediate, distance visual acuity and corrected distance visual acuity were the main outcome measures. Optical quality parameters measured using an optical quality analysis system, clinical records including age, sex, laterality, ocular dominance, and information related to refractory errors was also collected. Parameters related to the refractory errors were all uniformly based on the Barrett Universal II formula.
Results:
Of the 197 recruited patients, 111 and 86 were implanted with the ICB00 and ZCB00, respectively. No statistically significant differences in baseline parameters were observed between the two groups. While no statistically significant differences in distance visual acuity or optical quality were found between the two groups, compared to the ZCB00 group, the ICB00 group showed significantly higher intermediate visual acuity (p < 0.001) and near visual acuity (p < 0.05) 3 months postoperatively.
Conclusions
ICB00 provided superior intermediate vision and comparable distance performance and photic phenomena compared to a standard monofocal IOL.
