1.A Case of Childhood Systemic Lupus Erythematosus Showing Erythema Multiforme-like Eruption.

We present a case of childhood systemic lupus erythematosus(SLE) with renal disease in a 10-year-old girl with multiple, scattered, erythema multiforme-like eruptions on the face, trunk, forearms and lower legs. Serologic abnormalities included hypocomplementemia, high ANA & anti DNA titers, and LE cell test was positive. The histologic finding of the lesion showed leukocytoclastic vasculitis and direct immunofluorescent test revealed linear deposit of IgM at the dermoepiderrnal junction. Electron microscopy showed glomerular subepithelial depasition of eletron dense material in the kidney.

2.Ultrastructural Changes of Hair Follicles Induced by Cyclophosphamide in the Rat.

Authors studied the effects of cyciophosphamide, a potent inhibitor of nucleoprotein synthesis, to ivestigate the morphologic evidence of destructive actions to the hair. follicles. Sprague-Dawley rats were received 4 mg/kg of eyclophosphamide for 1 to 6 weeks, intraperitoneally, and examined light and electron microscopically. Light microscopically, distortion and constriction of the hair shafts, diminished diameter of the hair bulbs, and atrophy of. the hair matri.x were developed from 1 week. which were more prominent in second weeks and they were progressed after that time. Hairs were frequently fractured due to constriction of the hair shaf ts. Electron microscopically, cells of the hair pulp were decreased in number, and cells of the hair matrix were atrophied, Degenerative changes of the cellular organelles participating in grovth. and development of the hairs were noted, such as disordered formation of tricholyaline granules, diffuse atrophy and increased electron density of the inner root sheath, and loss of the glycogen and intercellular edema of the outer root sheath, but basal cells of the matrix showed minor changes relatively. From the above results, cyclophosphamide may specifically alter the matrix cells of the hair follicles and induces anagen hair losses, which may be reversible at a small dosage when the drug is discontinued, because basal cells of the matrix ahow rninor changes.

3.A Case of Syringoma Limited to the Vulva.

Vulva syringomas are rare in reviewing literature. In our case, a 29-year-old had multiple papular lesions involving both sides of the vulva. Microscopically the lesion showed dilated cystic sweat ducts some of which have small comma like tails resembling tadpoles. It is the purpose of this report to present a rare case of syringoma which was limited to the vulva without involvement of other sites.

5.Emotional Dysregulation, Attributional Bias, Neurocognitive Impairment in Individuals at Ultra-High Risk for Psychosis and with Schizophrenia : Its Association with Paranoia.

OBJECTIVES: Paranoia is a complex phenomenon, affected by a number of factors such as depression, trait anxiety, and attributional bias in schizophrenia. The aim of this study was to explore whether paranoia within continuum of clinical and subclinical states is associated with emotional dysregulation, attributional bias and neurocognitive impairment in whole individuals of normal controls, ultra-high risk (UHR) for psychosis and schizophrenia. METHODS: 101 normal controls, 50 participants at UHR for psychosis, and 49 schizophrenia patients were recruited. All subjects were asked to complete self-reported paranoia scale and emotional dysregulation scales including Rosenberg's self-esteem, Spielberg's state-trait anxiety inventory and Beck depression inventory. The attributional style was assessed by Ambiguous Intentions Hostility Questionnaire (AIHQ). Participants were also requested to complete the comprehensive neurocognitive battery. RESULTS: Multiple linear regression analysis showed that paranoia were found to be associated with emotional dysregulation (state anxiety, trait anxiety and depression), composite blaming bias in ambiguous situation, impairment of attention and working memory in whole participants [F (9, 190)=34.85, p<0.001, adjusted R2=0.61]. CONCLUSION: The main findings suggest that paranoia is a complex affective and cognitive structure that may be associated with emotional dysregulation, blaming bias and attention and working memory impairment in clinical and non-clinical paranoia.

7.Corneal Perforation Caused by Noninfectious Corneal Ulcer in a Patient with Toxic Epidermal Necrolysis.

PURPOSE: To report a case of corneal perforation caused by non-infectious corneal ulcer in a patient with toxic epidermal necrolysis (TEN). METHODS: A 29-year-old female visited the hospital with bullous lesions on the erythematous body surface. She also had severe conjunctivitis and keratitis in both eyes. She had been taken antibiotics and analgesics for nasopharyngitis. The patient was diagnosed with TEN. Although she was hospitalized and treated in the department of internal medicine, the left cornea was severely necrotized and became thinner. Slit-lamp examination showed focal descematocele in the central cornea. The result of corneal culture was negative and corneal infiltration that would suggest an infectious ulcer was not observed. RESULTS: The patient was in poor general health because of pneumonia. We performed tectonic amniotic membrane transplantation to prevent corneal perforation. Fibrovascular tissues were growing on the amniotic membrane from the surrounding conjunctiva. However, they did not cover the descemetocele area. Her general condition continued to worsen and she was transferred to the intensive care unit. The cornea was perforated and the patient died of septic shock. CONCLUSIONS: Corneal perforation may occur in a patient with TEN without secondary bacterial infection. Therefore, patients with TEN need to be informed of this serious complication and the possible need for more intensive treatment.

8.Impact of Work Environment and Organizational Justice on Job Satisfaction among General Hospital Nurses

Purpose:
This study investigated the impact of nurse practice environment and organizational justice on nurses’job satisfaction.
Methods:
We identified the factors between nursing work environment and organizational justice to job satisfaction for 189 nurses working at a general hospital in city C. Data were collected from June 1st to 15th, 2023, and analyzed using descriptive statistics, t-test, ANOVA, Pearson’s correlation coefficients, and multiple stepwise regression, using IBM/SPSS 27.0 for the Windows program.
Results:
The mean job satisfaction was 3.24±0.55 points on a 5-point scale. Multiple stepwise regression revealed that the factors affecting nurses’job satisfaction included nursing foundations for quality of care (β=.26, p=.005), staffing and resource adequacy (β=.40, p<.001), collegial nurse-physician relations (β=-.24, p=.007), and distributive justice(β =.27, p<.001).These variables explained 55.0% of job satisfaction.
Conclusion
The research findings indicate that higher job satisfaction is associated with a better nurse practice environment and positive perceptions of organization justice. These findings indicate that it is necessary to enhance the nurse practice environment and improve organizational justice to enhance job satisfaction among nurses.
9.Job Competencies and Educational Needs Perceived by New Community Health Practitioners.

PURPOSE: This study was performed to identify job competencies and needs for job education perceived by new community health practitioners. METHODS: This study used a qualitative research design. Eight new community health practitioners participated in this study. Data were collected through in-depth interviews and analyzed using content analysis. RESULTS: Five job competencies were identified in this study, including primary care, public health management, interpersonal relationship, teaching and counseling, and leadership. The contents of job education that they needed were management of major symptoms and chronic diseases, understanding and prescription of medications, emergency responses and care, management of endemic diseases, planning and management of public health programs, writing official documentsand computer works, and leadership training. The learning methods they preferred were connecting theory and practice, situation- or case-based learning, skill- or practice-based learning, and increased opportunities of clinical practice. CONCLUSION: The findings of this study provided the direction of job education for new community health practitioners. Job education for new community health practitioners needs to consider the job competencies and educational needs identified in this study.

10.Comparison of the Effects of Prophylactic Nonsteroidal Anti-inflammatory Drugs on Macular Edema After Cataract Surgery.

PURPOSE: To evaluate and compare the efficacy of prophylactic administration of topical nonsteroidal anti-inflammatory (NSAID) drugs on macular edema (ME) following cataract surgery. METHODS: The present open-label non-masked randomized (random number assignment) study was comprised of 90 eyes. Thirty eyes were administered Diclofenac sodium 0.1%, and 30 eyes were administered Pranoprofen 0.1% starting three days before surgery and continued for one month after surgery. The remaining 30 eyes made up the control group. The results consisted of the measurements of foveal thickness (FT), macular thickness (MT), and total macular volume (TMV), which were quantified by optical coherence tomography (OCT, Carl Zeiss Meditec) before and one month after phacoemulsification surgery. RESULTS: At one month, there were statistically significant differences in FT in the Diclofenac group (0.48+/-10.9 micrometer) and the Pranoprofen group (1.87+/-14.7 micrometer) (p<0.05) compared with the control group (11.65+/-18.6 micrometer). In addition, there was a statistically significant difference in MT between the control group (71.06+/-90.8 micrometer) and the Diclofenac group (15.19+/-36.1 micrometer) (p<0.05). However, no statistical difference in MT was noted between the control group and the Pranoprofen group (27.57+/-70.93 micrometer). Between the control group and the NSAID groups, only the Diclofenac group showed statistical differences in TMV throughout the observational period. CONCLUSIONS: Used prophylactically after cataract surgery, the NSAIDS eye drops were effective in reducing postoperative ME.
