3.A Case of Risperidone-induced Neuroleptic Malignant Syndrome.

Neuroleptic malignant syndrome(NMS) is an uncommon but potentially fatal idiosyncratic reaction to neuroleptics, characterized by muscular rigidity, fever, autonomic dysfunction, and altered consciousness. The major theories to explain NMS is central dopaminergic blockade, but it is unclear. Risperidone is a new antipsychotic drug, a benzisoxazole derivative that blocks dopamine D2 receptor and serotonin type 2 receptor. The comparatively greater serotonin-blocking activity is believed to give risperidone the specific property of not causing any more extrapyramidal side effects than conventional antipsychotics at the optimal dose of 4-8mg/day. It is postulated that risperidone is unlikely to cause NMS. Here, we report a case of risperidone induced neuroleptic malignant syndrome.

4.Vascularized Bone Graft for Spine and Limb Reconstruction

We analysed the thirty three cases of the free vascularized bone graft performed in the Department of Orthopaedic Surgery, Yonsei University College of Medicine from February 1982 to December 1989 and would like to report the results as follows. 1. There were 27 cases in men and 6 cases in women and the mean age was 27.3 years. 2. We have performed the procedure for the reconstruction of the large bony defects developed after massive trauma in 13 cases, core decompression of the avascular necrosis of the femoral head in 8 cases, tumoral resection in 6 cases, treatment of congenital pseudoarthrosis in 3 cases, and bone infection in 3 cases. 3. The most common recipient site was tibial shaft (18 cases, 54%) and the most common donor site was fibular shaft (23 cases, 69%). 4. The average length of bone defect (or grafted bone) in 23 cases of vascularized fibular graft was 9.8 cm and in 9 cases of vascularized osteocutaneous iliac bone graft, it was 5.8 cm. 5. The follow-up period was ranged from 13 months to 5 years 8 months (mean, 2 years 2 months). 6. All patients were evaluated clinically and roentgenographycally on the basis of fuctional recovery and bony union. Of the 33 cases, 31 cases (93%) showed satisfactory functional recovery with sound bony union. 7. It is highly expected that the vascularized bone graft is a one of the better alternatives for the reconstruction of the large bone defect and prevention of the early state of the avascular necrosis of the femoral head from progession.

5.Effect of Vegetable Juice Supplementation on Serum Lipid Profile and Antioxidant Activity in College Women.

The aim of this study was to assess the effect of 6-week vegetable juice supplementation (360 ml/day) on serum lipid profiles and antioxidant activity in college women. Twenty women (mean age: 21) with normal life style and dietary pattern and who are free of any specific diseases were recruited among the student in S women's university. The subjects consumed vegetable juice to take part in an uncontrolled clinical trial for a 6-week intervention period. While there was no difference in the concentration of total cholesterol and HDL-cholesterol, serum concentrations of TG and HDL/LDL ratio were significantly reduced by 22% (p = 0.013) and 6% (p = 0.007) respectively. Significant decrease in malondialdehyde (p = 0.000) was accompanied by an increase in the activity of serum antioxidant enzymes, such as GSHPx (p = 0.000), SOD (p = 0.007). It was also found that total antioxidant status was improved by 5.4% (p = 0.009). Serum parameters were all changed without affecting body mass index, waist and hip circumferences, or nutrient intakes after six week of supplementation. This study demonstrated that the supplementation regular meals with vegetable juice can favorably affect serum lipid profiles and antioxidant systems, and hence could contribute to reduce the risks of chronic diseases in college women.

7.A Comparative Study Examining the Obesity Indices, Psychological Well-being Index Scores, and Nutrient Intakes of Daughters According to Their Mother's Degree of Obesity.

The purpose of this study was to understand the effects a mother's degree of obesity has on her daughter's obesity index, psychological well-being index, and nutrient intakes. The daughters, as study subjects, were classified into two groups according to their mother's body mass index (BMI): an obese group [BMI>25 kg/m2, mother obesity group (MOG)] and a normal weight group [BMI<25 kg/m2, mother normal weight group (MNG)]. The daughters in MOG showed significantly higher percent ideal body weight values than the daughters in MNG (p<0.001). MOG and MNG had significantly different psychological well-being index scores in terms of being classified into a highly stressed group (p<0.001), potentially stressed group (p<0.05), and healthy group (p<0.001). Intakes of daily energy, protein, fat, thiamin, riboflavin, niacin, phosphorus, and iron were also significantly different (p<0.001) between MOG and MNG. In particular, the vitamin A and C intakes of the MNG daughters were significantly higher than those of the MOG daughters (p<0.001). Overall, the results showed that a mother's level of obesity was essentially related to her daughter's obesity index, psychological well-being index score, and nutrient intakes. Therefore, it seems necessary for mothers to maintain an ideal body weight to prevent obesity and diet related problems in their daughters.

9.Result of Intraarticular Fracture of the Calcaneus by Open Reduction

The Fracture of the os calcis is quite common in relation to fracture in other tarsal bone, and fractures involving the subtalar joint may cause serious and persistant disabilities. The treatment of these calcaneal fractures remains controversial So there is no universal agreement in the treatment of these fractures, especially involving the subtalar joint. Twenty displaced intraarticular fractures of calcaneus in 15 patients were treated by open reduction and internal fixation with a plate and screws and bone graft through a lateral approach to elevate the depressed lateral portion of the posterior facet at the Inchon Chritian Hospital from Jan., 1990 to Dec, 1991. Twenty feet in 15 patients of calcaneal fractures involving the subtalal joint were treated and were clinically analysed and following result were obtained. 1. The 20 cases were classified according to the Sanders classification, 9 in type II, 9 in type III and 2 in type IV. 2. The 20 cases were treated by open reduction andinteranl fixation with plate and screws and iliac bone graft through an Sanders lateral approach. The bone graft is packed under the elevated posterior to prevent late collapse of the central fragment and to supply structral stability. 3. The preoperative average Böhler's angle was 0.4°. The post operative average Böhler's angle was 23.4° which was increased significantly. 4. Based on assessment of result of Salama, excellenwas 6, good was 7, fair was 4 and poor was 3. 5. The most common complication after treatment was persistant pain, especially beneath the lateral malleolus.

10.Analyses of Dystrophin Gene and Sex Determination using PEP-PCR in Single Fetal Cells.

Recently, through the development of the primer extension preamplification(PEP) method which amplifies the whole genome, simultaneous multiple DNA analysis has become possible. Whole genome from each single cell can be amplified using 15 base oligonucleotide random primer. The greatest advantage of PEP-PCR is the ability to investigate several loci simultaneously and confirm results by analysing multiple aliquots for each locus. This technique led to the development of preimplantation genetic disease diagnosis using blastomere from early embryo, sperm, polar body and oocyte. In this study, we applied PEP-PCR in 20 cases of single amniocyte and 20 cases of single chorionic villus cell for the clinical application of the prenatal and preimplantational genetic diagnosis. We analysed 7 gene loci simultaneously which are 46, 47 exons related to dystrophin gene, two VNTR (variable number tandem repeat) markers using 5'toysIII, 3'CA related to dystrophin gene and DYZ1, DYZ3, DYS14 regions on chromosome Y. In all the tests, 97.5% of PEP-PCR amplifications with single cells were successful. We obtained 38/40 (95%) accuracy in gender determination through chromosome analysis comparison. Therefore, these results have significant implications for a sperm or oocyte analysis and prenatal or preimplantational genetic diagnosis.
