2.Clinical Study of Posterior Chamber Lens Implant in 200 Eyes.

We studied 193 patients(200 eyes) who underwent extracapsular cataract extraction with posterior chamber lens implantation from September 1983 to May 1986. We compared the results of planned extracapsular cataract extraction(ECCE) group with those of Kelmen phacoemulsification(KPE) group. The results as follows; 1) Surgical technique is planned extracapsular cataract extraction in 98 eyes, Kelman phacoemulsification in 97 eyes, secondary implant in 2 eyes and triple procedure for glaucoma in 3 eyes. 2) Postoperative visual acuity of 0.5 or better was achieved in 190 eyes(95%) and 1.0 or better in 147 eyes(73.5%). Average of visual acuity was 0.86 +/- 0.26 in planned ECCE, 0.96 +/- 0.19 in KPE and 0.91 +/- 0.23 in toto. There was significant correlation between two(p<0.01). 3) The mean spherical equivalent of postoperative corrective spectacle lens power was -0.97 +/- 1.0 in planned ECCE, -0.95 +/- 0.95 in KPE and -0.96 +/- 0.97 in toto. Postoperative refractive state was myopic in 162 persons(84%), emmetropic in 13 persons(7%) and hyperopic in 18 persons(9%). 4) Error of predicted required spectacle lens power was -0.21D +/- 0.95 in planned ECCE, -0.12D +/- 1.0 in KPE, -0.16D +/- 0.97 in toto. With the use of the standard formula described by Hoffer, our results for the accuracy of intraocular lens calculation was 77% +/- 1.0D / 92.7% +/- 2.0D / +2.77 to -2.61D. 5) Of 200 eyes, 162 eyes required correction of astigmatism with average of -0.93D +/- 1.12 in planned ECCE, -0.57D +/- 0.76 in KPE and -0.76D +/- 0.98 in toto. There was significant correlation between two(p<0.05). 6) Mean corneal refractive power changes was -0.14D +/- 0.47 in planned ECCE, 0.05D +/- 0.49 in KPE and 0.02D +/- 0.48 in toto. There was no significant correlation between two. 7) Correlation efficiency between postoperative corneal astigmatism and actual astigmatism by skiascopy was r=0.6874(p<0.01) in planned ECCE, r=0.1833 in KPE and r=0.5747(P<0.01) in toto.

3.Intraoperative Normovolemic Hemodilution in Patients Undergonig Posterolateral Spinal Fusion: Safety and Efficacy.

BACKGROUND: The implementation of intraoperative normovolemic hemodilution is a strategy used in an attempt to diminish the need for or obviate allogeneic transfusion and to avert the potential complications. The goal of this study was to evaluate the safety and efficacy of moderate intraoperative normovolemic hemodilution. METHODS: Fifteen patients scheduled for posterolateral spinal fusion underwent intraoperative normovolemic hemodilution with 10% pentastarch to a target hematocrit level of 25% (hemodilution group). All units of blood procured by hemodilution and additional allogeneic blood was transfused in the perioperative period to maintain the hematocrit level of>25%. We investigated the effect of hemodilution on whole blood coagulation as measured by the thromboelastography and, evaluated its efficacy of decreasing the need for allogeneic blood transfusion as compared to the nonhemodilution group, retrospectively. RESULTS: Hemodilution with pentastarch caused a decrease in response and coagulation time (p<0.01) but did not influence on the alpha angle and maximum amplitude on thromboelstogram. Amount of transfusion of allogeneic blood was 4.1 1.7 units for the hemodilution group and 5.0 1.5 units for the nonhemodilution group. Net red blood cell volume ""saved"" from hemodilution was about 120 ml. CONCLUSION: Moderate intraoperative normovolemic hemodilution with pentastarch does not affect the coagulability of whole blood. But its efficacy of decreasing the need for allogeneic blood transfusion is minimal.

5.A Case of Benign Mixed Tumor of the Lacrimal Gland.

A 19-year-old man visited out-patient department of the Ophthalmology in our hospital due to unilateral proptosis connected with excessive tearing and moderate swelling of the eyelid in the right eye, 2 years in duration. CT scan demonstrated a relatively well demarcated, single mass measuring 2.5 X 2.5cm in the posterotemporal portion of the right orbit, which had strongly enhanced, hyper dense, internal consistency. We underwent lateral orbitotomy for total removal of this mass under general anesthesia and histopathological diagnosis was made as a pleomorphic adenoma of the lacrimal gland. This patient is satisfied with good cosmetic and clinical result.

7.A Case of Lacrimal Monomorphic Adenoma.

The authors experienced a case of the monomorphic adenoma of lacrimal gland. Monomorphic adenoma is an extremely rare salivary gland tumor. A fifty eight year old Korean female was admitted to our Dept. of Ophthalmology because of multiple small nodular masses on her right upper eyelid and a large hard solitary mass in the supraorbital area since one year ago. On B-scan ultrasonogtaphy, round echo-free zone was detected in the superiortemporal orbit. Well defined hyperdense, enhanced round extraconal mass in the superiororbital area was detected by orbital CT scan. Two large well encapsulated, dark brown colored masses were extracted by supraorbital approach and lateral orbitotomy under general anesthesia. After removal of the masses, histopathologic examination confirmed the lacrimal monomorphic adenoma.

8.Two Cases of Bacteremia Caused by Leuconostoc citreum.

Two strains of vancomycin-resistant Leuconostoc species were isolated from blood cultures in two compromised patients. The isolates produced gas from Lactobacillus MRS booth, hydrolysed esculin, and produced no ammonia from arginine, thus fulfilling the major criteria as Leucorostoc spp. We recommend that clinical laboratories should perform susceptibility test to vancomycin for clinical isolates (especially from cerebrospinal fluid and blood) which resemble streptococci so as not to confuse Leuconostoc spp. with more commonly isolated pathogens such as streptococci.

9.The Smoking Status and Its Associated Factors of Some Army Soldiers.

OBJECTIVE: This study was performed to determine the smoking status and its associated factors in military life among young Korean men to provide date for smoking prevention in military areas as a part of the health promotion program for soldiers. METHODS: 1,033 infantrymen serving in 17 military units, which are located in suburbs of K city, were enrolled in this study. A questionnaire on the soldiers smoking status and other various characteristics was obtained from October 2nd to 12th, 2000. Logistic regression analysis was performed on the various variables in order to determine the factors related to smoking. RESULTS: 1) 830(80.3%) out of 1033 subjects had smoked cigarettes and 759 persons(73.4%) were regular smokers. 2) Among the 734 soldiers who started smoking before their military service, 714 soldiers(97.2%) continued to smoke after, while 45 soldiers(15.0%) among the 299 non-smokers started smoking after joining the army. 3) Through the logistic regression analysis, the education level, specialty in the army, smoking amongst the friends they met before military service, smoking amongst soldiers in the army, dating girl friends, drinking, satisfaction with their army lives and the presence of convenient smoking places at the barracks were significant factors related to the prevalence of smoking in soldiers. CONCLUSION: the factors related with incidence of smoking in the army are the complicated interactions among the individuals personal background, general charcterisitcs of the solider and the special environment in the barracks. Therefore, to improve national soldiers' health, an active non-smoking campaign and education should be pushed to discourage smoking in the barracks.

10.Virtual Gastroscopy Using Spiral CT in Gastric Lesions.

PURPOSE: To compare virtual gastroscopy using spiral CT with conventional endoscopy for the detection andevaluation of gastric lesions. MATERIALS AND METHODS: During a previous six-month period, 30 patients withpathologically-proven gastric lesions underwent conventional endoscopy and virtual gastroscopy using spiral CT.There were 18 cases of advanced gastric carcinoma, eight benign ulcers, and four submucosal tumors(two leiomyomas,two lymphomas). Source images of virtual gastroscopy were three-dimensionally reconstructed within an AdvantageWindows Workstation and virtual gastroscopy images were obtained using Navigator Software. On analysis, imageswere graded according to their quality (excellent, good, poor). Virtual gastroscopy images were interpreted by tworadiologists blinded to conventional endoscopic findings, and were subsequently compared with endoscopic findingsin terms of detectability and findings. In the cases of advanced gastric carcinoma, lesions were classifiedaccording to Borrmann's system. RESULTS: For virtual gastroscopy, overall image quality was excellent in 21cases(70%), good in five(17%), and poor in four(13%). Lesions were detected in 25 cases(83%). Among the 18advanced gastric carcinomas, virtual gastroscopy image quality was excellent in 14 cases(78%), good in two(11%),and poor in two(11%). Lesions were detected in 16 cases(89%). Two Borrmann type IV cases were not detected. Amongthe eight benign ulcers, virtual gastroscopy image quality was excellent in three cases(38%), good in three(38%),and poor in two(25%). The detection of lesion was possible in five cases(63%). In all submucosal tumors, virtualgastroscopy image quality was excellent. Lesions were detected in all cases. CONCLUSION: Virtual gastroscopyusing spiral CT is safe and noninvasive, and for the evaluation of gastric lesions may be complementary to axialCT. It successfully detects gastric lesions, and in depicting the pattern of gastric folds its image quality isexcellent.
