1.Morphologic Characterization of Polycystic Kidney in inv Transgenic Mouse.

The aim of this study was to characterize the morphology of a polycystic kidney which was found in 100% of the transgenic mice homozygous for inv mutation and to gain insight into the pathogenesis of inherited polycystic kidney disease during the pre- and postnatal periods. The fetal and postnatal kidneys from the homozygous and heterozygous transgenic mice were examined by the light, transmission and scanning electron microscopes, image analyzer, and an immunohistochemistry utilizing the antibodies specific for each segment of the renal tubules (Tetragonolobus purpureas, Arachis hypogaea, Tamm-Horsfall protein, AE1/AE3, EMA, vimentin, Phaseolus vulgaris) was performed to determine the site of origin of renal cysts. Two developmental phases of a cystic disease were identified. The first phase, seen in fetal kidneys, was characterized by dilatation mainly of the proximal tubules and a few distal tubules. The later phase, in postnatal period, was characterized by progressive enlargement of the kidneys due to mainly cystic change of the collecting ducts, which distorted the normal architecture of both cortex and medulla and almost completely replaced the renal parenchyma. The cystic dilatation involved all segments of the nephron and the collecting duct as well as the Bowman's spaces of glomeruli. The epithelial cell hyperplasia was found as a micropolyp formation within the renal cysts and an increase in PCNA positive cells. These findings suggest that a cyst is not simply a ballooning of a renal tubule and the stretching of cells, formerly thought to be due to an altered compliance of an abnormal basement membrane, but indeed the result of increased numbers of tubular epithelial cells.

2.A Study on the Self-Management and Physiological Measurements of Patients on Hemodialysis according to Their Age

PURPOSE: The study was conducted to investigate correlations between self-management and physiological measurements of patients on hemodialysis according to their age. METHODS: The subjects were 120 patients on hemodialysis at three hospitals in S and G cities. Data were collected using a structured questionnaire and medical records. The collected data were analyzed using descriptive statistics including t-test, ANOVA, and Pearson's correlation analysis with the SPSS 23.0 program. RESULTS: The levels of compliance with self-management were highest in the middle-aged group. There was no significant difference in physiological measurements between the different age groups. CONCLUSION: The results suggest that nurses should consider characteristics of different age groups to improve the levels of self-management and physiological measurements.

3.Chondroblastoma-like Extraskeletal Chondroma: A case report.

Extraskeletal chondromas are relatively uncommon benign cartilaginous tumors of the soft tissue and well known to pose a considerable diagnostic problem because of histological variations including the immature appearance of their tumor cells. Recently, we have experienced a case of extraskeletal chondroma mimicking benign chondroblastoma. The patient was a 47-year-old woman who complained of a painful subcutaneous swelling on the radial aspect of 4th proximal interphalangeal (PIP) joint in the left hand for 6 months. Radiologic examination of the 4th finger revealed a 1cm-sized soft tissue mass. Histologically, the tumor was characterized by a lobulated mass which was composed of dense proliferation of chondroblast-like cells admixed with a few multinucleated giant cells of osteoclastic type. However, there were focal areas of typical chondroma which showed lace-like intense calcification around the differentiated chondrocytes.

4.Ultrasound findings of subpial hemorrhage in neonates

Purpose:
Subpial hemorrhage (SPH) is a subtype of intracranial hemorrhage characterized by damage to the adjacent brain parenchyma. The aim of this study was to describe the sonographic features of SPH in neonates.
Methods:
The cranial ultrasound (US) findings of neonates with SPH confirmed by brain magnetic resonance imaging (MRI) were analyzed retrospectively. Initial and follow-up US and MRI scans were reviewed by two pediatric radiologists who were blinded to both clinical history and outcomes. The US features were compared with the MRI findings.
Results:
Sixteen patients were included (median gestational age, 38 weeks; range, 26 to 40 weeks; 69% term). SPH was detected most often in the temporal lobe (63%), and multiple SPHs were found in seven of 16 neonates, based on MRI. Acute SPH with an underlying venous infarct (UVI) was detected on US in 15 of 16 patients: small or large fan-shaped hyperechoic lesions (n=7 and 4, respectively) and gyriform hyperechoic lesions (n=4). The sonographic yin-yang sign was observed in three of the four large fan-shaped SPH cases. The accompanying findings on US were intraventricular hemorrhage (four out of six MRI-confirmed cases), and concurrent periventricular venous infarcts (five out of nine MRI-confirmed cases). In five patients, subpial cysts were observed on follow-up US or MRI (n=4 and n=4, respectively).
Conclusion
Acute SPH with UVI can appear as a peripheral fan-shaped or gyriform hyperechoic lesion on cranial US. SPH can be detected and suspected based on the US features of SPH with the accompanying findings.
5.Two Pediatric Cases of Dengue Fever Imported from Philippines.

Dengue fever is an important health problem for international travelers to all endemic areas. The steadily increasing numbers of tourists visiting endemic areas raise the risk of exposure, and imported dengue cases are increasingly observed in nonendemic area. Dengue has a wide spectrum of clinical presentations, often with unpredictable clinical evolution and outcome. While most patients recover following a self-limiting, non-severe clinical course, a small proportion progress to severe disease such as dengue hemorrhagic fever or dengue shock syndrome. Therefore, it is important to suspect dengue fever in every febrile patient returning from the tropics. Whenever it is suspected, a quick diagnosis and adequate managements are essential to avoid complications. We report two cases of imported dengue fever in Korean children presenting with fever, headache, nausea, and rash.

6.Lupus Erythematosus Profundus:Clinical and Histopathological Study.

BACKGROUND: Lupus erythematosus profundus (LEP) is an unusual clinical variant of lupus erythematosus (LE). It is unclear which part LEP occupied in the disease spectrum of LE. OBJECTIVE: Clinical and histopathological studies were performed on 19 patients with LEP in order to further define the clinical patterns, know the various serological findings, and review the histopathological features. METHODS: A retrospective review was carried out of the clinical records and histopathological specimens of 19 patients with LEP. RESULTS: The most common clinical features were indurated nodules or plaques on the cheek. There was a 37% positivity in the ANA test. Histopathogically epidermal changes as well as subcutaneous involvements were common. There were no cases of newly developed SLE during the follow up period of 41 months. CONCLUSION: Most patients with LEP have a relatively benign disease course, although a few develop systemic abnormalities and have abnormal laboratory findings.

9.Effects of the Instrument Pilates Exercise Based on the Schroth Exercise on the Cobb’s Angle, Angle of Trunk Rotation and Low Back Pain in Patients with Idiopathic Scoliosis: A Single Subject Study

Purpose:
This study examined the effects of the application of Schroth exercise-based instrument Pilates exercise on the Cobb’s angle, angle of trunk rotation, and low back pain of female patients with idiopathic scoliosis.
Methods:
Three patients with idiopathic scoliosis at a Cobb’s angle of 20° or more participated in this study. Among the single-subject experimental research designs, a reversal (ABA) design was performed. In particular, Schroth exercise-based instrument, Pilates exercise, was performed for 10 weeks, consisting of five weeks between the baseline and intervention one period and five weeks between the intervention 1 and intervention 2 periods, and then followed up for five weeks.
Results:
After the Schroth exercise-based instrument, Pilates exercise, was applied, the Cobb’s angle and the angle of trunk rotation decreased compared to the baseline in all subjects, and the follow-up period also showed a continuous decline. After Pilates exercise was performed, low back pain in subjects 1 and 2 was decreased in the intervention 1 period compared to the baseline. The level of low back pain in the intervention 2 period increased compared to the intervention 1 period, but a reduction was noted in the follow-up period. The low back pain in the subject was decreased in all intervention periods and the follow-up period.
Conclusion
Schroth exercise-based Pilates exercise improves the Cobb’s angle and the angle of trunk rotation for female patients with idiopathic scoliosis in their teens and 20s, and an effective intervention method is proposed for low back pain.
10.Gender Difference in Association with Socioeconomic Status and Incidence of Metabolic Syndrome in Korean Adults

BACKGROUND: The prevalence of metabolic syndrome in Korean is gradually increasing. Metabolic syndrome shows different patterns depending on gender and socioeconomic status. We investigated the differences in incidence of metabolic syndrome according to gender and socioeconomic status using a community cohort of middle-aged Korean. METHODS: The study included 5,544 subjects (2,733 men and 2,811 women) aged 40-69 years without metabolic syndrome, diabetes, or history of cardiovascular disease at baseline. The incidence of metabolic syndrome was estimated according to gender and socioeconomic status, and the association between socioeconomic status and incidence of metabolic syndrome was examined using Cox's proportional hazard regression analysis. RESULTS: The incidence density of metabolic syndrome was 53/1,000 person-years in men and 52/1,000 person-years in women during the mean follow-up period of 6.01 years. After adjusting for common risk factors, the hazard ratios (HRs) for metabolic syndrome for those with high, middle, and elementary school education levels compared to the college graduate or higher group (referent) were 1.07 (95% CI, 0.76-1.51), 1.73 (95% CI, 1.23-2.44), and 1.74 (95% CI, 1.23-2.46) in women, respectively, and compared to the more than 400 (10,000 won) group (referent), the HRs for metabolic syndrome according to income level were 1.56 (95% CI, 1.06-2.29), 1.86 (95% CI, 1.28-2.71), and 2.74 (95% CI, 1.87-4.00) in women, respectively. However, the association between socioeconomic status and incidence of metabolic syndrome was not significant in men. CONCLUSION: The association between incident metabolic syndrome and socioeconomic status was significant in middle-aged Korean women.
