Rearing is important to the growth and development of the child. Traditionally, nurturing is a maternal role which is expressive role. Maternal role was composed of mothering and maternicity. Rearing is a key concept of psychology, education and sociology discipline. Knowledge of the rearing is an essential component for the development of nursing as a science and a profession. The frist thing to study a concept is thought to be a concept analysis. So, in this study, concept analysis of the rearing was performed to clarify a concept of the rearing as a basis for the study of rearing afterward. The approach used for the concept analysis was the approach presented by Walker and Avant(1993). The defining attributes of rearing, identified in this study were (1) a series of caring activities in parent-child relation, (2) an essential of the growth and development of a child, (3) changeable according to time, place and object, (4) effort is necessary to the rearing, (5) positive or negative aspects (6) attitude, behavior, environment is sub-concepts of the rearing concept. The identified antecedents of the rearing was child-birth. The identified consequences of rearing were desirable or undesirable outcomes. Desirable outcome was the emotional satisfaction and growth in parent-child relation. Undesirable outcome was the emotional disorder and social maladaptation of the child. The empirical referents of the rearing could be the caring phenomena in parent-child relation.

The activity and growth of the accessory male sex gland depend on the androgen. The sex hormones effect on the growth of seminal vesicle and the prostate differently, but the relationship of the age and hormonal effect is not clear. To clarify the ageing effect on the male sex accessory gland, the morphometrical study has been done and the results analyzed with hormonal effect on the gland structure. Twenty-five seminal vesicle and prostate on bloc autopsy specimens[aged 22-68] were used. Middle portion of each side of the seminal vesicle and all blocks of the prostate horizontally divided at 3-5mm were embbeded in paraffin. Serial 7microm thick sections were prepared with hematoxiline and eosin, Gomori trichrome staining. For quantification, the image analyzer and the point counting method were used. There was not noticable change in the ratio of the parenchyma and the fibromuscular stroma in prostate until age of 60. At age of 20s, the actual ratio of the central, peripheral and transitional zone was accounted to 25%, 70%, 5% respectively, and it changed to 25%, 62%, 13% at 50`s. The percentage of the epithelial component in the glandual tissue was higher in the seminal vesicle. In the prostate, that was highest in the central zone. Stromal component was most abundant in the transitional zone. In both glands, the epithelial percentage decreased while the stromal percentage increased with age. The changing rate of the epitheilal and stromal compoenent was highest in the central zone. The luminal component changed only in the peripheral zone. The stromal component was most abundent in the transitional zone at all ages, even the increasing rate was highest in the central zone. The change in the proportion of each prostate zone and the different pattern of tissue compositional change in seminal vesicle and each prostate zone according to age represent the difference of hormonal and ageing effects on the seminal vesicle and each prostate zone.

A morphometric study on the trigeminal ganglion and the intracranial portion of the ophthalmic, maxillary and mandibular nerves was done in 50 Korean adult cadavers. The shape of the ganglion was classified into 7 types and semicircular shape was most common (54%). Mean width of the trigeminal root at porus trieminus was 6.08mm. The length of the trigemnal root from porus to the trigeminal ganglion was 8.28mm at ophthalmic portion, 10.16mm at maxillary portion, and 8.90mm at mendibular portion. Maximum width of the ganglion was 15.48mm and the breadth of the ophthalmic, and maxillary portion were 5.25mm and 5.26mm, respectively, and that of the mandibular portion (4.92mm) was narrow than the other portion. Intracranial ophthalmic nerve was 4.30mm of width and 17.89mm of length, and this nerve inclined average 39.7。 (range 14.56。) from sagital plane. It was observed infrequently that the cavernous sinus extended to the lateral border of the maxillary nervw and the emissary sinus ran under the mandibular nerve.

The composition and the structure of the sacrum present the sexual and the racial difference. The variation of the structure is meaningful in the administration of caudal anesthesia which used one of the safest and the most valuable means of local anesthesia. In this study, a morphological investigation of the sacrum in Korean adults was done using 145 dry sacral bones. Non-metrical morphological variations were observed and ten measurements were undertaken. The results of the study were as follows. 1. The five segmented sacrum appeared most commonly (62.1%). Six segmented sacrum appeared 37.2% and the incidence tended to increse according to the age. Four segmented sacrum was noted in one case (0.7%). 2. In 9.7%, deficiences existed in the upper dorsal wall of the sacral canal. In 5.5%, deficiences existed in the middle of the dorsal wall of the sacral canal. 3. The apex of the sacral hiatus was found mostly (28.9%) at the upper third of the body of fifth sacral vertebra. 4. The sacral hiatus was classified into five groups by shape with triangular being the most frequent. 5. There was a significant sexual difference in the antero-posterior and transverse diameters of the first sacral vertebral body, the length of the sacrum, and the length of the sacral hiatus. 6. The average sacral index I was 98.2 in male, and 104.4 in female. Sacral index I was the relationship between the greaster breath to the length of the sacrum. 7. The average sacral index II was 46.6 in male, and 44.8 in female. Sacral index II was the relationship between the first sacral body diameter to the greaster breath of the sacrum. A thorough knowledge of the anatomical features of the dorsum of the sacrum including above results will lead to the decrease in the number of failures in administration of caudal anesthesia.

Acute cerebellitis is a rare inflammatory syndrome and is one of the important causes of acute cerebellar dysfunction in children. The cerebrospinal fluid (CSF) IgG index is most often tested clinically in the diagnosis of multiple sclerosis. However, it is not specific to multiple sclerosis, and can be elevated in a variety of neurologic diseases. A 7-year-old boy with acute cerebellitis presenting with sudden onset headache and subtle cerebellar dysfunction demonstrated an elevated CSF IgG index (1.1) and an absence of oligoclonal bands. On the seventeenth day, the follow-up CSF IgG index was 0.71. Two-month and one-year follow-up magnetic resonance imaging revealed cerebellar atrophy, although the patient showed no neurologic deficit. To the best of our knowledge, this case report is the first to describe acute cerebellitis accompanied by an elevated CSF IgG index and an absence of CSF oligoclonal IgG bands. These findings suggest that a breach in the blood-brain-barrier might occur in acute cerebellitis.

OBJECTIVE: We investigated the effect of preeclampsia, birth weight, fetal sex, maternal body mass index(BMI) on cord blood leptin concentrations. This study is to test whether leptin would be increased in preeclampsia and to test cord leptin concentrations for correlation with maternal leptin concentrations. METHOD: Maternal blood samples were obtained from 56 normal pregnancies and 24 preeclampsia women prior to delivery. Umbilical cord blood were obtained from the all cases studied immediately after delivery. The total leptin levels were determined by radioimmunoassay. RESULTS: Mean maternal leptin level was 16.4+/-6.4ng/ml in normal pregnancy and was 19.4+/-7.9 ng/ml in preeclampsia. Mean cord leptin level was 5.4+/- 3.4ng/ml in normal pregnancy and 4.6+/-3.2ng/ml in preecalmpsia. Mean cord blood leptin level in female fetus was 6.6+/-3.7ng/ml and 4.8+/-2.8ng/ml in male fetus. CONCLUSION: This study shows that there are correlation with leptin level and fetal birth weight, body mass index, and fetal gender. Furthermore, physiologic role and mechanism of leptin of maternal serum and umbilical cord would be studied.

BACKGROUND: Though immunotherapy(IT) has become an effective rnethod in extrinsic allergic patients who didn't respond to pharmacologic therapy or couldn't avoid allergen, the mechanism, termination index and prognostic index of IT have not been clarified yet. METHOD: We selected 81 asthmatic children on immunotherapy with house dust mite (Dermatophagoides pteronyssinus and Dermatophagoides farinae). We measured the hematologic findings, the levels of serum IgG and IgE, allergen(house dust, Dermatophagoides pteronyssinus and Dermatophagoides farinae)-specific IgE concentrations, lymphocyte subsets and methacholine challenge test yearly during IT, and checked the radiographs of chest and paranasal sinus. RESULTS: Peripheral white blood cell count, the percentage of eosinophil and total eosinophil count decreased significantly after 2 years of IT. Serum IgG level increased significantly after 3 years of IT. Serum total and specific IgE levels decreased significantly after 3 years of IT, but they were still higher than the normal values. CD4+, CD8+, and B lymphocytes did not change with the IT, but CD3+ lymphocytes increased significantly after 2 years of IT. PC20-methacholine increased significantly after 1 year of IT, but no correlation was found between the duration of IT and bronchial hyperreactivity. Twenty-eight patients(34.6%) had abnormal findings on chest radiographs: 15 patients(53.6%) as bronchitis, 10 patients(35.7%) as bronchopneumonia, 2 patients(7.1%) as hyperinflation and 1 patient(3.6%) as atelectasis. Sixty-three patients(77.8%) had abnormal findings on paranasal sinus radiographs. In the follow-up radiographs of 49 patients, 28 patients(57.1%) showed improvement of paranasal sinusitis after 1 year of IT. CONCLUSION: This study showed some changes of the immunologic findings such as eosinophil count, IgG, IgE, allergen-specific IgE and CD3+ lymphocytes, and improvement of bronchial hyperreactivity and paranasal sinusitis' in asthmatic children during IT. These findings were closely related to clinical improvement.
