1.Histopathological Differences between Silicone Granuloma and Paraffinoma.

During the past two decades, silicone (polydimethylsiloxane) has become one of the most extensively applied biomaterials. Although pure silicone is relatively inert and usually causes only minimal tissue reactions, it has been reported to evoke a definite foreign body reaction. We studied five cases of silicone-induced granulomas in various sites; two in the breast, one in the breast and axillary lymph nodes, one in the subcutis of the abdomen, back and extremities and one in the eyeball, to illustrate the salient histopathologic features of reactions to silicone with particular emphasis to its differences from paraffin granuloma. For this, 17 paraffinomas were also studied. Tissue reaction to silicone liquid and gel was characterized by numerous round to oval empty cystic vacuoles, mild to moderate fat necrosis, foreign body reaction, a variable degree of mononuclear inflammatory cell infiltration and mild focal fibrosis. The cystic spaces were relatively uniform and showed a snow-man like appearance. In contrast to the silicone granulomas, the paraffinomas, also refered to as sclerosing lipogranulomas showed diffuse sclerosis and frequent calcification around the cystic vacuoles. The cystic spaces in paraffinomas were swiss cheese-like configuration, and the content of the cystic spaces was dirty and frequently calcified. However, there were certain similarities between these two types of granulomas particularly in the early phases of the reaction, therefore, the history of silicone injection or implant, is sometimes critical to the diagnosis of silicone granuloma. Despite great technologic advances in the manufacturing of prostheses and medical equipment, droplets and/or particles of silicone still escape into the body tissues in a variety of ways; therefores, the pathologist should always wonder whether the histologic reaction observed is due to silicone or to some other foreign material including paraffin.
2.Placetnal Findings of Septic Abortion Caused by Listeria Monocytogenes: A case report.

Since human listeriosis was firstly described by Nyfeldt in 1929, Listeria monocytogenes as a cause of septic abortion has been well known. It primarily affects pregnant woman and neonates, the elderly, and persons with immune-system dysfunction due to immunosuppressive drugs, malignant tumors or AIDS. Although several large epidemic listeriosis have been reported in the English literatures, it is still an underdiagnosed and underreported cause of congenital sepsis and septic abortion, because it is not always easy to isolate the organism in culture. There are 17 cases reports of Listeria monocytogenes infection in Korea, however, most of which were described about the bascteriologically proven cases. We describe placental findings of a septic abortion caused by Listeria monocytogenes at 15 weeks gestation in a 23-year-old primigravida woman. Placental examination showed characteristic and relatively specific patterns of granulomatous microabscesses composed of necrotic nuclear debris in the center and surrounding epithelioid histocytes enmeshed in fibrin between the acutely inflamed villi. There were numerous gram positive bacilli on Brown-Brenn stain.

3.Clear Cell Adenocarcinoma of the Urinary Bladder Accompanied by Vesical Endometriosis.

A case of clear cell adenocarcinoma arising in the female urinary bladder, which is accompanied by endometriosis of the urinary bladder and the uterus, is reported. The carcinoma protruded into the vesical lumen as a fungating mass, and had a tubulocyotic pattern. The tumor cell had intracytoplasmic glycogen and electron microscopically short microvilli on their surface, resembling clear cell acenocarcinoma of the female genital tract including ovary. This is the fourth case report of clear cell adenocarcinoma complicating vesical endometriosis, and may support the view that clear cell carcinome arises from endometriosis which, in turn, from the Mullerian remnant.

5.c-erbB-2 Oncoprotein Expression in Ductal Carcinoma in situ and Paget's Disease of the Breast.

A clinico-pathologic study with an immunohistochemical examination for c-erbB-2 expression in 54 cases of ductal carcinoma in situ and 16 cases of Paget's disease of the breast was performed. c-erbB-2 oncoprotein overexpression was observed in 45% (24/54) and 88% (14/16) of ductal carcinoma in situ and Paget's disease, respectively. The overexpression of c-erbB-2 oncoprotein was significantly correlated with the nuclear grade of tumors and inversely with the status of the estrogen receptor. c-erbB-2 was positive in 4 out of 5 patients with metastasis to axillary lymph nodes and 3 out of 4 patients who died of the disease. Prognostic significance of c-erbB-2 oncoprotein in ductal carcinoma in situ was highly suggested. The expression of c-erbB-2 oncoprotein in Paget's disease was well correlated with coexisting infiltrating or in situ ductal carcinoma. The high positive rate of c-erbB-2 oncoprotein in ductal carcinoma with Paget's disease could be understood with a recent hypothesis that c-erbB-2 oncoprotein is involved in promotion of cell motility and the spread of carcinoma cells.

Fourteen cases of double chambered right ventricle were diagnosed angiographically and of these nine caseswere confirmed after operation and autopsy at Seoul National University Hospital in recent four years since 1979.The clinical and radiological findings with the emphasis on the cinecardiographic findings were analysed. Thesummaries of the analysis are as follows; 1. Among 14 cases, 6 cases were male and 8 cases were female. Agedistribution was from 4 years to 36 years. 2. In chest X-ray findings, pulmonary vascularity was increased in 8cases, decreased in 4 cases, and nomral in 2 cases. Cardiomegaly was observed in 8 cases and other showed normalheart size. 3. In cinecardiography, 11 cases had interventricular septal defect. Among these 11 cases, VSD locatedin proximal high pressusre chamber was in 2 cases and located in distal low presssure chamber was in 9 cases. 4.The location of aberrant muscle bundle in sinus portion of right ventricule was in 8 cases. In the rest cases, andaberrant muscle bundle was located below the infundibulum of right ventricle. 5. For accurate diagnosis anddifferential diagnosis with other congenital cardiac anomalies such as Tetralogy of Fallot or isolated pulmonicstenosis, biplane cineangiography and catheterization is an essential procedure.

7.Chondrosarcoma: MR Imaging Findings Correlated with Pathologic Classification and Grade.

PURPOSE: To evaluate the MR imaging findings of chondrosarcomas by correlation with pathologic classificationand grade. MATERIALS AND METHODS: We performed MR imaging-pathologic correlation of nineteen chondrosarcomas. Conventional chondrosarcomas accounted for 15 cases (grade I : 6, II : 6, III : 3) and the mesenchymal and dedifferentiated types each accounted for two. MR signal intensity (SI) of the tumor on T1- and T2-weighted images(T1WI and T2WI, respectively), was classified as homogeneous or heterogeneous low-, iso- or high SI, and enhancing pattern as marginal, marginal and septal, marginal and nodular, or diffuse enhancement. RESULTS: Eighteen cases of chondrosarcomas (95%) showed homogeneous or heterogeneous low- or iso SI on T1WI and high SI on T2WI. Low gradeconventional chondrosarcomas showed marginal and septal (n=8/10) or marginal (n=2/10) enhancement on Gd-enhanced MR images. Grade III conventional chondrosarcomas showed marginal or marginal and nodular enhancement. Dedifferentiated and mesenchymal chondrosarcomas showed marginal and nodular or diffuse enhancement. CONCLUSION: Chondrosarcomas showed iso- or low SI on T1WI and high SI on T2WI. Marginal and septal enhancement was demonstrated on Gd-enhanced MR images of grade I and II conventional chondrosarcomas. If a tumor showed amarginal and nodular or diffuse enhancing pattern, this suggested it was a of high grade chondrosarcoma.

8.A Case of Black Adrenocortical Adenoma Causing Cushing's Syndrome with Contralateral Nonfuncioning Adenoma.

We report herein the case of a 38-year-old woman with Cushings syndrome caused by bilateral adrenocortical adenomas. The adrenal tumor on the left side hypersecreted cortisol and no findings of cortisol hypersecretion from the adrenal tumor on the right side were observed on bilateral adrenal vein samplings. Both adrenal tumors were resected and histologically without any findings of nodular hyperplasia. The left adrenal tumor was histologically diagnosed as a so-called black adenoma. These data imply that the adrenal adenomas developed primarily from the adrenal gland itself, and that one of the tumors was well differentiated and secreted excess hormones, while the other remained in cell proliferation without hypersecretion.

9.A study of contraction shrinkage of composite resins and ormocers with various curing times.

Ormocer has organic-inorganic compound polymers. One of advantages of ormocer is reduced polymerization shrinkage. The purpose of this study was to compare the amount of contraction shrinkage of composite resins and ormocers. Additionally, the time of each material when there is no further change of contraction shrinkage was analysed. Four brands of composite resins (P-60, Surefil, Z-250 and Denfil) and two brands of ormocers (Definite and Admira) were used. 20 seconds, 40 seconds and 60 seconds of curing times were given. Contraction shrinkage of them were measured using a linometer for 80 seconds. The effect of material and curing time to contraction shrinkage at the time of 80 seconds was analysed by two-way ANOVA. The effect of time to contraction shrinkage was analysed by one-way ANOVA, and the time when there was no further change of the contraction shrinkage was analysed. The results are as follows : 1. P-60, Definite, Z-250 and Denfil had no further change of contraction shrinkage from the time of 20 seconds, and Surefil and Admira had no further change of contraction shrinkage from the time of 10 seconds. 2. Statistical analysis revealed volumetric shrinkage varied among material (p<0.05). No significant difference of contraction shrinkage among different curing times was found, and there was no effect of interaction between materials and curing times to contraction shrinkage. 3. Definite and Admira showed the statistically same contraction shrinkage with those of Z-250 and P-60, which is higher than that of Surefil and lower than that of Denfil (p<0.05).
