2.Revival of Hop Japanese pollinosis in asthmatic subjects in Kyungki area.

BACKGROUND: Hop Japanese (Hop J) pollens are abundant in the air of Korea during the autumn season. Their significances as a source of allergenic sensitization have been underestimated in this country. MATERIAL AND METHOD: In other to observe clinical features of Hop J-sensitive asthmatic patients in this country, skin prick test with Hop J pollen was performed. The serum specific IgE antibodies to Hop J pollen antigen were detected by enzyme linked immunosorbent assay (ELISA) in positive responders (>2+ of A/H ratio) on skin prick test. To confirm the respiratory sensitization, bronchoprovocation test was performed in 17 asthmatic patients sensitive to this pollen. RESULT: Ten asthmatic subjects showed a significant bronchoconstriction following the inhalation of Hop J pollen extract (6 early and 4 dual astmatic responses) and all of them had high serum specific IgE bindings, with minimal bindings in negative responders. They have suffered from seasonal aggravation of asthmatic symptoms with or without rhinitis, and/or conjunctivitis symptoms. The skin reactivity to Hop J had more than 5+ of A/H ratio on skin prick test in nine positive responders, whlie negative responders showed from 1+ to 3+ response. Moreover, four (40%) asthmatic subjects showed a positive response to only the Hop J pollen on skin prick test and an isolated positive asthmatic response to the Hop J bronchoprovocation test. CONCLUSION: We believe that the Hop J pollen should be considered as an allergen during the Autumn season, and thus included in skin test batteries in this area. Some labelled having intrinsic asthma or rhinitis might be sensitized to this pollen or other unknown allergens.

3.A Study on the Health Risk Agents in the Workplace of a General Hospital.

To evaluate the health risk of the workplace environment of a general hospital toward the hospital workers, a questionnaire survey on the perception of risk at the workplace environment and environmental measurements at 27 locations with 240 workers in the hospital were made from October 25th, 1993 to October 30th, 1994. The results were as follows; 1. By the environmental measurements, 86 workers(36%) were found to be exposed to poor or harmful degree of general illumination, 193 workers(80%) were exposed to poor or harmful local illumination, 34 workers(14%) were exposed to poor or harmful degree of thermal condition and l80 workers(75%) were exposed to poor or harmful noise level, but nobody was exposed to poor or harmful dust and toluene concentration. Also nobody was exposed to poor or harmful level of radiation or anticancer/antibiotic agents. 2. The subjective perceptions on the environmental conditions felt by the workers were different from the objective findings by the environmental measurements. The workers underscored the poor illumination state and overscored the dust concentrations. Also workers oversensitized about the thermal condition, the noise level, the radiation level, the toluene concentration and anticancer/antibiotic agents. 3. The sources of noise were the dialogue(179 workers) and the office instruments(131workers). The sources of dust came from the clothes(108 workers) and the building materials(79 workers). 4. The questionnaire survey showed that the 28 workers were exposed to poor or harmful level of the antibiotics, 10 workers to alkali or acid and 6 workers to drug dust in the pharmacy but the above findings could not be proved by the environmental measurements.

4.Outpatient Distribution for Glaucoma Evaluation.

PURPOSE: To analyze the reasons for glaucoma evaluation and distribution of new patients visiting the glaucoma department. METHODS: In a retrospective study, 330 new patients underwent ocular examination using Goldmann applanation tonometry, gonioscopy, optic disc analysis, optical coherence tomography, and Humphrey perimeter under suspicion of glaucoma for the first time in the Glaucoma Department from January 2013 to December 2013. We analyzed the reasons and their diagnostic outcomes. RESULTS: The reasons for glaucoma evaluation were health screening (103 patients, 32.49%), other symptoms (102 patients, 31.55%), known glaucoma (56 patients, 17.67%), pre-refractive surgery evaluation (31 patients, 9.78%), family history (19 patients, 5.99%), and high myopia (6 patients, 1.89%). The diagnostic outcomes were as follows: glaucoma (139 patients, 43.85%), glaucoma suspect (60 patients, 18.93%), ocular hypertension (9 patients, 2.84%), neither glaucoma nor ocular hypertension (79 patients, 24.92%), normal (30 patients, 9.46%). The percentages of confirmed glaucoma according to the reasons for glaucoma evaluation were as follows: health screening, 26.21%; other symptoms, 40.20%; known glaucoma, 85.71%; pre-refractive surgery evaluation, 58.06%; family history, 15.79% and high myopia, 33.33%. CONCLUSIONS: The reasons for glaucoma evaluation were diverse. Glaucoma was confirmed in 43.85% of the patients and the predicted value of positive test for glaucoma including glaucoma suspect and ocular hypertension was 65.62%.

5.Lessons Learned from Optional Elective Course.

BACKGROUND: The College of Medicine at Korea University has lunched the new curriculum which incorporated optional elective courses in 2000. Optional elective course is consisted of 10 subjects. 8 subjects are conducted in classroom lectures and 2 of them are active experiences in research or community service. The purpose of this study is to identify the level of achievement of its own educational goals and objectives. Also, we tried to explore the possibilities of improving this course for the maximum educational effectiveness.. MATERIAL AND METHOD: We conducted a survey and the subjects of this study were second, and third year medical students who finished this course. RESULTS: 84.8% of respondents were aware of the goals of the course. 91.1% of them agreed this course is needed to enhance the effectiveness in undergraduate medical education. Most students agreed the course provide an opportunity to broaden their perspectives of field of medicine(81.4% of respondents). Also, this course provided good informations for students' future career choice(78.6% of respondents). However, students wanted to have more in-depth learning experience. INTERPRETATION: The implementation of the optional elective course at the Korea University, Medical College was successful. Despite of its initial success, the course has to be revised to allow students to get more in - depth study according their own interests.

6.Clear Cell Sarcoma of the Kidney: Report of two cases.

Clear cell sarcoma is a rare malignant rumor of the kidney which occurs in children and is differentiated from Wilms' tumor by its different clinicopathologic features and natural history. Previous studies indicate that this tumor may be of mesenchymal cell origin; however, this has not been proven conclusively. Further accumulation and study need to be conducted in order to clarify the histogenesis of this tumor. We report two cases of clear cell sarcoma of the kidney which occurred in a 2 and a half-year old and a 2-year old boy. This report places special emphasis on the clinicopathologic characteristics of these two cases including electron microscopic and immunohistochemical findings. Attempts were also made to differentiate the clinicopathologic aspects of clear cell sarcoma from Wilms' tumor and speculate on the histogenesis of this rumor.

7.The Comparison of Factors related to Performance of Breast Self-examination by Age Group in the Middle and Mature Aged Woman.

PURPOSE: This study aims to identify differences in breast self-examination (BSE) performance and influencing factors between woman-groups under and over 45 years old. METHODS: The subjects were 152 women aged from 35 to 65, who were recruited through convenient sampling in a metropolitan city. They were divided into two groups: under and over 45 years old. The data were collected using self-reporting questionnaires and analyzed by chi-square test, t-test, Pearson's correlation coefficient, and stepwise multiple regression. RESULTS: Experience of BSE education (chi-square=4.68, p=.030), BSE performance (chi-square=20.12, p<.001), confidence (t=-2.97, p=.003), and self-efficacy (t=-2.44, p=.016) were significantly higher in the group over 45 years (the older group) than the one under 45 years (the younger group). Self-efficacy (beta=.346, p=.004) and susceptibility (beta=.238, p=.002) were 17.6% of the variance in the younger group's BSE performance. On the other hand, significantly influencing factors on the older group's BSE performance were self-efficacy (beta=.500, p<.001) and BSE education (beta=.217, p<.001), which accounted for 25% of the variance in the BSE performance. CONCLUSION: We conclude that differentiated strategies of considering age should be established in nursing intervention to detect breast cancer early.

8.Effect of the Auricular Acupressure Therapy on Dysmenorrhea of Puberty Girls.

PURPOSE: The purpose of this study was to identify the effect of auricular acupressure therapy on dysmenorrhea of puberty girls. METHODS: This study was a pretest-posttest design with a nonequivalent control group. The subjects of this study were 61 girls who were middle and high school students in Seoul and the experiencing dysmenorrhea; 31 for the experimental group and 30 for the control group. The obtained data were analyzed by SPSS/WIN 12.0 program. RESULTS: The experimental group which applied the auricular acupressure therapy scored lower for low abdominal pain (F=136.13, p=.000), low back pain (F=62.79, p=.000). autonomic nervous system reactions (F=102.31, p=.000) and negative emotions on the menstruation (F=102.31, p=.000) compared to the control group. There were significant interaction effects of auricular acupressure on low abdominal pain, low back pain, autonomic nervous system reactions, and negative emotions on the menstruation between time and group. CONCLUSION: We observed that auricular acupressure therapy is effective for reliving pain and the symptom of dysmenorrhea. Based on these result, auricular acupuncture is useful by individual nursing intervention and improves the quality of life for dysmenorrhea of puberty girls.

10.Development and Preliminary Evaluation of Smartphone Application-Based Nutrition Education Material for Twin Pregnancy.

PURPOSE: To develop educational material about nutrition during antepartum period for women pregnant with twins by using smartphone applications. METHODS: A series of steps according to ADDIE model, including Analysis, Design, Development, Implementation, and Evaluation were used to develop smartphone application of antepartum nutrition care for women pregnant with twins. RESULTS: Based on experts' evaluation, the average score was 4.6±0.39 in total. Content score was 4.7±0.40. Interface design score was 4.5±0.55. Content score was higher than interface design score. User evaluation was conducted in the form of interview. Results of interviews revealed that users generally responded positively to the accuracy, understanding, and objectivity for content items of the smartphone app. As for the evaluation of consistency for the evaluation item of interface design, users answered “ordinary” or “generally yes”. For design suitability and accuracy of vocabulary, they answered ‘generally suitable’. CONCLUSION: The smartphone app developed through this study is expected to aid antepartum care for women pregnant with twins. It will also contribute to health promotion of both pregnant women and twin fetus.
