5.The sustaining effect of three polymers on the release of chlorhexidine from a controlled release drug device for root canal disinfection.

The aim of this in vitro study was to evaluate the suitability of using chitosan, poly (lactide-co-glycolide) (PLGA), and polymethyl methacrylate (PMMA) to control the release of chlorhexidine digluconate (CHX) from a prototype of controlled release drug device (CRD) for root canal disinfection. Four different prototypes with different formulations were prepared. Group A (n = 12); The device (absorbent paper point) was loaded with CHX as control. Group B (n = 12); same as group A, but the device was coated with chitosan. In Groups C and D, the device was treated in the same way as group A and then coated three times with 5% PMMA (Group C, n = 12), or coated three times with 3% PLGA (Group D, n = 12). The devices were randomly allocated to experimental groups of 12 each. All CRD prototypes were soaked in 3 mL distilled water. The concentrations of CHX were determined using a UV spectrophotometer. The surface characteristics of each prototype were observed using a scanning electron microscope. The result showed that release rate of CHX was the greatest in the non-coated group, followed by the chitosan-coated group, the PLGA-coated group, and the PMMA-coated group (P < 0.05). Pores were observed on the surface of the prototypes that were coated with PLGA and PMMA. When the pore size was smaller, the release rate was lower. This data indicate that polymer coating can control the release rate of CHX from the CRD prototypes.

6.Internal Carotid Artery Aneurysm Presenting with Unilateral Nasal Hemianopsia.

PURPOSE: To report a case of unilateral nasal hemianopsia caused by a large internal carotid artery aneurysm. CASE SUMMARY: A 56-year-old female presented with large cupping in the left optic nerve head detected incidentally during a routine check-up. She had no underlying systemic disease except hypertension. The best corrected visual acuity was 20/20 in both eyes and a slit-lamp examination showed no abnormal findings. Ophthalmoscopy showed cup/disc ratios of 0.6 in the right eye and 0.75 in the left eye. Relative afferent papillary defect or color vision defect was not observed. A Humphrey visual-field test indicated unilateral nasal hemianopsia in the left eye. Brain CT and angiography revealed a large 2.2-cm aneurysm on the left internal carotid artery. CONCLUSIONS: Internal carotid artery aneurysm should be considered as a possible cause of unilateral nasal hemianopsia in patients without intraocular lesion.

7.Effect of Visual Information by Ultrasound on Maternal-Fetal Attachment.

Providing visual information about the fetus to the mother by the ultrasound examination was found to be an effective nursing intervention to promote Maternal-Fetal Attachment. In keeping with the purpose of the study, to evaluate the effect of providing visual information by ultrasound on level of Maternal-Fetal Attachment, a non-equivalent experimental group quasi-experimental design was used. The data were collected using Cranley's Maternal-Fetal Attachment Scale(1981) with a research questionnaire that consisted of 16 items on general characteristics and 23 items on Maternal-Fetal Attachment from November 2, 2000 to August 11, 2001. Subjects were 126 pregnant women who were received visual information by ultrasound and 123 pregnant women who did not receive visual information by ultrasound after finishing examination. The data were analyzed by using the SPSS/PC+ window 10.0 version program. The results of this study were as follows: There was no statistical difference in general characteristics between both groups. The scores on Maternal-Fetal Attachment at second trimester show no statistical difference (t=1.123, p=0.263). The scores on Maternal- Fetal Attachment in both groups increased between the second trimester and third trimester. However, the increase was greater in the group receiving visual information by ultrasound as compared to the group which did not receive the visual information(t=-2.152, p=0.032). This result shows that providing visual information about the fetus by the ultrasound examination is effective in increasing Maternal-Fetal Attachment.

Pulmonary alveolar proteinosis (PAP) is a rare condition that is treated using whole lung lavage. A recent study suggested that granulocyte-macrophage colony stimulating factor (GM-CSF) plays roles in both the pathogenesis and treatment of PAP. We present a 69-year-old man with PAP who deteriorated despite bilateral whole lung lavage; that said, his symptoms, chest X-ray findings, and pulmonary function test improved after GM-CSF inhalation therapy over 12 months. GM-CSF therapy is an effective treatment modality for PAP.

10.Two Cases of Severe Hemolytic Transfusion Reactions Caused by Anti-Jkb Antibody.

We report two patients who suffered from hemolytic transfusion reactions due to anti-Jkb antibody: one showed acute- and the other showed delayed-type hemolysis. The first patient was a 40-year-old man who suffered from epilepsy after the operation for arteriovenous malformation 16 years ago. He received five units of red blood cells (RBC) after right temporal lobectomy. On the fifteenth postoperative day, fever and chill developed during transfusion of one unit of packed RBC, followed by dark urine and oliguria. The polyethylene glycol-Coombs test and enzyme test revealed anti-Jkb antibody which had not been detected on the pretransfusion specimen. The second patient was a 41-year-old man who was admitted for the reoperation of the prosthetic mitral valve. Because hemoglobin was 5.9g/dL at admission, he received five units of packed RBCs. Oliguria and laboratory findings consistent with hemolytic anemia were observed from the third day of transfusion. Anti-Jkb antibody was detected on antiglobulin phase. Both patients developed acute renal failure (ARF) and hemodialysis with conservative management were done. They finally recovered from ARF without any residual complications. Implementation of more sensitive pretransfusion tests should be considered to prevent rare, but serious hemolytic transfusion reactions.
