2.Treatment and Management of Sexually Transmitted Diseases in Sexually Assaulted Women.

The evaluation and management of the sexually assaulted women is a complex, multifaceted task. The purpose of the medical examination after a sexual assault is to assess the patient for physical injuries, and to collect evidence for forensic evaluation and possible legal proceedings. Laboratory samples should be obtained at the initial visit and should include testing for pregnancy, syphilis, and hepatitis B and human immunodeficiency virus infections. Treatment should address physical injuries, pregnancy prophylaxis, sexually transmitted diseases and psychosocial sequelae. Appropriate referral services should be initiated during the initial visit. Victims of sexual assault require appropriate care, follow-up, and information regarding their legal rights. The history should be confined to medically relevant facts and should be conducted in a safe and quiet environment. In many emergency departments, however, the emergency room doctors is responsible for the initial evaluation and management. Therefore, it is imperative that the emergency room doctors be familiar with the process and issues specific to the management of the victim of a sexual assault.

Owing to the advancement of imaging techniques which include the CT scan, it became easier to evaluate fracture patterns of calcaneal fractures accurately. Moreover, it is possible to obtain good results with operative treatment as a consequence of the development of good operative equipment and new operative technique. In 1988, Regazzoni and Benirschke in 1990, recommended L-shaped extensive lateral approach for calcaneus which provide extensive exposure of calcaneus and so allow easier reduction and fixation. We carried out L-shaped extensive lateral approach in 11 cases from June, 1993 to April, 1994. This approach did not produce any skin problems and allowed excellent anatomical reduction and fixation. But we experienced some severe causalgia on the heel region in several cases. We tried to analyse the cause of pain and concluded that it was the damage to the lateral calcaneal branch of the sural nerve. We are reporting the problems of tbis approach.

4.Preferential Suppression of the On Pathway by r-Aminobutyric Acid in the Catfish Retina.

The effects of r-aminobutyric acid(GABA) agonsits and antagonists were explored by the intracellular recording method to discern the preferential suppression of the ON component by GABA on the ON-OFF transient cell in the catfish (Ictalurus punctatus) retina. Experiments were performed in the superfused eyecup preparation. The animals were decapitated and pited before the eye, and the surrounding tissue was removed from the skull. The retina was exposed by excising the cornea, iris, and vitreous. This preparation rested on a wad of Ringer`s soaked cotton in contact with an Ag/Agcl reference electrode. Solutions were delivered through a manifold system that was connected to a pipette located near the absorbent wick. Electro-physiological recordings were made using standard intracellular electrodes filled with 2 M potassium acetate. The electrical signal was recorded with an amplifierand a penwriter, viewed on an oscilloscope, and stored on a data recorder. The light sources were red light-emitting-diode (LED) and the stimuli were full field illumination covering the cntire retina. GABA preferentially reduced ON light responses in ON-OFF transient cell. and GABA hyperpolarized bipolar cells, but the effects on ON bipolar cells were more sensitive than OFF bipolar cells. CACA and TACA, GABAc receptor agonist, did not act on bipolar cells. CACA and TACA, GABAc receptor agonists, hyperpolarized bipolar cells but the sensitivity deferences between ON and OFF bipolar cell were not observed. These results suggest that the preferential suppression of the ON component of the ON-OFF transient cell by GABA was resulted from the presynaptic mechanism that reduced bipolar cell input.

5.Uterine Arterial Embolization for the Treatment of Leiomyomas Accompanying with Adenomyosis.

OBJECTIVE: The purpose of this study was to make a guideline of uterine artery embolization for the treatment of uterine leiomyomas accompanying with adenomyosis in Korea. MATERIALS AND METHODS: We performed the retrospective study for 37 women who had uterine leiomyomas accompanying with adenomyosis. Bilateral uterine artery embolization was performed in 37 patients (age range 25-65) during 17 months with pain, hypermenorrhea, urinary frequency etc due to leiomyomas. Ultrasound imaging was performed before the procedure and at mean 6.9 months after the procedure. RESULTS: All procedures were technically successful. Mean clinical follow-up was 12.8 months. Minor complication occurred in 82% patients after the procedure. After imaging follow-up (mean, 6.9 months postprocedure), median uterine volume decreased 34.4%, and dominant myoma volume decreased 86%. There was no statistical difference in uterine volume reduction and dominant myoma size reduction whether occluding agents was polyvinyl alcohol, polyvinyl alcohol plus gelfoam, and gelfoam, and whether ultrasound measured Resistance Index value before the procedure was low or high. CONCLUSION: Primary candidates for uterine artery embolization include those with symptomatic uterine leiomyomas who no longer desire fertility but wish to avoid surgery or are poor surgical risks. To our study, uterine volume reduction and dominant myoma size reduction in patients who had adenomyosis were similar to previous other studies in patients who had not adenomyosis. Therefore adenomyosis should not be considered as a contraindication for uterine artery embolization. Because there is little data about subsequent reproductive potential after this procedure, it should not be routinely advocated for infertile women. Further investigation is warranted for occluding agents and Resistance Index.

7.Breastfeeding Initiation and Continuation by Employment Status among Korean Women.

PURPOSE: The objective of this study was to examine the factors associated with initiation and continuation of breastfeeding among Korean women in relation to their employment status. METHODS: Data were collected using a web-based self-administered questionnaire from 1,031 Korean mothers living in Seoul with babies younger than 24 months. Demographic characteristics, education on breastfeeding, rooming in, breastfeeding during hospital stay, and breastfeeding knowledge were examined. Multivariate logistic regression analyses were performed to identify factors associated with initiation and continuation at 1, 6 and 12 months according to mothers' employment status. RESULTS: Breastfeeding initiation rates were similar regardless of mothers' employment status. Continuation rates decreased for both groups of mothers, but were significantly lower among employed mothers at all duration points. Unemployed mothers who were able to keep their babies in the same room during the hospital stay were more likely to initiate breastfeeding. The factor that was consistently associated with breastfeeding continuation for all duration points among unemployed mothers was whether the mother breastfed during the hospital stay. Higher knowledge scores and having an infant with atopic dermatitis were also associated with breastfeeding continuation at 6 months and 12 months, respectively for unemployed mothers, and receiving education on breastfeeding was associated with 12-month continuation for employed mothers. CONCLUSION: These results emphasize the significant roles of hospitals for breastfeeding initiation and continuation, with rooming-in, initial breastfeeding practice and education during hospital stay as important practices. In addition, for working mothers to continue their breastfeeding, significant support from the workplace is crucial.
