1.Factors Affecting Physical Activity Levels among the Korean Elderly with Osteoarthritis: Focusing on Gender Differences

Background:
This study was conducted to provide basic data to explore strategies for promoting physical activity differentiated according to the gender of the elderly with osteoarthritis.
Methods:
This study was conducted using data from the Korea National Health and Nutrition Examination Survey (2014-2021). Those aged 65 or older and diagnosed with osteoarthritis were included, and a total of 2,915 people were analyzed (male=553 and female=2,362). The level of ph guidelines presented by the World Health Organization. Less than 600 metabolic equivalent of tasks (METs)-minutes/week or no physical activity was classified as light-intensity physical activity, and if METs-min/week was 600 or more, it was classified as moderate-to-vigorous physical activity. The data analyzed using Rao-Scott chi-square and multiple logistic regression ysical activity was calculated according to the Global Physical Activity Questionnaire analysisanalyses to account for the complex sampling design.
Results:
In the elderly male with osteoarthritis, the level of physical activity was low in the case of high age, no spouse, low education level, and poor subjective health condition. In the elderly women, age, residential area, and subjective health status were found to be significant influencing factors.
Conclusions
Differences in influencing factors by gender should be considered in the physical activity improvement intervention program for older adults with osteoarthritis. Furthermore, intervention studies must examine the effects of gender-specific programs on physical activity of older adults with osteoarthritis.
2.One Stage Spondylodesis for the Bursting Fracture of the Thoracolumbar Spine Through Unilateral Posterior Approach.

OBJECTIVE: The purpose of this study is to report the technical feasibility and the result of one stage operation for the bursting fracture of the thoracolumbar spine by posterior approach. METHODS: Seven patients underwent corpectomy and replaced the space with titanium mesh cage filled up with autologous bone pieces through unilateral posterior route after removal of the facet joint and the pedicle followed by pedicle screw fixation at the same surgical field. RESULTS: All patients tolerated the procedures well except one patient complicated with air embolism. Postoperative radiographs showed effective decompression and well placed instruments. Preoperative neurological deficits disappeared completely in two patients but some deficits persisted in the remainders. In two patients, epidural venous bleeding was troublesome and he needed transfusion of 5 pints of packed red blood cell. And another patient complicated with air embolism during operation was recovered with impunity after operation. To insert the mesh cage through the narrow corridor posterolaterally, it was unavoidable to sacrifice one spinal root. CONCLUSION: In spite of technical difficulty, one stage spondylodesis for the bursting thoracolumbar spine though unilateral posterior approach seem to be an effective operative procedure.

3.Analysis of 38 Long-Term Survivors after Liver resections for Hepatocellular Carcinomas.

BACKGROUND/AIMS: This study was conducted to develop an optimal strategy to achive long-term survival after liver resection for hepatocellular carcinoma. METHODS: Between July 1975 and March 1995, 109 patients who underwent liver resection for hepatocellular carcinoma at Inje University Seoul Paik Hospital were analyzed retrospetively. RESULTS: 38 patients (34.9%) survived longer than 5 years after the operation. Prognostic factors of statistical significance were the diagnostic clue, ICG R15, TNM stage, extent of tumor, intrahepatic metastasis, portal vein thrombosis, serosal infiltration, tumor grade, resection margin, Glissonian approach, radicality of operation, cancer recurrence and disease free interval. For the 63 cases with no intrahepatic metastasis, there was no survival difference between the lobectomy group and the segmentectomy/subsegmentectomy group(36.8% vs. 50.0%). In those cases with satellite nodules in one segment of the liver, 66.7% of the lobectomy group lived longer than 5 years while 17.6% of lessthan- lobectomy group survived long-term(p=0.025). CONCLUSION: For long-term survival, a systemic segmentectomy/ subsegmentectomy is adquate for those tumors without intrahepatic metastasis, while the presence of satellite nodules in one segment mandates a standard hepatic lobectomy.

4.Influence of implant misplacement on the success of the final prosthesis: Subjective evaluation by a prosthodontist of dental implants placed by an oral and maxillofacial surgeon.

PURPOSE: In many cases, the erroneous placement of a dental implant brings about undesirable results. Here, the effect of dental implant placement on the success of the final prosthesis was evaluated from the point of view of the prosthodontist. MATERIALS AND METHODS: All surgical operations were performed by the same oral surgeon with the same surgical protocol and all prosthodontic procedures were performed by the same prosthodontist. The problems faced by the prosthodontist, their causes, and their effect on prosthesis success were identified. The success of the final prostheses was evaluated by the same prosthodontist. RESULTS: Only 53% (238 implants in 105 patients) of dental implants were not associated with prosthodontic problems. Multiple implant placement (more than three implants) was associated more frequently with prosthodontic problems. CONCLUSIONS: The data indicate that the satisfactory construction of a prosthesis is highly dependent on the placement of the dental implant in the best possible position. It is strongly recommended that the oral surgeon and the prosthodontist engage in pre-operative discussions to establish a topdown treatment plan, as this will improve implant placement and ultimately the success of the prosthesis.

5.Ultrasonographic Evaluation of the Lymphedema.

BACKGROUND: Lymphedma occurs when tissue swelling develops through a failure of lymph drainage in the face of a normal capillary filtration. Little is known about water behavior in lymphedma. OBJECTIVE: Our purpose was to investigate, by means of ultrasound, the distribution of intradermal fluid in patients with lymphedema in response to 2 weeks physical therapy. METHODS: Ten patients with lymphedema were treated by 2 weeks' physical therapy. Before and after 2 weeks' treatment, circumference and ultrasonographic evaluation was done. Ultrasound images were obtained with a 20 MHz scanner. The echogenicity of the dermis and the skin thickness were quantified by in-built image analyzer. RESULTS: Low echogenic pixels showed the most noticeable and consistent change after 2 weeks' physical therapy. CONCLUSION: Ultrasonographic evaluation could be a useful tool in evaluating lymphedema.

7.Differential expression of Matrix Metalloproteinase (MMP)-2, -9 in normal and severe preeclamptic human placentas.

OBJECTIVE: The aim of the study was to investigate the differential expression of Matrix Metalloproteinase (MMP) -2,-9 in the normal and severe preeclamptic placenta. METHODS: Placentas were obtained from women undergoing cesarean section with normal (n=10) and severe preeclamptic (n=10) pregnancies. Semi-quantitative RT-PCR was done to detect the placental gene expression of MMP-2,-9. Western blot analysis was performed to identify MMP-2,-9 protein expression in each placenta. Immunohistochemical staining were employed to localize MMP-2,-9 in placental tissues. RESULTS: MMP-2,-9 genes were expressed in both normal and severe preeclamptic placenta. There were lower expressions MMP-2,-9 in severe preeclamptic placentas than in normal. MMP-2,-9 proteins were all present in each placental tissue. The expression for MMP-2,-9 was weaker in severe preeclamptic placenta than in normal. MMP-2,-9 were localized only to the trophoblast, and were also weakly positive in severe preeclamptic placenta compared with normal. CONCLUSIONS: MMP-2,-9 expressions were decreased in severe preeclampsia placenta compared to those from normal placenta. This study suggests that decreased expression of MMP-2,-9 may have a role in the development of severe preeclampsia.

8.A case of Breast Gigantism in a Patient with Wilson's Disease treated by Penicillamine.

We report a case of breast gigantism in a patient with Wilson's disease treated with penicillamine. A 19-year-old female with alleged Wilson's disease visited our hospital due to diffuse enlargement of both breasts. She had been treated with penicillamine 1,000 mg/day since her age of 15 after diagnosis of Wilson's disease. At the initial presentation, there were diffuse skin thickenings in both lower inner breasts and huge lesion which replaced almost all the breast parenchyma. After gun biopsy and excision for tissue diagnosis, fibroadenoma with ductal epithelial hyperplasia was diagnosed. Although daily dose of penicillamine was lowed to 500 mg/day, her symptom progressed. After 1 year of follow up, she and her parents strongly wanted to remove her breasts because of distorted body shape and weight of breasts. The patients underwent subcutaneous mastectomy with the designed incision of the reduction mammoplasty for the future mammoplasty. Although the breast gigantism is a rare side effect of penicillamine, female patients should be followed up cautiously for the possible change of breasts. Because penicillamine is no more the first choice for Wilson's disease, it would be better to avoid using penicillamine for Wilson's disease patients especially for the young females.

10.Usefulness of Ki-67 as a prognostic factor in lymph node-negative breast cancer.

PURPOSE: To evaluate the independent prognostic value of Ki-67 in lymph node-negative breast cancer and the usefulness of Ki-67 when it combined with St. Gallen classification as a guidance of adjuvant chemotherapy for node-negative cancer. METHODS: We retrospectively reviewed the data of 534 patients with lymph node-negative breast cancer who underwent curative surgery between 1998 and 2001 at our institution. Patients were classified according to the guideline of risk groups of St. Gallen consensus and the level of Ki-67 expression. Distant metastasis-free survival (DFS) rates were compared between groups. RESULTS: With a median follow-up of 55 months, the overall 5-year DFS rate was 91.5%. The 5-year DFS rates for patients with high and low Ki-67 tumors (cut-off value: > or = 10%) were 84.6% and 93.7%, respectively (p < 0.001). In a Cox regression model involving potential prognostic factors, high Ki-67 expression could independently predict the risk of distant recurrence (odds ratio = 2.0 [95% confidence interval, 1.03-3.93]). The 5-year DFS rates for patients with average and minimal risk group of St. Gallen classification were 89.3% and 97.5%, respectively. The average risk group was further divided into two subgroups with significantly different prognosis according to the Ki-67 expression (DFS rate: 84.2% vs. 91.5%; p = 0.007). CONCLUSIONS: Ki-67 was an independent prognostic factor in lymph node-negative breast cancer and the combination of Ki-67 expression and the St. Gallen classification could provide a more useful therapeutic guideline for lymph node-negative breast cancer patients.
