1.Use of separate single-tooth implant restorations to replace two or more consecutive posterior teeth: a prospective cohort study for up to 1 year.

PURPOSE: The aim of this study was to evaluate the periodontal and prosthodontic complications of multiple freestanding implants in the posterior jaws for up to 1 year of function. MATERIAL AND METHODS: Eight patients received 20 implants posterior to canines. Two or more implants were consecutively inserted to each patient. Single crowns were delivered onto the implants. Marginal bone loss, implant mobility, probing depth, and screw loosening were examined to evaluate the clinical success of such restorations for maximum 1 year of functional loading. RESULTS: All the implants performed well during the observation period. Neither periodontal nor prosthodontic complications were found except a slight porcelain chipping. While the marginal bone level was on average 0.09 mm lower around the implant after 6 months of loading, it was 0.15 mm higher after 1 year. CONCLUSION: Within the limits of this investigation, separate single-tooth implant restorations to replace consecutive missing teeth may clinically function well in the posterior jaw.

2.The Antioxidant Effect of Vitamin C and Deferoxamine on Paraquat Induced Lipid Peroxidation in Rats.

BACKGROUND: The toxicity of paraquat has been known to be caused by oxygen free radicals which leads to the lipid peroxidation and multiple organ failure. Although vitamin C has been known to be a potent antioxidant, recently there are numerous data which have shown that a low dose of vitamin C may act as a prooxidant due to the stimulation of the Fenton reaction with metal ions, which produces hydroxyl radicals. It has been reported that a deferoxamine in paraquat intoxication could reduce the production of the hydroxyl radicals by the inhibition of the Fenton reaction through the reduction of iron ion in tissue. The aim of this study was to evaluate the effect of the high and low dose of vitamin C and deferoxamine on lipid peroxidation and plasma TNF-alpha in paraquat intoxication. METHODS: Female Sprague-Dawley rats were divided into seven groups: control group which was not given paraquat(20mg/kg), P group which was given paraquat only, PVH group given paraquat and high dose of vitamin C(100mg/kg), PVL group given paraquat and low dose of vitamin C(10mg/kg), PVHD given paraquat, high dose of vitamine C and deferoxamine(100mg/kg), PVLD given paraquat, low dose of vitamin C and deferoxamine, and PD given paraquat and deferoxamine. Animals were killed at 6 and 24 hours after treatment. Malondialdehyde(MDA), superoxide dismutase(SOD) and glutathione(GSH) contents, catalase activity, plasma TNF-alpha, and histologic changes in the lung and liver tissue were measured. RESULTS: The lung histology in the PVH and PD or PVHD groups showed the significant decreases in the alveolar edema and interstitial thickness compared to the P group. The liver histology in the PVH and PVHD groups demonstrated marked differences in the central venous and sinusoidal dilatation compared to that of the P group. While the MDA levels of the lung and liver in the PVH and PD groups showed the significant reduction compared to that of the P group at 6 hours after treatment, all groups showed the significant changes compared to the P group at 24 hours. There was no significant change of the SOD levels of the lung and liver at 6 hours among all groups. At 24 hours, the SOD levels of the lung in PVH, PVL, and PVHD groups showed the significant increases compared to the P group. The increase of the SOD level in groups combined with deforoxamine, however, revealed a little reduction. The SOD level of the liver in PVH group only significantly increased compared to the P group at 24 hours. There was no significant change of the GSH level of the lung and liver among all groups at 6 hours. At 24 hours, the GSH level of the lung and liver were significantly increased in both PVH and PD group and PVH group, respectively, compared to the P group. Although the catalase activity of the lung was not significantly increased, that of liver was significantly increased in both PVHD and PD groups compared to the P group at 6 hours. The catalase activities of the lung and liver were significantly increased in PVH, PD, and PVHD at 24 hours. The concentrations of the Plasma TNF-alpha were slightly decreased at 6 hours and slightly increased at 24 hours compared to that of the P group, but they were not significant. CONCLUSION: This study showed that although the low dose of vitamin C had no effect, the high dose of vitamin C revealed a decrease of the MDA level and an increase of SOD, GSH, and catalase activity in the lung and lung and liver tissues, and the effect of the high dose of vitamin C increased with time. The administration of the deferoxamine with or without high dose of vitamin C, however, significantly showed the inhibition of the lipid peroxidation and antioxidant effect and low dose vitamin C decreased the effect of deferoxamine. The effects of the vitamin C and deferoxamine on plasma TNF-alpha were not clearly shown.

3.Comparison of Infective Endocarditis between Children and Adults with Congenital Heart Disease: A 16-Year, Single Tertiary Care Center Review.

PURPOSE: This study aimed to evaluate and compare the characteristics of infective endocarditis (IE) between children and adults with congenital heart disease (CHD) at a single tertiary care center. METHODS: In this retrospective medical record review, we extracted the demographic characteristics, diagnostic variables, and outcomes of patients diagnosed with IE and CHD between 2000 and 2016. RESULTS: We identified a total of 14 pediatric patients (nine male; median age at diagnosis, 3 years). Of the 14 patients, six had a history of previous open heart surgery, while four had undergone tetralogy of Fallot repair, with transannular patch or Rastelli procedure. Among the 10 children with positive blood cultures, the most common isolated organism was Staphylococcus spp. (8/10, 80%). Eleven adult patients had IE and CHD. Among the adult patients, only four were diagnosed with CHD before IE, and ventricular septal defect was the most common CHD. The most common isolated organism was Streptococcus spp. (6/11, 55%). Compared with adult patients, pediatric patients had a higher incidence of previously diagnosed CHD (P=0.001), with Staphylococcus spp. as the causative organism (P =0.027). The median duration between the onset of symptoms and diagnosis of IE was 9 days in children and 42 days in adults (P=0.012). CONCLUSIONS: Significant differences with regard to the diagnosis and progress of IE were observed between children and adults. Ageadjusted and systematic reassessment may be necessary for the diagnosis and management of IE.

Cowden syndrome (CS), also known as multiple hamartomas syndrome, is a rare hereditary autosomal dominant disorder caused by a germline mutation in the phosphatase and tensin homolog (PTEN) gene mapped on chromosome 10. The clinical features of CS are variable, primarily presenting as mucocutaneous lesions (99%). A mucocutaneous lesion, such as trichilemmoma of the face or keratosis of the extremities, is an important diagnostic marker for CS. CS has been reported to increase the incidence of benign and malignant neoplasms in the breast, thyroid, and gastrointestinal tract. The risk of developing malignancy in individuals with CS is up to 10 times higher than general population throughout an entire life time.

5.Leiomyoma of the Nasal Septum: a case report.

Leiomyoma is a tumor rarely occurring in the nose and the paranasal sinus. It constitutes about 1% of all benign tumors arising in the head and neck area. This is probably due to the paucity of smooth muscle in the nose. We experienced a case of leiomyoma originating in the right side of the nasal septum in a 50-year- man. It was successfully removed by endoscopic surgery, and we report this case with literature.

6.A Comparison Study of Cervical Flexion-Relaxation Ratio in the Normal and Forward Head Postures

Purpose:
This study aimed to examine the effects of forward head posture on the flexion-relaxation ratio (FRR) and muscle activity during sustained neck flexion and to investigate the correlation between craniovertebral angle and FRR.
Methods:
Nineteen subjects participated in this study and were allocated to a forward head posture (FHP) group or a non-forward head posture (NFHP) group. Craniovertebral angle (CVA) and FRR were measured in all subjects, and all participants performed a standardized cervical flexion-extension movement in two phases: Phase I, sustained cervical full flexion for 5s; and Phase II, cervical extension with the return to the starting position for 5s. The value of CVA has calculated three times, and the value of FRR was measured three times in order to obtain the mean value.
Results:
FRR values in the FHP and NFHP group were significantly different (p<0.05). Phase I was significantly different, but the Phase II was not significantly different between the FHP and NFHP group (p>0.05). There was a significant correlation between the muscle activity of Phase I and CVA (p<0.05). However, FRR and the muscle activity of the Phase II were not a significant correlation with the CVA (p>0.05).
Conclusion
FHP increases the muscle activity of the cervical erector spinae during sustained neck flexion and reduces FRR, which can cause fatigue in the cervical erector spinae. In addition, for those with a smaller CVA, muscle activity of the cervical erector spinae is increased during sustained neck flexion, which can increase neck muscle tension.
7.Comparison of the Static Balance Ability according to the Subjective Visual Vertical in Healthy Adults

Purpose:
The subjective visual vertical (SVV) test is used to evaluate the otolith function in the inner ear. This study compared the different balance ability according to the results of the SVV in healthy adults.
Methods:
This study recruited 30 normal healthy subjects who did not have neurological and musculoskeletal disorders. The subjects were divided into experimental and control groups based on the results of SVV: experimental group, >2°; control group, <2°. The static balance ability was evaluated using the Fourier Index, which could evaluate the balance capacity objectively and quantitatively.
Results:
The mean angle of the SVV in the experimental and control groups was 4.44° and 0.59°, respectively. In the result of the Fourier series, the F1 frequency band in the experimental group showed a significantly higher value under one condition compared to the control group (p<0.05). In the F2-4 and F5-6 frequency bands, the experimental group showed a significant increase in the Fourier series value under the four conditions compared to the control group (p<0.05). In the F7-8 frequency band, significantly higher values of the Fourier series were observed in the experimental group under the three different conditions (p<0.05).
Conclusion
These results showed increased trunk sway while maintaining static balance in the experimental group who showed a larger SVV angle compared to the control group. The SVV can be applied to evaluate the vestibular system and balance ability in normal adults.
8.Dorsal Neck Muscle Fatigue Affects Cervical Range of Motion and Proprioception in Adults with the Forward Head Posture

Purpose:
This study was to investigate the effect of dorsal neck muscle fatigue on the cervical range of motion (CROM) and proprioception in adults with the forward head posture (FHP).
Methods:
Thirty pain-free subjects were enrolled in this study. All subjects were measured the forward head angle by taking the capture of the sagittal plane of their upper body to determine the FHP. Subjects were distributed into two groups: the FHP group (n=14) and Control group (n=16). All subjects were measured the CROM and the Head repositioning accuracy (HRA) for joint proprioception before and after inducing muscle fatigue of the dorsal neck. The CROM and HRA were measured in neck flexion, extension, right-left lateral flexion, and right-left rotation. Sorenson’s test was used to induce muscle fatigue of the dorsal neck.
Results:
Total CROMs were significantly decreased after dorsal neck muscle fatigue in both groups (p<0.05). Total HRAs were significantly increased after dorsal neck muscle fatigue in the FHP group (p<0.05), but there were no significant differences in the control group (p>0.05). Total CROM changes were not significant differences between groups (p>0.05), but total HRA changes were significant differences between groups (p<0.05) except for right and left lateral flexion (p>0.05).
Conclusion
Immediate CROM and proprioception reduction after the dorsal neck muscle fatigue were observed in adults with the FHP. Therefore, FHP can significantly affect the CROM and positioning consistency of cervical proprioception.
