1.External Beam Radiation Therapy of Aden carcinoma of the Prostate.

The record of radiation therapy cases during a five-ear period at the University of Michigan Hospitals has been analyzed. Of a total of 73 Aden carcinoma of prostate, the majority belonged to Stage B and C which represented 49 and 20 cases, respectively. The mortality rate after irradiation was clearly related to the tumor stage. Local irradiation resulted in 88% of the local control of well-ifferentiated Aden carcinomas. Stage C cases had 50% mortality, whereas that of Stage B patients was 14%. Results of this study are in general agreement with previous data in terms of the local disease control after irradiation and provide a basis for conservative radiotherapy regimen as an approach in the treatment of localized prostatic carcinomas.

3.A case of PFAPA (periodic fever, aphthous stomatitis, pharyngitis, cervical adenitis) syndrome.

PFAPA (periodic fever, aphthous stomatitis, pharyngitis, cervical adenitis) syndrome is one of the causes of periodic fever in pediatrics with unknown etiology. It is characterized by abrupt onset of fever, malaise, aphthous stomatitis, pharyngitis and cervical adenitis without long-term sequelae. Laboratory findings of this sporadic and nonhereditary syndrome are so non-specific that the diagnosis is based on clinical findings. Oral prednisolone is quite effective in controlling the symptoms. We report a case of a 6-year-old girl who was diagnosed as having PFAPA syndrome after 2 years of episodes, by excluding other disease entities with similar clinical features. The patient was treated with oral prednisolone and her symptoms improved dramatically.

4.The experiences of system integration countries informing the potential unification of the Korean peninsula's healthcare system.

A "system integration country" is defined as the political unification to a single national system of a country divided for a certain period of time such as Germany, Yemen, and Vietnam. Vietnam is moving to adopt the South Korean health insurance system. The process by which the Vietnamese healthcare insurance system is introduced should be applied to establishing the North Korean healthcare system after the future unification of the Korean peninsula. Yemen's political instability has resulted in poor health care indicators. Before the unification of Germany, an agreement regarding healthcare was formed and vigorous mutual exchanges of health care issues occurred. Political unification is not in itself a perfect solution for health care issues. We must make thorough preparations and develop strategies for an upcoming integration of healthcare systems. Like the experience of the unified Germany, humanitarian exchanges and cooperation of public healthcare issues should continue. A unified Korea will be confronted with many hardships, especially in health care. Therefore, a practical plan for the healthcare system before and after the unification of the Korean peninsula is needed.

5.Sebaceous adenitis in a Jindo dog: Dermatologic and histopathologic findings.

A two-year-old, castrated male Jindo dog presented with a three-week history of non-pruritic alopecia and scales on dorsum. The dermatologic lesions were progressive and did not respond to antibiotics and treatment for ectoparasite. Silver-white scales were adhered to the hair shafts and a follicular cast was marked on a trichogram. Findings on histopathologic examination of the skin lesions were consistent with sebaceous adenitis. Treatment with systemic cyclosporine and topical shampoo treatment was recommended for long term management. This case report is the first to describe a sebaceous adenitis in our country and suggests combination treatment approaches to long term management of this disease.

6.Consumer Perception Of The Health Care Services In Yemen And Its Impact On Self-Medication Practice

Community pharmacists are expected to have the necessary knowledge to give advice on safe and appropriate drug use during self-medication. However, the profession of pharmacist in Yemen has become less trustworthy and less reliable. In addition, quality of medication has been raising a lot of questions among consumers and fake medications have been entering the country without quality control checks. The aim of this study was to determine the consumer perception of health care services provided and its impact on self-medication practice in Sana’a city, Yemen. A self-administered questionnaire containing open-ended and closed-ended questions was developed and distributed among 400 consumers attending 10 community pharmacies in Sana’a City. All data obtained from the questionnaires were coded, entered, and analysed using Chi-square test and multiple logistic regressions. Prevalence of self-medication was found to be 90.7 %. The majority of respondents stated that they do not trust the health care services provided by physician (68.8%), community drug dispensers (78.2%), Ministry of Health (70.5%) or the quality of medicine dispensed by community pharmacies (59.7%). The trust of health care services provided by physicians and community drug dispensers were found to be significant predictors of self-medication practice. Those who did not trust health care services provided by physicians were more likely to use self-medication compared to those who did not (OR= 21.212, CI 95% 2.678-168.001, p= 0.004). Those who did not trust health care services provided by community drug dispensers were more likely to use self-medication compared to those who did not (OR= 2.746, CI 95% 1.048-7.195, p= 0.04). Consumers in Sana’a City have a negative overall perception of the services provided by community drug dispensers, physician and Ministry of Health and the quality of medication. An urgent intervention from health care authorities to adopt and to implement a new national drug policy with necessary laws and regulations is needed

7.A Case of PFAPA Syndrome Mimicking Cyclic Vomiting Syndrome.

PFAPA syndrome is characterized by periodic fevers associated with aphthous stomatitis, pharyngitis, and cervical adenitis and is unusual in infants and children. We report on a case of PFAPA syndrome mimicking cyclic vomiting syndrome in a 42-month-old girl. She had experienced multiple episodes of cyclic vomiting with abdominal pain from age 20 to 30 months. When she was 30 months old, periodic fever with pharyngitis was combined with cyclic vomiting, and when 40 months old, aphthous stomatitis and cervical adenitis were added. These periodic symptoms and signs were not treated with prokinetics or antibiotics. Symptom duration of an episode was 3 days. After cimetidine therapy (150 mg three times daily for 6 months), her febrile and cyclic vomiting episodes ceased. At the time of writing she had not received therapy for 10 months and has remained well without periodic attack.

8.The changes of salivary gland after the ligation of the excretory duct in submandibular glands.

Obstructive adenitis of the salivary gland following salivary stone or infectious disease of the gland and surrounding tissues is a common disease. It is often difficult to decide whether to perform total excision of the gland or to consider conservative treatment. The present study was designed to investigate histological, histochemical changes of submandibular gland after ligating the excretory duct for identifying the results of gland duct obstruction. A group of 40 rat of Sprague-Dawley weighing about 200~220gm were used in the present study. 30 rats had ligation of the main excretory ducts of submandibular glands just at the exit from the glands. For controls, 10 rats had a sham operation without duct ligation. They were inducted into euthanagia state by intracardial Ketamine injection in 1, 2, 3, 5, 7, 14, 21, 28, 35 and 42 days after the ligation. In each ligation period, 3 animals were used for ligation and one animal was for control. The submandibular glands were dissected out at sacrifice and stained with HandE, PAS, mucicarmine stain and histological examination were carried out under the light microscope. After examination and comparison of all specimens, the results were as follows: 1. In the features of H&E stain, acini disappeared by degrees after the ligation of the excretary duct and interstitial cells were displaced into fibrous connective tissue. Salivary gland had been atrophied with enlarging ducts and proliferating ductal cells. 2. Through total experimental period, a lot of vessels were observed and the atrophy of serous gland was severer than that of mucous gland. 3. The deep portion of submandibular glands showed severe degeneration rather than superficial portion of them after the ligation. 4. The changes which had enlarged ducts and proliferating ductal cells were observed in entire gland and more prominent in serous gland than mucous gland after the ligation. 5. Although PAS and mucicarmine reactions were decreased gradually after the ligation with the lapse of time, since 2 to 3 weeks they were strong positive reactions on entire gland, especially on duct-like structure. So, we can suggest that salivary gland will be atrophied but, survived acini will be redistributed around the ducts.

9.Treatment of Unresectable Non-Small-Cell Lung Cancer with Curative Radiotherapy.

From 1979 to 1982, 80 patients with unresectable non-mall-ell lung cancer without metastasis were treated with high-ose radiotherapy to the primary and to regional lymph nodes with or without supraclavicular lymphatic in the Department of Therapeutic radiology, Seoul National University Hospital. Of these, 56 patients (70%) were completely evaluable, and 59 patients (74%) had squamous cell carcinoma, 13% large cell undifferentiated carcinoma and 8% Aden carcinoma. 21 patients (26%) had Stage II and 59 patients (74%) had Stage III. The complete and partial response rate in the high-ose (~ 6,000rad) radiotherapy was 70% with 19% complete response. 69 patients (86%) failed in the treatment, by the failure pattern, 64% had local failure alone, 35% had local failure and distant metastasis and 1% had distant metastasis alone. The failure rate in the thorax was 76% in squamous cell carcinoma, 40% in Aden carcinoma and 20% in large cell undifferentiated carcinoma. Preliminary result shows that actuarial survival at 1, 2 and 3 years were 56%, 26% and 20% in overall patients and 64%, 37% and 21% in Stage II and 54%, 21% and 18% in Stage III, respectively. Overall median survival was 14 months; 17 months in Stage II and 13 months in Stage III. 8 patients (10%) have lived a minimum of 2 years with no evidence of disease. There was no fatal complication confirmed to be induced by radiotherapy, so definitive high-ose radiotherapy was tolerated well without major problems and resulted in good local control and survival.

10.Mesenteric Lymphadenitis Due to Yersinia enterocolitica: A case report.

Mesenteric lymphadenitis due to Yersinia enterocolitica infection is not common in Korea. Although most cases of Yersinia enterocolitica-induced mesenteric adenitis are self limited, cardinal features of Yersinia enterocolitica-induced mesenteric adenitis are so similar to those of acute appendicitis that some of the patients undergo laparotomy with suspected appendicitis. The findings on laparotomy in such patients are usually enlarged mesenteric nodes with a normal or slightly inflamed appendix. Because histologic examination of the removed mesenteric lymph nodes reveals reactive hyperplasia in most cases, it is usually difficult to suspect Yersinia enterocolitica infection on morphology of the resected nodes. But suppurative granulomata of mesenteric lymph nodes, uncommonly encountered in Yersinia enterocolitica infection, strongly suggest yersinial infection. We report a case of mesenteric lymphadenitis in a 10-year-old boy, who underwent laparotomy with suspected acute appendicitis. The removed lymph node showed several suppurative granulomata in the cortex, suggesting yersinial infection. Serologic study confirmed Yersinia enterocolitica serotype O:3 infection.
