1.The effect of captopril on fibrinolytic system during the intimal hyperplasia after arterial balloon injury

Objective To study the effect of captopril on fibrinolytic system during the arterial intimal hyperplasia after ballon injury and the prevention of restenosis after transluminal angioplasty. Methods 36 rabbits were divided randomly into three groups:sham operation group (control group),injury group,medical treatment group (captopril+injury) each n =12.Captopril (2 mg?kg 1 ?d 1 p.o.) was given to 12 rabbits in medical treatment group from 1 day before to 30 days efter balloon injury.The injury group,12 rabbits underwent same ballon injury without medication.The tissue plasminogen activator (tPA),plasminogen activator inhibitor 1 (PAI 1) antigen level were measured by ELISA before,and 1,3,5,7,14,30 days after vascular injury.30 days after angioplasty pathomorphological examination was carried out.To observe the intimal thickness and extent of lumen stenosis.Results (1) The PAI 1 levels in the injury group were significantly higher than those in the medical treatment groups.(2)The intimal thickness and extent of lumen stenosis in medical treatment group was significantly lower than that in injury gropu.Conclusion Impaired fibrinolysis after ballon injured artery affect the repair process after vascular injury,which leads to restenosis.Captopril can prevent restenosis after angioplasty.
3.Effects of Total Alkaloids of Rhynchophylline on Baroreflex and Hypertensive Target Organs in Spontaneous Hypertensive Rats

Objective To investigate the effect of total alkaloids from Rhynchophylline on hypertensive target organs. Methods 40 spontaneous hypertensive rats (SHRs) were divided into 4 groups as follows: normal control group,positive control with Tianma Goutengyin granule at 750 mg?d-1 ,total alkaloids from Rhynchophylline at 2.5 mg?day-1 as low dosage,and at 15.0 mg?d-1 as high dosage.All the rats were fed up for 20 weeks,and baroreflex sensitivity (BRS,in ms?mmHg-1)was determined by revised Smyth’s method.Heart,brain and kidney of the rats were sectioned for histological analysis at the end of 20th week. Results In sixteenth weeks,BRS of positive control,Tianma Goutengyin granule group,total alkaloids at low dosage and high dosage were(0.27±0.05),(0.31±0.06),(0.35±0.08),(0.34±0.08) ms?mmHg-1,respectively.BRS in SHR was not decreased except in the positive control.The treatment groups were superior to the normal control group in structure of heart,but not in the brain and kidney of the rats. Conclusion Total alkaloids from Rhynchophyllinecould protect heart from hypertensive injury.
4.Effect of penehyclidine hydrochloride on serum interleukin-6 and tumor necrosis factor-alpha concentrations following tourniquet deflation in patients undergoing lower limb surgery

ObjectiveTo observe the effect of penehyclidine hydrochloride (PHC) on serum interleukin-6 (IL-6) and tumor necrosis factor-alpha (TNF-α ) concentrations following tourniquet deflation in patients undergoing lower limb surgery.Methods Thirty adult patients,scheduled for unilateral lower limb surgery,ASA classification [ - Ⅱ grades,were divided into control group and research group by random number table,each group of 15 cases.Before anesthesia 30 min,PHC in intravenous infusion of 0.01-0.02 mg/kg in research group,the corresponding volume in intravenous infusion of 0.9% sodium chloride in control group.Peripheral venous blood samples were collected immediately before tourniquet inflation (T0,baseline),immediately before tourniquet deflation(T1),30 min (T2) and 60 min (T3) after tourniquet deflation.Serum IL-6 and TNF- α concentrations were measured by enzyme linked immunosorbentassay.ResaltsSerum TNF- α change at the same time point after tourniquet deflation was not statistically significant between two groups (P > 0.05).Serum IL-6 concentration was decreased at each time point after tourniquet deflation compared with T0 in research group,while increased in control group.Serum IL-6concentration difference of T3 and T0 had statistically significant between research group and control group [ (-8.8 ± 5.6) ng/L vs.( 10.2 ± 6.7) ng/L,P< 0.05].ConclusionsPHC in advance can decrease serum IL-6 concentration after tourniquet deflation in patients undergoing lower limb surgery.
5.Analysis of the perioperative management of 20 kidney transplant recipients in non-transplant surgery

ObjectiveTo summarize and analyse the perioperative management especially theanesthesia of 20 kidney transplant recipients in non-transplant surgery. MethodThe anesthesia management of 20 kidney transplant recipients in non-transplant surgery was analyzed retrospectively. ResultsIn 20 cases, 1 case (5%) was performed under local anesthesia,4 cases (20%) were performed under intravertebral anesthesia and 15 cases (75%) were performed under general anesthesia. The operation time was 30-260 min, all cases were managed successfully. ConclusionIt is still a clinical challenge to deal with the surgical patients after kidney transplantation, and it needs fully understanding of the pathophysiological status of the patient and closely collaboration of transplant physicians, anesthesiologists and the surgeons.
6.Randomized controlled trial of palonosetron in preventing postoperative nausea and vomiting in gyneco-logical surgeries

Objective To compare the effects of a single injection of palonosetron and tropise-tron to prevent postoperative nausea and vomiting (PONV ) in gynecological surgeries. Methods Sixty patients undergoing elective major gynecological surgeries with general anesthesia (Apfel score ≥ 3 ) were included and randomized to group P (palonosetron ) and group T (tropisetron),30 patients in each group.All patients received general anesthesia with tracheal intuba-tion,and palonosetron 0.25 mg or tropisetron 5 mg were injected before anesthesia respectively in two groups.Intravenous hydromorphine was delivered for postoperative analgesia in all patients. PONV were evaluated and followed up for 72 hours.The degree of PONV was recorded and the com-plete response rate (CR)and complete control rate (CC)were calculated.Results The degree of PONV in 0-24 h and 24-48 h was milder in group P than in group T (P <0.05),while in 48-72 h the degree of PONV was similar between the two groups.In group P,5 patients had vomiting postopera-tively with the failure period of treatment of (1 9.6±9.4)h,and no patients needed rescue treatments. While in group T,1 9 patients suffered from vomiting with the failure period of treatment of (20.6± 4.5)h,and rescue treatments were delivered 3 times in total.The CR and CC of preventing PONV in 0-24 h,24-48 h and 0-72 h were significantly higher in group P than in group T (P <0.05).The CR and CC of the two groups were comparable in 48-72 h postoperatively.Conclusion A preoperative single dose of palonosetron 0.25 mg is better than tropisetron 5 mg in preventing PONV within 48hours after gynecological surgery.

AIM: To study the effects of Bingcha Embolus on the proliferation, apoptosis and cell cycle of astrocytes in vitro in terms of serum pharmacology. METHODS: The cerebral cortex astrocytes of neonatal Sprague-Dawley rats were cultivated in vitro, and the purified astrocytes were randomly devided into Bingcha Embolus groups (high dose, middle dose, low dose) and control group after being identified by means of immunocytochemical method. After the incubation of 48 h with sera, MTT was employed as a proper method to detect the proliferation of astrocytes. The ratio of apoptosis and different cell cycle phases were measured by Flow Cytometry. 8.Best time of mesenchymal stem cells transplantation for treatment of myocardial infarction in aged rabbits

Objective To investigate the best time of the mesenchymal stem cells(MSCs) transplantation after myocardial infarction(MI),in order to provide prophase experimental basis for treatment of the diseases correlating to cadiocyte necrosis by cells transplantation.Methods Thirty New Zealand big-ears albino rabbits,which aging were older than or equal to 36 months were selected(a 36-month rabbit is the advanced age rabbit,equaling a person 60 years old).Model of MI was made by ligating anterior descending branch.The model rabbits were randomly divided into six groups(control group,1 d group,1-week group,2-week group,3-week group;and 4-week group,n=5).200 ?L cells after marked were drawed and injected in multiple positions of myocardium after MI in 1 d group,1-week group,2-week group,3-week group and 4-week group separately.The rabbits in control group were injected with saline of equal quantity.The cardiac functional parameters,blood flow rate of atrioventricular valve and the hemodynamics at eighth week in various groups were measured.Results Compared with control group,the treatment of MSCs transplantation for MI improved the heart function.The cardiac functional parameters,blood flow rate of atrioventricular valve and the hemodynamics in 1 d group was higher than those in control group(P0.05).The cardiac functional parameters,blood flow rate of atrioventricular valve and the hemodynamics in 1-week,2-weeks and 3-weeks groups were better than those in 1 d group(P0.05).Conclusion In the condition of the same cell-density but the different time of MI,the transplantation time window is 1 d-3 weeks after MI,which is advantageous to the amelioration of heart function,the best time of transplantation was 1-3 weeks after MI.
9.Studies on the mechanism of the Arg327Ile mutant causing defect in the function of factor Ⅸ

Objective To investigate function of Arg327Ile (R327I) and Arg327Ala(R327A) FⅨ mutants and to study the molecular pathogenesis of haemophilia B(HB) caused by R3271 mutation.Methods Hygromycin-resistant cell line was screened and the secretion of FⅨ antigen into the medium was measured by ELISA.The cell line with appropriate expression levels of F Ⅸ antigen was selected for culture.Recombinant F Ⅸ (rF Ⅸ ) was purified from concentrated medium by two step methods of Q-Sepharose Fast Flow and anion exchange chromatography.The concentration and purity of rF Ⅸ were determined by ELISA and SDS-PAGE,respectively.The activation of wild-type ( WT),R327I and R327A of rFⅨ by FⅦa/TF/Ca2+ or FⅪa/Ca2+ was identified by Western blot in different time periods.The FⅨa and FⅧa complex formed by interaction with different concentrations of FⅧa was used to activate F X,the apparent dissociation constant (Kd) for FⅧa binding was calculated by the kinetic results.The kinetic data of the activation of FX by WT,R327I and R327A FⅨa with or without FⅧa were calculated.Results The amount of WT,R327I and R327A rFⅨ were 450,210,64 μg,and the purity of rFⅨ was confirmed by SDSPAGE.Both R3271 and R327A could be normally activated by FⅧa/TF/Ca2+ or FⅪa/Ca2+.Kd for FⅧa binding showed that the binding capacities of R327I and R327A were 4 and 5 times lower than WT,respectively.The catalytic efficiencies of R327I and R327A F Ⅸ a for F X were 6 and 8 times lower with FⅧa,and 3 and 7.4 times lower without F Ⅷ a,respectively.Conclusions R327I and R327A rF Ⅸ mutants impair their binding to the FⅧa.The site on R327 contributes to FⅧa binding.It is partly related to the activation of FX.The low FⅧa binding to R327I FⅨa may cause HB.
10.Study of molecular mechanisms of hereditary protein C deficiency in four Chinese families

Objective To investigate the phenotype, genotype and molecular mechanisms in four Chinese pedigrees with venous thrombosis caused by hereditary PC deficiency. Methods The plasma activity of PC: A, TPS: A and FPS: A of the probands and their family members were detected with chromogenic and coagulation assay. The antigen of PC and FPS were identified with ELISA. Thrombin generation tests were applied to indicate the coagulation status. All of the nine exons and intron-exon boundaries of PC gene and PS gene were amplified by PCR and directly sequenced for mutaiton investigation. Results Compound heterozygous mutations of L-34P, K150del and A209V with 36% of PC: A and 57% of PC: Ag were identified in proband 1. PC: A was 46% , PC: Ag was 64. 4% while TPS: A, FPS: A and FPS: Ag were 36% , 19.5% and 20.9% respectively in proband 2. Two independent heterozygous mutations of R147W in PC gene inherited from his mother and T519stop in PS gene inherited from his father were identified. The anticoagulant activity of Proband 2 and his parents were declined in thrombin generation assay. In proband with PS defeciency and his father, the inhibition of thrombin generation capacity was decreased with exogenous APC, while his mother did not have significant difference. In Proband 3, PC: A was 32% while PC: Ag was 48.42% . Two independent mutations of R147W and R178W in Exon 7 were detected. Compound heterozygous mutations of R178W and D255H,with 21% of PC : A and 18. 36% of PC: Ag were identified in the Proband 4. Conclusions Hereditary PC deficiency or combined PC and PS deficiency result in venous thrombosis in four Chinese families. Mutants of L-34P, A209V, R178W, R147W and D255H might be the molecular mechanisms of PC deficiency.
