1.The clinical application of comprehensive treatment and prognosis guide in children with febrile seizures caused by upper respiratory infections

Objective To investigate the effect of comprehensive treatment and prognosis guide in children with febrile seizures caused by upper respiratory infections.Methods 58 cases of children with febrile seizures caused by upper respiratory infections were divided into the two groups using a random number table method,and the control group received routine first aid and treatment,while the observation group took comprehensive treatment and prognosis guide.The clinical efficacy and recurrence were compared.Results The observation group's abatement of fever time,seizures disappear time and average length of stay was (30.15 ±2.78)h,(4.25 ±0.63)d and (6.51 ±0.52) d,which were significantly less than those of the control group [(45.43 ± 3.14) h,(7.11 ± 0.72) d and (10.02 ± 0.67) d] (t =10.21,9.62,10.30,all P < 0.05) ; The observation group's relapse rate was 13.79% after treatment,significantly lower than 31.03 % of the control group (x2 =11.23,P < 0.05).Conclusion The comprehensive treatment combined with prognosis guide has significant effect in the treatment of children with febrile seizures caused by upper respiratory infections,which can significantly improve clinical symptoms,reduce relapse after treatment.So it has a better clinical value.
2.Comparison of two surgical approaches used in elbow arthrolysis

Objective To compare the posterior midline approach and the combined lateral and medial approach usually used in elbow arthrolysis. Methods From January to December in 2009, 41 cases of elbow stiffness were treated and fully followed up by the same team in our hospital. The posterior midline approach was used in 21 cases, 15 males and 6 females, with an average age of 35.8 years (16 to 70 years). In this group, the total flexion-extension arc was < 30° in 12 cases, 31° to 60° in 7 cases, and 61° to 90° in 2 cases. Eight cases had ulnar symptoms. The combined lateral and medial approach was applied in 20 cases, 8 males and 12 females, with an average age of 38. 8 years (16 to 51 years). In this group, the total flexion-extension arc was < 30° in 11 cases, 31° to 60° in 7 cases, and 61° to 90° in 2 cases. Five cases had ulnar symptoms. Results The 41 cases were followed up for 4 to 15 months (average, 7.9 months). In the posterior approach group, the mean preoperative flexion-extension ROM (27.6°± 25.7°) was improved to the postoperative 111.4°± 25.6°, the mean preoperative rotation ROM ( 152.9°± 46. 9°) to the postoperative 168.1°± 19. 1°, and the mean preoperative MEPS score (65.5 ± 11.5) to the postoperative 95.7 ±6.4. In the combined approach group, the mean flexion-extension ROM was improved from 35.5°± 25.0° preoperatively to 116. 5°± 19.1° postoperatively, the mean rotation ROM from 138.0°±55.7° preoperatively to 148.5°± 45.6° postoperatively, and the mean MEPS score from 66. 3 ± 13.0 preoperatively to 97.3 ± 7.0 postoperatively. The differences were statistically significant between preoperation and postoperation in both groups ( P < 0. 05), but not statistically different between the 2 groups in the preoperative or postoperative values ( P > 0. 05). In the posterior approach group, 5 patients had huge hematoma, 3 had wound dehiscence at the posterior elbow, and the others all had mild hematoma. But in the combined approach group, no wound complications were noted. Conclusion The combined lateral and medial approach should be used as far as possible in the elbow arthrolysis to avoid the likely complications when the posterior midline approach is used.
3.The Protective Effects of Polypeptides from Chlamys farreri against UVB Radiation

Objective To study the protective effects of th e polypeptides isolated from Chlamys farreri(PCF)against ultraviolet B radiatins.Methods An oxidative damage model of HeLa epi thelial cell line irradiated by ultraviolet B(UVB 7.15?10 -5 J /cm 2 )was established.The HeLa cells were randomly divided into six groups:control group,UVB radiated groups(radiated control,0.5%PCF,1%PCF,2%PCF,1%Vit C).The enzymic activities of HeLa cells were tested by MTT methods,the apoptosis rate an d death rate of HeLa epithelial cells and the content of free calcium in those cells were tested by using flow cytometry(FCM).The contents of MDA and the activities of GSH-Px ,SOD,CAT in the supernatants of cells were determin ed by biochemical methods.Results The results indicated that polypept ides fromChlamys farreri could enhance the activities of GSH-Px,SOD,CA T and decrease the contents of MDAin the su pernatants of HeLa cells,increase t he concentration of cellular free calciumin the cells,and decrease the rates of apoptosis and cell death of the cells .Conclusions The PCF has the protective effects on the HeLa cells irradiated by ultraviolet B.The mechanism of th is effects of PCF may be mediated by e-liminating the free radicals,by increasing the activities of antioxidative enzymes to inhibit the oxidativ e damages caused by UVB .
4.Berberine-induced apoptosis in leukemia Molt-4 cell line and expression of NF-κB/p65 and Caspase-3

[Objective] To study the inhibition effect of berberine on cell proliferation in acute T lymphocytic leukemia cell line Molt-4,and its mechanisms.[Methods] The logarithmic growing cell lines were incubated with 0,10,100 μg/ml berberine for various hours.The apoptosis induced by berberine was detected by flow cytometry,eletrophoresis and microscopy.The protein pro-Caspase-3 and p65 were determined by Western bolt assay.[Results]Treated with berberine,Molt-4 cells were found with apoptosis bodies by microscopy.By flow cytometry,it was found that number of G2/M cells was increased and number of S cells was reduced.Necrosis band was found in DNA ladder electrophoresis.The expression of pro Caspase-3 was activated partly and p65 was down-regulated in Molt-4 cells treated with berberine.[Conclusion]Berberine can induce Molt-4 cells apoptosis, and NF-κB、Caspase-3 have been shown to regulate the apoptosis programme.
5.Determination of Paracetamol in Ganmaoqing by HPLC

Objective: To study the feasibility of the determination paracetamol in Gangmaoqing by HPLC. Methods: The paracetamol contents of the different samples of Ganmaoqing Tablets were determined by the dead stop titration and HPLC, respectively. Results: The dead stop titration was complex and the error was larger. Whereas HPLC was simple, quick with a good seperation and small error.Conclusion: HPLC is accurate and reliable and reproducible for the determination of paracetamol in Ganmaoqing Tablets.
6.Comparative genomic analysis of Salmonella type Ⅲsecreted candidate gene yiiG

Objective To study the distribution and evolution of yiiG, a Salmonella gene encoding a candidate type secretedsubstrate .Methods Salmonella genomes were comprehensively screened for yiiG distribution with se-quence alignment strategies .The evolutionary history of yiiG was traced .Comparative genomic analysis was per-formed to study the evolutionary mechanisms of yiiG gene acquisition , loss and duplication .RNA-seq data were combined to analyzing the correlation between yiiG and other virulence factors .A variety of bioinformatic tools were used for discovering the possible type Ⅲsecretion signals .Results yiiG distributed in S.enterica subsp.enterica but variable in other subspecies of S.enterica.No yiiG was found in S.bongori.Besides Salmonella, only a part of Shigella and E.coli strains were detected with yiiG homologs .The genomic locus of yiiG and its adjacency showed conservation among all Salmonella, E.coli and Shigella strains.In most of the serovars of S.enterica subsp.enteri-ca, there was a head-to-head tandem whole yiiG repeat sequence upstream the yiiG gene, which was renamed as yiiGRrc.RNA-seq analysis showed that yiiG gene expression level was highly correlated with T 3SS-related genes . Bioinformatic prediction also indicated the T 3SS effector signals in YiiG N-terminus.Conclusions yiiG represented an ancient genomic locus , which will be a hot spot where rearrangement events frequently happened .The function of yiiG could potentially be related with Salmonella virulence.Finally, a new protein-encoding gene (yiiGRrc) was newly identified that was closely related with yiiG, providing the target for further understanding the composition , function and function variation of yiiG gene family .
7.A method to predict clustered repeats in Salmonella genomes

Objective To develop a simple method identifying and illustrating clustered repeats in bacterial ge-nomes, and to observe the patterns of clustered repeats in Salmonella genomes.Methods Bacterial genomes were cut to be overlapped pieces of identical size with a sliding window strategy .Each piece of genome fragment was aligned against itself with BLAT integrated in PipMaker , which was further used to build collinearity figures . Collinearity figures were analyzed to identify the clustered repeats.Results With the new pipeline CRpred ( Clustered Repeat Predicter) , Salmonella typhimurium LT2 genome was screened, and in 151 clustered repeats were disclosed.Pattern analysis on these repeats indicated that there were five categories, including low-copy simple tandem repeats, high-copy simple tandem repeats, interspaced tandem repeats, reverse-complementary re-peats, and interspaced reverse-complementary repeats.Nine repeat regions in LT2 genome were discovered which could not be simply classified into the 5 categories defined above.Conclusions A new, simple and intuitive strategy is proposed to identify and show clustered repeats in genomes , providing clues for CRISPR , VNTR and oth-er repeat-related studies .
8.The clinical significance of serum brain natriuretic peptide in children with Kawasaki disease

Objective To study the serum levels of brain natriuretic peptide (BNP) and the correlation with the heart function in children with Kawasaki disease(KD), and to explore its clinical value for diagnosis of KD. Method A total of 43 children aged from 5 months to 8 years with mean age of (2.3 + 0.6) years with KD admitted from February 2007 to April 2009 were enrolled into this study as KD group, and patients with myocarditis, myocardiopathy, congenital heart disease and other primary heart disease were ruled out. Another 30 healthy children were taken as control health group. There were no significant differences in age and gender between two groups (P >0.05) .The serum levels of BNP were measured both in acute and recovery stages of KD by using ELISA. The serum levels of BNP in healthy children were measured randomly once. The left ventricular ejection fraction (LVEF), left ventricular shorten fraction ( LVSF), cardiac index (CI) and left ventricular inflow velocity through the mitral annulus (including E-velocity and A-velocity) were measured by using two-dimensional echocardiography in acute and recovery stages of KD. Data were analyzed with t -test and the linear regression analysis test. Results The serum level of BNP in acute stage was (517.26 + 213.40) ng/mL and was significantly higher than that in recovery stage (91.56 + 47.97) ng/mL, and higher than that in control group (91.56 + 47.97) ng/mL (P < 0.01). The levels of LVEF, LVSF and CI in the acute stage were significantly lower than those in the recoverystage ( P < 0.0%), but there was no significant difference in E/A between acute stage and recovery stage (P > 0.05). The BNP level had negative correlation with the levels of LVEF, LVSF and CI(r = -0.63, -0.52, and - 0.53, respectively, P < 0.05), but had no significant correlation with E/A (r = - 0.18, P > 0.05). Conclusions The serum levels of BNP increase significantly in the KD patients, and have negative correlation with the levels of LVEF, LVSF and CI. The detection of serum levels of BNP has an important significance for diagnosis of KD.
9.Diagnosis and treatment of elbow varus posteromedial rotational instability

Objective To report the early surgical outcomes of treating elbow varus posteromedialrotational instability (EVPRI),a pattern of traumatic elbow instability which has been recently described but incompletely understood. MethodsFrom December 2009 to April 2011,11 male patients with an EVPRI pattern were surgically treated in our hospital.Their average age was 33.8(from 22 to 40) years.They had 4 left and 7 right elbows affected.All had tenderness at the medial and lateral sides of the elbow and varus angulation of the elbow without dislocation.Their preoperative stress view X-rays and CT scans showed widened humeroradial joint space and fracture of the anteromedial facet of the coronoid process.We confirmed the diagnosis by applying yarus,pronated and axial stresses onto the forearm to evoke elbow dislocation,under fluoroscopy after anesthesia.In the initial operative treatment,the coronoid was repaired with a plate and K-wires applied to the medial surface of the coronoid,and a hinged external fixator was applied at the lateral side without repairing the lateral collateral ligament.Early rehabilitation was encouraged. Results All were followed up for an average of 14.4 (from 6 to 26) months.Each obtained an excellent result according to the Mayo Elbow Performance Index and recovered excellent elbow function.The average flexion was 137.8° ± 4.4° (from 130°to 140°), average extension 5.6°±7.3° (from 0 to 20°), average range of extension-flexion 132.2°±9.7°(from 120° to 140°),average pronation 87.8°±6.7°(from 70° to 90°),average supination 88.9° ± 3.3° (from 80° to 90°),and average range of rotation 176.7° ± 10.0° (from 150° to 180°).No complications such as varus subluxation of the elbow,infection and arthrosis occurred in this group. Conclusions Since EVPRI is a distinct type of elbow fracture-dislocation that must be recognized and adequately treated to restore good elbow function,inadequate or conservative treatment may cause subluxation,arthrosis or a poor outcome.Surgical treatment can achieve an excellent early outcome and avoid severe complications.
10.Treatment of Budd-Chiari syndrome

Objective To study the effect of various treatments for the Budd-Chiari syndrome (BCS). Methods The clinical data of different treatments in 260 BCS patients were analysed retrospectively. Results Of the 260 patients,273times of BCS underwent interventional treatment?operative treatment or the combination of operation and interventional therapy, including percutaneous transluminal angioplasty(PTA) or/and stenting of the inferior vena cava(IVC)in 149 cases (160 times), percutaneous transhepatic angioplasty or/and stenting of the hepatic venous in 15 cases (17 times ),the combination of the two methods in 10 cases,radical resection in room temperature in 8 cases, mesocaval shunt in 16 cases,mesojugular shunt in 5 cases,mesoatriumr shunt in 8 cases,IVC-atrium shunt in 6 cases,splenopneumoexy in 18 cases, mesocaval shunt combined with PTA and stenting of the IVC in 17 cases, and transcardic membranotomy combined with PTA and stenting of the IVC in 8 cases . Two patients died in the operation:One died of cardiac tamponade,and the another died of operrative hemorrhagic shock.During the follow-up period of 3 months~6 years,20 patients recurred,of whom 7 died of hepatic failure,the other patients recovered satisfacorily. Conclusions The interventional treatment is the first choice for BCS.For patients unable to get interventional treatments or failure for interventional treatments, operation or operation combined with interventional therapy should be considered.
