1.Effects of icariin-astragaloside IV-puerarin mixture on cognitive function and ferroptosis amino acid metabolism pathway in APP/PS1 HAMP-/-mice

AIM:To observe the effect of icariin-astragaloside Ⅳ-puerarin mixture(Yin-Huang-Ge mixture,YHG)on cognitive function and ferroptosis amino acid metabolism pathway in hepcidin(HAMP)knockout APPswe/PS1dE9(APP/PS1 HAMP-/-)mice.METHODS:The mice were divided into 7 groups:negative control(C57BL/6 mice)group,APP/PS1 group,APP/PS1 HAMP-/-group,APP/PS1+YHG group,APP/PS1 HAMP-/-+YHG group,APP/PS1+de-ferasirox(DFX)group,and APP/PS1 HAMP-/-+DFX group,with 6 mice in each group.The YHG and DFX were adminis-tered intragastrically,while the mice in C57 group,APP/PS1 group and APP/PS1 HAMP-/-group were given intragastric administration of distilled water,once a day for 2 months.The iron content in mouse brain tissues was detected by tissue iron kit.The morphological changes of the mitochondria in hippocampal neurons were observed by transmission electron microscopy.Morris water maze was used to detect the learning and memory ability of the mice.The content of neuronal nu-clear antigen(NeuN)in mouse brain tissues was detected by immunofluorescence staining.The expression of glutathione(GSH)in mouse brain tissues was detected by biochemical kit.The expression levels of glutamate-cysteine ligase catalytic subunit(GCLC)and glutamatase 2(GLS2)in mouse brain tissues were detected by Western blot.RESULTS:Compared with C57BL/6 mice,the brain iron content of APP/PS1 mice was significantly increased(P<0.01),the mitochondria were seriously damaged,the learning and memory ability was significantly decreased(P<0.05),the brain neurons were seri-ously damaged(P<0.01),and the expression levels of GSH,GCLC and GLS2 were significantly decreased(P<0.01).Compared with APP/PS1 mice,the brain iron content of APP/PS1 HAMP-/-mice was significantly increased(P<0.01),the mitochondria were seriously damaged,the learning and memory ability was significantly decreased(P<0.05),the brain neurons were seriously damaged(P<0.01),and the expression levels of GSH,GCLC and GLS2 were significantly decreased(P<0.05).After treatment with YHG and DFX,the brain iron content was significantly decreased(P<0.01),the mitochondrial damage was alleviated,the learning and memory ability was significantly increased(P<0.05),the brain neuron damage was alleviated(P<0.01),and the expression levels of GSH,GCLC and GLS2 were significantly increased(P<0.05).CONCLUSION:The YHG can improve the cognitive function of APP/PS1 HAMP-/-mice,and its mechanism may be related to the regulation of ferroptosis amino acid metabolism and the enhancement of antioxidant capacity.

Temporomandibular joint (TMJ) diseases are common clinical conditions. The number of patients with TMJ diseases is large, and the etiology, epidemiology, disease spectrum, and treatment of the disease remain controversial and unknown. To understand and master the current situation of the occurrence, development and prevention of TMJ diseases, as well as to identify the patterns in etiology, incidence, drug sensitivity, and prognosis is crucial for alleviating patients′suffering.This will facilitate in-depth medical research, effective disease prevention measures, and the formulation of corresponding health policies. Cohort construction and research has an irreplaceable role in precise disease prevention and significant improvement in diagnosis and treatment levels. Large-scale cohort studies are needed to explore the relationship between potential risk factors and outcomes of TMJ diseases, and to observe disease prognoses through long-term follw-ups. The consensus aims to establish a standard conceptual frame work for a cohort study on patients with TMJ disease while providing ideas for cohort data standards to this condition. TMJ disease cohort data consists of both common data standards applicable to all specific disease cohorts as well as disease-specific data standards. Common data were available for each specific disease cohort. By integrating different cohort research resources, standard problems or study variables can be unified. Long-term follow-up can be performed using consistent definitions and criteria across different projects for better core data collection. It is hoped that this consensus will be facilitate the development cohort studies of TMJ diseases.
3.Grain-sized moxibustion inhibits the progression of Alzheimer disease in 5XFAD transgenic mice

Objective: To investigate whether grain-sized moxibustion at Xinshu (BL15) and Shenshu (BL23) can alleviate cognitive decline and other pathologic features in early-stage Alzheimer disease (AD) using transgenic mice with 5 familial AD mutations (5XFAD). Methods: The genotype of transgenic mice was detected by polymerase chain reaction. A total of 40 transgenic mice (1.5 months old) were randomly and equally allocated to an AD model group (5XFAD group) or a grain-sized moxibustion group (5XFAD + GM group), with 20 wild-type (WT) mice (C57BL/6J) serving as the normal control group (WT group). Mice in the 5XFAD + GM group were treated by grain-sized moxibustion at bilateral Xinshu (BL15) and Shenshu (BL23). Mice in the WT group and 5XFAD group received no treatment but were restrained to ensure exposure to a similar experimental condition. Cognitive function and memory were assessed with the Morris water maze and Y-maze tests. The amyloid β 40 (Aβ40) and amyloid β 42 (Aβ42) levels in the brain were evaluated by enzyme-linked immunosorbent assay; amyloid plaque deposition in brain tissue sections was detected by thioflavin-S staining; the expression of glial fibrillary acidic protein (GFAP), cluster of differentiation 11b (CD11b), brain-derived neurotrophic factor (BDNF), and choline acetyltransferase (ChAT) in the hippocampus and prefrontal cortex was analyzed by immunohistochemistry. Results: In the Morris water maze test, compared with the 5XFAD group, mice in the 5XFAD + GM group had a shorter escape latency and more target area crossings and spent more time in the target quadrant (P<0.05). In the Y-maze test, compared with the 5XFAD group, the number of training times of the 5XFAD + GM group was significantly decreased (P<0.05), together with more correct responses (P<0.05). Compared with the 5XFAD group, the levels of Aβ40 and Aβ42 in the brain tissue of the 5XFAD + GM group were significantly lower (P<0.05); in the hippocampus and prefrontal cortex, the total number of amyloid β plaque deposition were significantly lower (P<0.05); the expression levels of GFAP and CD11b were significantly reduced (P<0.05); and the expression levels of ChAT and BDNF were significantly increased (P<0.05).Conclusion: Grain-sized moxibustion at Xinshu (BL15) and Shenshu (BL23) greatly improves learning and memory functions, decreases the levels of Aβ40 and Aβ42, inhibits amyloid β plaque deposition, decreases the expression of GFAP and CD11b, and increases the expression of ChAT and BDNF in AD mice to inhibit the progression of AD.

Objective:In general, patients with seropositive rheumatoid arthritis (RA) are considered to show an aggressive disease course. However, the relationship between the two subgroups in disease severity is controversial. Our study is aimed to compare the clinical characteristics and prognosis of double-seropositive and seronegative RA in China through a real-world large scale study.Methods:RA patients who met the 1987 American College of Rheumatology (ACR) classification criteria or the 2010 ACR/European Anti-Rheumatism Alliance RA classification criteria, and who attended the 10 hospitals across the country from September 2015 to January 2020, were enrolled. According to the serological status, patients were divided into 4 subgroups [rheumatoid factor (RF)(-) anti-cyclic citrullinated peptide (CCP) antibody (-), RF(+), RF(+) anti-CCP antibody(+), anti-CCP antibody(+)] and compared the disease characteristics and treatment response. One-way analysis of variance was used for measurement data that conformed to normal distribution, Kruskal-Wallis H test was used for measurement data that did not conform to normal distribution; paired t test was used for comparison before and after treatment within the group if the data was normally distributed else paired rank sum test was used; χ2 test was used for count data. Results:① A total of 2 461 patients were included, including 1 813 RF(+) anti-CCP antibody(+) patients (73.67%), 129 RF(+) patients (5.24%), 245 RF(-) anti-CCP antibody(-) patients (9.96%), 74 anti-CCP antibody(+) patients (11.13%). ② Regardless of the CCP status, RF(+) patients had an early age of onset [RF(-) anti-CCP antibody(-) (51±14) years old, anti-CCP antibody(+) (50±15) years old, RF(+) anti-CCP antibody(+) (48±14) years old, RF(+)(48±13) years old, F=3.003, P=0.029], longer disease duration [RF(-) anti-CCP antibody(-) 50 (20, 126) months, anti-CCP antibody(+) 60(24, 150) months, RF(+) anti-CCP antibody(+) 89(35, 179) months, RF(+) 83(25, 160) months, H=22.001, P<0.01], more joint swelling counts (SJC) [RF(-) anti-CCP antibody(-) 2(0, 6), Anti-CCP antibody(+) 2(0, 5), RF(+) anti-CCP antibody(+) 2(0, 7), RF(+) 2(0, 6), H=8.939, P=0.03] and tender joint counts (TJC) [RF(-) anti-CCP antibody(-) 3(0, 8), anti-CCP antibody(+) 2(0, 6), RF(+) anti-CCP antibody(+) 3(1, 9), RF(+) 2(0, 8), H=11.341, P=0.01] and the morning stiff time was longer [RF(-) anti-CCP antibody(-) 30(0, 60) min, anti-CCP antibody(+) 20(0, 60) min, RF(+) anti-CCP antibody(+) 30(10, 60) min, RF(+) 30(10, 60) min, H=13.32, P<0.01]; ESR [RF(-) anti-CCP antibody(-) 17(9, 38) mm/1 h, anti-CCP antibody(+) 20(10, 35) mm/1 h, RF(+) anti-CCP antibody(+) 26(14, 45) mm/1 h, RF(+) 28(14, 50) mm/1 h, H=37.084, P<0.01] and CRP [RF(-) anti-CCP antibody(-) 2.3 (0.8, 15.9) mm/L, Anti-CCP antibody(+) 2.7(0.7, 12.1) mm/L, RF(+) anti-CCP antibody(+) 5.2(1.3, 17.2) mm/L, RF (+) 5.2(0.9, 16.2) mm/L, H=22.141, P<0.01] of the RF(+)patients were significantly higher than RF(-) patients, and RF(+) patients had higher disease severity(DAS28-ESR) [RF(-) anti-CCP antibody(-) (4.0±1.8), anti-CCP antibody(+) (3.8±1.6), RF(+) anti-CCP antibody(+) (4.3±1.8), RF(+) (4.1±1.7), F=7.269, P<0.01]. ③ The RF(+) anti-CCP antibody(+) patients were divided into 4 subgroups, and it was found that RF-H anti-CCP antibody-L patients had higher disease severity [RF-H anti-CCP antibody-H 4.3(2.9, 5.6), RF-L anti-CCP antibody-L 4.5(3.0, 5.7), RF-H anti-CCP antibody-L 4.9(3.1, 6.2), RF-L anti-CCP antibody-H 2.8(1.8, 3.9), H=20.374, P<0.01]. ④ After 3-month follow up, the clinical characteristics of the four groups were improved, but there was no significant difference in the improvement of the four groups, indicating that the RF and anti-CCP antibody status did not affect the remission within 3 months. Conclusion:Among RA patients, the disease activity of RA patients is closely related to RF and the RF(+) patients have more severe disease than RF(-) patients. Patients with higher RF titer also have more severe disease than that of patients with low RF titer. After 3 months of medication treatment, the antibody status does not affect the disease remission rate.
5.Serum IL-32 level in children with bacterial pneumonia and its relationship with disease severity

Objective:To investigate the serum interleukin-32(IL-32) level in children with bacterial pneumonia and its relationship with the severity of the disease.Methods:From January 2019 to December 2019, 300 children with bacterial pneumonia in Taizhou Cancer Hospital were selected as the pneumonia group, and 300 healthy children in our hospital were selected as control group.The serum levels of IL-32, IL-6, IL-8 and tumor necrosis factor-α(TNF-α) were measured by double antibody sandwich enzyme-linked immunosorbent assay.Results:The serum levels of IL-32[(212.46±16.74)ng/L], IL-6[(0.29±0.06)μg/L], IL-8[(0.41±0.08)μg/L] and TNF-α[(1.15±0.11)μg/L] in the pneumonia group were higher than those in the control group[(116.51±14.52)ng/L, (0.07±0.02)μg/L, (0.09±0.03)μg/L, (0.14±0.03)μg/L](all P<0.05). The serum levels of IL-32[(128.76±15.42)ng/L], IL-6[(0.15±0.04)μg/L], IL-8[(0.16±0.05)μg/L] and TNF-α[(0.29±0.06)μg/L] in children with pneumonia during recovery were lower than those in early stage[(212.46±16.74)ng/L, (0.29±0.06)μg/L, (0.41±0.08)μg/L, (1.15±0.11)μg/L](all P<0.05). The levels of serum IL-32[(232.16±16.02)ng/L], IL-6[(0.37±0.08)μg/L], IL-8[(0.54±0.09)μg/L], TNF-α[(1.42±0.15)μg/L] in the severe pneumonia group were higher than those in the non-severe pneumonia group[(184.26±16.31)ng/L, (0.23±0.05)μg/L, (0.26±0.06)μg/L, (0.79±0.12)μg/L](all P<0.05). There were positive correlation between serum IL-32 level and IL-6( r=0.602), IL-8( r=0.624), TNF-α( r=0.593) levels in children with pneumonia(all P<0.05). Conclusion:The serum IL-32 level is elevated in children with bacterial pneumonia.The serum IL-32 level can reflect the therapeutic effect, severity and inflammatory response of children with bacterial pneumonia.
6.Effect of ADOPT-based individualized nursing on mental health and adaptive ability of patients undergoing orthognathic surgery

Objective:To explore the effect of individualized nursing based on the ADOPT problem solving model on the mental health and adaptive ability of patients undergoing orthognathic surgery.Methods:150 patients undergoing orthognathic surgery from January 2017 to April 2019 in the Oral and Maxillofacial Surgery of the First Affiliated Hospital of Zhengzhou University was selected as subjects by convenient sampling, they were divided into observation group and control group with 75 cases each. The mental health of patients before the intervention was surveyed and compared with the domestic adult norms. The control group received routine inpatient care, and the observation group received ADOPT model individualized care. Intervention was continuous from admission to 3-month follow-up. Symptom Check List-90 (SCL-90) and Social Psychological Adjustment Scale (SPAS) were performed to evaluate and compare psychological health symptoms and adaptability in both groups.Results:The scores of the five dimensions of obsessive-compulsive symptoms, interpersonal sensitivity, anxiety, hostility, and paranoia on the SCL-90 of 150 patients before the intervention were higher than those of domestic adult norms, the differences were statistically significant ( P<0.05) . After the intervention, the obsessive-compulsive symptoms, interpersonal sensitivity, anxiety, hostility, and paranoia in the observation group were lower than those in the control group with statistically significant differences ( P<0.05) . The dimissions scores and total scores of SPAS of the observation group were higher than those of the control group, and the differences were statistically significant ( P<0.05) . Conclusions:The application of individualized nursing based on ADOPT problem-solving model in patients undergoing orthognathic treatment can improve their mental health, especially obsessive-compulsive symptoms, interpersonal sensitivity, anxiety, hostility, and paranoia.
7.Effect of losartan and irbesartan on blood pressure and ISI in female hypertension patients complicated with hyperuricemia

Objective To compare the effects of losartan and irbesartan on blood pressure,blood uric acid,insulin sensitivity and insulin resistance in female hypertensive patients complicated with hyperuricemia.Methods From August 2015 to December 2017,100 females of hypertension complicated with hyperuricemia who hospitalized in the Third People's Hospital of Ningxia were enrolled in the study.All the patients were divided into two groups according to the random digital table,with 50 cases in each group.The observation group was treated with losartan,and the control group was treated with irbesartan for 8 weeks.The total effective rate and adverse reactions were compared between the two groups.Also the changes of blood pressure,serum uric acid,fasting glucose (FBG),fasting insulin (FINS),high sensitivity C-reactive protein (hs-CRP),insulin sensitivity index (ISI) and insulin resistance index (HOMA-IR) were compared between the two groups after treatment.Results There was no statistically significant difference in the total effective rate between the two groups [92.0% (46/50) vs.90.0% (45/50)] (P > 0.05).Before treatment,there were no statistically significant differences in blood pressure,serum uric acid,FBG,FINS,hs-CRP,ISI and HOMA-IR between the two groups (all P > 0.05).After treatment,the systolic blood pressure and diastolic blood pressure in the two groups were (133.09 ± 10.11) mmHg vs.(131.54 ± 11.01) mmHg and (82.76 ± 6.23) mmHg vs.(83.75 ± 6.88) mmHg,which were lower than those before treatment (observation group:t =19.742,10.606,control group:t =18.925,-9.956,all P < 0.05).But there were no statistically significant differences between the two groups (all P > 0.05).After treatment,the serum uric acid in the observation group was lower than that in the control group [(387.21 ± 25.56) μmol/L vs.(429.67 ± 27.44) μmol/L] (t =8.006,P < 0.05).The hs-CRP,FINS,HOMA-IR,ISI in the observation group were (4.92 ± 1.02) rmg/L,(15.92 ± 3.01) mU/L,(1.71 ± 0.24),(1.047 ± 0.095),which in the control group were (4.54 ± 1.00) mg/L,(17.23 ± 3.20) mU/L,(1.65 ± 0.27),(1.140 ± 0.083).After treatment,the hs-CRP,FINS,HOMA-IR in the two groups were decreased (all P < 0.05),while ISI was increased (P < 0.05).Furthermore,the improvement of FINS,ISI and HOMA-IR in the observation group was better than those in the control group (t =2.109,-5.213,3.191,all P < 0.05).Conclusion Both losartan and irbesartan can improve clinical symptoms,lower blood pressure and improve insulin resistance in female hypertension patients complicated with hyperuricemia,and losartan is more effective than irbesartan.
8.Influence of graphic type health education combined with empowerment theory on phonetic function and swallowing function in tongue cancer patients after surgery

Objective To evaluate the influence of graphic type health education combined with empowerment theory on phonetic function and swallowing function in tongue cancer patients after surgery. Methods A total of 86 tongue cancer patients after surgery in the First Affiliated Hospital of Zhengzhou University from June 2014 to June 2017 were selected by convenience sampling method, and divided into control group and experimental group, with 43 cases in each group. The control group received graphic type health education, while the experimental group received graphic type health education combined with empowerment theory. The phonetic function and swallowing function were collected and compared before and after intervention between two groups. Results Before the intervention, there was no statistical significance in the difference of patients' phonetic function between the two groups (P>0.05). However, after the intervention, the phonetic function improved than before, and it in the experimental group was higher than that in the control group (P<0.05). The difference in swallowing function between the two groups was not statistically significant before the intervention (P> 0.05). After the intervention, swallowing function of the two groups were higher than those before intervention, and the experimental group was higher than that of the control group (P<0.05). Conclusions Graphic type health education combined with empowerment theory is helpful for the recovery of phonetic function and swallowing function in tongue cancer patients after surgery,it is worthy of clinical promotion.
9.Analysis of clinical characteristics of 26 patients with melioiclosis spleen abscesses in Hainan province

Objective To analyze the clinical features of melioidosis spleen abscess.Methods A retrospective analysis of 26 cases with melioidosis spleen abscess at Hainan People′s Hospital between October 2002 and March 2015 was performed.Data with general conditions, clinical characteristics, imaging features, treatment and outcome were all extracted and analyzed.Results High fever (100.0%) and splenomegaly (73.1%) were the main clinical symptoms and signs.Laboratory examinations showed that white blood cell and neutrophil percentage increased, with a mean value of 12.6×10.9/L and 0.86, respectively.88.5% cases had diabetes mellitus which was the main underlying disease.Apart from spleen abscess, pulmonary infection (73.1%) and liver abscess (38.5%) were the majority.Imaging showed multiple small abscess (92.3%).All patients had at least one positive blood culture of Burkholderia pseudomalleiand were treated with imipenem, ceftazidime and sulfamethoxazole alone or in combination.Hospitalization days ranged between 8 to 65 days with the mean of 29.5 days.Twenty-three cases (88.5%) were cured, the remaining 3 cases (11.5%) abandoned treatment and were all lost for follow-up.Conclusions Melioidosis should be suspected in patients from epidemic areas, who have diabetes and imaging shows spleen abscess and other organ infections including pulmonary infection and liver abscess.Microbiological culture is needed immediately.
10.Effect of isoflurane on expression of hippocampal ubiquitin C-terminal hydrolase-L1 mRNA in aged rats

Objective To evaluate the effect of isoflurane on the expression of hippocampal ubiq-uitin C-terminal hydrolase-L1(UCH-L1)mRNA in aged rats. Methods Thirty-six pathogen-free healthy male Sprague-Dawley rats, aged 20 months, weighing 500-700 g, were divided into 2 groups using a ran-dom number table: control group(group C, n=12)and isoflurane group(group I, n=24). In group I, 2% isoflurane was continuously inhaled for 2 h. Twelve rats were selected at 24 and 72 h after stopping isoflurane inhalation(T1,2), and Y-maze test was performed to evaluate cognitive function. The animals were sacrificed after the end of Y-maze test and their hippocampi were removed for determination of UCH-L1 expression(by immuno-histochemistry)and UCH-L1 mRNA expression(by real-time polymerase chain reaction). Results Compared with group C, the total number of training achieving the standard was sig-nificantly increased at T1(P<001 or 005), and the expression of UCH-L1 mRNA was down-regulated, and no significant change was found at T2in group I(P>005). Conclusion The mechanism by which isoflurane induces cognitive decline may be related to down-regulating the expression of UCH-L1 mRNA in hippocampal tissues of aged rats.
