1.Application of psychological intervention on anxiety of patients with obstructive jaundice

Objective To investigate the influence of psychological intervention on the anxiety of patients with obstructive jaundice and supply theoretical reference for psychological evaluation and nutsing.Methods 129 patients with obstructive jaundice was divided into the intervention group(64 cases)and the control group(65 cases).The control group adopted routine treatraent method such as anti-infection,liver protection treatment,enhancement of coagulation mechanism and supporting treatment.The intervention group received psychological nursing,relaxation training and distraction of attention based upon routine nursing.The degree of anxiety and pruritus was obseved and recorded in the two groups.Results The degree of anxiety in the intervention group was evidently lower than that of the control group(P<0.01).The degree of pruritus in the intervention group was 18.75%,which was statistically different from that of the control group(35.38%),P<0.05.Condusion Adoption of psychological intervention could effectively reduce the degree of anxiety and pruritus of patients with obstructive jaundice.
2.Long-term continuous maintenance of peripherally inserted central catheter in one patient

Objective To summarize our experiences in the long-term continuous maintenance of the peripherally inserted central catheter (PICC) in a patient with intestinal fistula.Method The systematic holistic nursing care has been provided according to the nursing practices to a patient with intestinal fistula since 2006,during which eitht PICCs were used successively.Results The eight PICCs were placed for 3-14 months.The 1 st,3rd,5th,and 7th cathers were retained for 12,13,11,and 14 months.The second catheter was retained for 6 months,and then removed due to increased body temperature (38.7℃).The 4th cather was retained for 3 months,then catheter-related bloodstream infection was suspected due to high body temperature (39℃) ; culture for fungi at the tip showed positive results.The 6th catheter was retained for 7 months,inserting from jugular vein to the superior vena cava; however,it was removed due to puncture point local purulent changes at the puncture site.The 8th catheter was applied for 8 months,but was rmoved due to tube occlusion,which responded poorly to urokinase.Conclusion The application of PICC in patients with fistula can relieve the pain and improve the quality of life.
3.Effect of nursing project management on reducing the incidence of perioperative pressure injury during laparoscopic pancreaticoduodenectomy

Objective To explore the effect of nursing project management on reducing the incidence of perioperative pressure injury during laparoscopic pancreaticoduodenectomy. Methods The pressure ulcers conditions of 41 cases undergoing laparoscopic resection of pancreatic duodenal from December 2014 to July 2015 were investigated to find out the causes of the incidence of pressure ulcers, and we made pertinent measures for continuous quality improvement. From August 2015 to March 2016, 45 patients underwent laparoscopic pancreatoduodenectomy received nursing project management nursing care. Two groups of patients in terms of the incidence of pressure ulcers, pressure ulcer risk assessment accuracy and implementation rate of patient skin specification handover were compared. Results The incidences of laparoscopic resection of pancreatic duodenal ulcer decreased from 24.39% before the improvement of the project to 2.22%, the accuracy of nurse on the laparoscopic resection of pancreatic duodenal ulcer risk assessment increased from 75.60% to 95.55%, the implementation rate of patient skin specification handover increased from 82.93% to 97.78%, the differences were statistically significant (P<0.05). Conclusions The implementation of nursing program can reduce the incidence of pressure injury and improve the quality of nursing in perioperative period of laparoscopic pancreaticoduodenectomy.
4.Application of comfort clinostatism in radiofrequency ablation of patients with liver cancer

Objective To study the effect of the intervention procedure of comfort clinostatism on the related complications and feelings of patients with liver cancer who underwent RFA .Methods The patients were divided into intervention group (448 cases) and control group (264 cases).The control group patients who received treatment from June 2012 to December 2013 were treated without nursing intervention , and doctors designed the clinostatism and accomplished the treatment.The intervention group patients who received treatment from January 2014 to June 2015 were treated with nursing intervention .The special nurse would accomplish the treatment beyond the procedure of comfort clinostatism.The complications were observed and recorded.Psychological feeling were evaluated by the patients themselves.The charge nurse would tabulated data .Results In the intervention group , the degrees of the data significantly were improved which consisted of psychological anxiety , the degree of psychological anxiety , the length of time to maintain the same posture , and the incidence of complications . Besides, the overall satisfaction of patients was significantly improved (P<0.01).Conclusions The process oriented, standardizing and hommizational procedure of the comfortable clinostatism can effectively avoid the common complications , enhance the relationship between doctors and patients , and raise the overall satisfaction of patients.
