1.Identification of the metabolites of Sinisan extract in rat plasma, urine, feces and bile after intragastric administration.

Sinisan is a widely used traditional Chinese medicine (TCM) in treating various diseases; however, the in vivo metabolic profile of its multiple components remains unknown. In this paper, ultra-high performance liquid chromatography-quadrupole time-of-flight mass spectrometry (UPLC-Q-TOF/MS) was applied to identify the metabolites of Sinisan extract in rat plasma, urine, feces and bile after intragastric administration. Using MS(E) and mass defect filter techniques, 41 metabolites of 10 parent compounds (naringin, naringenin, hesperidin, neohesperidin, liquiritin, liquiritigenin, glycyrrhizic acid, glycyrrhetinic acid, saikosaponin a and saikosaponin d) were detected and tentatively identified. It was shown by our results that these compounds was metabolized to the forms of hydroxylation, glucuronidation, sulfation, glucuronidation with sulfation and glucuronidation with hydroxylation in vivo.
2.Metabolism of naringin produced by intestinal bacteria.

Naringin has been reported to possess a wild range of biological activities. However, the route and metabolites of naringin produced by intestinal bacteria are not well understood. In this paper, different bacteria were isolated from human feces and their abilities to convert naringin to different metabolites were studied. Ultra performance liquid chromatography-quadrupole time-of-flight mass spectrometry (UPLC-Q-TOF/MS) with automated data analysis software (MetaboLynx) was applied to fast analysis of naringin metabolites. Using MSE and mass defect filter techniques, three metabolites were detected and tentatively identified. The results indicated that acetylation, hydrolyzation and hydrolyzation with hydrogenation were the major metabolic pathways of naringin in vitro. Then, we studied the gene sequence of the 16S rRNA of the bacteria by extraction of genomic DNA of the strain, PCR amplification and clone of the 16S rRNA. The consequence proved that Enterococcus sp.30, Bacillus sp.46, Escherichia sp.54 and Escherichia sp.63 have the peculiar metabolism characteristic of naringin.
3.Study on curative effect of assisted surgical treatment based on 3D printing technology on complex Pilon fractures

Objective:To explore the curative effect of assisted surgery based on 3D printing technology on complex Pilon fractures.Methods:From June 2017 to June 2018, 52 patients with complex Pilon fracture treated in the General Hospital of Armed Police Navy were selected in the study.The patients were divided into study group ( n=26) and control group ( n=26) according to different operation mode.The control group was treated with traditional open reduction and internal fixation, and the study group was supplemented with 3D printing technology.The differences of treatment effect, operation time, intraoperative hemorrhage, fracture healing time, hospitalization time, incidence of complications, ankle function score and VAS pain score between the two groups were compared. Results:The total effective rate of the study group was 88.5%(23/26), which was higher than 65.4%(17/23) of the control group (χ 2=3.900, P=0.048). The mean operation time ( t=-3.770, P<0.001), mean fracture healing time ( t=-2.206, P=0.032) and mean hospitalization time ( t=-9.542, P<0.001) of the study group were shorter than those of the control group, and the amount of intraoperative bleeding ( t=-20.226, P<0.001) was less than that of the control group, the differences were statistically significant.The total incidence of complications in the study group was 15.4%(4/23), which was lower than 42.3%(11/23) in the control group (χ 2=4.591, P=0.032). With the development of time, the ankle function score of the two groups increased gradually, and the VAS pain score decreased gradually ( P<0.05). One month after operation, the ankle function score of the study group was higher than that of the control group ( t=2.340, P=0.023), but there was no statistically significant difference between the two groups in VAS pain score ( t=-0.278, P=0.782). At the last follow-up, the ankle function score of the study group was higher than that of the control group( t=2.760, P=0.008), and the VAS pain score of the study group was lower than that of the control group( t=-3.633, P<0.001), the differences were statistically significant. Conclusion:3D printing assisted surgery is better than traditional open reduction and internal fixation in the treatment of complex Pilon fracture, with shorter operation time, fracture healing time and hospitalization time, less intraoperative bleeding, lower incidence of complications and better postoperative recovery.
4.Correlation of serum growth differentiation factor-15 with coronary slow flow phenomenon and atherosclerosis

Objective To investigate the correlation of serum growth differentiation factor-15(GDF-15)with coronary slow flow phenomenon(CSFP)and coronary atherosclerosis(AS).Meth-ods A total of 190 patients undergoing first-time coronary angiography in our hospital from January 2019 to June 2022 were recruited,and then according to definite diagnosis,divided into CSFP group(n=60),AS group(n=70)and NC group(normal coronary flow,n=60).Their gen-eral data,risk factors for coronary heart disease,clinical biochemical indexes and electrocardio-gram were collected.The serum GDF15 level was measured by ELISA.Results Age(OR=1.065,95%CI:1.014-1.119,P=0.021),smoking history(OR=0.330,95%CI:0.132-0.823,P=0.001)and GDF-15(OR=1.006,95%CI:1.003-1.009,P=0.018)were independent influencing factors of AS.Age(OR=0.956,95%CI:0.926-0.988,P=0.024)and GDF-15(OR=1.003,95%CI:1.000-1.006,P=0.031)were independent influencing factors of CSFP.The GDF-15 level was significantly higher in the moderate or severe AS group than the CSFP group and the mild AS group(867.02±222.82 ng/L vs 568.21±163.03 ng/L and 635.41±214.95 ng/L,P<0.01).Serum GDF-15 level was positively correlated with hs-CRP level(r=0.228,P=0.014).Conclusion GDF-15 is highly expressed in the patients with CSFP and with AS.With the increase of GDF-15 level,the severe the degree of AS gradually.GDF-15 is highly correlated with hs-CRP.
5.Study on metabolites of ferulic acid and gallic acid compatible with danggui and chishao in rat plasma and urine.

OBJECTIVETo study the major metabolites of ferulic acid and gallic acid compatible with Danggui Chishaoyao in rat plasma and urine.
METHODThe metabolites of ferulic acid and gallic acid in rat plasma and urine were analyzed after oral administration of compatible Danggui Chishaoyao using UPLC-Q-TOF-MS.
RESULTOn the basis of the mass information, it was inferred that in vivo metabolites of ferulic acid were be in the form of methylation products, sulfate conjugation products and glucuronidation conjugation products and so on; meanwhile, gallic acid was mainly transformed into eduction products and methylation products.
CONCLUSIONThere are kinds of phase I and phase II metabolites of ferulic acid and gallic acid in rat plasma and urine, which provide a basis for its efficacious materials and action mechanism.

6.Batch Inspection of Sildenafil and its Analogues in Anti-fatigue Health Foods by HPLC-MS/MS

Objective:To detect 11 kinds of illegally added chemicals in anti-fatigue health foods including sildenafil ,tadalafil, vardenafil , acetildenafil , homosildenafil , hydorxyhomosildenafil , norneosildenafil , aminotadalafil , pseudovardenafil , thioaildenafil and noracetildenafil.Methods:An HPLC-MS/MS method was used with LUNA C18(250 mm ×4.6 mm, 5μm)as the analysis column and triethylamine phosphate , acetonitrile and methanol as the mobile phase with gradient elution at the flow rate of 1ml.min-1 .An ESI+scan and an MS/MS mode were applied .The qualitative and quantitative analysis were carried out by HPLC-MS/MS.Results:Through the detection of 100 batches of samples , 85 batches were only found illegally added chemical substance sildenafil mainly at the concentration of 5-15 mg per granule .Conclusion:The method of HPLC-MS/MS can be used as a powerful screening tool for the rapid drug screening in basic level drug control laboratories to reduce chemical substances illegally added in anti -fatigue health foods .
7.Antibiotic resistance and molecular characterization of Vibrio cholera strains isolated from an outbreak of cholera epidemic in Jiangsu province.

OBJECTIVETo assess the antibiotic resistance and molecular characterization of cholera strains and to provide basis for clinical treatment and prevention of cholera.
METHODS4 stains isolated from an outbreak of cholera epidemic in Huai'an City in Jiangsu province in September 2010 were characterized using antibiotic susceptibility, biotype analysis, virluence genes detection, ctxB gene sequencing, and PFGE analysis.
RESULTSThe 4 strains were all resistant to sulphamethoxazole/trimethoprim, erythromycin, streptomycin. High drug susceptibility of the samples was found to 6 kinds of antibiotics such as amikacin, norfloxacin, ciprofloxacin, gentamicin, chloramphenicol, ampicillin. The isolates expressed phenotypic traits of both serogroup O1 ogawa and El Tor and carried 9 kinds of virulence genes, ctxA, ace, zot, toxR, tcpI, ompU, rtxC, tcpA, and hlyA gene. They were also identified as harboring the classical ctxB genotype based on amino acid residue substitutions. The PFGE profiles of NotI showed a single banding pattern, while SfiI's was 2 banding patterns.
CONCLUSIONThe bacterium type of Vibrio cholerae causing the epidemic outbreak of cholera belonged to the atypical EL Tor variant which was also identified as toxicogenic strain. The mapping of the strains prompted that there should be the common contamination source. Drug sensitivity test can guide the clinical drug use, in order to reduce the emergence of resistant strains.
