1.MR Arthrography of the Labral-Capsular-Ligamentous Complex: Normal Anatomy, Anatomic Variations, and Pitfalls - Preliminary Study.

PURPOSE: To evaluate anatomic variations and pitfalls of the labral-capsular-ligamentous complex in the shoulder joint for the proper interpretation of magnetic reasonance arthrographic images. MATERIALS AND METHODS:To determine the presence of sublabral sulci, undercutting of the base of the glenoid labrum by the articular cartilage, and the proximity of the middle glenohumeral ligament to the anterior glenoid labrum, 56 MR arthrograms of 41 asymptomatic volunteers were prospectively evalvated for labral shape and capsular insertion. We also evaluated the axillary fold, which was often confused with a loose body. RESULTS: The anterior and posterior parts of the labra, varied but their shape showed several dominant features; triangular(72%, 36%, respectively), rounded(13%, 35%), cleaved(8%, 1%), notched(2%, 0%), flat(5%, 24%), and absent(0%, 4%). Anterior capsular insertion was type 1 in 82% of cases, type 2 in 13% and type 3 in 5%, whereas posterior insertion was type 1 in 62%, type 2 in 36% and type 3 in 2%. We could also detect many pitfalls, such as undercutting of the base of theglenoid labrum by the articular cartilage(29%), sublabral sulci(25%), a prominent axillary fold(38%), and the middle glenohumeral ligament in proximity to the anterior labrum(5%). CONCLUSION: Our study revealed wide variability in the MR arthrographic appearance of the labral-capsular-ligamentous complex in asymptomatic shoulders. A good understanding of normal variation and pitfalls of the normal shoulder may be helpful pathologic condition in case of glenouhumeral instability.

2.A Study of Factors Influencing Weight Control Behavior in Adolescent Females.

This study was conducted to provide information about weight control behavior in adolescent females. To explain the behavior intention of dieting, conceptual framework based on "Social Support, Control and the Stress Process Model" and "Theory of Reasoned Action" was used. The survey was carried out by self-questionnaires with 463 female high school and college students in Daegu. Analysis of data was done using mean, correlation and multiple regression analysis with the SAS computer program. A society preoccupied with thinness gives a burden to women, and this burden may stress dissatisfaction with body image. Social perception of ideal body image except parents' perception, and salient others' perception, and salient others' expectation of subjects' body image except parents' expectation, were much thinner than normal figures in this study. The influencing factors for behavior intention of dieting of the subjects were perceived stress and attitude toward diet behavior, especially beliefs of behavioral outcome. Influencing factors related to perceived stress-that is dissatisfaction of body image-were current figure, social perception of body image, effect of mass communication and others' estimation of subjects' body image with self-comparison with others, in order.

3.Cytokine Gene Expression Associated with Bone Destruction in Cholesteatoma Tissues.

BACKGROUND AND OBJECTIVES: Cholesteatoma is composed of hyperproliferative keratinizing epithelium in the middle ear cavity. Although the mechanism of bone destruction by cholesteatoma has not been fully elucidated, it has been reported that activation of several cytokines followed by osteoclast activation might have a major effect on bone resorption in cholesteatoma. Among the several cytokines, it has been suggested that the overexpression of tumor necrosis factor-alpha (TNF-alpha), interleukin-1alpha (IL-1alpha) and interleukin-6 (IL-6) in the cholesteatomatous tissue is correlated with bone destruction, and transforming growth factor-beta1 (TGF-beta1) and interleukin-4 (IL-4) are associated with osteoclast inhibitory activity. To determine the potential role of several cytokines in the pathogenesis of cholesteatoma and its bone destruction, we investigated mRNA expression of four cytokines in the cholesteatomatous tissue. MAERIALS AND METHODS: We investigated mRNA expression of cytokines in 20 cholesteatomatous tissues as well as in the normal postaural skins using RT-PCR (reverse transcriptase-polymerase chain reaction). RESULTS: 1) Out of 20 cholesteatomatous tissues, six were mRNA expressions of IL-1alpha, four of IL-6, ten of TGF-beta1 mRNA, but no expressions of IL-4. There was no gene expression except six cases for TGF-beta1 in the normal postaural skin. 2) Three cases that expressed IL-1alpha, IL-6, and TGF-beta1 showed severe bone destruction, whereas no bone destruction was noted in 5 cases that expressed only TGF-beta1. 3) There was no significant correlation between the duration of disease and the individual cytokine expression. CONCLUSION: These findings suggest that IL-1alpha and IL-6 may play a major role in the proliferation and growth of cholesteatoma as well as the excitatory effect for bone destruction by cholesteatoma, and TGF-beta1 might have a protective effect for bone destruction.

4.Novel glutathione-containing dry-yeast extracts inhibit eosinophilia and mucus overproduction in a murine model of asthma.

BACKGROUND/OBSECTIVE: Airway inflammation by eosinophils, neutrophils and alveolar macrophages is a characteristic feature of asthma that leads to pathological subepithelial thickening and remodeling. Our previous study showed that oxidative stress in airways resulted in eosinophilia and epithelial apoptosis. The current study investigated whether glutathione-containing dry yeast extract (dry-YE) ameliorated eosinophilia, goblet cell hyperplasia and mucus overproduction. MATERIALS/METHOD: This study employed 2 µg/mL lipopolysaccharide (LPS)- or 20 ng/mL eotaxin-1-exposed human bronchial epithelial cells and ovalbumin (OVA)-challenged mice. Dry-YE employed in this study contained a significant amount of glutathione (140 mg in 100 g dry yeast). RESULTS: Human bronchial epithelial cell eotaxin-1 and mucin 5AC (MUC5AC) were markedly induced by the endotoxin LPS, which was dose-dependently attenuated by nontoxic dry-YE at 10-50 µg/mL. Moreover, dry-YE inhibited the MUC5AC induction enhanced by eotaxin-1, indicating that eotaxin-1-mediated eosinophilia may prompt the MUC5AC induction. Oral supplementation with 10-100 mg/kg dry-YE inhibited inflammatory cell accumulation in airway subepithelial regions with a reduction of lung tissue level of intracellular adhesion molecule-1. In addition, ≥ 50 mg/kg dry-YE diminished the lung tissue levels of eotaxin-1, eosinophil major basic protein and MUC5AC in OVA-exposed mice. Alcian blue/periodic acid schiff staining revealed that the dry-YE supplementation inhibited goblet cell hyperplasia and mucus overproduction in the trachea and bronchiolar airways of OVA-challenged mice. CONCLUSIONS: Oxidative stress may be involved in the induction of eotaxin-1 and MUC5AC by endotoxin episode and OVA challenge. Dry-YE effectively ameliorated oxidative stress-responsive epithelial eosinophilia and mucus-secreting goblet cell hyperplasia in cellular and murine models of asthma.

BACKGROUND: Systemic lupus erythematosus(SLE) frequently has cardiovascular complications. Pericardial inflammation and effusion. ventricular dysfunction, valvular disease and coronary atherosclerosis may result in morbidity. While the pathologic findings in SLE have been well described, the antemortem documentation of the cardiac complications has been less frequent than the postmortem findings would support, we therefore sought to study how echocariography may improve the clinical assessment of cardiac involvement in 40 consecutive patients with SLE. SUBJECTS AND METHODS: This study began in 1990 with a prospective design. An appointment was scheduled with all patients whose SLE was diagnosed at the outpatient clinic and during the admission of the internal medicine service at the Kang Nam St Mary's hospital These were 40 female patients, with a mean age of 32+/-11 years(+/-SD). The average duration of disease was 37+/-32months, the average dosage of prednisolone per day was 20+/-17mg. M-mode and 2-D echocardiogram were carried out to the 40 SLE patients. RESULT: Seventeen(43%) of the SLE patients had pericardial effusion. Reduced left ventricular contractility was evident in 8 patients(20%). Abnormality of diastolic dysfunction to be checked by A/E ratio was in 3 patients(8%). No patients had echocardiographic evidence of pulmonary arterial hypertension. Clinical findings of myocardial dysfunction in these patients included dyspnea(30%), jugular venous distension(20%), rales(10%) and S3 gallop(8%). Valvular abnormality of SLE patients included mitral regurgitation in five patients(13%) and aortic regurgitation in two patients(5%). Electrocardiogram showed myocardial ischemia, atrial fibrillation and sick sinus syndrome in 3, 1 and 1 patients respectively. CONCLUSION: In this study, pericardial effusion were the most common echocardiopraphic abnormalities in Patients with SLE. The incidence of valvular and myocardial disease were relatively common.

Anorexia nervosa is a kind of dietary disorder which causes amenorhea, weight loss, behavioral changes, and its hormonal status is typical hypogonadotrophic hypogonadism. The patients with anorexia nervosa showed many hormonal and chemical abonormal findings, like decresed follicular stimulating hormone, decreased lutealizing hormone and estrogen, increased cortisol, hypokalemia, hypercarotenemia. But tissue mineral contents of anorexia nervosa were not reported. We experience the typical increase of tissue copper contents in three anorexia nervosa patients in recent days, so report them with reviews of literatures.

7.Autologous Stem Cell Transplantation using a Modified TAM Conditioning Regimen for Clinically Aggressive Non-Hodgkin's Lymphoma.

PURPOSE: High-dose chemotherapy (HDT) and autologous stem cell transplantation (ASCT) have been used for the treatment of clinically aggressive non-Hodgkin's lymphoma (NHL). However, the superiority of specific conditioning regimens has not yet been established. The present study evaluated the efficacy and toxicity of a conditioning regimen involving fractionated total body irradiation (TBI), and the use of Ara-C and melphalan (TAM) for clinically aggressive NHL. MATERIALS AND METHODS: Between March 2002 and December 2004, 31 patients with aggressive NHL received fractionated TBI with a dose of 12 Gy over 3 days, and were administered 9 g/m2 Ara-C and 100 mg/m2 melphalan followed by autologous peripheral blood stem Cell Transplantation at the Catholic Hematopoietic Stem cell transplantation Center Korea. Patients that responded to first line chemotherapy and achieved complete remission (CR), or were in a first sensitive relapse were defined as having less advanced disease, while the other patients were defined as having more advanced disease. RESULTS: Objective responses were obtained in 24 of 31 patients (77.4%), comprising complete remission in 19 patients (61.3%) and partial remission in 5 (16.1%) patients. The median follow-up time was 28 months (range 1~62 months). At 3 years, the overall survival and event-free survival (EFS) rates were 62.3% and 47.3%, respectively. Patients with less advanced disease and more advanced disease showed 3-year EFS rates of 73.3% and 22.5 %, respectively (p=0.006). Early (within the first 100 days) treatment-related mortality occurred in 3 (9.7%) patients. Of the 31 total patients, 15 (48.4%) developed grade 3 mucositis, 22 (70.9%) developed neutropenic fever, and two (6.5%) developed interstitial pneumonia syndrome >grade 3. CONCLUSION: The modified TAM conditioning regimen and ASCT appear to be a feasible treatment regimen for clinically aggressive NHL, particularly for patients with less advanced disease.

8.Establishment of Active Identification and Management System for Potential Brain Dead Donors in Life-link Center.

BACKGROUND: The purpose of this study was to promote organ donation by active identification and proper management of brain-dead donor with collaborating network system and to assume operating expenses in the setting of independent organ procurement organization (IOPO) in Korea. METHODS: Seoul National University Hospital and Gachon University Gill Hospital worked together as regional OPO during 8 months from April to December 2008. RESULTS: We constructed cooperative network system with five base-hospitals by MOU (memorandum of understanding). We visited 138 hospitals 223 times and built up brain-dead organ donation. Among total 265 dead patients in intensive care unit (ICU), 95 (36%) patients were considered as potential organ donors, but only 14 (14.7%) donated their organs actually. During the previous 8 months, there were 67 contacts for potential donor evaluation and total 100 solid organs were actually procured from 31 brain-dead donors except 4 cases. We also established and applied a flow chart and critical pathway of potential brain-dead donor. It was worthy of notice to manage 3 brain-dead donors and successfully procured their organs without donor transportation to HOPO. Apart from operating and depreciation expenses, we could estimate the expenses loss of mean 850,000 won per organ in the current system. CONCLUSIONS: Our results showed hope for success of IOPO in Korea which would be founded in the near future. Besides persistent active relationship with regional hospitals, a certain degree of financial support or other means such as increase of organ fee and medical insurance coverage should be considered.

9.Clinical outcomes of erlotinib, gefitinib, or pemetrexed in patients with non-squamous, non-small-cell lung cancer.

BACKGROUND/AIMS: This study compared the clinical benefits of epidermal growth factor receptor tyrosine kinase inhibitors (EGFR TKIs) with pemetrexed to identify the clinical parameters that correlated with response. METHODS: A retrospective chart review examined patients who were 1) treated with EGFR TKI or pemetrexed, 2) diagnosed with advanced non-squamous non-small-cell lung cancer, and 3) previously treated with platinum-based chemotherapy in Soonchunhyang Bucheon Hospital. RESULTS: Sixty-one patients (18 erlotinib, 18 gefitinib, 25 pemetrexed) were investigated from February 2002 to August 2009. The median follow-up period was 37 months (7~97 months). Overall, their median age was 63 years, 41 patients were non-smokers, 57 patients had adenocarcinoma, and 55 patients were at stage IV. Twenty-one patients received the study drugs as second-line chemotherapy, and others as third-line or more. No significant differences in the overall response rate (erlotinib 33.3% vs. gefitinib 38.9% vs. pemetrexed 20.0%) and progression-free survival (erlotinib 1.9 months vs. gefitinib 3.0 months vs. pemetrexed 2.9 months) were found among the three groups. Female gender was related to a good response to EGFR TKIs (p=0.047). Skin rash in the erlotinib group (p=0.037) and adenocarcinoma in the pemetrexed group (p=0.02) were related to improved progression-free survival. Few side effects were reported. CONCLUSIONS: Both EGFR TKIs and pemetrexed therapy for non-squamous non-small-cell lung cancer were efficient and tolerable after the failure of first-line platinum-based chemotherapy. Further prospective studies are needed to validate the predictive role of the suggested clinical parameters in this study.

10.A Case of Solitary Involved NK-T Cell Lymphoma on the Gallbladder.

Extranodal NK-T cell lymphoma is a subtype of non-Hodgkin's lymphoma (NHL) and this most commonly affects the nasal and paranasal cavities. Primary lymphoma of the gallbladder is extremely rare and solitary relapsed extranodal NK-T cell lymphoma of the gallbladder has not yet been reported in Korea. We experienced a case of a solitary relapsed extranodal NK-T cell lymphoma of the gallbladder. One year earlier, a 55-year-old man was diagnosed with extranodal NK-T cell lymphoma of the anus, and he underwent six cycles of chemotherapy with CHOP (cyclophosphamide, adriamycin, vincristine and prednisone), and he achieved complete remission. The patient was admitted for right upper quadrant pain. Computed tomography (CT) performed on readmission revealed gallbladder wall thickening. Fluorodeoxyglucose-positron emission tomography (FDG-PET) showed hypermetabolic lesions along the gallbladder wall. The specimen obtained at cholecystectomy revealed CD3(+) and CD56(+) lymphoma, which is characteristic of NK-T cell lymphoma.
