1. Changes of sex hormone levels in male acne patients with normal serum testosterone and effect of antiandrogen therapy

Objective: To observe the sex hormone levels in adolescent male acne patients with normal serum testosterone, and to evaluate the curative effect of antiandrogen therapy. Methods: Totally 40 adolescent male acne patients with normal serum testosterone were evenly randomized into two groups, the treatment group received finasteride plus life-style intervention, and the control group received life-style intervention only. The curative effects were compared between the two groups, and the levels of serum testosterone, dihydrotestosterone (DHT), free testosterone (FT), sex hormone binding globulin (SHBG), and estradiol (E2), and the adverse effects were observed before and after treatment. The curative effects in the treatment group were also observed 3 months after drug withdrawal. Another 40 healthy adolescent males served as normal group for comparison of the baseline hormone levels with ance patients. Results: The patients in our group had significantly higher levels of body fat content, E2 (P<0.05), DHT, and FT (P<0.01), and lower level of SHBG (P<0.05) compared with those in the normal control group. The overall effective rate of the treatment group was 90%, which was significantly higher than that of the control group (25%, P<0.01); the effective rate of the treatment group was 85% 3 months after drug withdrawal. The levels of DHT (P<0.01) and FT (P<0.05) in the treatment group were significantly decreased and the levels of SHBG was significantly increased (P<0.05) after treatment. There were no severe adverse effects in both groups. Conclusion: The adolescent male acne patients with normal serum testosterone have higher body fat content, DHT, FT, E2 and lower SHBF levels. The therapy using finasteride plus life-style is effective) it can greatly improve the serum level of SHBF and reduce adverse effects, with low recurrent rate after drug withdrawal.
2.The clinical features and outcomes in 21 patients with ocular adnexal mucosa-associated lymphoid tissue lymphoma

Objective To evaluate the clinical features,therapies and prognosis in patients with mucosa-assoeiated lymphoid tissue (MALT) lymphoma in ocular adnexal marginal zone (OAML).Methods A retrospective analysis was made upon clinical data from 21 patients with OAML admitted into Beijing Tongren Hospital from June,2008 to December,2011.Results There were 12 (57.1 %) men and 9(42.9%) women,with a median age of 57 (23-79) years old.Majority of patients had localized pathological changes.Among them,16 patients (76.2%) were in stage Ⅰ E,and 5 (23.8%) in stage Ⅳ E.Surgical resection as the sole treatment was performed in 13 patients (61.9%),and positron emission tomography CT(PET-CT) imaging demonstrated normal fluorine 18-fluorodeoxyglucose (FDG) uptake after surgical resection,who were managed with no further therapy.All the 13 patients were followed up for median 14 (5-38) months,and all in complete remission.Combination chemotherapy was given to 8(38.1%) patients.Three patients in stage Ⅰ E treated with COP (cyclophosphamide,vineristine and prednisone) or CHOP (cyclophosphamide,adriamycin,vincristine and prednisone) were all in partial remission.Five patients in stage ⅣE were treated with COP/CHOP in combination with rituximab,and all in complete remission.The 3-year overall survival rate and disease-free survival rate in the total patients were 100.0% and 74.9% respectively.Conclusions The patients with OAML generally have localized disease,show indolent clinical course,and present low lymphoma-related mortality.Surgical resection is a very important treatment in the patients with local disease.Systemic chemotherapy should be considered in patients at advanced stages.Rituximab in combination with chemotherapy can improve the remission rate.
3.The relationship between obesity and microalbuminuria among general population in Beijing area

Objective To investigate the correlation between microalbuminuria (MAU) and obesity and its indexes, including BMI, waist circumference(WC), and waist-to-hip ratio(WHR) , among partial community population in Beijing. Methods A total of 2080 subjects who took physical examination in Beijing, including 810 men and 1270 women with a mean age of(50. 9 ± 13. 1 )years, were enrolled. The informed consent has been achieved from each patients. BMI and WHR were calculated based on collected data of height, weight, WC, and hipline. Urine albumin-creatinine ratio(ACR) within the range of 30-300mg/g was classified as MAU. The subjects were divided into normal albuminuria ( NAU ) group and MAU group. The correlations between MAU and different obesity indexes including BMI, WC and WHR, were analyzed. Results Among the 2080 subjects, there was a positive correlation between BMI (r = 0. 1276,P<0.01) and ACR, and WC (r = 0.0840, P <0.01) and ACR. WHR and ACR was irrelevant ( P > 0. 05 ). In univariate analysis, there was significant difference in BMI ≥ 28 kg/m2 ( OR = 2. 02 ) and WC ≥85 cm (male) or≥80 cm (female) (OR = 1.69 ) between NAU group and MAU group (P < 0. 05 ).There was no significant difference in BMI 24-< 28 kg/m2, and WHR ≥0. 90 (male) or ≥0. 85 (female)between NAU group and MAU group( P≥0. 05 ). Multivariate logistic regression analysis revealed that BMI ( OR = 1.06) was an isolated independent risk factor of MAU from age ( OR = 1.01 ), female ( OR = 1.42),systolic blood pressure (OR=1.01), TC (OR=1.93) and HDL-C (OR=0.54). Conclusions Obesity is an independent risk factor of MAU among partial community population in Beijing. The correlation between different obesity indexes and MAU also differs.

Objective To explore the value of dexamethasone(DEX) for neuronal cell injury and death by observing the effect of DEX on excitatory amino acid(EAA) and monoamine neurotransmitter in cerebral tissue of neonatal rat with hypoxia-ischemia.Methods Hypoxic-ischemic neonatal rat models were established,the levels of EAA and monoamine neurotransmitter in cerebral tissue were analyzed by using capillary electrophoresis and fluorospectrophotometry method.The rats were divided into 4 groups: small dose DEX group pre-treated with DEX(0.5 mg/kg) prior to hypoxia-ischemia,large dose DEX group pre-treated with DEX(10 mg/kg) prior to hypoxia-ischemia,HIE group and shamful operation group.Results The levels of EAA and monoamine neurotransmitter contents in HIE group were significantly higher than those in shamful operation group(P0.05).EAA contents of large dose DEX group greatly decreased compared with HIE group (P
5.Studies on Stability of Microbial Flocculant and Its Application to Municipal Thickened Sludge Dewatering

The stability of microbial flocculant (MBF) produced by Aspergillus sojae and its application to municipal thickened sludge dewatering were studied. The results showed that the MBF had high heat and acid-base endurance with high flocculating activity in a wide range of pH from 1.5 to 12. The MBF retained 96% of flocculating activity after 35 days preservation at 4℃, but in different pH the flocculating activity difference was very apparent after 35 days store at room temperature. The experimental results also demon- strated that the MBF was better than PAM and PAC in reducing specific resistance filtration. The optimal dose of MBF used for intensifying thickened sludge dewatering is 7%(volume fraction). And the more the volume of sludge is treated, the less the cost of MBF for unit volume sludge treatment.

Objective To assess the diagnostic predictive value of Wells score and modified Geneva score for acute pulmonary embolism by prospective case series and to explore a more suitable scoring system for Chinese population.Methods All the patients suspected of pulmonary embolism (PE) and received CT pulmonary angiography (CTPA) were enrolled consecutively in Fuxing Hospital,Capital Medical University,China,from June 2009 to August 2011.Before CTPA test or on condition that test results were unknown,clinical scoring was assessed prospectively by the Wells score and the modified Geneva score.The probability of PE in each patient was assessed and the patients were divided into low,moderate and high probability groups according to the clinical scores.The result of CTPA was used as the diagnostic gold standard for PE.Diagnostic accuracy in each group was analyzed.The predictive accuracy of both scores was compared by AUCROC curve.Results A total of 139 patients met our enrollment criteria and 117 eligible patients entered our study at last.PE was diagnosed in 47 patients by CTPA with an overall prevalence of 40.2%.Prevalence of PE in the low,moderate and high pretest probability groups assessed by the Wells score and by the simplified modified Geneva score were 7.1% (3/42),42.9% (21/49),88.5% (23/26)and 10.0% (3/30),48.1% (37/77),7/10,respectively.AUCROC curves for the Wells score and the simplified modified Geneva score were 0.872 ( 95% CI 0.810-0.933 ) and 0.734 ( 95% CI 0.643-0.825 )respectively,with a significant difference ( P =0.005 ).Conclusion The Wells score is more accurate for clinical predicting acute PE than the modified Geneva score.
8.Synthesis and investigation on antidiabetic activity of 4-(1-aryl-3-oxo-5-phenylpentylamino) benzenesulfonamide.

Searching for new antidiabetic lead compound, 4-(1-aryl-3-oxo-5-phenylpentylamino) benzenesulfonamides were designed and synthesized directly by three component one-pot condensation of 4-phenyl-2-butanone and sulfanilamide with some aromatic aldehydes at an yield of 23%-97%. The chemical structures of the twelve new Mannich bases were confirmed by 1H NMR, 13C NMR, FTIR, ESI-MS and HR-MS. The screening results of antidiabetic activity indicated that most of these title compounds possess alpha-glucosidase inhibitory activity, among which compound le is the strongest one. And compound 11 possesses good peroxisome proliferator-activated receptor response element (PPRE) agonist activity. The structure-activity relationship of these new beta-amino ketones containing benzenesulfonamide unit was also discussed preliminarily.
