1.Hand hygiene management among health care workers in stomatology medical institutions in Zunyi

Objective To understand the current management situation of hand hygiene (HH)among health care workers(HCWs)in stomatology medical institutions in Zunyi,and provide basis for the scientific management of HH.Methods In October-December 2014,the on-site observation and questionnaire survey were adopted to investigate current management situation of HH in 127 stomatology medical institutions in Zunyi.Results The rates of qualified installation of sink,faucet,detergent,alcohol-based hand rub,and hand-drying facilities were 60.63%, 11 .02%,92.13%,22.05%,and 37.80% respectively.Except detergent and sink,the installation of other HH supplies in stomatology medical institutions in Zunyi were not ideal.The correct rate of HH among HCWs was 33.07%(42/127),qualified rates of HH methods among HCWs in departments of stomatology,comprehensive out-patient departments, stomatological clinics,and teeth-cleaning clinics were 50.00%,23.08%,23.53%,and 0 respectively.Conclusion HH facilities are not perfect in primary stomatology medical institutions in Zunyi,health ad-ministrative departments’supervision on HCWs’HH is inadequate,the awareness rate of HH knowledge and com-pliance to HH among HCWs are both low.
2.Endoscopic ultrasound-guided fine needle aspiration with different needle types for solid pancreatic masses: a prospective and randomized comparative trial in two centers

Objective To compare the diagnostic yield of endoscopic ultrasound-guided fine needle aspiration (EUS-FNA) for solid pancreatic masses performed with three different needle types through the cytological results.Methods All patients with solid pancreatic masses larger than 2cm from December 2010 to May 2011 were enrolled,and divided into two groups according to different access of EUS-FNA,trans-gastric approach with 19-,22-and 25-gauge needles (n =42) and trans-duodenal approach with 22-and 25-gauge needles (n =10).In both groups,EUS-FNA was performed with randomization of needle types.During the puncture,the suction,the number of movements,and the depth of insertion were fixed.At the end of the puncture,a liquid-based cytological (LBC) preparation was used to fix the specimen.One cytopathologists was assigned to make the diagnosis and comparison.Results Technical success was 100% and no procedure related complications occurred.No statistically significant differences were observed in different needles in terms of all cytological parameters between two groups (P > 0.05).However,the 25-gauge needle showed a trend towards a higher sensitivity,specificity,positive predictive value,negative predictive value and accuracy.Conclusion There is no significant difference in yield of cytological results between different needle types,although 25-gauge needle shows a relative superiority.
3.Analysis of STR loci expression after allogeneic cord blood hematopoietic stem cell transplantation.

The purpose of this study was to analyze the STR loci expression after allergenic cord hematopoietic stem cell transplantation in patient with Ducennes muscular dystropy (DMD) patient. PCR-SSO was used to identify the HLA antigens and alleles, STR-PCR was used to detect the chimera status. Quantity analysis of donor chimeras was performed by multiplex PCR amplification of STR marker and capillary electrophoresis with fluorescence detection. The results showed that patient appear to be HLA identical to the donor cord blood at the tested level. Persistent full donor chimerism was found in breast bone marrow. The patient with stable MC (DC < 5%) had a probability of long term survival with molecular remission MC status appeared in forearm muscle, tongue, liver, spleen, stomach, right temporal lobe, diaphragmatic muscle, bronchus, left ventricle and right kidney. In conclusion, the donor gene can express in parenchymatoas organs, the donor chimerism was detected in breast bone marrow and some other organs.

4.Molecular and epidemiological study on viral diarrhea among infants in Lanzhou.

OBJECTIVETo study the epidemiologic characteristics of viral diarrhea in children under 5 years old in Lanzhou, understand the four major virus in children of distribution.
METHODSIn the first hospital of Lanzhou university from Jul 2009 to Jun 2010,we collected 290 stool specimens from children with diarrhea and 114 asymptomatic controls. Rotavirus was detected by ELISA,further strain characterization was carried out by nested PCR. The human calicivirus, astrovirus, adenovirus were detected by RT-multiplex PCR and PCR.
RESULTSAt least one of the four viral agents was found in 60% of the specimens. Rotavirus, human calicivirus, adenovirus, and astrovirus were identified in 39.31%, 11.38%, 10.69%, and 4.83% in 290 specimens respectively. Rotavirus G3 was the most prevailing serotype, P [8] was the most common genotype. In the 114 control samples, 7 sample was positived for calicivirus, 5 samples were positived for human adenovirus and 1 sample was positived for astrovirus.
CONCLUSIONThe results indicated clearly the impact of viral agents causing diarrhea and the importance of long-term systematic surveillance.

5.Effect of cyclosporine and simulect mono and combination therapy on cardiac allo-transplantation in rats.

OBJECTIVE:
To observe the effect of cyclosporine and simulect mono or combination therapy on cardiac allo-transplantation in rats.
METHODS:
Recipients with allografts were treated with different doses of cyclosporine and/or simulect after cardiac allo-transplantation. Graft survival time was observed; the histopathological examination of graft tissues was performed; and levels of serum IL-2 and IL-4 were determined.
RESULTS:
Mono or combination therapy with cyclosporine and/or simulect increased the survival of cardiac allografts. With the prolongation of survival time of the grafts, the rejection of grafts was moderated. The serum IL-2 level increased in acute rejected grafts; the serum IL-4 level increased evidently in long survival grafts.
CONCLUSION
Cyclosporine and simulect have an effect in the prolongation of cardiac allograft survival in rats, and the combination therapy shows an evident synergistic effect.

6.Immune tolerance induced by combined heart-thymus transplantation with intrathymic inoculation of thymocytes in rats.

OBJECTIVE:
To explore the role of allo heart and thymus transplantation by intrathymic inoculation of thymocytes.
METHODS:
Wistar recipients were given intrathymic injection of allo thymocytes (2 x 10(7)) 14 days before the heart and/or thymus transplantation. Graft survival, histopathology, levels and mRNA expressions of IL-2, IL-4 in serum and cardiac-grafts were investigated.
RESULTS:
Heart transplantation and heart-thymus composite transplantation with the treatment of CysA for 7 or 14 days prolonged graft survival. Heart transplantation and heart-thymus composite transplantation with intrathymic thymocytes injection induced the long-term survival of allo-grafts transiently immunosuppressed with CysA; IL-4 maintained at high levels but IL-2 kept at low levels in grafts in long-term survivals.
CONCLUSION
Intrathymic inoculation of allo thymoctyes can induce immune tolerance for both cardiac transplantation and heart-thymus combined transplantation in rats. Thymus graft may play a role in the induction and maintenance of central tolerance.

7.High fat diet induced the expression of SREBP-1, TGF-beta1 and alpha-SMA in renal tubular cells and extracellular matrix accumulation in Wistar rats.

OBJECTIVETo explore the effect of high fat diet on the expression of sterol regulatory element binding protein-1 (SREBP-1), transforming growth factor-beta1 (TGF-beta1) and alpha-smooth muscle actin (alpha-SMA) in renal tubular cells and extracellular matrix accumulation in Wistar rats.
METHODSThe Wistar rats were treated with high fat diet for 12 weeks and renal lipid deposit was detected by the method of Oil Red O staining. The immunohistochemistry and Western blot were used to investigate the expression of SREBP-1, TGF-beta1, alpha-SMA and fibronectin (FN) protein. The expression of SREBP-1 mRNA was determined with in situ hybridization. Masson staining was for the detection of extracellular matrix (ECM) accumulation.
RESULTSThe weight of rats raised by high fat diet increased, in company with the high serum glucose, serum triglyceride and serum insulin. The Oil Red O staining revealed that the renal proximal tubular epithelial cells showed significant lipid droplet in high fat diet rats. SREBP-1 protein and mRNA were located in the renal tubular cells and the expressions of high fat diet rats were higher than those of normal control rats. They were respectively 1.88 times and 1.85 times than those of normal control group. TGF-beta1 and alpha-SMA protein were also located in renal tubular cells and high fat diet up-regulated the expression of them. ECM accumulation was detected with Masson staining and the result showed that high fat diet treatment increased interstitial ECM product and FN protein was found high expression.
CONCLUSIONHigh fat diet may induce lipid droplet deposit in renal tubular cells by up-regulation of the expression of SREBP-1, which causes ECM accumulation by increasing the expression of TGF-beta1 and alpha-SMA.

8.Repairing tibial post-traumatic osteomyelitis with bone and skin defect by Ilizarov technique at stage I.

OBJECTIVETo explore clinical effects of Ilizarov technique at stage I for repairing tibial post-traumatic osteomyelitis with bone and skin defect.
METHODSFrom June 2010 to December 2013,44 patients with tibial post-traumatic osteomyelitis with bone and skin defect were treated with Ilizarov technique at stage I . Among them, there were 35 males and 9 females aged from 18 to 70 years old with an average of 42.5 years old. Bone defect ranged from 4 to 16 cm, skin defect ranged from 3 cm x 4 cm to 5 cm x 16 cm. The operation was performed debridement thoroughly, removed inflammatory bone section, osteotomy invasively, install circular external fixator by Ilizarow technique; screw nut were rotated at 1 week after operation, and prolonged 0.5 to 1.0 mm everyday. Wound surface, new born callus and bone healing were observed to evaluate clinical effects.
RESULTSAll patients were followed up from 11 to 36 months with an average of 18.5 months. Bone defect after osteotomy was from 6 to 22 cm with an average of 11.5 cm; the time of wound healing time ranged from 21 to 79 d with an average of 38 d; bone defect healing time was from 8 to 15 months with an average of 12.5 months. All patients were cured, no recurrent infection, refracture and shorten of calf deformity were occurred.
CONCLUSIONRepairing tibial post-traumatic osteomyelitis with bone and skin defect by llizarov technique at stage I has advantages of less trauma, low inflammatory recurrence rate, could avoid multiple complex operation, and receive definite curative effect.

9.Effect of DNA methyltransferases 1 inhibition on proliferation, invasion, and metastasis in ACC-M line.

OBJECTIVETo investigate the effect of DNA methyltransferases 1 (DNMT-1) inhibition on the ACC-M cells in vitro and in vivo and discuss the role of DNMT-1 in the development, invasion and metastasis of salivary adenoid cystic carcinoma (SACC).
METHODSACC-M cells of stable DNMT-1 inhibition were established in a previous research. In vitro, the growth and invasion of ACC-M cells which stably inhibited DNMT-1 were detected and analyzed by methyl thiazolyl tetrazolium (MTT) growth curve, flow cytometry, plating efficiency and invasion assay. In vivo, the growth and metastasis of ACC-M cells which persistently inhibited DNMT-1 were observed and analyzed by subcutaneous injection and tail vein injection into the nude mice.
RESULTSIn vitro, the doubling time [(34.7 +/- 2.1) h], S phase fraction [(17.4 +/- 1.7)%], plating efficiency [(43.0 +/- 1.3)%] of ACC-M cells was significantly different from those of blank [(26.2 +/- 3.1) h, (31.5 +/- 2.0)%, (71.0 +/- 4.7)%], empty load control [(28.4 +/- 3.9) h, (39.0 +/- 2.0)%, (66.0 +/- 5.2)%], P < 0.05, and the invasion ability was not significantly different among these groups (P > 0.05). In vivo, the subcutaneous tumor forming rate (6/10), volume [(2.18 +/- 0.83) mm(3)], weight [(0.0156 +/- 0.0046) g] of ACC-M cells was also significantly lower than that of blank [10/10, (155.44 +/- 1.67) mm(3), (0.0724 +/- 0.0157) g], empty load control [10/10, (147.46 +/- 1.73) mm(3), (0.0729 +/- 0.0177) g], P < 0.05, but the rate of lung metastasis was not significantly different among these groups (P > 0.05), and the masses (2.0 +/- 0.5), diameter (70.0 +/- 20.3) microm of ACC-M cells was significantly lower than that of blank [(28.0 +/- 5.5), (195 +/- 25.4) microm], empty load control [(27.0 +/- 4.5), (190.0 +/- 19.9) microm], P < 0.05.
CONCLUSIONSInhibition of DNMT-1 is able to inhibit the proliferation and metastasis of ACC-M cells in vitro and in vivo.

10.External therapy of plasma cell mastitis by jiuyi powder using partial least-squares discriminant analysis: a safety analysis.

OBJECTIVETo evaluate the safety and the clinical value of external use of jiuyi Powder (JP) in treating plasma cell mastitis using partial least-squares discriminant analysis (PLSDA).
METHODSTotally 50 patients with plasma cell mastitis treated by external use of JP were observed and biochemical examinations of blood and urine detected before application, at day 4 after application, at day 1 and 14 after discontinuation. Blood mercury and urinary mercury were detected before application, at day 1, 4, and 7 after application, at day 1 and 14 after discontinuation. Urinary mercury was also detected at 28 after discontinuation and 3 months after discontinuation. The information of wound, days of external application and the total dosage of external application were recorded before application, at day 1, 4, and 7 after application, as well as at day 1 after discontinuation. Then a discriminant model covering potential safety factors was set up by PLSDA after screening safety indices with important effects. The applicability of the model was assessed using area under ROC curve. Potential safety factors were assessed using variable importance in the projection (VIP).
RESULTSUrinary β2-microglobulin (β2-MG), urinary N-acetyl-β-D-glucosaminidase (NAG), 24 h urinary protein, and urinary α1-microglobulin (α1-MG) were greatly affected by external use of JP in treating plasma cell mastitis. The accuracy rate of PLSDA discriminate model was 74. 00%. The sensitivity, specificity, and the area under ROC curve was 0. 7826, 0. 7037, and 0. 8084, respectively. Three factors with greater effect on the potential safety were screened as follows: pre-application volume of the sore cavity, days of external application, and the total dosage of external application.
CONCLUSIONSPLSDA method could be used in analyzing bioinformation of clinical Chinese medicine. Urinary β2-MG and urinary NAG were two main safety monitoring indices. Days of external application and the total dosage of external application were main factors influencing blood mercury and urine mercury. A safety classification simulation model of treating plasma cell mastitis by external therapy of JP was established by the two factors, which could be used to assess the safety of external application of JP to some extent.
