1.Clinical diagnostic value of serum amyloid A in patients with lung cancer

Objective To evaluate the clinical value of SAA by detecting their expression levels in patients with lung cancer and the analysis of the relativity of SAA for early diagnosis.Methods There are 243 cases specimens obtained from lung cancer patients who were newly diagnosed and without any treatment in Zhejiang Cancer Hospital from April 2014 to June 2015.The 243 lung cancer individuals were 147 male, and 96 female, their ages ranged from 29 to 85 years, with an average age of 63 years.The distribution of pathological type was as follows:95 patients were adenocarcinoma, 102 patients were squamous carcinoma, and 46 patients were small cell carcinoma.The distribution of TNM staging systems was as follows: 59 patients in stage 1and stage 2, 54 patients in stage 3, and 130 patients in stage 4.While 179 cases physical examination as the control.There were 94 individuals male, and 85 individuals female.Their ages ranged from 26 to 86 years, with an average age of 61 years.By using latex enhanced immune turbidimetric method, serum SAA concentrations in patients with lung cancer and healthy controls were checked on the Hitachi-7600 automated chemistry analyzer ( Hitachi ).The comparisons of all analyses values between healthy controls and lung cancer were estimated by two independent samples nonparametric tests ( Mann-Whitney U).The association between SAA and lung cancer prognostic factors such as age, smoking status and metastasis, was evaluated by spearman correlation and multivariate analysis.Results The median and interquartile spacing of SAA concentration was 42.36 mg/L (9.35, 74.22) in lung cancer patients.While 24 mg/L ( 3.25, 21.45 ).The median level of SAA in lung cancer patients (42.36 mg/L) were significantly higher than in healthy controls (11.24 mg/L), and difference reached statistically significant (Z=-2.403,P=0.006).Nevertheless, there was no significant difference in SAA concentrations among the different pathological types(Z=-1.013, P=0.339), ages (Z=0.578, P=0.458) and gender(Z=0.726, P=0.246) of lung cancer patients.While the level of SAA in has smoking status (Z=-2.282, P=0.013) and distant metastasis (Z=-2.138, P=0.017) of lung cancer was higher.By drawing ROC curve, the cut off value of SAA in distinguishing lung cancer with healthy control was 14.48 mg/L.Meanwhile, the AUC was 0.811, the accuracy is 89.12% and the sensitivity was 88.73%.Serum concentration was positively related with smoking status ( r =0.331, P =0.018 ) and distant metastasis ( r =0.372, P=0.015 ) by Spearman correlation analysis .Conclusion Serum SAA concentrations may contribute to the auxiliary diagnosis of lung cancer, evaluate the clinical stage and distant metastases of lung cancer.
2.Effect of oral alloy post and core on magnetic resonance imaging.

OBJECTIVEThis study aims to observe and compare the influences of the casting alloy post and cores on magnetic resonance imaging (MRI).
METHODSSix casting alloy post and sores were fabricated for the experimental groups, and zirconia post and core served as the control. The seven types of post and cores were of the same size and shape. Each post and core was orally fastened to a volunteer and then imaged with sequences of T1-weighted spin echo imaging (SE-T1WI), T2-weighted turbo spin-echo imaging (TSE-T2WI), and diffusion weighted imaging (DWI).
RESULTSCobalt chromium alloy post and core generated severe artifacts, whereas gold palladium alloy post and core generated no obvious artifacts, and no difference was observed between the zirconia and the gold palladium alloy post and cores (P > 0.05). The pure titanium alloy post and core produced mild artifacts. The post and cores of the titanium, nickel-cadmium, and gold platinum alloys generated moderate artifacts with no significant differences (P > 0.05).
CONCLUSIONDifferent metal post and cores have different influences on MRI. With the exception of that generated by cobalt chromium alloy, the artifact generated by the single-alloy post and core does not influence the image of the normal head and neck.

3.A clinical study of the use of MCT/LCT and LCT in neonatal infants with hepatic dysfunction

Objectives: To compare the effect of different fat emulsions on the blood lipid and liver function in neonates with hepatic dysfunction. Methods:40 babies with hyperbilirubinemia(≥51.3 mmol/L) were randomly divided into two groups ,receiving the fat emulsion of 20% Intralipid (LCT group) or 20% Lipofundin (MCT/LCT group) at the dose of 2.0 g/(kg?d) for 7 days. Blood lipid 〔total cholesterol(TC), triglyceride(TG)〕 and the serum biochemical indexes relative to liver function were measured before and after the period of 7 days. Results:The levels of TC did not change remarkably in two groups. 4.Relation between serum uric acid and metabolic syndrome and coronary artery disease in essential hypertension.

Objective To assess the relationship between serum uric acid (SUA)and metabolic syndrome and coronary artery disease(CAD).Methods 232 untreated subjects with essential hypertension were divided into two groups(CAD and no CAD)by coronary angiography.All subjects were free of myocardial infarction,cardiomyopathy,valvular disease, atrial fibrillation,aortic dissection,and renal disease.Results Compared to no CAD group,age,diabetes,triglycerides and SUA in CAD group were higher.There was a significant association between SUA and the severity of CAD (P=0. 015).However,after adjustment for concomitant risk factors of cardiovascular disease,SUA was not an independent risk factor of CAD(P=0.151).In sex-specific analysis,there was a trend of SUA to be an independent risk factor of CAD in women(P=0.062),but it was no statistic significance.The highest quartile level of $UA tended to be associated with increased risk for CAD,but it was not statistically significant either(OR=2.52,P=0.075).SUA was closely associated with metabolic syndrome and diabetes in woman.Conclusion In untreated patients with essential hyperteusion,SUA was associated with metabolic syndrome and the severity of CAD,but it is not an independent risk factor of CAD,the raised SUA may be only a marker of insulin resistance.
5.The effects of commonly used dental cast metal posts and cores on MRI

Objective:To observe the influence of commonly used dental cast metal posts and cores on magnetic resonance imaging (MRI)for head and neck.Methods:Six kinds of posts and cores made of metal alloy (Ni-Cr alloy,Co-Cr alloy,titanium alloy,pure titanium,Pd-Au alloy and Au-Pt alloy)were respectively fixed on the mucosa of left palate of a volunteer.The maxillofacial region of the subjects was scanned by routine head sequences SE-T1WI and TSE-T2WI respectively.Results:ANOVA of factorial design analy-sis indicated significant differences of artifacts among metal materials(F=153.63,P<0.05),scanning sequences(F=70.94,P<0.05)and the number of cast posts and cores(F=102.73,P<0.05).In both sequences the size of metal artifacts were as following:Pd -Au alloy>Co-Cr alloy>Ni-Cr alloy>Au-Pt alloy>titanium alloy>pure titanium;two posts and cores>one post and core;SE-T1WI>TSE-T2WI.Conclusion:Different cast metal posts and cores have different influences on MRI artifact.Reasonably choosing the metal materials and scanning sequences can minimize artifacts on MRI.
6.The radiosensitizing effects of cryptotanshinone on H22 hepatoma-bearing mice

Background and purpose:Quinonoids can change the cell cycle distribution of tumor cells, and effect the radiosensitizing. This study aimed to investigate the radiosensitization effects, cell cycle and apoptosis of cryptotanshinone on H22 hepatoma-bearing mice. Methods:The mouse hepatoma H22 model was established, then divided into blank control group, irradiation alone group, high dose of cryptotanshinone group, cryptotanshinone (low, medium and high)+IR groups. After irradiated, observed the growth of tumor’s conditions, record epigenetic tumor irradiation time, calculated the delay time of tumor growth and enhancement factor (EF). After 22 days, mice were killed, stripped tumor, and calculated the inhibition rate. The cell cycle distribution and apoptosis were measured by lfow cytometry. Results:Cryptotanshinone (low, medium and high) groups inhibited the tumor growth better than the blank group, and had the signiifcant radiosensitizing effect. The enhancement factor was 1.22, 1.43,2.19, respectively. Cells were treated with cryptotanshinone which had significant effects on cell cycle, and induced apoptosis, which indicated signiifcant G2/M phase arrest and a decrease in S phase. Conclusion:Cryptotanshinone inhibited the tumor growth and had the radiosensitizing effects on H22 hepatoma-bearing mice. One of the mechanism may be that it might make significant G2/M phase arrest and S phase decreased, and induced apoptosis. So cells were more sensitive to radiation.
7.Cervical pedicle screw fixation for cervical spinal cord injury without fracture and dislocation

Objective To summarize the efficacy of cervical pedicle screw fixation in the treatment of cervical spinal cord injury without fracture and dislocation.Methods From September 2007 to March 2010,21 patients with cervical spinal cord injury without fracture and dislocation combined with developmental cervical stenosis were managed by pedicle screw fixation in unstable segments,and then single open-door laminoplasty in stenosis segments.The clinical outcomes were reviewed and summarized.Results All the patients received follow-up of 1-3 years.Two patients had superficial wound infection,which was cured by dressing change.One presented intra-incisional hematoma one week post-operatively and was cured by hemostasis and drainage.One had door-reclosing without symptom aggravation.There was neither perforation of pedicle screws nor breakage,loosening or displacement of internal fixation.The curvature of cervical lordosis at follow-up had significant rectification in comparison with that at preoperation,but showed unobvious change as compared with that immediately after operation.According to the modified Japanese Orthopedic Association (JOA) score,the neurological score improved from preoperative 4-15 points to 9-17 points at the final follow-up,which revealed more than 80% of recovery in 13 patients,50% -80% of recovery in six and 5% -50% of recovery in two,with the average recovery rate of 75%.Conclusion For unstable segments in patients with cervical spinal cord injury without fracture and dislocation combined with developmental spinal stenosis,posterior pedicle screw fixation can avoid the risk of further spinal cord injury in single open-door laminoplasty and restore the physiological curvature of cervical vertebrae,which provides solid foundation for backward drift of spinal cord and creates favorable condition for recovery of spinal cord function.
8.The effect of pvdQ gene on Pseudomonas aeruginosa swarming motility

Objective To investigate the effect of pvdQ(PA2385)gene on Pseudomonas aeruginosa swarming motility.Methods The plasmid pME6032 with pvdQ gene was constructed and identified,then transformed into Pseudomonas aeruginosa PAO1 using the eleetroporation to build pvdQ-overexpression strain.The pME6032-PAO1 strain was constructed with the same method.The cloning plasmid pEX18Gm containing sacB was successfully used to construct unmarked deletion mutant of pvdQ gene and pvdQ mutant strain. Bacteria were inoculated in LB and were cultured overnight.The clones were measured for the diameter of the swarming zone.The statistical analysis was done using one-factor ANOVA.Results Strains of pvdQ-overexpression and pvdQ-mutant were successfully constructed and confirmed by polymerase chain reaction(PCR).The four strains were compared for the swarming motility by changes in diameter:PAO1(20.52±1.80)mm,pME6032-PAO1 strain(19.39±2.10)mm,pvdQ-overexpression strain(51.20±2.16)mm,pvdQ-mutant strain(3.30±0.55)mm.The diameter of pME6032-PAO1 strain was not significantly different from that of wild strain PAO1(t=-0.1493,P>0.05).However,the diameter of pvdQ(Q-mutant strain was significantly shorter than that of wild strain PAO1(t=2.8525,P<0.05).while the diameter of pvdQ-overexpression strain was longer than that of the wild strain PAO1(t=1.4230,P<0.05).Conclusions pvdQ gene may be involved in regulating the swarming motility of Pseudomonas aeruginosa,which can promote Pseudomonas aeruginosa swarming motility.
9.Treatment of multiple myeloma complicating femoral neck fracture

Objective To evaluate the effect on treatment of multiple myeloma complicating femoral neck fracture by surgical intervention combined with drugs.Methods Twelve patients with multiple myeloma complicating femoral neck fracture were treated by cemented total hip arthroplasty femoral head shank.All of patients were received zoledronic acid treatment,at the same time,7 cases of them treated by MPT regimen (melphalan,methylprednisolone and thalidomide),3 by bortezomib combined with dexamethasone,and 1 by autologous peripheral blood stem cell transplantation.Results Twelve patients could tolerate surgery,postoperative pain was significantly reduced.Assessed by using Harris hip function score after 6 months operation,2 cases were excellent,7 cases were good,3 cases were common,excellent and good rate was 75% (9/12).All patients were followed up for 8 months to 3 years,1 case of local recurrence after 13 months.1-year overall survival rate was 100%,2-year overall survival rate was 83%,and 3-year overall survival rate was 67%.Conclusions Multiple myeloma complicating femoral neck fracture,using bone cement in total hip arthroplasty femoral head shank to clear the local tumor lesions,receive reconstruction capability,rapid postoperative recovery,pain relief,and opportunities for further comprehensive treatments.The bisphosphonate can promote new bone formation and prevent further fractures.The surgical intervention combined with chemotherapy could relieve the symptoms,reduce tumor cell burden,improve quality of life and prolong survival time.
10.The clinical analysis of the pneumonia caused by acinetobacter baumannii

Objective To observe the clinical characteristics, risk factors, and sensitivity to antibiotics of nosocomial infections caused by acinetobacter baumannii. Methods Data were retrospectively collected from all isolated 37 strains acinetobacter. The clinical features, results of suptum culture and test of drug sensitivity were reviewed. Results The 37 acinetobacter baumannii strains mainly distributed in intensive care unit (ICU), most of them had the risk factors of receiving invasive treatment, mechanical ventilation, ect. The antibiotics imipenem, amikacin, pipercillin/tazobactam showed good efficacy for patients with acinetobacter infection, but other antibiotics had highly drug resistant rate. 5 were dead. The mortality of nosocomial infections caused by multi-drug resistant acinetobacter was 41.7%, which was much higher than the non-multi-drug resistant's (2 dead, the mortality was 8%). Conclusion Acinetobacter is one of the most important multi-drug resistant pathogen in nosocomial infections. Antimicrobial agents should be chosen according to antimicrobial susceptibility teat results. Patients who have the risk factors of nosocomial infections caused by acinetobacter should have suptum culture and antibiotic susceptibility studies as soon as possible.
