1.Progress in the personalized medicine using pharmacometabonomics.

Pharmacometabonomics, as an emerging branch of system biology, has been increasingly used in personalized medicine and showed broad prospects. By means of metabonomics, the complicated and detailed metabolic profile of the patient is described, thus providing more detailed description of the disease phenotype. With this understanding, response of different individuals to the drugs are predicted or evaluated through inherent genetic information of the individual combined with the environmental factors. As a result, appropriate drugs and dosage are chosen, which greatly promotes the realization of the individualized therapy goals. This article describes the emerging field of pharmacometabonomics, and the research results of personalized medicine based on the pharmacometabonomics in recent years are reviewed in detail.

2.Diagnosis of HIV infection in otolaryngology: a case report.

In the article we described a case of 61-year-old male with pharyngeal paraesthesia for 3 months. Physical examination: lean physique; vast uneven white membrane above hard palate, soft palate and pharynx mucous membrane, not easy to wipe and extend to the throat. The neck without cervical lymph node enlargement. Blood routine test: WBC 4.92 x 10(9)/L, N 64.3%, L 18.7%, EO 7.1%. RBC 4.08 x 10(12)/L, PLT 181 x 10(9)/L. No significant abnormal in the other blood biochemical indexes, tumor marker and immune indexes; blood bacteria culture: negative; blood culture: negative; sputum culture (3 times): all negative; anti-HIV screening test: positive, serum HIV testing: positive(the test done by Shanghai Pudong new area's centers for disease control and detection). The incidence of HIV/AIDS is still low at present, so the diagnosis of HIV/AIDS can be ignored easily by the otolaryngology doctor. If the patient with oral cavity and pharyngeal ulcer delayed healing, the doctor should be alert to,HIV/AIDS infection. We should check serum HIV antibody to eliminate or confirm HIV/AIDS earlier.

3.Advances in laparoscopic sphincter-preserving surgery for low rectal cancer

The incidence of low rectal cancer, which severely threatens human health, has been increasing annually in China. Sur-gical operation is the most effective treatment for low rectal cancer. In-depth analyses on the physiology and anatomy of the rectum and the biological characteristics of low rectal cancer have led to the wide application of anus preservation operation as a treatment for this disease. Anus preservation operation is widely used, especially because the application of the laparoscopic technique has recently be-come a popular topic in colorectal surgery. Alternative laparoscopic sphincter-preserving procedures for this disease include the follow-ing: laparoscopic anterior resection; and laparoscopic anterior resection with inter-sphincter resection or with prolapse technique. Colorectal anastomosis with double-stapler technique, hand-sewn coloanal anastomosis, and colonic J-pouch-anal anastomosis are com-mon reconstruction procedures. Each procedure has its own adaptive range, advantages, and disadvantages. Radical surgery must be pri-oritized, but we should also consider the patients' life quality when selecting a mode of surgery.
4.Clinical comparative study of modified versus traditional posterior lumbar interbody fusion for lumbar spinal stenosis

Objective To compare the clinical outcomes of modified and traditional posterior lumbar interbody fusion (PLIF) in treatment of lumbar spinal stenosis.Methods A retrospective review was made on 47 patients who sustained monosegmental L4.5 stenosis due to lumbar degenerative revise and characterized by typical intermittent claudication and single or both limb numbness and pain.The patients were randomly allocated to Group A and Group B using the lottery method.Group A (n =24) consisted of 10 males and 14 females at mean age of 52 years (range,47-66 years) and the course of disease averaged 13 months (range,9-23 months).Group B (n =23) contained 8 males and 15 females at mean age of 53 years (range,49-67 years) and the course of disease averaged 11.5 months (range,6-22months).Modified PLIF including spinous process replantation combined with canal H-shaped bone grafting for posterior column reconstruction and interbody fusion was performed in Group A.Traditional PLIF,namely posterior spinal decompression and interbody fusion,was performed in Group B.Waist function rehabilitation after surgery of the patients in the two groups was assessed using the Oswestry disability index (ODI) and postoperative results were analyzed.Results There were no significant differences between Groups A and B regarding the operation time [(1 90 ± 15.66) min vs (170 ± 11.32) min] and intraoperative blood loss [(980 ± 35.58) ml vs (879 ± 21.25) ml] (P > 0.05).Mean period of follow-up was 20 months (range,12-28 months).Postoperative results in Group A were graded as excellent in 19cases,good in three,fair in two and poor in zero,with excellence rate of 88%.While in Group B,the results were excellent in 13 cases,good in four,fair in four and poor in two,with excellence rate of 74%(P < 0.05).ODI score and symptom improvement rate after operation were better in Group A than in Group B (P < 0.05).Conclusion The modified PLIF including spinous process replantation combined with canal H-shaped bone grafting for posterior column reconstruction is relatively an ideal surgical technique for treatment of lumbar spinal stenosis and deserves wide promotion.

6.A comparison study of TCD vs MRI VE-ASL in the evaluation of collateral circulation of cerebral arter-ies

Objectives To evaluate the performances of transcranial Doppler (TCD) and vessel-encoded arterial spin labeling MRI perfusion imaging (VE-ASL) in the evaluation of collateral circulation of cerebral arteries. Methods Thirty patients with unilateral ICA or MCA stenosis diagnosed by TCD and carotid duplex ultrasound and confirmed by MRA underwent VE-ASL. Peak systolic velocity (PSV) of bilateral MCA in the same depth, distal to the stenosis site, was recorded. Cerebral blood flow (CBF) in bilateral MCA territories was measured by VE-ASL. The detection rates of collater-al flow using TCD and VE-ASL were compared. Results TCD revealed that PSV in the ipsilateral and contralateral MCA were (31.6 ± 10.5) cm/s and (83.1 ± 9.2) cm/s, respectively. VE_ASL revealed that CBF in the ipsilateral and contralateral MCA territory were (22.5±9.8) mL·min-1·100g-1 and (31.7±8.3) mL·min-1·100g-1, respectively. The PSV ratio of the ipsi-lateral/contralateral MCA was significantly lower than the CBF ratio of the ipsilateral/contralateral MCA(0.37 ± 0.173 vs. 0.66±0.141, P=0.001). The detection rates of collateral flow using TCD were lower than those using VE-ASL. The detec-tion rate was 26.7% vs. 70% on TCD vs. VE-ASL in anterior collateral circulation (P=0.001), was 16.7% vs. 60% (P<0.001) on TCD vs. VE-ASL in posterior collateral circulation. The total display rate of collateral flow was 36.7%vs. 86.7%on TCD vs. VE-ASL (P<0.001). Conclusions TCD is inferior to VE-ASL in evaluating the collateral circulation because of the limitations including the anatomical variations of the circle of Willis and formation of leptomenigeal anastomoses.
7.Differential expression profile of long non-coding RNA in the lipopolysaecharide-induced inflammation of monocyte-derived macrophages

Objective To analyze the expression profile of long non-coding RNA (lncRNA) in the lipopolysaecharide (LPS)-induced inflammation of monocyte-derived macrophages.Methods Peripheral blood mononuclear cells were derived from healthy donor and induced into macrophages. The macrophages were divided into blank control group and LPS (1 mg/L) stimulated 12 hours group. Culture supernatants and cell pellets were harvested in each group, enzyme linked immunosorbent assay (ELISA) was used to assay the production changes of interleukins (IL-1β and IL-6), and tumor necrosis factor-α (TNF-α) in the supernatant. The technique of lncRNA microarray was used to test the lncRNA expression profile in LPS-induced inflammation of macrophages and control macrophages. The raw data of lncRNA were pretreated for normalization. Five lncRNA expressions were validated by real-time quantitative reverse transcription-polymerase chain reaction (qRT-PCR). Furthermore, qRT-PCR was used to detect the expression of NR_028034 in macrophages after LPS-induced inflammation.Results ① The contents of IL-1β (ng/L:562.93±61.17 vs. 59.74±15.68), IL-6 (ng/L: 702.46±92.31 vs. 71.66±18.25) and TNF-α (ng/L: 794.50±63.89 vs. 85.12±22.07) in the LPS group were significantly higher than those in the blank control group (allP < 0.01). These results indicated that the inflammatory model of human macrophages was constructed successfully. ② Compared with blank control group, and 1479 lncRNA which have more than 2 folds variation and significant difference (P < 0.05) by statistical analysis was defined as lncRNA with differential expression. Among these lncRNA, LPS group showed 953 up- regulated and 526 down- regulated genes by 2 folds and 49 up- regulated and 35 down- regulated genes by 5 folds. ③ qRT-PCR results were generally consistent with the microarray data. ④ The expression of NR_028034 was increased by (4.41±0.65), (11.56±2.04), (18.58±1.36) folds compared with blank control group at 3, 6, 12 hours after LPS stimulation (allP < 0.01).Conclusions These data show a significantly altered lncRNA expression profile in the LPS-induced inflammation of monocyte-derived macrophages, suggesting that lncRNA may be involved in regulation of macrophages inflammatory response.
8.Effect of exercise training on amyloid-beta peptide and β-secretase in the hippocampus of the rats with vascular dementia

ObjectiveTo study the effect of exercise training on β-amyloid polypeptide (Aβ) and β-secretase(BACE) in the hippocampus of the rats with vascular dementia (VD).Methods 30 Sprague-Dawley (SD) rats were carried out to an exercise group (n =10 ),a model group (n =10 ),and a sham-operation group ( n =10 ).VD rat models were made by the ligation of bilateral common carotid arteries.Morris water maze test were carried out 4 weeks after the operation to assess the ability in learning and memory of the rats and Aβ and β-secretase (BACE)expression was detected in the hippocampus of the rats using immunohistochemical techniques.ResultsIn the Morris water maze test,the model group showed reduction in the learning and memorizing ability,with obvious longer escape latencies ( ( 101.34 ± 19.67 ) s,(95.42 ± 23.89 ) s,( 89.39 ± 22.67 ) s,( 90.12 ± 19.77 ) s,respective-ly) than that of sham-operation group ( ( 62.13 ± 11.38 ) s,( 24.84 ± 13.69 ) s,( 16.98 ± 12.51 )s,( 11.41 ± 8.93 ) s,correspond-dingly) (P < 0.05 ),and the exercise group was improved in the learning and memorizing ability ( corresponding to ( 80.15 ± 21.56 ) s,( 51.24 ± 20.91 ) s,( 43.78 ± 22.36) s,( 45.67 ± 20.87 ) s ),compared with the model group(P<0.05).The grey values of Aβ in the hippocampus of the rats for the exercise group was ( 130.12 ± 19.01 ),( 116.77 ± 23.67 ) for the model group and ( 148.44 ± 17.67 ) for the sham-operation group(P< 0.05).The grey values of BACE in the hippocampus of the ratsfor the exercise group were( 131.21± 25.25 ),( 120.53± 10.21 ) for the model group(P< 0.05 ) and ( 162.38 ± 28.11 ) for the sham-operation group (P < 0.05).ConclusionExercise training can lower the expression of BACE and Aβ in the hippocampus of rats with VD,therefore improving the learning and memory ability of rats with VD.

Objective To investigate the effects of rehabilitation training on hippocampal amyloid-beta peptide (Aβ) and insulin-degrading enzyme (IDE) levels in vascular dementia (VD).Methods Thirty female Sprague-Dawley rats were randomly assigned to a rehabilitation group (n =10),a model group (n =10) or a sham-operation group (n =10).An experimental VD model was established in the rats of the first 2 groups by bilateral common carotid artery permanent ligation.The rats in the rehabilitation group then received 1 h of rehabilitation training daily.Learning and memory were assessed at 4 weeks aftet the operation.Immunohistochemical staining was used to detect Aβ and IDE expression in the hippocampus dentate gyrus (DG) area.Results The rats in the rehabilitation group showed significantly better learning ability compared with the model group.The expression of Aβ in the rehabilitation group was significantly less than in the model group.The expression of IDE in the rehabilitation group was significantly greater Conclusion Rehabilitation can accelerate the recovery of learning and memory in VD,at least in rats The mechanism is possibly related to decreased accumulation of Aβ in the hippocampus due to up-regulation of the expression of IDE.
10.Studies on quality standard of Fushengkang Capsule

Object To establish the method for quality control of Fushengkang Capsule. Methods Semen Livistonae Chinensis, Fructus Camptothecae Acuminatae and Rhizoma Curcumae in this capsule were identified by TLC. The content of camptothecine was determined by HPLC. Condition of HPLC was: Dikma Diamonsil C 18 column (250 mm? 4 6 mm, 5 ?m) the mobile phase was a mixture of methanol water (55∶45), the flow rate was 0 8 mL/min, and detection wavelength at 254 nm. Results The spots of TLC were clear and concentrative. There was a good linear relationship for camptothecine within the range of 0 050 6 - 0 253 0 ?g. The average recovery was 96 90%, RSD=1 74% (n=6). Conclusion This method is sensitive, accurate, reproducible and exclusive. It can be used for quality control of Fushengkang Capsule
