1.Study on apolipoprotein E gene polymorphism and mtDAN4336 mutation in patients with Alzheimer's disease

Objective To study the distribution of Apolipoprotein E(ApoE) gene polymorphism and mtDAN*!4336 mutation in patients with sporadic Alzheimer's disease(AD),and analyse the difference of frequency of ApoE genotype from some other studies.Methods The polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) technique was used to analyze the ApoE allele genotype and mtDAN mutation of 127 AD patients and 138 healthy elderly controls.Results In AD group, we found 2.4% of the patients with 2 ApoE4 allele gene, 18.1% of the patients with 1 ApoE4 allele gene, and 79.5% of the patients without ApoE4 allele gene. However, in control group, the frequency was 0.7%, 10.1% and 89.2% respectively. There was a significant difference between the two groups( P
2.Overexpression of vascular endothelial growth factor in epidermis of human skin ulcerous margin

Objective To investigate the expression of vascular endothelial growth factor(VEGF) in human skin ulcer margin. Methods Ulcer biopsy samples were stained with anti VEGF and CD34 antibodies by immunohistochemistry .Results VEGF was not expressed in normal epidermis. The ulcer specimen was divided into normal,peripheral,marginal and central parts. Epidermis of normal part was not stained. VEGF was obviously expressed in peripheral upper epidermis of ulcer and lower epidermis of ulcer margin. Central part was not stained due to shedding of epidermis. And also CD34 staining demonstrated that dermal capillaries increased obviously and lumen dilated, which was marked in the periphery and the margin of ulcer. Conclusions VEGF is secreted in skin ulcerous epidermis and promotes angiogenesis and vascular dilataion which play a role in ulcer healing.

Objective]To summarize Professor Chen Huade’s clinical experience on treatment of hyperactivity of liver yang type vertigo by acupuncture. [Method]By combining with expositions of past dynasties, collecting and analysis of the clinical cases of liver yang type vertigo, to summarize the etiology and pathogenesis of liver yang type vertigo and Professor Chen’s treatment characteristics, and take a case as evidence.[Result]In Professor Chen’s opinion, the basic pathogenesis of hyperactivity of liver yang type vertigo is hepatorenal Yin deficient liver Yang syndrone. Belong to excess in the upper and deficiency in the lower, asthenia in origin and asthenia in superficiality. Based on the theory of “treating disease from the root”,cure this disease by the body acupuncture plus eartip bloodletting therapy, each had a good curative effect.The evidence showed, after the body acupuncture plus eartip bloodletting therapy ten times, vertigo symptom relieved obviously, blood pressure dropped to normal range. In the following up of 3 months, none had a recurrence. [Conclusion]Because of its cheap, non-toxic side effects, significant efficacy, and the characteristics of both symptoms and root causes curing, it is gradually accepted by more and more patients.Professor Chen Huade’s treatment on hyperactivity of liver yang type vertigo by acupuncture has significant effect in clinic which is worth spreading.
4.Analysis on the Clinical Application of Human Serum Albumin in Inpatients in Our Hospital in 2014

Objective:To evaluate the clinical application of human serum albumin in the inpatients in our hospital to promote its rational use. Methods:Totally 1 183 cases were selected from the inpatients treated with human serum albumin from January to De-cember in 2014, the clinical department, classification of diseases, age distribution and relevant concentration checks of serum albumin in the inpatients were analytically reviewed. Results: Totally 26 clinical departments were involved in the use of human serum albu-min,mainly in the digestive departments and ICU. The elderly patients aged above 65 years accounted for great proportion (47. 68%). In addition, all patients had relevant concentration checks of serum albumin. Conclusion:The patients with digestive diseases tend to have the largest consumption proportion of human serum albumin in our hospital. It is important for physicians to strictly follow the indi-cations of medication in using human serum albumin.
5.Research on speckle reduction in ophthalmology high frequency ultrasound imaging

Objective Speckle is the inherent problem exists in B-Mode ultrasound image,and effective speckle noise reduction will improve the image quality and contribute greatly to clinical doctors to diagnose,especially in fine ophthalmic examination with high frequency ultrasound.Methods This paper proposed a new speckle reduction method based on the Laplacian pyramid and multiscale analysis.In the Laplacian pyramid,true clinical features were separated from noise,according to the different bandpass ultrasound image characteristics in each layer,and the advanced eight directions speckle reduction anisotropic diffusion (SRAD)was adapted to suppressed the speckle noise,and the identified noise and the extracted features were selectively emphasized by suitable edge,coherence and contrast enhancement filtering from fine to coarse scales.The performance of the proposed method was compared to the traditional SRAD method and coherence-enhancing diffusion method by measuring the equivalent number of looks (ENL) and Time cost.Results The ENL and Time cost value of the proposed method were higher compared to the SRAD and the cedif method,i.e 1.172 3 vs 1.122 3,0.929 3 and 0.864 0 vs 1.396 0,1.468 3.Conclusions In summary,the proposed method can more effectively remove the speckle noise while preserving the edge and details of the images.
6.The effect of CYP 3A5 genotypic analysis of donor from cardiac death donation on the individualized administration of Tacrolimus

Objective To investigate the effect of the genotypic analysis of donor from cardiac death donation on the initial dose of Tac for liver transplant recipients and provide individualized administration for the early use of Tac in liver transplantation patients.Method Thirty recipients with a different genotype of CYP3A5 from cardiac death donors were collected from March 2010 to February 2013.The matched recipients were randomly divided into experiment group and control group.There was an adjustment of initial doses of Tac according to the donors' different CYP3A5 genotypes in experiment group but not in control group.Result In experiment and control groups,the average Tac blood concentrations at the 7th day after operation were (7.47 ± 1.83) and (8.68 ± 5.14) ng/mL,and the percent of recipeints reaching the optimal Tac concentrations was 72.2% and 38.9%,respectively (P<0.05).In experiment and control groups,22.2% and 55.6% recipients needed adjustments of Tac concentrations respectively (P<0.05).Conclusion Individualized adjustment of Tac initial doses of recipients according to cardiac death donors' different CYP3A5 genotypes was benefit for reaching optimal concentrations as soon as possible and could decrease the rate of rejection,and reduce the side effects of Tac.
7.Detection and identification of aldehyde dehydrogenase as a indicator in the assessment of liver quality in rabbits with brain death

Objective To explore the differential proteins between livers of control and brain dead grups,and to provide an experimental basis for the assessment of liver quality in brain dead rabbits.Methods 60 healthy male New Zealand rabbits were divided into two groups.The brain dead group (n=30) contained rabbits 2 hours (B1),6 hours (B2),and 8 hours(B3) after brain death.The sham group (n=30) contained groups of 2 hours (C1),6 hours (C2),and 8 hours (C3).At the end of the relevant experiments,blood samples and liver tissues were collected.The level of ALT and AST were determined by an automatic biochemistry analyzer and the morphologic changes of the livers were detected by HE staining.The differentially expressed proteins were screened and identified by two-dimensional gel electrophoresis,PDQuest software,matrix-assisted laser desorption ionization time of flight mass spectrometry,and the NCBI database.Results In 8 hour brain dead group,the level of ALT increased comparing with 6 h (P<0.05),but there was no significant statistical difference in the other groups.Under real time observation with the light microscope,the livers of the brain dead groups had increased edema and infiltration of lymphocytes in the portal area,especially in the 8 hour group.However,infiltration of neutrophils also appeared in the 8 hour control group and all groups had no damage in the liver cell.There were 10 kinds of differentially expressed proteins through the two-dimensional gel electrophoresis,mass spectrometry analysis,and database query.One protein of interest was ALDH2,which showed a gradually decreasing expression in the liver when the braid dead time increased.Conclusion Brain death could lead to no damage of liver function and little damage to liver morphology.The identified protein ALDH2 may be related with liver injury after brain death and could be a new indicator in the assessment of liver quality in brain dead rabbits.
8.Establishment of brain death model for rabbits and their pathophysiological changes

Objective To explore the way to establish the brain death model for rabbits and pathophysiological changes before and after brain death.Methods 80 healthy male New Zealand rabbits were divided into brain death group (n =60) and sham operation group (n =20),The 60 brain death rabbits were established by increasing intracranial pressure in a modified,slow,and intermittent way.The sham operation rabbits were only maintained with anesthesia.Results The 56 brain death rabbits were established successfully and maintained for 10 h with the respiration and circulation supports.2 rabbits died due to anesthetic accident,the other 2 died because of improper pressure.The surgical success rate is 93.3% (56/60).The changes of mean artery pressure (MAP) and heart rate (HR) in brain death group were more significant than in sham operation group:MAP and HR fluctuated and showed the increased tendency.The mean MAP and HR during increasing intracranial pressure were (400.24±18.36) mm Hg (1 mm Hg=0.133 kPa) and (258.00 ± 25.70) beats/min respectively,which was significantly higher than before and after increasing intracranial pressure (P<0.05).Conclusions The brain death model for rabbits could be established by increasing intracranial pressure in a modified,slow,and intermittent way successfully and maintained 10 h.the MAP and HR before and after brain death showed characteristic changes.The model is helpful to the further observation of organ changes in brain dead state.
9.Systemic review about the effect of the application of continuity nursing in community diabetes

Objective Systemic analysis the effect of the application of continuity nursing in community diabetes.Methods Retrieve documents from the PubMed,Cochrane Library,EMBASE,Science Direct,CBM,WANFANG Data and CNKI.Dating from the dates of establishment to May,2014.Randomized controlled trials (RCTs) on continuity of patient care for patients with diabetes were included.Two reviewers independently screened the literature according to the inclusion and exclusion criteria,extracted the data and assessed the quality of the included studies,and then meta-analysis was performed by RevMan 5.1 software.Results 5 RCTs (156 cases with diabetes) were involved into this Meta-analysis.The experimental group was superior to the control group in FBG,2h PBG,HbA1c and the level of treatment compliance,with statistically significant differences,P<0.05.The descriptive analysis showed that the differences in the score of diabetes knowledge and the quality of life were statistically significant,P<0.05,because the data could not be converted.Conclusions The continuity nursing can improve the compliance and the quality of life of community diabetes,ameliorate certain biochemical indicators.Study of large sample and RCTs should be take because of the existence of some restraining factors.
10.Speech Perception in Patients with Cochlear Implants and Hearing Aids in Opposite Ears

Objective The aim of the present study was to investigate the ability to perceive syllables phrases ,mono-syllabic words ,disyllabic words ,when using a CI and an HA in non -implanted ears (bimodal fitting) ,and the relationship be-tween aided thresholds in the HA ear and bimodal effectiveness .Methods Eighteen children who consistently used a bimodal fitting participated in the study .The loudness of speech in the HA ear matched that with implanted ear ,and the aided thresh-olds in the HA ear were obtained .The recognition rate of syllables phrases ,monosyllabic words ,and disyllabic words was tested under the aforementioned two modes in both the quiet and noisy backgrounds .Results The speech performance of children for monosyllabic words were 82 .67% ± 12 .23% ,83 .61% ± 12 .22% ,for disyllabic words 76 .00% ± 16 .13% ,78 .11% ± 14 .84% , for syllables phrases 60 .11% ± 17 .18% ,65 .43% ± 16 .76% ,with a CI alone or with bimodal fitting in a quiet environment ,re-spectively .In a noisy environment ,monosyllabic words were 75 .50% ± 14 .12% ,76 .83% ± 14 .15% ,and disyllabic words were 68 .22% ± 17 .15% ,77 .18% ± 16 .83% ,and syllables phrases were 49 .39% ± 19 .26% ,56 .33% ± 19 .55% .The results sug-gested that speech performance in a quiet or a noisy environment was significantly better with bimodal hearing than with a CI a-lone .All were statistically significant except the recognition rate of monosyllabic words in quiet background (P<0 .05) .Signifi-cant negative correlations were found between aided thresholds at 250 and 500 Hz ,and the bimodal advantages were noticeable for the some speeching perception .Conclusion Bimodal hearing enhances speech performance for deaf patients .The low -fre-quency residual hearing in the HA ear may play a major role in enhancing auditory and speech performance .
