1.Video-assisted thoracoscopic surgery for traumatic hemothorax

Objective To evaluate the application of video-assisted thoracoscopic surgery (VATS) in the treatment of traumatic hemothorax. Methods We performed VATS in 60 cases of traumatic hemothorax with medium amount of bleeding or above from March 2000 to March 2004, including 12 cases accompanying the prodrome of shock and 6 cases associated with injuries of liver or spleen. Unilateral VATS was carried out in 48 cases, all of which were completed under thoracoscope except 3 cases of heart injuries were supplemented with mini-incision repair. Bilateral VATS was conducted in 6 cases and the combination of thoracoscopy with laparoscopy was required in 6 cases. Results The operation time was 45~175 min (mean, 105 min) and the postoperative hospital stay was 7~13 days (mean, 11 days). No complication occurred. Conclusions VATS in the management of traumatic hemothorax is safe, effective and minimally invasive, with a relatively short operation time and quick recovery.

3.Application of Centurion Vision System in subluxated cataract

Objective To evaluate the safety and efficacy of Centurion Vision System for subhixated cataract surgery.Methods Clinical data of 15 eyes with lens subluxation treated with Centurion Vision System were retrospectively reviewed.Ocular variables including visual acuity,intraocular pressure,corneal endothelial cell counts (CEC),central foveal retinal thickness (CFRT),intraocular lens (IOL) position,as well as intraoperative and postoperative complications were observed.Results The best corrected visual acuity (BCVA) was 0.6 or better in 10 eyes (66.7%) 3 months after surgery,while the BCVA of the other 5 eyes was between 0.2 and 0.6 mainly due to pre-existing macular degeneration caused by high myopia or trauma.CEC and CFRT values demonstrated a statistically significant difference between the preoperative and all postoperative follow-ups (all P < 0.05).Three months after surgery,no serious surgeryrelated complications,such as retinal detachment,IOL dislocation,suprachoroidal hemorrhage,macular edema,were observed.Conclusion Centurion Vision System can facilitate phacoemulsification in eyes with subluxated lenses,and this technique is safe and effective due to providing excellent outcomes and avoiding vitreoretinal complications.
4.Identification of Dendrobii Caulis basing on ITS sequence.

Isolation of high-quality genomic DNA from dried and processed crude drug is the key for the DNA identification of Dendrobii Caulis. The DNA extract of Dendrobii Caulis was firstly compared using different method to isolate genomic DNA from dried and processed crude drug, including commercial DNA extracted kit and CTAB method. Using modified CTAB method (extracted from large samples), the genomic DNA was successfully isolated from Dendrobii Caulis, including Huangcao and Fengdou. The ITS regions were amplified using the purified DNA as template, and then cloned and sequenced. These ITS sequences were compared with data from Genbank database and our lab, 14 Dendrobium species and five similar species were identified from "Huangcao" and "Huangcao" slice, while six species and three similar species from "Fengdou" according to their sequence similarity. The study demonstrated that the dried Dendrobii Caulis could be identified using DNA molecular method, which could overcome deficiencies and limitations of traditional identification method and has a certain application prospects.

5.Research Progress on DNA Molecular Identification of Plants in the Genus Chrysanthemum

This review summarized the research status on DNA molecular identification of plants in the genus Chrysanthemum. Some case studies on representative DNA molecular identification techniques, which included ge-nomic in situ hybridization (GISH), DNA molecular markers and DNA barcoding, were described. Simultaneously, the merits, demerits and development of the techniques were discussed. The above work provides evidence for the identi-fication and resource utilization of plants in the genus Chrysanthemum.
6.Modified FOLFOX4 vs docetaxel-oxaliplatin program neoadjuvant chemotherapy in postoperative stage Ⅲ gastric cancer

Objective To compare the efficacy and safety of modified FOLFOX4 program with docetaxel-oxaliplatin-5-fluorouracil (DOF) program as neoadjuvant chemotherapy in stage Ⅲ gastric cancer patients.Methods In 86 cases of stage Ⅲ gastric cancer patients,42 received modified FOLFOX4 chemotherapy,44 cases were treated by DOF program.After two cycles of chemotherapy,the treatment effect and adverse reactions were evaluated.Results The total effectiveness in modified FOLFOX4 group and DOF group was 60% (25/42) and 68% (30/44) respectively (P > 0.05).Tumor control rate was 83% (35/42) and 89% (39/44) respectively (P > 0.05).The incidence of nausea,vomiting and leukopenia was higher in DOF group than modified FOLFOX4 group (P < 0.05).The D2 lymph node dissection rate between modified FOLFOX4 group and DOF group was 80% (20/25) and 87% (26/30) respectively (P > 0.05),the R0 resection rate was 72% (18/25) and 83% (25/30) respectively (P > 0.05).In 43 R0 resection patients using FOLFOX4 the median survival time was 38.7 months,1-,3-year survival rate was 90%,and 60% ; while that in DOF group the median survival time was 39.6 months,1-,3-year survival rate was 95%,and 75%.Multivariate analysis showed that postoperative TNM stage only was an independent risk factor for prognosis.Conclusions TNM stage was the independent risk factor for prognosis of gastric carcinoma patients after radical gastrectomy.
8.Changes of β-endorphin in both of plasma and CSF and its significance in children with acute encephaledema following infection of central nervous system

To explore the role of β-endorphin (β-EP) in the pathogenesis of the acute encephaledema, the levels of β-EP in both of plasma and CSF were determined by radioimmunoassay in 69 children with infection of central nervous system consisting of 39 cases with encephaledema and 30 cases without encephaledema, respectively. Another 19 cases without intracranial infection were as the control group. The results showed that the levels of plasma and CSF β-EP in the encephaledema group (50.74 ng/L±26.60ng/L,62.72ng/L±39.23ng/L) were significantly higher than those in without encephaledema group (32.78 ng/L±21.2ng/L,34.13ng/L±30.26ng/L)and the normal group (14.83ng/L±6.55ng/L,9.77ng/L±6.33ng/L),respectively (P<0.01).It is concluded that β-EP plays an important role in the occurrence and development of encephaledema in children with the infection of central nervous system.
9.Amino acid substitution or insertion patterns in penicillin-binding protein 2 in Neisseria gonorrhoeae isolates with reduced susceptibility to ceftriaxone

Objective To investigate the amino acid patterns in penicillin-binding protein 2(PBP2)in Neisseria gonorrhoeae isolates with reduced susceptibility to ceftriaxonc.and the relationship between the amino acid patterns and reduced ceftriaxone susceptibility.Methods DNA was extracted from 13 clinical isolates of N.gonorrhoeae.including 11 strains with decreased susceptibility to ceftriaxone and 2 sensitive isolates.The full-length penA gene encoding the penicillin-binding protein 2 was amplified and sequenced.BLASTn and BLASTx programs were used to assess the insertion and substitution patterns of nucleotides in penA gene and of amino acids in PBP2,respectively.Results BLASTn analysis revealed insertion or substitution of 18-38 nucleotides in the penA gene of gonococcal isolates with reduced ceftriaxone susceptibility.As shown by BLASTX analysis.there were five patterns of amino acid substitution or insertion in PBP2 of the 11 isolates with reduced ceftriaxone susceptibility.However.mosaic structure of PBP2 was not found in any of these isolates.Conclusion Mosaic PBP2 seems not to be the major factor contributing to the decrease in susceptibility of N.gonorrhoeae to ceftriaxone.
10.Medium-term follow-up of minimally invasive percutaneous nephrolithotomy in patients with renal function impairment

Objective To evaluate the medium-term outcomes of minimally invasive percutaneous nephrolithotomy in patients with renal function impairment.Methods Data of 811 patients who underwent minimally invasive percutaneous nephrolithotomy between Jan.2009 and Dec.2011 were retrospectively collected.Seventy-eight patients had a preoperative estimated GFR of 30-59 ml/(min · 1.73m2).The minimum follow-up was 1 year.Patients were divided into group 1 (improved or stable disease) and group 2 (worsened disease).Patient age was (42.8 ± 16.3) and (45.3 ± 14.2) separately,with male patients 41 and 7,female patients 25 and 5 respectively.Body mass index were (24.3 ± 4.1) kg/m2 and (25.4 ±5.2) kg/m2,history of open surgery or ESWL were 19 and 4.Mild hydronephrosis were 22 and 4.Staghorn stone were 22 and 5.Multiple access were 13 and 2.Operative time was (78.2 ± 30.4) min and (80.3 ±32.3) min.Stone-free rate 1 month postoperatively were recorded.Multiple regression analysis was used.Results The stone free rate at 1 month postoperatively was 85% by CT scan.Preoperative eGFR was (38.4±12.8) ml/(min · 1.73m2),and 1 ycar postoperative eGFR was (45.1 ±15.8) ml/(min · 1.73m2)in 78 patients.Renal function had improved in 31% of patients,but it remained the same or deteriorated in 54% and 15%,respectively at 1 year follow-up.On multivariate regression analysis,diabetes was associated with the deterioration of renal function.Conclusions At medium-term follow-up,renal function was maintained or improved in 85% of patients with chronic kidney disease who underwent minimally invasive percutaneous nephrolithotomy.Minimally invasive percutaneous nephrolithotomy could be an effective and safe option for the patients with chronic renal function impairment.Diabetes mellitus was independent predictive factor of renal function impairment.Therefore,patients with diabetes should be followed up carefully.Medical management is needed if necessassry.
