Objective To investigate the relationship between angiotensin converting enzyme (ACE) gene insertion/deletion (I/D) polymorphism and the severe morbidity of neonates. Methods According to the Neonatal Critical Score (draft), neonates were divided into three groups:control group (n=116), non-critical group (n=237) and critical group (n=98). ACE genotype was determined by PCR. Results The DD genotype was significantly higher in critical group compared with the other two. 2.Application of magnifying colonoscopy in diagnosis and treatment of colorectal neoplastic lesions

ObjectiveTo evaluate the clinical value of magnifying endoscopy in diagnosis and treatment of colorectal benign neoplastic lesions.Methods78 colorectal lesions in 61 patients were examined with magnifying colonoscopy after indigo carmine dye, and a pit pattern diagnosis was made for every lesion according to Kudo's classification.All the lesions were totally resected, and the specimen were sent for pathologic examinations.ResultsThe diagnostic sensitivity of neoplastic lesions was 98.4% and specificity was 85.7% when types Ⅰ and Ⅱ represented the pit pattern of nonneoplastic lesions, whereas types Ⅲ, Ⅳ, and Ⅴ represented adenoma and early colorectal cancer. The overall accuracy in differentiating adenoma and early colorectal cancer from nonneoplastic lesions was 96.2%.94.5% of adenomarous lesions were treated by colonoscopy.ConclusionThe magnifying colonoscopy can provide an instantenous accurate diagnosis of tumorous lesions in colon and rectum. Synchronize, minimally invasive and curative treatment is possible to be completed by using it for a large number of lesions.
3.Study on the clinical features and prognostic factors of primary anorectal malignant melanoma

Objective:To analyze the influence of clinicopathological features on the prognosis of anorectal malignant melanoma (AMM), and to establish a more accurate prognosis prediction model.Methods:From January 1, 2006 to December 31, 2018, at Fudan University Shanghai Cancer Center, the data of 89 patients diagnosed with AMM and underwent operation were retrospectively analyzed. The Cox proportional hazard regression model was used to analyze factors affecting the prognosis. Backward elimination was used to select variables, Nomogram prognosis prediction model was established and verified internally, and the consistency index was calculated.Results:Among 89 AMM patients, 65 (73.0%) were female, 78(87.6%) were <70 years old, and the most common tumor location was the rectum (48.3%, n=43), followed by the anal canal (31.5%, n=28) and the anorectal canal (20.2%, n=18). Thirty-eight (42.7%) patients directly received abdominal-perineal resection (APR), 37 patients (41.6%) received interferon-based immunotherapy, and 11 patients (12.4%) received both systemic chemotherapy and immunotherapy. The 3-year disease-specific survival (DSS) rate was 41.6%(37/89), and the 5-year DSS rate was 31.5%(28/89). The results of multivariate Cox analysis showed that age ≥70 years old and T2 stage were risk factors of the prognosis of AMM patients (hazard ratio ( HR)=11.29, 4.83; 95% confidence interval ( CI) 2.89 to 44.13, 1.66 to 14.11; both P<0.01), while neurovascular invasion, immunotherapy treatment, systemic chemotherapy combined with immunotherapy, APR after extensive local resection, directly APR and APR combined with inguinal lymphadenectomy were protective factors of the prognosis ( HR=0.09, 0.23, 0.10, 0.13, 0.26, 0.02; 95% CI 0.02 to 0.34, 0.10 to 0.57, 0.02 to 0.49, 0.03 to 0.52, 0.08 to 0.90, 0.00 to 0.27; all P<0.05). The Nomogram model was further established with age, gender, tumor location, T stage, distant metastasis, medication chemotherapy and surgical treatment. The results of the Nomogram model internal verification indicated that the accuracy of the model in predicting 1-year, 3-year and 5-year DSS was good, and the consistency index was 0.749, which was significantly higher than the consistency index of traditional TNM stage (0.607). Conclusions:Most AMM patients are <70 years old, and the majority of them are female. The common location of AMM is rectum, and many patients receive immunotherapy. Age ≥70 years old and T2 stage are risk factors affecting the prognosis of AMM patients. Neurovascalar invasion, immunotherapy based comprehensive treatment, APR after extensive local resection, directly APR and APR combined with inguinal lymphadenectomy are protective factors of the prognosis. Nomogram prognosis prediction model established based on the clinicopathological features and treatment of AMM patients has higher accuracy and clinical reference value than the traditional TNM stage system.
4.Detection of Sodium Caprylate in Human Serum Albumin by Precolumn Derivation Reversed Phase High Performance Liquid Chromatography

A method was developed for the determination of sodium caprylate in human serum albumin by reversed phase high performance liquid chromatography(RP-HPLC) after pre-column derivatization. The caprylic acid, extracted from human serum albumin by hexane, was treated with ω-bromoacetophenone and 18-crown-6 for 30 min at 50 ℃, and analyzed on a Nova-Park C_(18)(150 mm×3.9 mm, 4 μm) column with methanol-water(75∶ 25, V/V) as the mobile phase. The internal standard was enanthic acid, the flow rate was 1.0 mL/min and the detection wavelength was 262 nm. The extraction yield of caprylic acid was 97.9% and that of enanthic acid was 98.2%. The linear range of sodium caprylate was 9.00×10~(-4)-1.44×10~(-2) mol/L(r=0.9995). The average recovery of caprylic acid was 99.7%, RSD was less than 0.9%. The present method is reliable and relatively simple and can be used for the determination of sodium caprylate in human serum albumin.
5.Preparation and Characterization of Butylaminopropyl Silica Monolithic Column for Capillary Electrochromatography

A novel silica monolithic stationary phase functionalized with butylaminopropyl ligands for capillary electrochromatography(CEC) has been presented. The monolithic capillary columns were prepared by a sol-gel) process and subsequent a chemical modification. The amino groups on the surface of the stationary phase are meant to generate a substantial anodic electroosmotic flow (EOF). The butyl and propyl groups provide) hydrophobic properties. To evaluate the column performance, effects of buffer pH and organic modifier content on the EOF and electrochromatographic retention behavior of alkylbenzenes, organic acids and anilines were investigated. The monolithic stationary phase exhibited reversed phase (RP) chromatographic behavior toward neutral solutes. The model organic acid anion solutes were separated by the mixed mode mechanism, which comprised RP interaction, weak anion-exchange, and electrophoresis. Basic compounds such as anilines) were well separated on the butylaminopropyl silica monolithic column without peak tailing.
6.Titanium surface covered with antimicrobial coating:antibacterial ability and biocompatibility

BACKGROUND:Titanium al oy with good biocompatibility, corrosion resistance and mechanical properties have been widely used in clinic. How to give its excel ent antibacterial properties so as to cope with plant-associated infections has become a research focus in recent years. OBJECTIVE:To review the principle, techniques, classification and relative merits of antimicrobial coating. METHODS:A computer-based search of Scopus database and VIP database was performed by the first author to retrieve relevant articles published from January 1990 to January 2014 using the keywords of“titanium al oy, plant, antibacterial, coating”. RESULTS AND CONCLUSION:Coatings can be classified into antibiotic coating, non-antibiotic organic antimicrobial coating, inorganic antibacterial coating, anti-adhesion coating, antibacterial bioactive polymer coating, al of which have better biocompatibility, but also have their limitations. Current studies concerning antimicrobial coatings mainly focus on how to enhance the binding force between antimicrobial coating and the substrate as wel as how to get a good anti-bacterial ability, biocompatibility, high wear resistance and persistence;antibacterial phase structure and distribution effects on the bacterial colonization. The antibacterial phase structure and distribution is the key factor for the antimicrobial properties of titanium al oys with either entirely added anti-bacterial elements or surface coating.
7.Establishement of phenobabital addiction rat model by modified drug admixed food method

Objective To establish the phenobarbital addiction rat model by the modified drug‐admixed‐food(DAF) method . Methods Rats were selected as the experimental animal .The modified DAF method was adopted to construct the model .The initial dose of phenobarbital 75 mg/kg was given mixed and the dase was gradually increased day by day ,which continued for 45 d .The es‐tablished model was verified by the precipitated withdrawal method and the natural withdrawal method for judging whether the model construction succeeding .Results The convulsion rate in the DAF precipitated withdrawal group was higher than that in the control group(P< 0 .01) ;the withdrawal symptoms scores in the DAF natural withdrawal group were higher than those in the con‐trol group(P< 0 .05) ;the relieving degree of body mass in the two withdrawal groups was greater than that in the control group (P< 0 .05) .The brain section in the mode group revealed obvious cellular apoptotic change .Conclusion DAF is simple and feasible for establishing the phenobarbital addiction rat model .
8.Clinical analysis of 72 patients with cardiogenic cerebral embolism.

Objective To study the etiology,clinical features and prognosis of cardiogenic cerebral embolism.Methods Cardiogenic cerebral embolism patients in the ward of Department of Cerebral vessels from April 2007 to December 2007 were registered prospectively,and then the clinical data of the patients after 3 months follow-up were analyzed.Results 72 patients were included,49 patients had atrial fibrillation.At the end of the 3 month-follow-up,15 patients were dead,the total mortality was 20.8%.25 patients must depend on others to live on.Conclusion Atrial fibrillation was the most common etiology of cardiogenic cerebral embolism.Internal carotid artery systerm was easier to form embolism than vertebrobasilar systerm.Compared with vertebrobasilar systerm,internal carotid artery systerm was much worse on nerve function impairment and prognosis.Positive measures must be taken to prevent and treat primary cardiopathy,and reduce the rate of cardiogenic cerebral embolism.
9.Relation between Chinese Medical Constitutions and Chemotherapy-induced Leucopenia in Breast Cancer Patients: a Clinical Study.

OBJECTIVETo analyze the relationship between Chinese medical constitutions and chemotherapy-induced leucopenia (CIL) of primary breast cancer patients.
METHODSTotally 306 breast cancer patients undergoing adjunctive chemotherapy for the 1st time, and effective 291 breast cancer patients were recruited in this study.Nine Basic Constitutional Scale was used before first chemotherapy. Chinese medical constitutions were classified and quantitatively scored. The highest grading for any item of adverse reactions in each case during the whole chemotherapy course was recorded after chemotherapy. Data were statistically analyzed using SPSS16.0.
RESULTSThere was no significant difference in CIL between different chemotherapy regimens and various Chinese medical constitutions of breast cancer patients (P > 0.05). Yang deficiency constitution is one risk factor for CIL. The higher the score of yang deficiency constitution, the more severe the CIL.
CONCLUSIONSYang deficiency constitution was correlated with the degree of CIL. The higher the score of yang deficiency constitution, the greater the risk of III-IV grade CIL in breast cancer patients.

10.Effects of anaesthetic concentration of sevoflurane on TM3 mouse leydig cell viability

Objective To investigate the effects of anaesthetic concentration of sevoflurane on TM3 mouse leydig cell viability.Methods TM3 mouse leydig cells were randomly divided into 3 groups ( n =24 dishes each):control group (group C),2% and 5% sevoflurane groups (groups SEV1 and SEV2 ).The cells were collected after being exposed to sevoflurane or 95 % room air + 5 % CO2 for 2,4 or 6 h (T1-3) for microscopic examination with optical binocular inverted microscope.The number of live cells was counted by using cell counting kit8.Gene chips were used to indentify differentially expressed genes between group C and group SEV2 after being exposed to air and 5 % sevoflurane for 6 h respectively.Results The leydig cell viability was significantly decreased at T3 in group SEV2 as compared with groups C and SEV1.Morphological changes were found only in group SEV2.A total of 45 genes were identified to be differentially expressed in group SEV2 as compared with group C.The level of expression of prostaglandin-endoperoxidase synthase 2 gene (Ptgs2),chemokine (C-C motif) ligand 2(CCL2) gene and dual specificity phosphatase1 (Dusp1) gene increased by at least 4 times in group SEV2.Conclusion Sevoflurane can inhibit the cell viability of TM3 mouse leydig cell in concentration dependent manner through abnormal expression of Ptgs2,CCL2 and Dusp1 genes.
