2.THE PROTECTIVE EFFECTS OF VITAMIN B_(12) ON SPATIAL COGNITIVE ABILITY IMPAIRED BY HOMOCYSTEINE IN RATS

Objective: To observe the relationship between homocysteine(Hcy)and spatial cognitive ability and to evaluate whether vitamin B12 supplementation could protect spatial cognitive ability in rats. Methods: The rats were randomized into three groups. The experimental group was given methionine subcutaneously. The intervention group was given methionine and vitamin B12 subcutaneously. The control group received isometric normal saline. After being raised for 8 weeks, all rats were examined for plasma Hcy and serum vitamin B12 and scores of Y-maze test. The brain tissues of hippocampus were checked immunohistochemically for acetylcholinesterase (AChE). Results: The experimental group developed hyperhomocysteinemia and descreased spatial cognitive ability significantly (P0.05). Conclusion: Homocysteine can impair the spatial cognitive ability in rats and vitamin B12 is effective in lowering Hcy level and protect the spatial cognitive ability of rats.
3.Minimally invasive surgery in patients with middle-upper advanced gastric cancer

Objective To investigate the effect of minimally invasive surgery in patients with midge-upper advanced gastric cancer (AGC). Methods 167 patients with middle-upper AGC underwent total gastrectomy+D2 lymph node dissection from January 2009 to December 2013 were enrolled in the study and divided into laparoscopic assisted total gastrectomy (LATG) group (n = 57) and open total gastrectomy (OTG) group (n = 110). Baseline data, operative, postoperative data and follow-up result were compared between the two groups. Results There were no significant differences in baseline data between the two groups (P > 0.05); The intraoperative blood loss and incision length in LATG group were significantly lower than that in OTG group (P < 0.05). There were no significant differences in operation time, ratio of intraoperative blood transfusion, distance between the upper margin and the tumor, the number of lymph node dissection between two groups (P > 0.05); Postoperative first aerofluxus time, ecovery liquid diet time, postoperative hospitalization days and VAS score of pain in 3 d after operation in LATG group were significantly lower than that in OTG group (P < 0.05). There was no significant differences in incidences of postoperative complication between two groups (P > 0.05); There was no patient dying within 30 d after surgery.The median follow-up time was 38.2 months (1~58 months). Postoperative 1, 3 year overall survival rates in LATG group and OTG group were 87.7% and 83.6%, 71.9% and 64.5%, and there were no significant differences between the two groups (P > 0.05). Conclusion For patients with middle-upper AGC, LATG+D2 lymph node dissection has lots of advantages including less intraoperative blood loss, shorter incision, faster postoperative recovery, shorter hospital stay and so on, which is safe and effective, and their short-and long-term results are satisfactory.
5.Progress on treatment and research of quadrilateral plate fractures of acetabular.

Acetabular is an important human joint for weight bearing. Quadrilateral plate is a crucial structure of medial acetabulum with special morphology and important function. Quadrilateral plate fractures are common fracture in acetabulum. Quadrilateral plate fracture is hard to expose and reduction because it is in the medial of acetabulum. At the same time,the bone in the quadrilateral plate is not easy to fixed for thinning bones and adjacent to the articular cavity. The operator should know well about the anatomy and choose the suitable internal fixation. After quadrilateral plate fractures, the femur head maybe displace medially even break into pelvis. That make reduction and treatment always be a challenge. With different kinds of fractures,the efficacy of treatment is not the same. This paper intend to review the relation of anatomic features,approaches, internal fixations, key point of treatment and efficacy.

7.Optimum target plasma concentration of propofol in preventing adverse effects of carboprost tromethamine in patients undergoing caesarean section

Objective To investigate the optimum target plasma concentration of propofol in preventing the adverse effects of carboprost tromethamine in the patients undergoing caesarean section.Methods One hundred and twenty-eight nulliparous parturients who were at full term with a singleton fetus,of American Society of Anesthesiologists physical status Ⅰ or Ⅱ,aged 24-37 yr,weighing 54-78kg,scheduled for elective caesarean section under combined spinal-epidural anesthesia,were randomly divided into 4 groups (n =32 each) using a random number table:control group (group C),and different concentrations of propofol groups (P1-3 groups).Carboprost tromethamine 250 μg was injected into the body of the uterus,and propofol with the target plasma concentrations of 0.8,1.2 and 1.6 μg/ml was simultaneously given by target-controlled infusion in P1,P2 and P3 groups,respectively,and normal saline was infused at a rate of 0.5 ml · kg-1 · h-1 in group C.The occurrence of cardiovascular events was recorded from the end of carboprost tromethamine administration until the end of surgery.The relatedadverse effects after carboprost tromethamine administration,and Ramsay sedation score at 15 mm after carboprost tromethamine administration were recorded,and satisfactory sedation was defined as Ramsay sedation score of 2.The occurrence of complications associated with combined spinal-epidural anesthesia was recorded during the postoperative follow-up.Results Compared with group C,the incidence of carboprost tromethamine-related adverse effects was significantly decreased in P2 and P3 groups,the rate of satisfactory sedation was significantly increased in P1 and P2 groups,the incidence of hypotension and tachycardia was significantly decreased in group P1 (P<0.05),and no significant change was found in the incidence of carboprost tromethamine-related adv erse effects in group P1,and in the rate of satisfactory sedation in group P3 (P> 0.05).Compared with group P1,the incidence of carboprost tromethaminerelated adverse effects was significantly decreased in P2 and P3 groups,the rate of satisfactory sedation was significantly increased in group P2,and the rate of satisfactory sedation was significantly decreased in group P3 (P<0.05).Compared with group P2,the rate of satisfactory sedation was significantly decreased (P<0.05),and no significant change was found in the incidence of carboprost tromethamine-related adverse effects in group P3 (P>0.05).No cardiovascular events were found in group P2,and the incidence of hypotension was 9% in group P3.Conclusion The optimum target plasma concentration of propofol in preventing the adverse effects of carboprost tromethamine is 1.2 μg/ml in the patients undergoing caesarean section.
8.INFLUENCE OF TAURINE ON LEARNING AND MEMORY CYTOKINES IN RATS INTRAHIPPOCAMPALLY INJECTED WITH ?-AMYLOID

Objective: To investigate the effect of taurine on learning and memory impairment, cytokines secretion in rats intrahippocampally injected with ?-amyloid (A?) 1-40. Methods: SD rats were randomly divided into control group, A? injected group, taurine (0.3g/kg?d, 0.6g/kg?d) groups. The rats were fed with taurine for 7 days, and then subjected to bilateral intrahippocampus injection of A?1-40 or vehicle. Two weeks later, all rats performed Morris water maze test. The contents of IL-6, TNF-? were checked by way of radio-immunity assay for hippocampus samples. Results: Compared with A?model group, the escape latency and distance were significantly reduced in taurine (0.6g/kg?d) group; the ratio of swimming distance in the target quadrant to that in the whole pool of the probe trial; the content of cytokines of IL-6 and TNF-?in hippocampus were reduced significantly. Conclusion: Taurine can effectively attenuate the cognitive dysfunction caused by A?1-40 in rats. The reduced cytokines content in hippocampus might contribute to this effect.
9.Screening for Y chromosome azoospermia factor region microdeletions using modified multiplex PCR

Objective To establish an universal primer-multiplex PCR system for diagnosis of Y chromosome AZF region microdeletions in 262 patients with non-obstructive azoospermic and severe oligozoospermic male infertility. Methods In each panel of multiplex PCR, YUP and YCP containing a fragment of non-human DNA sequence at their 5' ends were designed. The universal primers and chimiric primers were employed for the amplification at the same multiplex PCR system to screen for the Y chromosome AZF region ( a, b and c) microdeletions in 262 non-obstructive azoospermic and severe oligozoospermic male infertility patients. Results Thirty-three out of 262 patients (12. 60% ) were detected with Y chromosome AZF microdeletions. Among them, 27 cases were AZF c microdeletions and 6 ones were AZF b + c microdeletions. These results were in agreement with the results from EMQN method. There was no false-positivity. The gel electrophoresis for detection of multiple STS from both methods showed that the sY84,sY86, sY127, sY134, sY254, sY255, SRY bands were homogeneous and clear with similar brightness. Conclusion The modified multiplex PCR is suitable for screening of Y chromosome AZF microdeletions in non-obstructive azoospermic and severe digozoospermic male infertility patients.
