2.Toxicological mechanisms of lung injurant and poisoning management

The lung is one of the important target organs of chemical poisoning. Chemical toxins may be absorbed into body through blood circulation of the lungs,inducing systemic damage. Mean?while,they may also directly damage the lungs themselves. Lung injury is usually rapid and life-threatening. This review systematically introduced the toxicological mechanisms and poisoning management of lung injury agents.
3.Effect of bladder instillation of drugs on postoperative recurrence of superficial bladder cancer

Objective To compare and analyze the Brucea javanica oil emulsion,hydroxy camptothecin bladder perfusion of recurrence after operation of superficial bladder cancer.Methods 80 cases with superficial blad-der cancer were randomly divided into group A and group B (n=40 ).Group A received intravesical instillation of Brucea javanica oil emulsion,group B received intravesical instillation of hydroxycamptothecin.The serum tumor marker levels were compared between two groups,and the recurrence rate and adverse reactions in the two groups were observed.Results At 1 day before operation,postoperative 7d,the serum Dickkopf-1 (DKK-1 ),Dickkopf-2 (DKK-2),Dickkopf-3(DKK-3),Dickkopf-4(DKK-4),vascular endothelial growth factor A(VEGFA), vascular endothelial growth factor B(VEGFB),vascular endothelial growth factor C(VEGFC)of the two groups had no statistically significant differences (P>0.05).Postoperative 6 months,the serum levels of DKK-1,DKK-2, DKK-3,DKK-4,VEGFA,VEGFB,VEGFC in group A were (20.62 ±2.84)g/L,(29.15 ±6.32)g/L,(18.66 ± 4.53)g/L,(32.34 ±6.42)g/L,(80.72 ±9.16)g/L,(60.43 ±16.73)g/L,(70.31 ±8.41)g/L,respectively, which were lower than those of group B [(27.42 ±3.76)g/L,(38.16 ±4.34)g/L,(30.37 ±5.35)g/L,(47.26 ± 7.72)g/L,(146.31 ±12.44)g/L,(83.14.14.07)g/L,(91.58 ±7.82)g/L,respectively,t=-2.92,-2.58,-2.82,-2.41,-2.80,-2.75,-2.70,all P<0.01].The recurrence rate of group A was 7.5%(3/40),which was significantly lower than 15.0%(6/40)of group B(6/40)(χ2 =6.11,P <0.05).The incidence rate of adverse reac-tions of group A was 12.5%,which was significantly lower than 27.5% of the control group(χ2 =5.46,P<0.05). Conclusion Compared with hydroxyl camptothecine vesical perfusion,the effect of intravesical instillation of Brucea javanica oil emulsion in preventing the postoperative recurrence of superficial bladder cancer is more significant,can effectively reduce the levels of serum tumor marker,with less adverse reactions,it is worth application and promotion.
4.Exploration of the mechanism for performance appraisal and allocation of primary healthcare institutions in Suichang county of Zhej iang province

In view of such outcomes of the performance pay practice at primary healthcare institutions as decline of motivation of medical workers,the county explored the mechanism of graded performance appraisal and allocation,against the macro background of ceiling setting for the total of performance pay andbalance subsidy and total amount controlfiscal policy.The following measures are taken,namely a dual linkage to relate the performance appraisal outcome of a township hospital with both allocation of fiscal subsidy and setting of the merit-based performance pay standard per person;higher ratio of merit-based performance pay;floating of the performance pay level per person for the institution in question;separation of the performance pay of the chief leader from others;and subsidies for remote mountainous areas.It is proposed by the author to build a routine increase mechanism for both the total of performance pay and fiscal subsidies,to combine specific subsidy with pay for services,and to join funding compensation with service performance.These measures are expected to create a new mechanism for performance pay allocation,featuring more pay for more work and more pay for better performance.
5.The molecular-targeted therapy in advanced differentiated thyroid cancer

Along with the extensive understanding of differentiated thyroid cancer (DTC), its distinct molecular genotype and signaling pathway was revealed, which improved the development of molecular-targeted therapy. Regarding radioactive iodine-refractory DTC, multiple multikinase inhibitors including sorafenib and lenvatinib, which target vascular endothelial growth factor (VEGF) pathway, were proved to be effective. Moreover, selective inhibitors and redifferentiation agents were shown to be promising. In the future, individual genetic testing would provide more speciifc information in directing individualized molecular-targeted therapy.
6.Role of Clinical Pharmacists in Modern Pharmaceutical Care

OBJECTIVE:To investigate the status quo of clinical hospital pharmacy in hospitals in Fujian province.METHODS:A total of 575 questionnaires were given out in 10 general hospitals to investigate the status quo of clinical pharmacy and the role of physicians and nurses(primary nurses)in the pharmaceutical care.RESULTS:The hospitals in Fujian attached great importance to clinical pharmacy,but physicians and nurses(primary nurses)had a poor knowledge on the clinical pharmaceutical care and lacked trust in pharmacists' capability.CONCLUSION:The status quo of clinical hospital pharmacy in hospitals in Fujian province is far from perfect,the key to improve clinical hospital pharmacy level is to improve pharmacists' professional quality.
7.Effects of Rhizoma Sparganii and Rhizoma Curcumae on the Angiogenesis of Newly Born Tissue in Artificial Sponge in Rats

OBJECTIVE:To explore the inhibitory effects and mechanism of Rhizoma Sparganii and Rhizoma Curcumae on the angiogenesis of new granulation tissue in sponge.METHODS:The experimental endometriosis in female rats was established with sponge implantation.The effects of Rhizoma Sparganii and Rhizoma Curcumae on the angiogenesis of new granulation tissue in sponge were observed after intragastric medication for 2 consecutive weeks.The staining of endothelial cell specific Ⅷ factor and vascular endothelial cell growth factor(VEGF) was carried out by immunohistochemistry method.The expression of VEGF mRNA was detected by RT-PCR assay.RESULTS:In rats receiving high dosage of Rhizoma Sparganii and Rhizoma Curcumae(SC),the area of new granulation tissue in sponge,the number of blood vessels,the expression of VE-GF and VEGF mRNA were all significantly lower than in model group(P

10.Drug-induced Exfoliative Dermatitis:Literature Analysis of 240 Cases

OBJECTIVE:To probe into the drug categories causing high incidence of exfoliative dermatitis,and to explore the methods for the prevention and cure of this disease.METHODS:240 exfoliative dermatitis cases publicly reported in domestic journal between 1998 and 2008 were reviewed retrospectively and analyzed statistically.RESULTS:Among the total 240 cases,144 were male vs.96 female,which was chiefly induced via oral and intravenous routes;a total of 19 drugs categories(133 kinds)were involved,dominating the first 3 places were anti-infective drugs,central nervous system drugs and antipodagric drugs.Allopurinol,carbamazepine and phenobarbital were top three drugs among all kinds of drugs inducing exfoliative dermatitis.15 patients(6.25%)died.Exfoliative dermatitis was chiefly treated using stosstherapy in which a large dose of the corticosteroids combined antihistamine drugs and calcium gluconate were used,meanwhile antibiotic drugs was used as an anti-infective therapy to reduce the death secondary to septicemia.CONCLUSION:It is urgent to tighten monitoring on the use of key drugs in clinical practice to avoid and decrease the incidence of exfoliative dermatitis induced by anti-infective drugs.
