5.Regulating mechanisms of homeohox gene HOXA7 in tumor genesis and development

Homeobex gene,derived firstly from Drosophila, has the ability of regulating normal embry-onic development and proliferation and differentiation of adult tissue cells. Recent studies demonstrate that HOXA7 can be high and low expressed in adult tumor tissues by means of genetic fusion and genetic silencing to regulate genesis and development of tumor cells. To study its expression and regulation in tumor will be helpful for tumor diagnosis ,treatment and prognosis.

7.Progress on oral Helicobacter pylori infection and its diagnostic techniques

Helicobacter pylori (Hp) is of high prevalence and can cause lifelong infection.Hp can parasite in oral cavity,which may be not only an important cause of recurrent gastrointestinal diseases,but also affect Hp eradication in stomach.Bacterial culture,rapid urease test (RUT) and polymerase chain reaction (PCR) have been widely used in diagnosis of gastrointestinal Hp infections.While due to the difference of microbial environments in oral cavity and in stomach,there exit big disparities when they are used to detect Hp in oral cavity.Saliva Hp urease antigen detection method (HPS) is a graphic immune chromogenic assay.Hp salivary culture with HPS test can help to confirm the existence of Hp in oral cavity,so as to improve Hp eradication rate through concurrent treatment of gastrointestinal and oral Hp infections.The status of oral Hp infection,its correlation with gastric Hp infection,and new techniques of oral Hp detection and culture are reviewed in this article.
8.Clinical diagnosis value of colonoscopy and pathological examination for the etiology of colon and rectum ulcer

Objective To explore the clinical diagnosis value of colonoscopy and pathological examination for the etiology of colon and rectum ulcer.Methods 120 patients with colorectal ulcer were examined by colonoscopy and pathological examination,and differential diagnosed.Results The rate of endoscopic diagnosis combined pathological diagnosis for ulcerative colitis,tuberculosis,crohn's disease,amebic,chronic bacterium dysentery were different from the endoscopic diagnosis rate and pathological diagnosis rate,the differences were significant (x2 =12.37,13.58,15.02,17.77,18.39,P < 0.05 ) ; In the nausea lymphoma,colorectal cancer,endoscopic diagnosis combined pathological diagnosis with quite a statistically significant difference between the rate of endoscopic diagnosis( x2 =10.95,11.40,P < 0.05 ).Conclusion For the colorectal ulcer disease,colonoscopy combined pathological examination was an ideal means of inspection.
9.Clinical effectiveness study of laparoscopic and open radical gastrectomy for gastric cancer patients

Objective To investigate the clinical effectiveness of laparoscopic and open radical gastrectomy for gastric cancer patients.Methods Seventy-two gastric cancer patients who underwent laparoscopic or open radical gastrectomy were enrolled in this study.Laparoscopic radical gastrectomy group had 38 cases,open radical gastrectomy group had 32 cases.The operating time,length of incision,bleeding volume during operation,postoperative hospital stay and the level of C reactive protein were compared.Results All patients were performed operation successfully.No patient transferred open operation in laparoscopic radical gastrectomy group.No death because of operation.The patients in laparoscopic radical gastrectomy group took longer operating time,but shorter length of incision,less bleeding volume during operation,there was significant difference (P < 0.05).But the number of clearing lymph nodes between two groups had no significant difference (P > 0.05).Compared with open radical gastrectomy group,the patients in laparoscopic radical gastrectomy group had shorter exhaust time,early out-of-bed activity,shorter postoperative hospital stay,lower rate of complication,there was significant difference (P < 0.05).The level of C reactive protein before operation had no significant difference between two groups (P > 0.05),and level of C reactive protein had the tendency of improve previously and decrease lately.The level of C reactive protein in laparoscopic radical gastrectomy group was significantly lower than that in open radical gastrectomy group at the postoperative 1st,3rd,5th day[(9.33 ± 0.27) mg/L vs.(11.29 ± 0.42) mg/L,(7.16 ± 0.18) mg/L vs.(9.87±0.65) mg/L,(4.38 ±0.41) mg/Lvs.(6.97±0.51) mg/L](P<0.05).Conclusion Laparoscopic radical gastrectomy is useful in treating gastric cancer for it's advantage of better security,better effectiveness,quicker recover and less trauma.
10.Some Special Problems concerning Idiopathic Hypoparatbyroidism

There have been more than 100 cases of idiopathic hypoparathyroidism(IHP) reported in our country. The 8 cases reported in this paper showed some special features in the clinical manifestations, the biochemical parameters, and the response to treatment.Clinical, one case was complicated with Klinefelter's syndrome. Another case suffered from subcorneal pustulosis prior to the onset of idiopathic hypo-parathyroidism. Two cases were misdiagnosed as epilepsy and admitted to the department of neurology. Generalized exfoliative dermatitis was induced in one case by large doses of antiepileptic agents to control the convulsive seizures.Biochemically, both CPK and LDH were elevated in one case. Serum calcium was as low as 2.4 mg/dl in one case, and serum phosphrus was as high as 11.7 mg/d1 in another. These phenomena were rarely encounted in IHP.Therapeutically, transplantation of homogenous parathyroid gland with blood vessels was the method of choice. Two cases were treated in this way with good results. A third case received a dog's parathyroid gland, which also showed good results. The mechanism of heterogenous parathyroid transplantation needs further investigation.In addition, the diagnosis and differential diagnosis of IHP were discussed.
