6.Alteration of AngⅡ, hs-CRP and BNP in patients with atrial fibrillation and its clinical significance
Chinese Journal of Primary Medicine and Pharmacy 2013;20(10):1497-1499
Objective To explore the clinical signifcance of changes of serum Ang Ⅱ,hs-CRP and BNP levels and the relationship with atrial fibrillation(AF),and the risk factors of AF recurrence. Methods 100 patients with AF were selected in this study, and they were divided into two groups:paroxysmal atrial fibrillation group (n =54),durative atrial fibrillation group(n =46),and 50 healthy subjects were selected as control. The levels of serum Ang Ⅱ,BNP were detected by radioimmunoassay, and hs-CRP was detected by immunoturbidimetry. Results The levels of Ang Ⅱ,hs-CRP and BNP in atrial fibrillation patients were significantly increased as compared to control group (P < 0.01).The levels of Ang Ⅱ,hs-CRP in the durative atrial fibrillation group were higher than those in the paroxysmal atrial fibrillation group. The levels of serum Ang Ⅱ,hs-CRP were positively correlated with serum BNP level (r =0.563,0.678,all P <0.01).The level of serum Ang Ⅱ was positively correlated with serum hs-CRP level(r =0.626,P < 0.01).Multiple logistic regression analysis revealed that hs-CRP and BNP were the important risk factors of atrial fibrillation recurrence[OR value =2.42,95% CI(1.84 ~ 5.26) ;OR value =2.14,95% CI(1.37 ~.86)].Conclusion The serum AngⅡ,hs-CRP and BNP may play important roles in the development of AF.hs-CRP and BNP may be the independent risk factors of AF.The detection of hs-CRP and BNP may be related to recurrence prognosis in patients with AF.
7.Application value of cancer gene c-erbB-2 and its protein product expression in evaluation of the diagnosis and prognosis of breast cancer with lymph node metastasis
Chinese Journal of Primary Medicine and Pharmacy 2013;20(21):3251-3253
Objective To discuss the application value of cancer gene c-erbB-2 and its protein products P185 in evaluation of the diagnosis and prognosis of breast cancer with lymph node metastasis, and to explore the relationship between c-erbB-2 expression and tumor pathology classification, relapse. Methods 112 cases with mammary disease were selected as the research subjects.According to different clinical diagnosis, they were divided into breast cancer lymph node metastasis group, breast cancer without metastasis group and mammary gland benign pathological change group. The c-erbB-2 gene and carcinoembryonic antigen(CEA) were detected by immunohistochemical method,enzyme-linked immunosorbent(ELISA) and electrochemical luminescence method. The c-erbB-2 protein(P185)and CEA of the three groups were compared, the sensitivity and specificity of c-erbB-2 expression and CEA in the diagnosis of breast cancer with lymph node metastasis were analyzed. At the same time, the relationship between cerbB-2 expression and tumor pathology classification, relapse was analyzed. Results P185 and CEA index of breast cancer lymph node metastasis group and breast cancer without metastasis group were significantly higher than those of mammary gland benign pathological change group(t =7.093,6.443,9.232,8.508,all P < 0.0 1),and P185 index of breast cancer lymph node metastasis group was significantly higher than without metastasis group (t =2.281,P <0.05),and there was no significant difference of CEA between the two breast cancer groups (t =0.406,P > 0.05).The sensitivity of c-erbB-2 expression in dignosis of breast cancer lymph node metastasis had no difference with CEA (x2 =0.632,P > 0.05),but the specificity was significantly higher than CEA(x2 =11.900,P < 0.01).The positive expression rate of c-erbB-2 and tumor pathology classification had no obvious relation:Postoperative recurrence rate of c-erbB-2 positive expression patients was significantly higher than the negative expression patients(x2 =5.141,P <0.05).Conclusion c-erbB-2 gene and its protein product has good sensitivity and specificity in the diagnosis of breast cancer lymph node metastasis, and has obvious correlation with postoperative breast cancer recurrence, has important reference value in evaluation of the diagnosis and prognosis of breast cancer.
8.Progress on diagnosis and treatment of multiple primary carcinomas of lung combined with other organs
Cancer Research and Clinic 2014;26(2):139-141
The multiple primary carcinoma (MPC) is defined as more than 2 or up carcinomas occured with in same or multiple organs in an individual.Along with the science development and advancement of human society, the life-span of human being has been prolonged, and more of MPC are identified. But the misdiagnosis of MPC as the metastases of primary tumors or missed diagnosis will mislead therapeutic regimens and exacerbate the diseases. Therefore, the accurate differentiation of MPC is becoming increasingly important. This article mainly reviews the epidemiology, diagnostic criteria, etiology, frequent sites of involvement, classification, and diagnostic protocols of MPC.
9.The levels of tumor necrosis factor-alpha、IL-6 and adiponectin associated with insulin resistance in type 2 diabetes mellitus
Chinese Journal of Primary Medicine and Pharmacy 2011;18(10):1308-1309
Objective To explore the relationships between Serum levels of tumor necrosis factor-alpha TNFα,1L-6,adiponectin and insulin resistance in type 2 diabetes. Methods 80 T2DM patients and 40 healthy controls were recruited.Levels of serum TNF-α,IL-6,adiponectin,plasma glucose, insulin and BMI were measured.Insulin resistance was evaluated by HOMA-IR.Results The levels of TNF-α and IL-6 in T2DM group were significantly higher than that in control group(all P<0.05);The levels of adiponeetin in T2DM group was significantly lower than that in control group(P<0.05).The levels of TNF-α and IL-6 were positively correlated with HOMA-IR(P<0.05),adiponeetin was egatively correlated nwith HOMA-IR(P<0.01).Conclusion TNF-α,IL-6 and adiponectin were closely related with insulin resistance, and should take parts in development of T2DM.
10.Challenges in Drug Discovery for Thiazolidinedione Substitute.
Acta Pharmaceutica Sinica 2011;1(3):137-142
Thiazolidinedione (TZD) is a powerful insulin sensitizer in the treatment of type 2 diabetes. It acts as a ligand to the nuclear receptor PPARγ (peroxisome proliferator-activated receptor-gamma) and induces transcription of PPARγ responsive genes. TZD controls lipid synthesis and storage in adipose tissue, liver and many other tissues through PPARγ. Derivatives of TZD, such as rosiglitazone (Avandia) and pioglitazone (Actos), are more powerful than metformin or berberine in insulin sensitization. Although they have common side effects such as weight gain and edema, these did not influence the side effects in general. However, recent findings of risk for congestive heart failure and bladder cancer have indeed significantly impaired their future in many countries. European countries have prohibited those drugs and in 2011, US will terminate application of rosiglitazone in clinics and hospitals. The multiple country actions may mark the end of TZD era. As a result, there is a strong demand for identification of TZD substitute in the treatment of type 2 diabetes. In this regard, literature about PPARγ ligands and potential TZD substitute are reviewed in this article. Histone deacetylase (HDAC) inhibitor is emphasized as a new class of insulin sensitizer here. Regulators of SIRT1, CREB, NO, p38, ERK and Cdk5 are discussed in the activation of PPARγ.
