1.Intussusception in an Adult due to Inverted Meckel's Diverticulum with Ectopic Pancreatic Tissue.

Adult intussusception is rare involving of only 1% of the causes of bowel obstruction. We report a case of a 39-year-old female with intussusceptions due to inverted Meckel's diverticulum. She visited our hospital for diffuse abdominal pain during 1 week and aggravated abdominal pain for 2 days. Vital signs were stable, and there was periumbilical tenderness. She had no history of abdominal operation. CT scan showed a 3.7x2.1 cm of fatty mass with focal intussusception in the distal ileum. When the emergency operation was performed, the patient was found to be suffering from ileocolic intussusception. A manual reduction of intussusception showed inverted Meckel's deverticulum at 65 cm proximal to the ileocecal valve, and the segmental resection of small bowel including a Meckel's diverticulum was performed. Pathologic examination revealed a Meckel's diverticulum containing a 0.6x0.6 cm sized aberrant pancreas.

2.Natural course of childhood chronic immune thrombocytopenia using the revised terminology and definitions of the international working group: a single center experience.

BACKGROUND: The immune thrombocytopenia (ITP) criteria were newly standardized by the International Working Group. Thus, we analyzed the natural course of childhood chronic ITP to predict the prognosis based on the revised criteria. METHODS: The medical records of children with chronic ITP from May 2000 to February 2013 in our institute were reviewed. RESULTS: Forty-seven children with chronic ITP who were not undergoing corticosteroid therapy were included. Their initial platelet count was 23+/-25x10(9)/L, and age at diagnosis was 6.3+/-4.1 years. The follow-up period was 5.4+/-3.7 years. Among them, 44.7% (21/47) showed spontaneous remission and maintained a platelet count > or =100x10(9)/L. And 66.0% (31/47) maintained a platelet count > or =50x10(9)/L until the last follow-up date. The time periods required for the platelet count to be maintained > or =50x10(9)/L and > or =100 x10(9)/L were 3.1+/-2.7 and 3.6+/-2.7 years. Age at diagnosis in the > or =50x10(9)/L group (5.7+/-4.4 years) was significantly lower than the age at diagnosis in the <50x10(9)/L group (7.4+/-3.3 years) (P=0.040). And follow-up period was the factor influencing prognosis between the > or =100x10(9)/L group and <50x10(9)/L group (P=0.022). CONCLUSION: Approximately 45% of children with chronic ITP recovered spontaneously about 3-4 years after the diagnosis and 2/3 of patients maintained a platelet count > or =50x10(9)/L, relatively safe state. Age at diagnosis of ITP and follow-up period were the factors influencing prognosis in this study.

3.Self-Injurious Thoughts and Behaviors Interview-Korean Version: Psychometric Properties

Objective:
There is currently no structured interview tool developed that comprehensively evaluates self-injurious thoughts and behaviors (SITB) in Korea. The Self-injurious Thoughts and Behaviors Interview (SITBI) collectively measures suicidal ideation, plans, gestures, attempts, and non-suicidal self-injuries (NSSI). The SITBI’s reliability and validity have been established with it being widely used in English speaking countries. This study evaluated the psychometric validity of the Korean version of the SITBI (SITBI-K).
Methods:
The SITBI’s validity as a diagnostic assessment tool for NSSI and suicidal behavior disorder (SBD), as defined by the Diagnostic and Statistical Manual of Mental Disorders-5th edition, was examined. Analyses were performed on 108 university students reporting experiences of suicidal thoughts and behaviors (female 84.26%, mean age=22.10, ±SD 3.33).
Results:
The SITBI-K displayed excellent interrater reliability, with a credible test-retest reliability at two months. Construct validity examined the correlation between the SITBI-K’s modules and approved the self-report results. Appropriate convergent and discriminant validities were obtained for suicidal ideation, plans, gestures, attempts, and NSSI.
Conclusion
The SITBI-K showed excellent psychometric validity at a level comparable to the original. Its clinical utility for both NSSI and SBD diagnoses was confirmed.
4.A Pilot Study on the Validity and Reliability of the Korean Version of the Oswestry Disability Index in a Farming Community.

OBJECTIVES: The Korean version of the Oswestry Disability Index (ODI) is the most widely used tool validated instrument for measuring and the level of disability associated with low back disorders. We wanted to validate use of the Korean version of the ODI in Korean farmers with low back pain. METHODS: The object of this study was to examine the reliability and validity of the Korean version of the ODI. The Korean version was tested on 53 farmers (62.3+/-10.3 years of age) with low back-related disorders. We investigated the Test-retest reliability, internal consistency, and construct validity of the ODI. The Test-retest reliability was assessed in 30 farmers within a time interval of approximately 14 to 18 days. Differences between the Korean version of the ODI and the Short Form 12 (SF-12), which includes 8 domains (general health, physical functioning, role-physical limitation, bodily pain, role-emotional limitation, mental health, vitality, social functioning) were analyzed for construct validity. The correlation of the Korean version of the ODI with the SF-12 was analyzed, as well. RESULTS: The intra-class correlation coefficient of test-retest reliability was 0.854. Reliability estimated by the internal consistency reached a Cronbach's alpha of 0.879. The correlation between 7 domains of the SF-12 except for the mental health domain and the Korean version of the ODI was statistically significant (p<0.05). Four domains (general health, physical functioning, role-physical limitation, bodily pain) that measured physical status all showed high correlations (p<0.01), as did a domain that measured mental status (role-emotional limitation) (p<0.01). CONCLUSIONS: The results showed that the Korean version of the ODI is a reliable and valid instrument to measure the level of disability in Korean farmers with low back-related disorders. The use of this instrument is recommended for future clinical trials in Korea.

6.Evaluation of PostNeoadjuvant Chemotherapy Pathologic Complete Response and Residual Tumor Size of Breast Cancer: Analysis on Accuracy of MRI and Affecting Factors

Purpose:
To evaluate the accuracy of MRI in predicting the pathological complete response (pCR) and the residual tumor size of breast cancer after neoadjucant chemotherapy (NAC), and to determine the factors affecting the accuarcy.
Materials and Methods:
Eighty-eight breast cancer patients who underwent surgery after NAC at our center between 2010 and 2017 were included in this study. pCR was defined as the absence of invasive cancer on pathological evaluation. The maximum diameter of the residual tumor on post-NAC MRI was compared with the tumor size of the surgical specimen measured pathologically. Statistical analysis was performed to elucidate the factors affecting pCR and the residual tumor size-discrepancy between the MRI and the pathological measurements.
Results:
The pCR rate was 10%. The diagnostic accuracy of MRI and the area under the curve for predicting pCR were 90.91% and 0.8017, respectively. The residual tumor sizes obtained using MRI and pathological measurements showed a strong correlation (r = 0.9, p < 0.001), especially in patients with a single mass lesion (p = 0.047). The size discrepancy between MRI and the pathological measurements was significantly greater in patients with the luminal type (p = 0.023) and multifocal tumorson-mass enhancement on pre-NAC MRI (p = 0.047).
Conclusion
MRI is an accurate tool for evaluating pCR and residual tumor size in breast cancer patients who receive NAC. Tumor subtype and initial MRI features affect the accuracy of MRI.

8.A Clinical and Statistical study of Pregnancy and Delivery in the Mid-teenager mothers.

OBJECTIVE: This study is designed to help pregnancy and delivery of mid-teenager mothers by analyzing the clinical features of pregnancy and delivery of mid-teenager mothers ranging from 13 to 16 years old. METHODS: This study includes 77 mothers aged 16 years and younger among the 3,863 mothers who delivered in our hospital from January 1998 to December 2005. We selected 100 mothers who aged between 17 and 19 years, 100 mothers who aged between 20 and 29 years as control groups respectively during the same period and statistical analysis is applied to compare the results. RESULTS: In this study, mid-teenagers constitutes 1.99% of all pregnant women. The number of antenatal care, the number of women experienced abortion, and age of menarche were significantly low in the mid-teenager mothers. Especially, the risks of anemia and meconium staining were significantly higher and the rate of cesarean section was significantly lower than those of the mothers in twenties. The risks of prematurity, low birth weight infant, preterm labor, and birth canal injury were higher than those of control groups, but there were no significant differences in three groups. CONCLUSION: Mid-teenager mothers were known to be high risk mothers in obstetrics due to immaturity in physics and low socioeconomic status. However, the risks can be decreased by adequate prenatal care, sufficient nourishment and maintenance of good habits based on medical and socioeconomic supports.

9.Pathogenic Etiology and Clinical Indictors of Bacterial Infectoin in Febrile Infants Aged Less than 3 Months: A Single Institute Stduy.

To analyze the infectious causes and clinical symptoms of febrile infants aged less than 3 months presenting to a Pediatric Emergency Medical Center (PEMC) and to propose more efficient, evidence-based management and treatment. We conducted a retrospective study of 462 febrile infants aged less than 3 months who visited PEMC at Keimyung University Dongsan Medical Center from January 2015 to June 2016. Infants' sex, age, fever duration, and laboratory findings, including bacterial or viral pathogens, were recorded. To evaluate clinical signs, one point per sign was given for grunting, decreased activity, and the presence of cyanosis; total scores were compared between the bacterial infection (BI) and non-bacterial infection (NBI) groups. BI was diagnosed in 118 (25.5%) infants, and no BI was diagnosed in 344 (74.5%) infants. Escherichia coli was the most frequently isolated pathogen, accounting for 80.5% (n = 95) of all infections (n = 118). Statistically significant differences in sex, the duration of fever, sign scores, white blood cell count, neutrophil count, lymphocyte count, and C-reactive protein (CRP) level were found between the BI and NBI groups. The cut-off value for CRP was 1.445 mg/dL, with sensitivity and specificity values of 76.3% and 75.9%, respectively, in BI patients, as determined by the receiver operating characteristic curve. As more PEMCs are being built in Korea, hospital accessibility is better; thus, it may be possible to perform outpatient management of young, febrile infants aged younger than 3 months without antibiotics and lumbar puncture using individual sign scores and laboratory findings.

10.Usefulness of Routine Screening for Infants with Single Umbilical Artery.

PURPOSE: Single umbilical artery is usually detected by antenatal fetal sonography or by postnatal primary physical exams. Routine screening test is usually recommended because of frequent association with abnormalities in multiple organs or in chromosomes. This study was performed to find the necessity of evaluating combining anomalies and chromosomes in neonates with single umbilical artery. METHODS: Retrospective analysis had been performed based on admission records and laboratory data, including simple X-ray, echocardiography, abdominal sonography, and chromosomal study, of 6, 636 neonates born last decade from September, 1993 to August, 2003 at Ewha Womans University hospital. RESULTS: There were 73 (1.1%) neonates born with single umbilical artery that matched the incidence in general population. Congenital malformation was observed in 28 neonates which included cardiovascular (n=16, 21.9%), gastrointestinal (n=6, 8.2%), geni tourinary (n=5, 6.8%), musculoskeletal (n=4, 5.5%), central nervous system (n=1, 1.4%), and other (n=1, 1.4%) abnormalities. No chromosomal abnormalities were detected in this group. CONCLUSION: Because the incidences of structural abnormalities in genitourinary system, gastrointestinal system and cardiovascular system were high as previously reported, it is highly recommended to perform imaging studies including echocardiography and abdominal sonography to screen these anomalies. However necessity of the chromosomal study should be reconsidered based on large study population in isolated single umbilical artery patient.
