1.Comparison of Benign and Malignant Microcalcifications.

PURPOSE: To evaluate morphologic characteristics of the microcalcifications on mammography that were confirmed pathologically. MATERIALS AND METHODS: Forty five cases of microcalcifications on mammography (fifteen cases of benign lesion, thirty cases of maiignancy) were retrospectively reviewed. RESULTS: The number of microcalcifications within 1 cm2 were more than 5 in 22 cases (73%) of 30 malignancy and less than 5 in 11 cases (73%) of 15 benignity. The heterogeneity of microcalcifications were seen in 26 cases of malignancy (87%) and the homogeneity of microcaicifications were 11 cases of benignity (73%). The morphologic characteristics of the microcalcifications were linear-V shape in 9 cases (30%), punctate shape in 8 cases (27%), fine stippled shape in 7 cases (23%), and round dot shape in 6 cases (20%) of malignancy and, round dot shape in 11 cases (73%), punctate shape in 2 cases (13%), and linear-V shape in 2 cases (13%) of benignity. CONCLUSION: Numerous irregular microcalcifications that are heterogenous in size and morphology were strong indicators of malignancy.

2.Rotational Deformity after Closed Interlocking nailing of Femoral Fracture

Closed interlocking nailing has become a common method of treatment of femoral fractures in adults. Interlocking nailing had a tendency toward rotational deformity and shortening. Therefore, we determined anteversion of the femoral neck after closed IM nailing of the femoral fractures. Femoral rotational deformity in both leg was determined by the construction of a line through the femoral neck and another through the transcondylar plane of the distal portion of the femur, using the CT scanning. We performed a retrospective study of 15 patients treated with closed IM nailing for femoral shaft fractures from 1989 to 1994. 1. The average differences in anteversion angle was 12.9 degree. 2. 3 cases(50%) in 6 cases that have difference more than 15° are accompanied with ispilateral injury of the knee joint and the lower leg. 3. Angle difference of the rotational deformity of proximal or distal femoral fractures is 15.5° which is 3.5° larger than that of fractures of femoral midshaft, 12.0°.

3.Anterior impingement of the Ankle Treated by Arthroscopic Removal of bony Spur

From September 1992 to April 1994, we reviewed 7 patients after arthroscopic surgery for anterior impingement in the ankle, who complained ankle pain and limitation of motion. All patients were mem and main symptoms were severe pain at full dorsiflexion of ankle and limitation of motion. Bony spur was located on the anterior aspect of tibia, and it was possible to resect the tibial bony spur arthroscopically without distraction devices. Mean operation time was 55 minutes. Pain, swelling and limitation of motion were much improved. There was no postoperative complication. Arthroscopic resection of the anterior tibial bony spur is an effective treatment for anterior impingement in the ankle.

4.Subacute Hematogenous Ostemyelitis in Children

Subacute hematogenous ostemyelitis is mostly likely due to an altered relationship between host and pathogen, associated with increase host resistance, decreased virulence of the causative organism and/or antibiotics modification. In a retrospective study of the hematogenous ostemyelitis in 156 cases, 19 cases of them were the subacute form of the ostemyelitis. Pain is the most consistent symptom and most of the constitutional symptoms were usually mild. However, the ESR was often elevated. From our data, the following results were obtained. 1. The most common skeletal involvement was the distal part of the tibia(8 out of 19 patients). 2. The causative organism was staphylococcus aureus by the bacterial culture of the specimens. 3. In the literature, acute hematogenous ostemyelitis rarely cross the physis in patients older than 18 months of its age. However, subacute ostemyelitis frequently does cross the physis(5 out of 19 patients). 4. Cross physeal lesions were healed without any permanant damage to the growth plate in our cases(5 out of 19 patients). 5. All patients except for one case were treated by curettage and antibiotics. 6. The mush higher incidence of the cross physeal lesions may be expected by MRI in identifying the lesion. 7. Seven out of 19 patients had radiographic findings similar to eosinophilic granjuloma, Ewing's sarcoma or osteosarcoma.

6.Buschke-Ollendorff Syndrome: A Case Report

The osteopoikilosis is commonly known as harmatoma in which metaphyseal and epiphyseal area of long bones and the bone of pelvis, hands, feet and et al, contain islands of dense cortical bone with normal Harversian system without any symptoms. The radiologic findings of the osteopoikilosis is multitude of oval or well-circumscribed areas of increased density, 2 to 10 mm in size, in symmetrical distribution and normal uptake in bone scan. Buschke-Ollendorff syndrome, which is transmitted by autosomal dominant, is characterized by the association of osteopoikilosis and connective tissue nevi which are popular and symmetrically distributed lesions on chest, back, buttock, thigh or arm. Authors report three Buschke-Ollendorff syndrome, studied by CT scan, MRI, bone scan and bone and skin biopsy, among five patients associated with osteopoikilosis found by simple radiologic study from ten members in one family with their pedigree.

7.Analysis of Research Status on Disease of Women and Children in North Korea: Literature Analysis Through Academic Journals

Purpose:
Since the division of the Korean Peninsula into South and North Korea in 1948, the gaps in the medical system and the socioeconomic status between these 2 countries has expanded. The purpose of this study was to analyze the distribution of different areas of research, as well as the types of research that was conducted in the North Korean medical journal, ‘Pediatrics, Obstetrics and Gynecology.’ This study aimed to specifically investigate the medical research status of healthcare in women and children.
Methods:
This study analyzed a total of 949 papers that were published in the North Korean medical journal, ‘Pediatrics, Obstetrics and Gynecology.’ Papers from January 2015 to December 2019 were included. For the analysis, the 8th Korean Standard Classification of Diseases, disease name, subspecialty classification, and research classifications were extracted from the papers.Result: Of the 949 papers included in the analysis, 495, 366, and 88 studies were conducted in the major fields of obstetrics and gynecology, pediatrics, and on breast surgery, respectively. Within the field of obstetrics and gynecology, the most studied subspecialties were maternal and fetal medicine, general gynecology, and gynecologic oncology. To a lesser extent, the following subspecialties were reported: gynecologic endocrinology (6.7%), infertility (6.3%), and urogynecology (approximately 1%).
Conclusion
Although North Korean papers differ from those published worldwide (due to government intervention), they are still a useful source of information as they are easier to access than alternative data sources. The results from this study have implications for understanding the distribution of research on different areas of healthcare, as well as the burden of healthcare in North Korea.
8.A Case of Acute Liver Failure Associated with Methimazole in Patient with Underlying Liver Disease.

We report a case of methimazole-induced acute hepatic failure, which occurred 4 days after initiation of drug in a 51-year-old man with hyperthyroidism. Liver function was evaluated before taking methimazole, total bilirubin was 14.2 mg/dL. This finding suggested toxic hepatitis d/t herbal medication or unknown liver disease. He was treated with methimazole with increasing doses from 15 to 45 mg/day, he developed liver failure gradually, despite of suspending methimazole. From the time of admission, his liver function test was abnormal and liver cirrhosis was suspected by liver sonography. With aggravated liver function, he died of renal failure, sepsis of unknown origin and respiratory failure. Fulminant hepatitis rarely occurs in methimazole users, and spontaneous recovery is expected. But this case shows that methimazole-induced hepatotoxicity with possible underlying liver disease could increase the risk of poor outcomes. And in case of continued deterioration of liver function, prompt liver transplatation should be considered.

9.Fungal Ball in Sinus: Multi-Center Study in Daegu.

BACKGROUND AND OBJECTIVES: Fungal sinusitis has been reported increasingly in immunocompetent patients and the fungus ball is the most frequent and best recognized form of fungal sinusitis. Fungus ball is usually unresponsive to appropriate medical treatment and surgery is the treatment of choice. The aim of this study is to evaluate the incidence, clinical symptoms and signs, and CT findings, the proportion of the patients with fungus ball. MATERIALS AND METHODS: In retrospective study, we reviewed medical records and CT findings of 255 patients with fungus ball confirmed by histopathologically among 6,217 patients who had undergone endoscopic sinus surgery between 1995 and 2004 at five medical centers in Daegu. RESULTS: The symptoms of fungus ball were nasal obstruction (64.4%), rhinorrhea (56.7%), PND (49.2%), cheek pain (26.4%) and foul odor (25.1%). Fungus ball was mostly encountered in only one maxillary sinus (76.4%) of an otherwise healthy person and preoperative computerized tomography (CT) revealed mottled calcific densities within the involved sinus in 55.2% of cases. The success rate of fungus ball surgery by endoscopy was 98.4%. CONCLUSION: This is the retrospective study to evaluate the prevalence of fungus ball (nearly 5%) among the endoscopic sinus surgery. To make a diagnosis of fungus ball, a high index of suspicion to unilateral symptomatic chronic sinusitis, often painful, unresponsive to appropriate medical treatment is necessary and endoscopic sinus surgery is and should remain the mainstay of treatment.

10.Full-mouth rehabilitation by immediate implantation combined with orthognathic surgery: a clinical report.

Clinical therapy that combines full-mouth rehabilitation with immediate implantation and orthognathic surgery poses a challenge to prosthodontists. This clinical report describes a multidisciplinary approach to the diagnosis and treatment of a patient presenting with skeletal discrepancy and rampant caries. The results thus achieved indicate that full-mouth rehabilitation by fixed immediate and early loading implantation accompanied by orthognathic surgery can be a predictable and effective treatment procedure.
