1.Antimicrobial resistance profile of the gram-negative bacilli isolated from urinary tract infections

Objective? To?investigate?the?antimicrobial?resistance?and?beta-lactamase?production?profile?in?the?gram-negative?bacilli?isolated from urinary tract infections in the Second People's Hospital of Gansu Province during the period from 2014 to 2015. The results will provided to clinicians for better antimicrobial treatment. Methods? The?bacterial?isolates?were?identified?via?conventional?laboratory?tests?or?automatic?identification?systems?and?subjected?to?antimicrobial?susceptibility?testing?by?using?Kirby-Bauer?method.?Three-dimensional test was used to detect the enzymes conferring antimicrobial resistance. The susceptibility testing results were interpreted according to the CLSI breakpoints issued in 2012. Results A total of 987 gram-negative strains were isolated from urinary tract infections, including E. coli (51.6%, 509/987), Enterobacter (11.0%, 109/987), P. aeruginosa (10.5%, 104/987), K. pneumoniae (9.9%, 98/987), P. mirabilis (9.3%, 92/987), C. freundii (4.7%, 46/987), and other gram-negative bacilli (2.9%, 29/987). ESBLs were produced?in?494?(50.1?%)?of?the?987?strains?of?gram?negative?bacilli.?Preliminary?screening?test?identified?243?AmpC?beta-lactamases?producing?strains,?and?135?(13.7?%)?strains?were?confirmed?by?three-dimensional?test.?Both?ESBLs?and?Amps?beta-lactamases?were?produced in 16 (1.6%) strains. The prevalence of ESBLs-producing strains was 79.6% in E. coli, 34.6% in P. aeruginosa and 37.8% in K. pneumoniae?isolates.?Metallo-β-lactamase?or?KPC?beta-lactamase?was?not?identified.?The?antimicrobial?resistance?was?serious?in?gram-negative bacilli, which showed relatively low resistance rate to imipenem (0.02%), amikacin (10.6%), cefoperazone-sulbactam (23.8 %), and nitrofurantoin (25.2 %). E. coli prevalence of levofloxacin,?ciprofloxacin?resistance?respectively?were?89.8%,?91.8%. The gram-negative bacilli from urinary tract also showed various levels of resistance to cephalosporins. Conclusions The gram-negative bacilli isolated from urinary tract infections are mainly E. coli. The gram-negative isolates show high level antimicrobial resistance and high prevalence of beta-lactamases. Imipenem, amikacin, cefoperazone-sulbactam, and nitrofurantoin still have very high antibacterial activity against these isolates in vitro.
2.Infection after Ophthalmic Operation in 276 Patients with AIDS:An Observation

OBJECTIVE To observe the infection after ophthalmic operation in 276 patients with AIDS.METHODS This trial involved 276 patients(276 eyes) with AIDS.They were divided into 2 groups according to surgical sites:112 patients(112 eyes) were in the first group who were undergone eyelid or conjunctival operation and 164 patients(164 eyes) were in the second group who were undergone cataract or glaucoma operation.There were 325 patients(325 eyes) who did not suffered with AIDS in the control group: 130 patients(130 eyes) were in the third group who were undergone eyelid or conjunctival operation and 195 patients(195 eyes) were in the fourth group who were undergone cataract or glaucoma operation.The infections were observed after operation.RESULTS There were 30 cases(26.79%),8 cases(4.88%),12 cases(9.23%) and 8 cases(4.10%) suffered with infection after operation in four groups respectively.There were 38 infected cases(13.77%) in patients with AIDS and more than that in patients who did not suffered with AIDS(20 cases and 6.15% ?2=9.07,P
3.Rapid Determination of Content of 3-Butylphthalide in Angelicae Sinensis Radix by Near-infrared Diffuse Reflection Spectrum

Objective To establish a quantitative model to rapidly determine content of 3-butylphthalide in Angelicae Sinensis Radix based on near-infrared diffuse reflectance spectroscopy (NIR). Methods Totally 145 batches of samples of Angelicae Sinensis Radix from 14 areas of Gansu and Yunnan Provinces were collected and dried in the shade. With the content of 3-butylphthalide determined by RP-HPLC-DAD as reference, the NIR spectra of 145 batches of Angelicae Sinensis Radix were collected by integrating sphere diffuse reflectance, and the PLS (partial least squares) quantitative analysis model was established between 7415-4056 cm-1. Results The Rc (correlation coefficient between actual content and predicted content), RMSEC (the root-mean-square error of calibration), RMSEP (the root-mean-square error of prediction), Rcv (cross-validation correlation coefficient), RMSECV (the root-mean-square error of cross-validation), and average predicted recovery reached 0.976 7, 0.614 3, 0.706 5, 0.897 8, 1.155 4 and 102.08%, respectively. Conclusion This prediction model was accurate, reliable, and can be used to quantitatively determine the content of 3-butylphthalide in Angelicae Sinensis Radix.
4.Neutralizing anti-CD44 antibodies suppresses the growth of B16 cells and enhances AKT-mediated glycolytic metabolism in melanoma

Objective CD44, a cell surface glycoprotein, plays an important role in tumor growth and glycolysis.The aim of this study was to investigate the effects of neutralizing CD44 antibodies on the growth and glycolytic metabolism of B16 cells in melanoma in vitro.Methods B16 cells were treated with control antibodies (50 μg/mL) or different concentrations of CD44 antibodies (2, 10, and 50 μg/mL) for 24 hours, followed by examination of the activation of the AKT pathway in the B16 cells by Western blot.Then the tumor cells were also treated with control antibodies (50 μg/mL) or CD44 antibodies (50μg/mL) after pretreated with API-2 (4 μmol/L) in a parallel test.After 48 hours of treatment, the expression of lactate dehydrogenase A (LDHA) in the B16 cells and the level of lactate in the culture supernatant were detected by immunofluorescence and colorimetry, respectively.Lastly, the B16 cells were treated with control antibodies (50μg/mL), API-2 (4 μmol/L), CD44 antibodies (50μg/mL), or API-2 + CD44 antibodies for 96 hours, followed by measurement of the proliferation of the cells by MTT and their apoptosis by AO/EB and AnnexinV staining.Results In comparison with the control antibody group, the level of AKT phosphorylation (p-AKT) in the B16 cells showed a concentration-dependent increase in the 2, 10, and 50 μg/mL CD44 antibody groups (1.00±0.25 vs 2.51±0.32, 3.89±0.46, and 4.07±0.42, P<0.01), and the expression of LDHA was increased by (2.13±0.24) times, with the lactate level in the culture supernatant significantly elevated from (35.32±3.24) to (56.34±8.19) mmol/L (P<0.01) after 96 hours of treatment with 50 μg/mL CD44 antibodies.Treatment with API-2+CD44 antibodies, however, suppressed the increase in the LDHA expression and reduced the level of lactate.Compared with the control antibody group, the proliferation rate of the B16 cells was markedly decreased in the API-2, CD44 antibody, and API-2+CD44 antibody groups ([103±12.91] vs [84.87±19.35], [71.35±16.23], and [41.16±9.15]%, P<0.05), while the apoptosis rate remarkably increased ([5.23±0.96] vs [13.65±4.27], [19.21±3.53], and [43.21±7.87]%, P<0.01).Conclusion Neutralizing the function of CD44 in the B16 cells in vitro can inhibit the growth of the cells and promote AKT-mediated glycolytic metabolism, while suppressing the AKT pathway may enhance the antitumor activity of the CD44 antibody.

An HA-1-DC nucleic-acid vaccine was constructed to induce anti-leukemia effect after hematopoietic stem cell transplantation (HSCT). DCs were generated from HSCT donors in vitro, and its immunologic activity was assayed by using flow cytometry and mixed lymphocytes reaction. HA-1 gene was electroporated into the cultured DCs to construct a DC nucleic-acid vaccine. After transfection for 48 h, the expression of HA-1 protein could be detected by using Western blot. The DCs were cultured with syngenic lymphocytes to induce specific cytotoxic T lymphocytes (CTLs). The cytoxicity of the CTLs was detected by LDH assay. The results showed that The DCs derived from peripheral blood monocytes (PBMCs) expressed the phenotype of DCs, and were effective in stimulating proliferation of the allogenic lymphocytes. After electroporating for 48-h, HA-1 protein was detected by using Western blot. The cytotoxicty of inducing CTLs was higher than the control group. It was suggested that minor histocompatibility antigen HA-1 could be considered as a target of immunotherapy against leukemia after HSCT.

7.Comparative study of the efficacy and safety between mono-chemotherapy and targeted therapy for patients over 80 years old with advanced non-small cell lung cancer

Objective To compare the efficacy and safety between mono-chemotherapy and targeted therapy as first-line regimen for patients over 80 years old with advanced non-small cell lung cancer (NSCLC).Methods 108 NSCLC patients aged ≥80 years in our hospital were divided into two groups according to the therapeutic program,of which group A was mono-chemotherapy group (n =60) and group B was targeted therapy group (n =48).The primary endpoint was overall survival (OS) and progression free survival (PFS),while secondary endpoint was objective response rate (RR),disease control rate (DCR) and safety.Results The median PFS,and median OS in group B were significantly longer than those in group A (P =0.013,0.025).The 1-year survival rate of group B was significantly higher than that of group A (P =0.001),and there was no significant difference between the two groups (P =0.605).There was also no statistically significant difference in RR between the two groups (P =O.408).DCR in Group B was significantly higher than that in group A (P =0.043).The incidence of skin rash in group B was significantly higher than that in group A,and the incidence of adverse reactions and hematological toxicity in group A was significantly higher than that in group B.Conclusions Compared with single-agent chemotherapy,epithelial growth factor receptor-tyrosinekinase (EGFR-TKI) targeted therapy has longer PFS and OS benefits for patients ≥ 80 years with advanced NSCLC,and was well tolerated by patients.

A 53-year woman and her 18-year daughter presenting with bone pain, bone fractures, bone deformities and short stature were admitted to Gansu Provincial People′s Hospital in March 2021. Laboratory tests showed low blood phosphorus, low renal phosphorus threshold, normal or low blood calcium, and normal or increased PTH. The high-throughput sequencing indicated heterozygous mutations of the PHEX gene (Phosphate-regulating gene with Homology to Endopeptidases on the X chromosome) in two patients, which was not detected in other family members; finally the diagnosis of X-linked dominant hypophosphatemic rickets/osteomalacia(XLH)was confirmed for these two patients. Treated with neutral phosphorus solution and Rocaltrol, bone pain was relieved completely in the younger patient, but not for her mother due to long disease course and severe complications. Because of the large heterogeneity of the disease there are high missed diagnosis and misdiagnosis rates for XLH. In this paper a pedigree of XLH is reported with literature review.
10.The effect of nursing intervention on peri-operative percutaneous coro-nary angioplasty and stent implantation

Objective To observe the effects of percutaneous coronary angioplasty and stent implantation in periopera-tive nursing intervention effect. Methods From January 2012 to January 2014 year in our hospital, 64 cases of percuta-neous transluminal coronary angioplasty and stent implantation were observed, according to different nursing patients were randomly divided into intervention group (n=33﹚ and the control group(n=32﹚,the control group adopted random symptomatic care, the intervention group focused on the implementation of perioperative nursing, included preopera-tive preparation,and psychological nursing,observation and nursing cooperation during the operation, postoperative complications and nursing, observation of life, patients,the SAS score, SDS score, the incidence and nursing satisfac-tion, complications were compared between the two groups before and after treatment. Results After treatment, two groups of SAS, SDS scores were significantly decreased than before treatment(P<0.05﹚, and the SAS, SDS scores of pa-tients in the intervention group were lower than control group (P<0.05﹚. After interventional,nursing satisfaction rate was 93.75% in the intervention group,nursing satisfaction rate was 71.88%,was significantly higher than the con-trol group (χ2=6.348,P<0.05﹚. After intervention, the incidence of complications of intervention group was 12.50%, the control group was 31.25%, the incidence of postoperative complications of the intervention group was significantly lower than the control group(P<0.05﹚. Conclusion Percutaneous coronary angioplasty and stent implantation of perioper-ative nursing intervention measures can improve nursing satisfaction, improve the patients' negative psychological shape body, reduce the incidence of complications, is conducive to the recovery of patients with the disease.
